We present an unusual case of portal vein thrombosis with a splanchnic hemangioma secondary to acute biliary pancreatitis. We report a 45-year-old patient, who has systemic arterial hypertension in treatment, was admitted for abdominal pain in the epigastrium, with irradiation to the right hypochondrium, accompanied by nausea and vomiting of 10 occasions of bile content, physical examination with pain in the right hypochondrium, Murphy positive. We have laboratory studies with a lipase of 788, so a diagnosis of pancreatitis is made with an etiology to be determined. The laboratories suggestive of acute biliary pancreatitis (lipase 788.71);an imaging study was subsequently performed (ultrasonography) with the result of stone in the common bile duct. A laparoscopy was performed with relative improvement, so he was discharged and returned 20 days after surgery due to abdominal pain of the same intensity in the left hypochondrium. Ending his hospitalization with a splenectomy for splenic hemangioma with portal vein thrombosis. 展开更多

Neutron radiography is a crucial nondestructive testing technology widely used in the aerospace,military,and nuclear industries.However,because of the physical limitations of neutron sources and collimators,the resulting neutron radiographic images inevitably exhibit multiple distortions,including noise,geometric unsharpness,and white spots.Furthermore,these distortions are particularly significant in compact neutron radiography systems with low neutron fluxes.Therefore,in this study,we devised a multi-distortion suppression network that employs a modified generative adversarial network to improve the quality of degraded neutron radiographic images.Real neutron radiographic image datasets with various types and levels of distortion were built for the first time as multi-distortion suppression datasets.Thereafter,the coordinate attention mechanism was incorporated into the backbone network to augment the capability of the proposed network to learn the abstract relationship between ideally clear and degraded images.Extensive experiments were performed;the results show that the proposed method can effectively suppress multiple distortions in real neutron radiographic images and achieve state-of-theart perceptual visual quality,thus demonstrating its application potential in neutron radiography. 展开更多

Artificial intelligence(AI)and deep learning are becoming increasingly powerful tools in diagnostic and radiographic medicine.Deep learning has already been utilized for automated detection of pneumonia from chest radiographs,diabetic retinopathy,breast cancer,skin carcinoma classification,and metastatic lymphadenopathy detection,with diagnostic reliability akin to medical experts.In the World Journal of Orthopedics article,the authors apply an automated and AIassisted technique to determine the hallux valgus angle(HVA)for assessing HV foot deformity.With the U-net neural network,the authors constructed an algorithm for pattern recognition of HV foot deformity from anteroposterior highresolution radiographs.The performance of the deep learning algorithm was compared to expert clinician manual performance and assessed alongside clinician-clinician variability.The authors found that the AI tool was sufficient in assessing HVA and proposed the system as an instrument to augment clinical efficiency.Though further sophistication is needed to establish automated algorithms for more complicated foot pathologies,this work adds to the growing evidence supporting AI as a powerful diagnostic tool. 展开更多

Owing to the immobility of traditional reactors and spallation neutron sources,the demand for compact thermal neutron radiography(CTNR)based on accelerator neutron sources has rapidly increased in industrial applications.Recently,thermal neutron radiography experiments based on a D-T neutron generator performed by Hefei Institutes of Physical Science indicated a significant resolution deviation between the experimental results and the values calculated using the traditional resolution model.The experimental result was up to 23%lower than the calculated result,which hinders the achievement of the design goal of a compact neutron radiography system.A GEANT4 Monte Carlo code was developed to simulate the CTNR process,aiming to identify the key factors leading to resolution deviation.The effects of a low collimation ratio and high-energy neutrons were analyzed based on the neutron beam environment of the CTNR system.The results showed that the deviation was primarily caused by geometric distortion at low collimation ratios and radiation noise induced by highenergy neutrons.Additionally,the theoretical model was modified by considering the imaging position and radiation noise factors.The modified theoretical model was in good agreement with the experimental results,and the maximum deviation was reduced to 4.22%.This can be useful for the high-precision design of CTNR systems. 展开更多

BACKGROUND Giant hepatic cavernous hemangioma with multiple satellite nodules is a rare subtype of hepatic cavernous hemangioma,the most common vascular liver tumor.We report on a tumor with unusual histologic features:(1)Finger-like infiltration pattern;(2)lack of encapsulation;(3)blurred tumor/liver interface;and(4)massive satellitosis-referring to the article“Hepatic cavernous hemangioma:underrecognized associated histologic features”.CASE SUMMARY A 60-year-old man presented with increasing uncharacteristic abdominal discomfort and mildly elevated blood parameters of acute inflammation.Imaging revealed an unclear,giant liver tumor of the left liver lobe.A massive vascular tumor with extensive satellitosis broadly infiltrating the adjacent liver parenchyma was resected via hemihepatectomy of segmentsⅡ/Ⅲ.Histopathological diagnosis was giant hepatic cavernous hemangioma with multiple satellite nodules,featuring unusual characteristics hardly portrayed in the literature.Retrospectively,this particular morphology can explain the difficult pre-and perioperative diagnosis of a vascular liver tumor that is usually readily identifiable by modern imaging methods.CONCLUSION This case emphasizes the exact histological workup of tumor and tumor-induced parenchyma changes in radiologically unclassifiable liver tumors. 展开更多

BACKGROUND Primary abdominal and retroperitoneal cavernous hemangioma is a vascular tumor and rarely seen in the clinic.Due to the lack of specific imaging features,retroperitoneal cavernous hemangioma cannot be diagnosed accurately.Some symptoms may develop with the enlargement of lesion volume or the occurrence of complications such as rupture or oppression.We report here a special case who was admitted with chronic abdominal pain.Admission examination suggested a retroperitoneal lymphatic duct cyst.Laparoscopic resection of the retroperitoneal mass was performed,and histological examination confirmed retroperitoneal cavernous hemangioma.CASE SUMMARY The patient was a 43-year-old Tibetan woman with intermittent left lower abdominal pain and discomfort 3 years ago.Ultrasonography revealed a cystic mass in the retroperitoneum with clear boundaries,internal septa,and no blood flow signal.Computed tomography(CT)and magnetic resonance imaging(MRI)showed an irregular space-occupying mass in the retroperitoneum,and retroperitoneal lymphatic cyst was considered.Plain CT scanning showed multiple cystlike hypo-intense shadows in the retroperitoneum,partially fused into a mass,and no obvious enhancement was found on enhanced scanning.MRI showed multiple irregular clump-like long T1 and long T2 signal shadows above the pancreas,within which linear short T2 signal shadows were seen.Diffusionweighted imaging sequence showed hypo-signal shadows,without obvious enhancement on enhanced scanning.Ultrasound,CT,and MRI all suggested the possibility of retroperitoneal lymphatic cyst.However,the patient was finally diagnosed with retroperitoneal cavernous hemangioma by pathological examination.CONCLUSION Retroperitoneal cavernous hemangioma is a benign lesion,and it is difficult to make a diagnosis preoperatively.Surgical resection may be the only treatment,which not only allows histopathological confirmation as a diagnostic purpose and excludes any risk of malignancy,but also avoids invasion of adjacent tissues,oppression,and other complications as a therapeutic goal. 展开更多

BACKGROUND Postoperative complications of triceps surae intramuscular hemangioma surgery with talipes equinovarus have rarely been described,and the evidence for treatment is limited.The purpose of this case study was to report the new application of the Ilizarov technique,which successfully treated talipes equinovarus in adults after triceps surae intramuscular hemangioma.CASE SUMMARY A 29-year-old woman treated with the Ilizarov technique for talipes equinovarus in the right leg after triceps surae intramuscular hemangioma surgery.The equinus deformity was roughly corrected after 2 years of follow-up,without significant secondary sequelae.CONCLUSION Talipes equinovarus caused by postoperative sequelae of intramuscular hemangioma was successfully corrected by the Ilizarov technique.The Ilizarov technique may be used for treating talipes equinovarus caused by various causes. 展开更多

BACKGROUND Congenital infantile fibrosarcoma(CIF)and congenital hemangioma(CH)have similarities on prenatal ultrasound and are rare.CASE SUMMARY We report 3 cases of fetuses with superficial hypervascular tumors,confirmed by postnatal pathology as CIF(1 case)and CH(2 cases,including 1 in a twin fetus).In Case 1,a mass with a rich blood supply in the fetal axilla was discovered by prenatal ultrasound at 28+0 wk of gestation.The postpartum pathological diagnosis was CIF,the mass was surgically removed,and the prognosis of the child was good.In Case 2,at 23+1 wk of gestation,a mass was discovered at the base of the fetus’s thigh on prenatal ultrasound.The postpartum pathological diagnosis was CH.After conservative treatment,the mass shrank significantly.Case 3 occurred in a twin fetus.At 30+0 wk of gestation,prenatal ultrasound revealed a bulging mass with a rich blood supply on the abdominal wall of one of the fetuses.Three weeks later,the affected fetus died,and the unaffected baby was successfully delivered by emergency cesarean section.The affected fetus was pathologically diagnosed with CH.CONCLUSION Prenatal ultrasound can provide accurate information,such as the location,size and blood supply of a surface mass in a fetus.We found similarities between CIF and CH in prenatal ultrasound findings.Although it is difficult to distinguish these conditions by prenatal ultrasound alone,for superficial hypervascular tumors that rapidly increase in size in a short period,close ultrasound monitoring of the fetus is required to quickly address possible adverse outcomes. 展开更多

Pancreatic cavernous hemangioma complicated with chronic intracapsular spontaneous hemorrhage:A case report and review of literature

BACKGROUND Pancreatic cavernous hemangioma(pCH)is a rare type of benign vascular tumor.Making the right diagnosis is challenging due to low clinical suspicion and the lack of existing cross-sectional imaging tools to distinguish it from other pancreatic lesions.CASE SUMMARY We describe a male patient,age 18,who presented with a pCH.Computed tomography,magnetic resonance imaging,and ultrasound showed cystic space in the tail of the pancreas.A dark spot sign on the T2 weighted image sequence was observed.Clinically,a mucinous cystic neoplasm with hemorrhage was suspected preoperatively by combining imaging,and the operative indication was clear.The patient underwent a distal pancreatic tumor resection under laparoscopic control.Immunohistochemical staining for CD31 and CD34 was positive;D2-40 was positive in interstitial lymphatic vessels and negative in vascular epithelial cells;and calcium-binding protein was negative.The results support the diagnosis of pCH combined with chronic intracapsular spontaneous hemorrhage.No complications or recurrences were observed during the follow-up period.CONCLUSION Chronic spontaneous hemorrhage may occur in pCH,which may greatly influence the accuracy of diagnosis using imaging modalities.Surgical resection for uncertain pCH seems reasonable with a good outcome. 展开更多

目的比较腹腔镜辅助下血管瘤剥除术与解剖性肝切除术治疗巨大肝血管瘤(HH)患者的疗效。方法2020年1月~2022年6月我院诊治的HH患者68例,接受腹腔镜下血管瘤剥除术治疗34例,接受腹腔镜下解剖性肝切除术治疗34例。常规检测血生化指标。结... 目的比较腹腔镜辅助下血管瘤剥除术与解剖性肝切除术治疗巨大肝血管瘤(HH)患者的疗效。方法2020年1月~2022年6月我院诊治的HH患者68例,接受腹腔镜下血管瘤剥除术治疗34例,接受腹腔镜下解剖性肝切除术治疗34例。常规检测血生化指标。结果剥除组手术时间、术中出血量、腹腔引流量和住院花费分别为(144.6±37.2)min、(402.3±132.5)mL、(257.2±48.6)mL和(3.2±0.7)万元,均显著短于或低于切除组【分别为(169.5±42.8)min、(537.4±155.8)mL、(335.4±71.2)mL和(3.6±0.6)万元,P<0.05】;在术后3 d,剥除组血清ALT和AST水平分别为(71.6±14.8)U/L和(48.8±9.2)U/L,均显著低于切除组【分别为(83.1±15.7)U/L和(57.6±10.6)U/L,P<0.05】;术后,剥除组出现胸腔积液、切口液化、切口感染和胆漏等并发症发生率为17.6%,与切除组的26.5%比,无显著性统计学差异(P>0.05)。结论采取腹腔镜辅助下血管瘤剥除术与解剖性肝切除术治疗巨大HH均为有效的手术方案,但血管瘤剥除术更有利于缩短手术时间,减少出血量和腹腔引流量,可根据情况适当选择。 展开更多

目的评估X线平片对股骨头坏死ARCO3A期分期的准确性,并分析其误分期的原因。方法回顾性分析2017年1月至2023年7月同期行髋关节正、蛙式位X线及CT检查,且髋关节CT分期为ARCO3A期的158例股骨头坏死患者,其中14例为双侧ARCO3A期,共计172例A... 目的评估X线平片对股骨头坏死ARCO3A期分期的准确性,并分析其误分期的原因。方法回顾性分析2017年1月至2023年7月同期行髋关节正、蛙式位X线及CT检查,且髋关节CT分期为ARCO3A期的158例股骨头坏死患者,其中14例为双侧ARCO3A期,共计172例ARCO3A期股骨头坏死,F/M=67/91,平均年龄43.7±11.8岁。观察髋关节X线正、蛙式位图像进行ARCO分期并统计结果。结果髋关节X线分期为ARCO 3A期139例,误分期为ARCO 2期33例,准确率为80.8%;对照同期髋关节CT图像,X线误分期病例中未发现软骨下骨折6例(18.2%)、未发现坏死区骨折4例(12.1%)、未发现股骨头局限塌陷23例(69.7%)。结论X线对股骨头坏死ARCO 3A期分期的准确性高,但仍存在误分期的状况。在临床工作中,正位结合蛙式位,仔细观察关节面局部轻微不光整及软骨下、坏死区骨折等征象有助于提高X线平片对股骨头坏死ARCO3A期分期的准确性。 展开更多

目的:归纳和分析影响胸部CT图像质量的因素,进一步对照分析常规剂量组和低剂量组两组图像质量有无差异,以论证胸部CT低剂量检查的可行性。方法:选取150例胸部CT查体的患者,A组为常规剂量组,管电压120kV,管电流自动调节;B组为低剂量组,... 目的:归纳和分析影响胸部CT图像质量的因素,进一步对照分析常规剂量组和低剂量组两组图像质量有无差异,以论证胸部CT低剂量检查的可行性。方法:选取150例胸部CT查体的患者,A组为常规剂量组,管电压120kV,管电流自动调节;B组为低剂量组,管电压100kV,管电流自动调节。参照胸部CT的图像质量评价标准对常规剂量组和低剂量组图像质量进一步作出主观评价。对辐射剂量、信噪比等客观数据进行统计学处理,以P<0.05为差异有统计学意义。并与主观评价相比较。结果:主、客观评价两组图像质量均无统计学意义,A组辐射剂量与B组辐射剂量有统计学意义。结论:影响图像质量的因素包括患者的因素、设备的因素以及操作者的因素,胸部CT采用100kV低剂量检查可以作为常规扫描方案,获得的图像可以满足影像诊断的要求,具有可行性。 展开更多

肝脏硬化性血管瘤的CT和MRI表现分析

目的 探讨肝脏硬化性血管瘤(HSH)的影像表现。方法 回顾性分析2013年至2021年复旦大学附属中山医院经病理证实的10例HSH患者的影像学资料6例行MR检查,2例行CT检查,2例行MR+CT检查。图像分析包括病灶数目、部位、大小、形态、边缘、密度... 目的 探讨肝脏硬化性血管瘤(HSH)的影像表现。方法 回顾性分析2013年至2021年复旦大学附属中山医院经病理证实的10例HSH患者的影像学资料6例行MR检查,2例行CT检查,2例行MR+CT检查。图像分析包括病灶数目、部位、大小、形态、边缘、密度或信号、强化方式、相邻肝包膜等影像学特征,并测量病灶与肝实质的ADC值,两者ADC值比较采用独立样本t检验。结果 10例患者均为单发病灶,大小(35.8±44.4)mm,8例类圆形、1例类椭圆形、1例分叶状,边缘均清楚。CT平扫均为低密度,2例轻度均匀持续强化,2例向心性强化;T1WI呈低、稍低信号,T2WI呈高信号、3例病灶内见低信号,增强扫描4例向心性强化,2例轻度环形持续强化,1例延迟强化,1例类似环形“快进快出”强化,病灶与肝实质的ADC值差异有显著统计学意义(P<0.01),3例出现相邻肝包膜皱缩现象。结论HSH影像表现不典型但具有一定特点:动脉期环形强化,门脉期及延迟期强化不减退,病灶ADC值高于肝实质ADC值。 展开更多

目的:探讨DWI联合3D-ASL技术在鞍旁脑膜瘤与海绵状血管瘤的鉴别诊断价值。方法:回顾性分析经手术病理证实的鞍旁脑膜瘤与海绵状血管瘤各20例的MRI资料,分析平扫、增强扫描、DWI、3D-ASL图像特点,并测量最小ADC值及最大相对脑血流量(rCBF... 目的:探讨DWI联合3D-ASL技术在鞍旁脑膜瘤与海绵状血管瘤的鉴别诊断价值。方法:回顾性分析经手术病理证实的鞍旁脑膜瘤与海绵状血管瘤各20例的MRI资料,分析平扫、增强扫描、DWI、3D-ASL图像特点,并测量最小ADC值及最大相对脑血流量(rCBF)。结果:鞍旁脑膜瘤多呈类圆形,T_(2)WI多呈等信号,增强扫描以早期中等程度强化常见,常见瘤周水肿及脑膜尾征;DWI多呈等高信号,最小ADC值(0.67~1.12)×10^(-3)mm^(2)/s,平均(0.76±0.13)×10^(-3)mm^(2)/s;多呈中高灌注,rCBF为5.072~9.683,平均7.062±3.571。海绵状血管瘤多呈哑铃状,T_(2)WI多呈高信号,增强扫描以充填式显著强化为主,未见瘤周水肿及脑膜尾征;DWI多呈低信号,最小ADC值(1.48~1.83)×10^(-3)mm^(2)/s,平均(1.56±0.25)×10^(-3)mm^(2)/s;多呈低灌注,rCBF为1.356~3.014,平均1.696±1.028。2组肿瘤形态、T_(2)WI信号、DWI信号、最小ADC值、强化方式、灌注信号及rCBF的差异均有统计学意义(均P<0.05)。结论:DWI联合3D-ASL技术在鞍旁脑膜瘤与海绵状血管瘤的鉴别诊断中具有较高的应用价值。 展开更多

目的探讨鼻中隔相关性鼻出血的有效治疗方法和治疗效果。方法回顾分析本院2019年7月至2022年4月住院治疗的120例鼻出血患者,均经鼻内镜检查明确出血点且与鼻中隔相关,分别用鼻腔填塞(观察组)、电凝止血(对照组)和鼻中隔矫正及电凝止血术... 目的探讨鼻中隔相关性鼻出血的有效治疗方法和治疗效果。方法回顾分析本院2019年7月至2022年4月住院治疗的120例鼻出血患者,均经鼻内镜检查明确出血点且与鼻中隔相关,分别用鼻腔填塞(观察组)、电凝止血(对照组)和鼻中隔矫正及电凝止血术(试验组),比较分析三组治疗效果。结果试验组首次止血成功率100%,术后3个月随访无再出血者;对照组首次止血成功率80.95%,8例需再次出血,术后3个月随访再出血率13.51%;观察组首次止血成功率77.42%,7例需再次填塞后止血,术后3个月随访再出血率32.26%。三组比较差异具有统计学意义(P<0.05)。结论鼻中隔相关性鼻出血在全身麻醉鼻内镜下电凝止血联合鼻中隔矫正手术治疗鼻腔出血较单纯电凝止血和鼻腔填塞止血效果更佳,且降低鼻腔填塞物导致患者鼻面部胀痛不适感,且治愈率高,在患者条件允许情况下值得临床推广。 展开更多
