OBJECTIVE: To explore the effects of standardized tertiary rehabilitation on the activities of daily living in stroke patients with hemiplegia within 6 months after attack. DESIGN: A clinical observation. SETTING: Department of Rehabilitation Medicine, Huashan Hospital of Fudan University. PARTICIPANTS: Eighty-two outpatients and inpatients with acute stroke were selected from the Department of Neurology, Shanghai Huashan Hosptial from January 1999 to June 2003, including 49 males and 33 females, 40 - 80 years of age, with a mean age of (65 ±11) years old. Inclusive criteria: According to the diagnostic standards for cerebrovascular diseases set by Fourth National Academic Meeting for Cerebrovascular Disease in 1995, the patients were diagnosed as new attack of cerebral infarction or cerebral hemorrhage, and confirmed by CT or MRI to be initial patients; They should be accorded with the following conditions, including within 1 week after stabilization of life signs, Glasgow coma score 〉 8 points, 40 - 80 years of age, with disturbance of limb function. Informed consents were obtained from all the patients or their relatives. Exclusive criteria: Patients were excluded due to active liver disease, liver and kidney malfunction, congestive heart failure, malignant tumor, history of dementia, failure in respiratory function, tetraplegia; cerebral infarction or cerebral hemorrhage for more than 3 weeks; unable to be followed up due to in other cities and provinces; psychiatric history; deafness and muteness. According to the will of the patients or their relatives, the patients who accepted the standardized rehabilitation program were enrolled as the treatment group (n =42), and the others as the control group (n =40). Approval was obtained from the ethical committee of the hospital. METHODS: All the patients were given routine therapies of internal medicine after admission. According to the conditions of Brunnstom recovery 6-phase evaluation, the patients in the treatment group were trained with the pre-designed comprehensive standardized rehabilitation program for corresponding period. At early period (within about 1 month after attack), the patients received rehabilitative interventions in the Department of Emergency or Department of Neurology, once a day, 45 minutes for each time, 5 times a week; At middle period (about 1 - 3 months after attack), the patients received rehabilitative interventions in the rehabilitation ward or center, once to twice a day, 30 - 45 minutes for each time, 4 - 5 days a week; At late period (about 3 - 6 months after attack), the patients received rehabilitative intervention mainly assisted by rehabilitation physician in the community, relatives and volunteers, 3 - 4 times a week, and they were followed in the house or outpatient department once every two weeks. MAIN OUTCOME MEASURES: The patients were evaluated blindly by the same rehabilitation physician using scale of modified Barthel index at admission and 1, 3 and 6 months after attack respectively. RESULTS: Totally 82 patients with acute stroke were enrolled, and 3 cases in the treatment group missed, including 2 cases died at 1 month after admission, and 1 case refused the follow up l0 days later, all the others were involved in the analysis of results. The scores of modified Barthel index at corresponding time points after admission in the treatment group were all obviously higher than those in the control group (P 〈 0,01), and the score differences were also obviously higher than those in the control group (P 〈 0,01). The activities of daily living at admission and 1, 3 and 6 months after admission in the treatment group were 22,50%, 46.43%, 75,95% and 89,52% of that of normal people respectively, and those in the control group were 17.09%, 25,77%, 43,38% and 55,00% respectively, The activities of daily Diving at admission and 1, 3 and 6 months in the treatment group were 131.66%, 180.17%, 175.08% and 162.76% of those in the controlgroup. As compared with at admission, the percentage of the score difference to the total score at the ends of the 1^st, 3^rd and 6^th months were 23.93%, 53.45% and 67.02% in the treatment group, while 8.67%, 25.36% and 36.98% in the control group. CONCLUSION: Standardized tertiary rehabilitation can obviously promote the activities of daily living in stroke patients with hemiplegia.展开更多

It also compared five-tone therapy with Tibetan Singing bowl music therapy. The ancient music therapy is a kind of medical means which is oriented to the treatment effect. At first, it was mainly based on the music medical behavior of Taoist, and then gradually evolved into the modern treatment application of folk pentatonic therapy. On the basis of inheritance and development of ancient traditional music treatment, contemporary medicine proposed a new individualized music treatment. Therefore, the theory of five-tone therapy has updated the research on traditional Chinese music treatment theory and system, and will provide reference for the establishment of accurate music treatment system in the future.展开更多

As one of the external treatment options in traditional Chinese medicine (TCM), massage therapy has achieved an ideal curative effect in the treatment of the sub-health state. This paper summarizes the articles on massage treatment for the treatment of patients in the sub-health state from the past decade. It mainly includes the current understanding of sub-health, treatment ideas and specific methods, as well as clinical classification of treatment modalities. From the perspective of treatment ideas and specific methods, most massage therapies are performed under the guidance of the holistic concept of TCM, and the concept of treatment based on syndrome differentiation (four diagnostic methods of TCM: inspection, auscultation, inquiry, and palpation). In this treatment, not only are the therapeutic functions of meridians and acupoints emphasized, but the selection of the therapeutic time is also performed according to the "Ziwuliuzhu" and the "Lingguibafa"(the acupuncture method based on time in ancient China). According to the different clinical symptoms of sub-health, it can be divided into three types: physical sub-health, psychological sub-health, and social adaptive sub-health. Massage therapy eliminates pain, relieves physical fatigue, and has a good therapeutic effect on physical sub-health. It can alleviate anxiety and depression, promote psychosomatic pleasure of patients, and has a satisfactory regulatory effect on the psychological sub-health symptoms. However, basic research on using modern medical theories and the means to explain its mechanism of action is relatively weak. Therefore, only constantly improving the quality of clinical observation and strengthening basic research can provide more and better evidence for the clinical application of massage therapy in treating the symptoms of the sub-health status.展开更多

Recently, there has been a growing evidence of its potential benefits in the treatment of various diseases. The CT has been constantly developing with the emergence of various modern and improved cupping devices. It is now evident that cupping could adjust the Qi, blood, Yin and Yang, dredge the meridian, as well as relieve the effect of illness, achieve fitness and relative equilibrium of Yin-Yang. It exerts its effects through the negative pressure suction via the mechanical and thermal stimulations, and the negative pressure effects induced by cupping. This article gives an overview of CT practice, its historical development, as well as its therapeutic effects and mechanism. Furthermore, a new and updated classification of CT was briefly introduced.展开更多

METHODS: A total of 140 CHF patients admitted to our hospital were randomly divided into combined group (Qiangxin Decoction and cardiac resynchronization, n = 70) and routine group (cardiac resynchronization, n = 70), and they were treated for 2 courses (16 weeks), with 8 weeks as a course. The improvement of heart function classification (HYHA) was observed before and after treatment, and the therapeutic effects were evaluated according to the improvement of heart function classification (markedly effective, effective, invalid, and worsening). The traditional Chinese medicine (TCM) diagnostic criteria was the main symptoms (0 to 6 points), secondary symptoms (0 to 3 points), tongue picture (0 to 1 point) and pulse condition (0 to 1 point), and the higher the score was, the more obvious the symptoms were. The left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD), left ventricular ejection fraction (LVEF) and cardiac output (CO) were observed and recorded according to the heart color ultrasound before and after treatment. And the Minnesota Living with Heart Failure Questionnaire (MLHFQ) was used to evaluate the life quality before and after treatment, and the highest score of the scale was 105 points. The higher the score was, the obvious the symptoms were, and the worse the life quality was. And the adverse reactions were recorded in the 2 groups. RESULTS: After the treatment, the total effective rate in the routine group was significantly lower than that in the combined group (75.71% vs . 94.30%), and there was statistically significant difference (P < 0.05). After the treatment, the TCM syndrome score in the routine group was higher than that in the combined group ((14.37±3.59) points vs.(10.53±3.11) points), and there was statistically significant difference (P < 0.05). After the treatment, the levels of LVEDD and LVESD in the routine group were higher than those in the combined group, and there were statistically significant differences (P < 0.05). The levels of LVEF and CO in the routine group were lower than those in the combined group, and there were statistically significant differences (P < 0.05). After the treatment, the score of MLHFQ scale in the routine group was higher than that in the combined group ((57.38±8.53) points vs.(46.39±7.14) points), and there was statistically significant difference (P < 0.05). CONCLUSION: Qiangxin Decoction combined with cardiac resynchronization therapy has good echocardiographic evaluation in patients with chronic heart failure.展开更多

文摘ExtremeIy high frequency (EHF) puncture is a new method of treatment based uponbiological effects of low power electromagnetic waves of millimetric band and acupuncture theory oftraditional Chinese medicine (TCM). It is non-invasive, painless and effective. Extensiveness and variety of the specific sensations (Deqi phenomenon) during EHF-puncture re-flect intensive reactions of the patient’s organism and provide individualization of therapy. EHF-puncture is a step of integration of traditional Chinese with modern Western medicines.
文摘Acupuncture is effective for the treatment of apoplectic hemiplegia, and the acupoint on the paralytic side is often selected for acupuncture. However, some doctor prefers healthy-side needling with quite satisfactory results. We study and review the treatment of apoplexy by healthy-side needling to further improve its therapeutic effects.

文摘BACKGROUND: At present, there are many studies on the rehabilitation therapy of stroke patients with hemiplegia, but there is deficiency of corresponding standardized rehabilitation program. CONCLUSION: Standardized tertiary rehabilitation can obviously promote the activities of daily living in stroke patients with hemiplegia.

文摘From the perspective of the application of medical value orientation music therapy, this paper summarized the ancient Chinese sound therapy, such as shaman music therapy and five-tone therapy. Therefore, the theory of five-tone therapy has updated the research on traditional Chinese music treatment theory and system, and will provide reference for the establishment of accurate music treatment system in the future.

文摘Sub-health state is a kind of low-quality state between health and disease, and will lead to disease if not intervened in time. Therefore, only constantly improving the quality of clinical observation and strengthening basic research can provide more and better evidence for the clinical application of massage therapy in treating the symptoms of the sub-health status.

文摘Cupping therapy (CT) is an ancient traditional and complementary medicine practice. Furthermore, a new and updated classification of CT was briefly introduced.
文摘OBJECTIVE: To explore the echocardiographic evaluation of patients with chronic heart failure (CHF) after Qiangxin Decoction combined with cardiac resynchronization therapy. CONCLUSION: Qiangxin Decoction combined with cardiac resynchronization therapy has good echocardiographic evaluation in patients with chronic heart failure.
