The Impact of Health Information Technology on Hospital Performance: A Systematic Integrative Literature Review

Objective: To review, categorise, and synthesise findings from literature on health information technology (HIT) functionalities, HIT use, and the impact of HIT on hospital performance. Materials and Methods: We condu... Objective: To review, categorise, and synthesise findings from literature on health information technology (HIT) functionalities, HIT use, and the impact of HIT on hospital performance. Materials and Methods: We conducted a systematic integrative literature review based on a compre-hensive database search. To organise, categorise and synthesise the ex-isting literature, we adopted the affordance actualization theory. To align the literature with our research framework, we used four categories: 1) the functionalities of HIT and how these functionalities are measured;2) use and immediate outcomes of HIT functionalities;3) different perfor-mance indicators and how HIT functionalities affect them;and 4) what hospital characteristics influence the outcome of hospital performance. Results: Fifty-two studies were included. We identified four types of HIT. Only ten studies (19.2%) define the use of HIT by explicitly meas-uring the use rate of HIT. We identified five dimensions of hospital per-formance indicators. Every dimension showed mixed results;however, in general, HIT has a positive impact on mortality and patient readmis-sions. We found several hospital characteristics that may affect the rela-tionship between HIT and hospital-level outcomes. Discussion: Further efforts should focus on embedded research on HIT functionalities, use and effects of HIT implementations with more performance indicators and adjusted for hospital characteristics. Conclusion: The proposed framework could help hospitals and researchers make decisions regard-ing the functionalities, use and effects of HIT implementation in hospi-tals. Given our research outcomes, we suggest future research opportuni-ties to improve understanding of how HIT affects hospital performance. . 展开更多

The subversive nature of information war lies not only in the information itself, but also in the circulation and application of information. It has always been a challenge to quantitatively analyze the function and effect of information flow through command, control, communications, computer, kill, intelligence,surveillance, reconnaissance (C4KISR) system. In this work, we propose a framework of force of information influence and the methods for calculating the force of information influence between C4KISR nodes of sensing, intelligence processing,decision making and fire attack. Specifically, the basic concept of force of information influence between nodes in C4KISR system is formally proposed and its mathematical definition is provided. Then, based on the information entropy theory, the model of force of information influence between C4KISR system nodes is constructed. Finally, the simulation experiments have been performed under an air defense and attack scenario. The experimental results show that, with the proposed force of information influence framework, we can effectively evaluate the contribution of information circulation through different C4KISR system nodes to the corresponding tasks. Our framework of force of information influence can also serve as an effective tool for the design and dynamic reconfiguration of C4KISR system architecture. 展开更多

The presence of numerous uncertainties in hybrid decision information systems(HDISs)renders attribute reduction a formidable task.Currently available attribute reduction algorithms,including those based on Pawlak attribute importance,Skowron discernibility matrix,and information entropy,struggle to effectively manages multiple uncertainties simultaneously in HDISs like the precise measurement of disparities between nominal attribute values,and attributes with fuzzy boundaries and abnormal values.In order to address the aforementioned issues,this paper delves into the study of attribute reduction withinHDISs.First of all,a novel metric based on the decision attribute is introduced to solve the problem of accurately measuring the differences between nominal attribute values.The newly introduced distance metric has been christened the supervised distance that can effectively quantify the differences between the nominal attribute values.Then,based on the newly developed metric,a novel fuzzy relationship is defined from the perspective of“feedback on parity of attribute values to attribute sets”.This new fuzzy relationship serves as a valuable tool in addressing the challenges posed by abnormal attribute values.Furthermore,leveraging the newly introduced fuzzy relationship,the fuzzy conditional information entropy is defined as a solution to the challenges posed by fuzzy attributes.It effectively quantifies the uncertainty associated with fuzzy attribute values,thereby providing a robust framework for handling fuzzy information in hybrid information systems.Finally,an algorithm for attribute reduction utilizing the fuzzy conditional information entropy is presented.The experimental results on 12 datasets show that the average reduction rate of our algorithm reaches 84.04%,and the classification accuracy is improved by 3.91%compared to the original dataset,and by an average of 11.25%compared to the other 9 state-of-the-art reduction algorithms.The comprehensive analysis of these research results clearly indicates that our algorithm is highly effective in managing the intricate uncertainties inherent in hybrid data. 展开更多

In this paper, we focus on the power allocation of Integrated Sensing and Communication(ISAC) with orthogonal frequency division multiplexing(OFDM) waveform. In order to improve the spectrum utilization efficiency in ISAC, we propose a design scheme based on spectrum sharing, that is,to maximize the mutual information(MI) of radar sensing while ensuring certain communication rate and transmission power constraints. In the proposed scheme, three cases are considered for the scattering off the target due to the communication signals,as negligible signal, beneficial signal, and interference signal to radar sensing, respectively, thus requiring three power allocation schemes. However,the corresponding power allocation schemes are nonconvex and their closed-form solutions are unavailable as a consequence. Motivated by this, alternating optimization(AO), sequence convex programming(SCP) and Lagrange multiplier are individually combined for three suboptimal solutions corresponding with three power allocation schemes. By combining the three algorithms, we transform the non-convex problem which is difficult to deal with into a convex problem which is easy to solve and obtain the suboptimal solution of the corresponding optimization problem. Numerical results show that, compared with the allocation results of the existing algorithms, the proposed joint design algorithm significantly improves the radar performance. 展开更多

The results showed that, the three physical quantities oscillate between their upper and lower bounds, where the number of oscillations increases as the Rashba interaction strength increases. The exchanging rate of these three quantities depends on the Rashba strength, and whether the entangled state is generated via direct/indirect interaction. Moreover, the coherence parameter can be used as a control parameter to maximize or minimize the three physical quantities. 展开更多

The paper addresses the decentralized optimal control and stabilization problems for interconnected systems subject to asymmetric information.Compared with previous work,a closed-loop optimal solution to the control problem and sufficient and necessary conditions for the stabilization problem of the interconnected systems are given for the first time.The main challenge lies in three aspects:Firstly,the asymmetric information results in coupling between control and estimation and failure of the separation principle.Secondly,two extra unknown variables are generated by asymmetric information(different information filtration)when solving forward-backward stochastic difference equations.Thirdly,the existence of additive noise makes the study of mean-square boundedness an obstacle.The adopted technique is proving and assuming the linear form of controllers and establishing the equivalence between the two systems with and without additive noise.A dual-motor parallel drive system is presented to demonstrate the validity of the proposed algorithm. 展开更多

Introduction: Head and neck cancers represent a significant burden of cancer, collectively ranking fourth for cancer incidence and second for cancer mortality worldwide. Objective: To study the epidemiological and histopathological aspects of head and neck cancers in the ENT-CCF department of Ignace Deen National Hospital. Materials and Methods: This was a descriptive retrospective study covering a period of 5 years from September 1, 2016, to September 30, 2021. Results: We collected 43 cases of head and neck cancers, representing a hospital frequency of 1.85%. The mean age was 45.21 ± 13.23 years. Males predominated, accounting for 65.12%, compared to 34.88% for females, with a male-to-female ratio of 1.87. Housewives accounted for 27.90%, followed by farmers at 23.26% and traders/merchants at 18.60%, with 41.86% being uneducated. The main risk factors were tobacco (37.21%) and alcohol (34.88%). Pain (53.49%), dysphonia (51.16%), and dyspnea (39.53%) were the main presenting symptoms. Squamous cell carcinoma (74.42%) and large cell lymphoma (18.60%) were the most commonly cited histological types. Conclusion: Head and neck cancers remain relatively common in our country. Improving prognosis depends on early diagnosis of the condition, resuscitation resources, and prompt management. 展开更多

Recommendation Information Systems(RIS)are pivotal in helping users in swiftly locating desired content from the vast amount of information available on the Internet.Graph Convolution Network(GCN)algorithms have been employed to implement the RIS efficiently.However,the GCN algorithm faces limitations in terms of performance enhancement owing to the due to the embedding value-vanishing problem that occurs during the learning process.To address this issue,we propose a Weighted Forwarding method using the GCN(WF-GCN)algorithm.The proposed method involves multiplying the embedding results with different weights for each hop layer during graph learning.By applying the WF-GCN algorithm,which adjusts weights for each hop layer before forwarding to the next,nodes with many neighbors achieve higher embedding values.This approach facilitates the learning of more hop layers within the GCN framework.The efficacy of the WF-GCN was demonstrated through its application to various datasets.In the MovieLens dataset,the implementation of WF-GCN in LightGCN resulted in significant performance improvements,with recall and NDCG increasing by up to+163.64%and+132.04%,respectively.Similarly,in the Last.FM dataset,LightGCN using WF-GCN enhanced with WF-GCN showed substantial improvements,with the recall and NDCG metrics rising by up to+174.40%and+169.95%,respectively.Furthermore,the application of WF-GCN to Self-supervised Graph Learning(SGL)and Simple Graph Contrastive Learning(SimGCL)also demonstrated notable enhancements in both recall and NDCG across these datasets. 展开更多

The advent of the big data era has presented unprecedented challenges to remedies for personal information infringement in areas such as damage assessment,proof of causation,determination of illegality,fault assessment,and liability.Traditional tort law is unable to provide a robust response for these challenges,which severely hinders human rights protection in the digital society.The dynamic system theory represents a third path between fixed constitutive elements and general clauses.It both overcomes the rigidity of the“allor-nothing”legal effect evaluation mechanism of the“element-effect”model and avoids the uncertainty of the general clause model.It can effectively enhance the flexibility of the legal system in responding to social changes.In light of this,it is necessary to construct a dynamic foundational evaluation framework for personal information infringement under the guidance of the dynamic system theory.By relying on the dynamic interplay effect of various foundational evaluation elements,this framework can achieve a flexible evaluation of the constitutive elements of liability and the legal effects of liability for personal information infringement.Through this approach,the crisis of personal information infringement in the era of big data can be mitigated,and the realization of personal information rights as digital human rights can be promoted. 展开更多

Hospital Information System addresses the entire major functional areas of modern multi-specialty hospitals. The package enables improved patient care, patient safety, efficiency and reduced costs. It provides easy access to critical information, thereby enabling management to make better decisions on time. Aims: The short-term objectives of the on-line computerized system are to reduce costs and improve the accuracy and timeliness of patient care, accounting and administration, record keeping, and management reporting. The long-term goal is to build and maintain patient database for analysis of data to facilitate decision-making process. Methods: To run the system it requires some Hardware & Database for IT Department. The technique involves Patient Registration System, Finance & Accounts, Human Resource Management System, Laboratory, Out Patient Management System, and Inpatient Management System. Results: In the mid seventies, a complex was established at Shahbag area in the name of Bangladesh Institute of Research and Rehabilitation in Diabetes, Endocrine and Metabolic Disorders (BIRDEM). From 1982 BIRDEM was designated as the WHO collaborating centre for research on prevention and control of diabetes. It is first of its kind outside Europe. Now from 2000 BIRDEM has successfull developed & implemented some crucial part of HIS System like Patient Admission & Billing System, Finanace & Account System, Human Resourse Management & Payroll System, Store Inventory & Procurement Management System, Labrotary Management System, Assets Management System, Radiology & Imaging Management System. Conclusions: It can thus be seen that deploying IT can help the medical profession in improving its quality of service and thus automatically increasing the preparedness and defensiveness. Of course, it is of vital importance that the software must have the right type of modularity and openness so that it is manageable, maintainable and upgradeable. They can perform the complex task of matching, tabulating, calculating, retrieving, printing and securing the data as required. Three-way decision(T-WD)theory is about thinking,problem solving,and computing in threes.Behavioral decision making(BDM)focuses on effective,cognitive,and social processes employed by humans for choosing the optimal object,of which prospect theory and regret theory are two widely used tools.The hesitant fuzzy set(HFS)captures a series of uncertainties when it is difficult to specify precise fuzzy membership grades.Guided by the principles of three-way decisions as thinking in threes and integrating these three topics together,this paper reviews and examines advances in three-way behavioral decision making(TW-BDM)with hesitant fuzzy information systems(HFIS)from the perspective of the past,present,and future.First,we provide a brief historical account of the three topics and present basic formulations.Second,we summarize the latest development trends and examine a number of basic issues,such as one-sidedness of reference points and subjective randomness for result values,and then report the results of a comparative analysis of existing methods.Finally,we point out key challenges and future research directions. 展开更多

Introduction: Today, information technology is considered as an important national development principle in each country which is applied in different fields. Health care as a whole and the hospitals could be regarded as a field and organizations with most remarkable IT applications respectively. Although different benchmarks and frameworks have been developed to assess different aspects of Hospital Information Systems (HISs) by various researchers, there is not any suitable reference model yet to benchmark HIS in the world. Electronic Medical Record Adoption Model (EMRAM) has been currently presented and is globally well-known to benchmark the rate of HIS utilization in the hospitals. Notwithstanding, this model has not been introduced in Iran so far. Methods: This research was carried out based on an applied descriptive method in three private hospitals of Isfahan—one of the most important provinces of Iran—in the year 2015. The purpose of this study was to investigate IT utilization stage in three selected private hospitals. Conclusion: The findings revealed that HIS is not at the center of concern in studied hospitals and is in the first maturity stage in accordance with EMRAM. However, hospital managers are enforced and under the pressure of different beneficiaries including insurance companies to improve their HIS. Therefore, it could be concluded that these types of hospitals are still far away from desirable conditions and need to enhance their IT utilization stage significantly. 展开更多

Although numerous advances have been made in information technology in the past decades,there is still a lack of progress in information systems dynamics(ISD),owing to the lack of a mathematical foundation needed to describe information and the lack of an analytical framework to evaluate information systems.The value of ISD lies in its ability to guide the design,development,application,and evaluation of largescale information system-of-systems(So Ss),just as mechanical dynamics theories guide mechanical systems engineering.This paper reports on a breakthrough in these fundamental challenges by proposing a framework for information space,improving a mathematical theory for information measurement,and proposing a dynamic configuration model for information systems.In this way,it establishes a basic theoretical framework for ISD.The proposed theoretical methodologies have been successfully applied and verified in the Smart Court So Ss Engineering Project of China and have achieved significant improvements in the quality and efficiency of Chinese court informatization.The proposed ISD provides an innovative paradigm for the analysis,design,development,and evaluation of large-scale complex information systems,such as electronic government and smart cities. 展开更多

Government information sharing(GIS)refers to that act of required or provided for duty government information,commercial information and public welfare information,and it is a basic issue of government services.However,the existing GIS has low transparency and is lack of flexibility between different departments.Aiming at such problems,this paper takes blockchain as a solusion,and systematically summarizes the development of digital GIS,the advantages and challenges of blockchain and its theoretical research and practical applications.Specifically,it reviews e-government interactive structure,big data and other solutions,analyses their imperfections,and puts forward blockchain-based solutions.The blockchain improves government service efficiency and data security,meanwhile it faces challenges in throughput and supervision.In order to meet these challenges,researchers propose solutions based on three data exchange scenarios:government-to-government,government-to-enterprise and government-toindividual.In addition to researches,blockchain-based GIS is put into use.The electronic licensing project of Nanjing(China)government is taken as an example to illustrate that blockchain has the ability to openly and securely verify,track,and exchange data,so as to improve management ability and public service level of government agencies. 展开更多

Introduction: The aim of the study was to describe the immunohistochemical aspects of breast cancers at Douala General Hospital, Cameroon. Methodology: This was a descriptive study with retrospective data collection, conducted from January 1<sup>st</sup> 2010 and December 31<sup>st</sup> 2019. It was focused on histologically proven breast cancers followed up at Douala General Hospital. Results: We collected 285 cases of breast cancer, all female, representing an annual frequency of 28.5 cases. The mean age of the patients was 48 ± 13 years. The most frequent histological form was infiltrating ductal carcinoma (83.9%), with grade II predominating (38.6%). Immunohistochemical analyses were performed on 89 patients (31.23%). The triple-negative subtype was the most common class (35.95%), followed by Luminal A (32.60);Human Epidermal Receptor 2-class and Luminal B tumors were observed at a frequency of 13.48% each. Treatment consisted mainly of surgery (78.60%) combined with chemotherapy (65.97%) and/or radiotherapy (63.16%). Hormonal treatment was used in only 10.18% of patients. Targeted therapy was exceptional (4%). Conclusion: Triple-negative phenotype is the most common. However, potentially hormone-sensitive tumors account for almost half of all patients, who are relatively young. Individualized treatments are rare. It is important to systematize these analyses for all breast cancers, with a view to appropriate management in our environment. 展开更多

Needs and Opportunities for Information in Patients with Metastatic Breast Cancer Attending a Tertiary Hospital in Tanzania—A Qualitative Study

Background: The majority of breast cancer patients in Tanzania present with advanced disease, and a significant proportion has metastatic breast cancer (MBC) on presentation or develops it during the course of their follow-up. With few treatment options to choose from, such patients often benefit from empathic support and access to information to help them make treatment decisions based on their individual circumstances and needs. Patients with MBC have been shown to present with unique physical, social and psychological needs that require additional time, counselling and availability of health care providers in addition to the routine options available to other patients. In resource-limited settings, the needs of such patients are often unknown and unaddressed, which adds to the anxiety associated with the diagnosis and its treatment. Materials and methods: This descriptive qualitative study was conducted using 3 focus group discussions with a total of 17 participants with metastatic breast cancer (MBC) attending Ocean Road Cancer Institute in Dar es Salaam, Tanzania. Participants were purposively selected for the study from outpatient clinics and inpatient wards. A semi-structured FGD guide was used to moderate discussions and analysis was done using a thematic approach. Results: The median age of participants was 51 (range 33 - 81 years) with an average of 4 months (range 1 - 12 months) from diagnosis of BC to the interview. 4 (24%) were diagnosed with MBC on first presentation (denovo). Participants spoke about the importance of accurate BC-related information in allowing timely referral and treatment both in the community and within the health system. They recognized the role of mass and social media in increasing awareness about BC and identified myths surrounding cancer treatment especially mastectomy. Correct and timely information at points of care, through media platforms and via ambassadors/patient support groups was perceived as a means to avoiding delays and securing early and effective treatment. Conclusion: Patients with MBC in Tanzania have many unmet informational needs in relation to their disease. Accurate BC-related information is important in allowing early detection and diagnosis. At the community level, provision of information through established media platforms and the use of patient advocates may help to enable early referral and treatment of patients. 展开更多

Accounting Information System(AIS),which is the foundation of any enterprise resource planning(ERP)system,is often built as centralized system.The technologies that allow the Internet-of-Value,which is built onfive aspects that are network,algorithms,distributed ledger,transfers,and assets,are based on blockchain.Cryptography and consensus protocols boost the blockchain plat-form implementation,acting as a deterrent to cyber-attacks and hacks.Blockchain platforms foster innovation among supply chain participants,resulting in ecosys-tem development.Traditional business processes have been severely disrupted by blockchains since apps and transactions that previously required centralized struc-tures or trusted third-parties to authenticate them may now function in a decentra-lized manner with the same level of assurance.Because a blockchain split in AIS may easily lead to double-spending attacks,reducing the likelihood of a split has become a very important and difficult research subject.Reduced block relay time between the nodes can minimize the block propagation time of all nodes,resulting in better Bitcoin performance.In this paper,three problems were addressed on transaction and block propagation mechanisms in order to reduce the likelihood of a split.A novel algorithm for blockchain is proposed to reduce the total pro-pagation delay in AIS transactions.Numerical results reveal that,the proposed algorithm performs better and reduce the transaction delay in AIS as compared with existing methods. 展开更多
