Objective: To identify the mutations of iduronate-2-sulfatase (IDS) gene, to reveal its mutation features, and to establish a basis for genetic counseling and prenatal gene diagnosis of Hunter syndrome. Methods: Urine...Objective: To identify the mutations of iduronate-2-sulfatase (IDS) gene, to reveal its mutation features, and to establish a basis for genetic counseling and prenatal gene diagnosis of Hunter syndrome. Methods: Urine glycosaminoglycans (GAGs) assay, PCR and DNA sequencing were performed to detect mutation of IDS gene of the patient and his parents. Results: The result showed that the patient was: DS(++), HS(++), KS(-), CS(-), and that both of his parents were negative. A frame-shift deletion mutation (1062 del 16) was identified in exon 7 of the patient's IDS gene. His parents' genotypes were normal. Conclusion:The patient's mutation was not inherited by his parents but a novel one. The mutation probably altered the primary structure and tertiary structure of IDS enzyme protein remarkably and lowered the activity of IDS enzyme greatly. Therefore it is supposed to be the direct cause of the disorder.展开更多

Nowadays,the usage of socialmedia platforms is rapidly increasing,and rumours or false information are also rising,especially among Arab nations.This false information is harmful to society and individuals.Blocking an...Nowadays,the usage of socialmedia platforms is rapidly increasing,and rumours or false information are also rising,especially among Arab nations.This false information is harmful to society and individuals.Blocking and detecting the spread of fake news in Arabic becomes critical.Several artificial intelligence(AI)methods,including contemporary transformer techniques,BERT,were used to detect fake news.Thus,fake news in Arabic is identified by utilizing AI approaches.This article develops a new hunterprey optimization with hybrid deep learning-based fake news detection(HPOHDL-FND)model on the Arabic corpus.The HPOHDL-FND technique undergoes extensive data pre-processing steps to transform the input data into a useful format.Besides,the HPOHDL-FND technique utilizes long-term memory with a recurrent neural network(LSTM-RNN)model for fake news detection and classification.Finally,hunter prey optimization(HPO)algorithm is exploited for optimal modification of the hyperparameters related to the LSTM-RNN model.The performance validation of the HPOHDL-FND technique is tested using two Arabic datasets.The outcomes exemplified better performance over the other existing techniques with maximum accuracy of 96.57%and 93.53%on Covid19Fakes and satirical datasets,respectively.展开更多

On the basis of some uniformly prior estimates of the solution to the viscous weakly dissipative (gμHS2) system, we show that the semi-group of the solution operator {S(t)}t≥0 has a bounded absorbing set. BACKGROUND Bow hunter’s syndrome(BHS)is a rare but surgically treatable cause of vertebrobasilar insufficiency due to dynamic rotational occlusion of the vertebral artery.Typically,patients present with posterior cir...BACKGROUND Bow hunter’s syndrome(BHS)is a rare but surgically treatable cause of vertebrobasilar insufficiency due to dynamic rotational occlusion of the vertebral artery.Typically,patients present with posterior circulation transient ischaemic symptoms such as presyncope,syncope,vertigo,diplopia,and horizontal nystagmus,but irreversible deficits,including medullary and cerebellar infarctions,have also been described.CASE SUMMARY A 70-year-old patient presented an acute onset of vertigo and gait instability triggered by right head rotation.His medical history included previous episodes of unilateral left neck and occipital pain followed by light-headedness,sweating,and blurred vision when turning his head,and these episodes were associated with severe degenerative changes in the atlanto-dens and left atlanto-axial facet joints and right rotation of the C2 cervical vertebrae.Brain magnetic resonance imaging revealed the presence of acute bilateral cerebellar ischaemic lesions,while static vascular imaging did not reveal any vertebral artery abnormalities.Dynamic ultrasonography and angiography were performed and confirmed the presence of a dynamic occlusion of the vertebral artery V3-V4 segment when the head was rotated to the right secondary to left C1-C2 bone spur compression.Surgical decompression led to complete resolution of paroxysmal symptoms without neurological sequelae.CONCLUSION BHS should be considered in cases of repeated posterior circulation transient ischaemic attack or ischaemic stroke,particularly when associated with high cervical spine abnormalities.展开更多
BACKGROUND In bow hunter’s syndrome(BHS),also known as rotational vertebral artery(VA)syndrome,there is dynamic/rotational compression of the VA producing verte-brobasilar insufficiency.Most occurrences involve atlan...BACKGROUND In bow hunter’s syndrome(BHS),also known as rotational vertebral artery(VA)syndrome,there is dynamic/rotational compression of the VA producing verte-brobasilar insufficiency.Most occurrences involve atlantoaxial rather than mid-cervical VA compromise,the latter being rarely reported.Herein,we detail successful VA decompression at mid-cervical spine,given a departure from its usual course.CASE SUMMARY The patient,a 45-year-old man,presented to our hospital with occipital headache and vertigo.Computed tomography angiography showed anomalous C4 entry of right VA,with compression upon head rotation to that side.Thyroid cartilage and anterior tubercle of C5 transverse process were visibly at fault.We opted for sur-gery,using an anterior cervical approach to remove the anterior tubercle.Patient recovery was uneventful and brought resolution of all preoperative symptoms.CONCLUSION BHS is an important consideration where aberrant coursing of VA and neurologic symptoms coexist.展开更多
A key benefit of publishing illness narratives is giving voice to sick or dying people,in order to promote a deepened understanding of patients’experiences of illness.Reading this kind of stories can also help health...A key benefit of publishing illness narratives is giving voice to sick or dying people,in order to promote a deepened understanding of patients’experiences of illness.Reading this kind of stories can also help healthcare providers to reflect about facing the difficult concept of“a good death”(a good way to die).Following considerations arise from a close reading of Franz Kafka’s incomplete short story entitled“The Hunter Gracchus”(1917)and,in particular,from the condition in which he puts the protagonist,the Hunter Gracchus.Kafka creates a completely surrealistic story concerning the artificial extension of persistently compromised vital functions of a person affected by an irreversible pathology.The protagonist,who seems to be aware of his poor condition,is really distressed by living the dying process spread over a long time and so diluted.Realistic connotations of this short story were not clear at Kafka’s time,but,nowadays,because of advances in medicine,with the availability of medications or invasive interventions developed to prolong life,many patients can experience a condition similar to hunter Gracchus’s one.We believe,therefore,that could be important reading and then reflecting on this story,especially assuming the hunter’s point of view,for the purpose of developing a new approach to the current problem of the“dehumanization”of dying.To make the reflection process as free as possible,we thought that it would have been better to give the voice to the Poet,and for this reason,we tried to avoid adding words of“wisdom”to support one thesis or another.In our opinion,reflecting upon the specific experience of a person who is practically dead but still not allowed to die could encourage health-care providers to consider meaning and consequences of serious illnesses,exploring their feelings about life-sustaining treatments and death.展开更多

This is how the colonization of the Arctic began. The domestic dog appeared in this region about 25 - 30 thousand years ago as a result of the severe climatic conditions in which both hunting tribes and the ancestors of the domestic dog had to live. A wolf is a predator, an able hunter. Therefore, most likely wolves were originally trained to hunt with humans—the leaders of the hunting pack. Thus, in northeast Asia, there was a direct relationship between man’s colonization of the Arctic, the appearance of the hunting dog, and a decrease in the number of mammoth, bison, and other large animals in the region, as a result of hunting when whole herds were killed.展开更多
