Methods:A total of 92 cases of patients with coronary heart disease with mild to moderate hypertension were selected as the object of observation, according to the random data table, they were divided into the control group (n=46) and observation group (n=46), and patients in the control group were treated with Nifedipine Controlled Release Table therapy, on this basis, the observation group patients were given telmisartan treatment, two groups were treated for 6 months. The levels of the blood pressure, inflammatory factors, vascular endothelial function and left ventricular function compared between the two groups before and after treatment. Results: There were no significant differences in the levels of SBP, DBP, hs-CRP, TNF-α, NO, ET-1, LVEF, LVEDD and LVESD in the two groups before treatment. After treatment, two groups of SBP, DBP, hs-CRP, TNF-α, ET-1, LVEDD and LVESD levels were significantly lower than those in the same group before treatment, and after treatment, the levels of SBP, DBP, hs-CRP, TNF-α, ET-1 and LVESD in the observation group were significantly lower than those in the control group, while there were no significant difference in the level of LVEDD between the two groups after treatment;Compared with level in the group before treatment, the levels of NO and LVEF in the two groups were significantly increased, and the observation group [(82.13±19.01) μmol/L, (52.83±7.45)%] was significantly higher than the control group ((67.37±13.08) μmol/L, (49.47±6.96)%)Conclusion: Telmisartan combined with Nifedipine Controlled Release Table in treating coronary heart disease with mild to moderate hypertension, can effectively control blood pressure, reduce the inflammatory stress, improve vascular endothelial function and left ventricular function of patients, has an important clinical value.展开更多

Methods: A total of 143 elderly hypertensives were enrolled and divided into diabet...Objective: To investigate blood pressure variability of Elder hypertensives with type 2 diabetes and its relationship with cognition. Methods: A total of 143 elderly hypertensives were enrolled and divided into diabetic group (59 cases) and non-diabetic group (84 cases). The difference of general clinical characteristics, biochemical parameters, carotid ultrasound, a neuropsychological Scales and 24-hour ambulatory blood pressure (24hABPM) parameters between the two groups of subjects were compared. Then, the two groups (diabetic group and non-diabetic group) were further divided into (Mild cognitive dysfunction) subgroup (MMSE>26) and normal cognition subgroup (MMSE≤26), respectively. On the basis of MMSE scores, the difference of the parameters of ABPM between the two subgroups was analyzed. Results: Compared with the control group, 24hSBP, 24hPP, dSBP, dPP, nSBP, nPP, 24hSSD, dSSD, nSSD, 24hSCV, dSCV and nSCV were significantly higher in the diabetic group (p<0.05). However, cognition was lower in the diabetic group. No significant difference was found in the circadian pattern of blood pressure between the two groups. 24hSSD, dSSD, nSSD, 24hSCV, dSCV, nSCV were significantly higher in the MCI subgroup than normal cognition subgroup in both diabetic and non-diabetic groups(p<0.05), and they were negatively associated with scores of MMSE, the correlation coefficient were -0.235,-0.246,-0.341,-0.158,-0.222,-0.238 (0.001≤P<0.05). Conclusion: The study showed that in the elderly with hypertension, the mean systolic blood pressure and blood pressure variability were both higher in the diabetic group, and the cognition was lower instead. Whether or not with diabetes, blood pressure variability was always higher in the MCI subgroup. Blood pressure variability increased in patients with diabetes, and was associated with cognitive decline.展开更多
BACKGROUND Nonalcoholic fatty liver disease(NAFLD)encompasses a variety of liver conditions impacting individuals who consume minimal or no alcohol.Recently,traditional Chinese medicine has been gradually used to trea...BACKGROUND Nonalcoholic fatty liver disease(NAFLD)encompasses a variety of liver conditions impacting individuals who consume minimal or no alcohol.Recently,traditional Chinese medicine has been gradually used to treat mild to moderate fatty liver,among which Dendrobium nobile Lindl.powder has been affirmed by many doctors and patients to be effective.However,there is limited research on combining this treatment with standard therapies for mild to moderate NAFLD.AIM To survey the effect of combining Dendrobium nobile Lindl.powder with standard treatment on liver function and lipid metabolism disorder in patients with mild to moderate NAFLD.METHODS Eighty patients with mild to moderate NAFLD participated in this retrospective study,classified into two groups:The observation group(n=40)and the control group(n=40).In November 2020 and November 2022,the study was conducted at People’s Hospital of Chongqing Liang Jiang New Area.The control group received standard treatment,while the observation group received Dendrobium nobile Lindl.powder based on the control group.The study compared differences in traditional Chinese medicine clinical syndrome scores,liver fibrosis treatment,liver function indicators,lipid levels,and serum inflammatory factor levels before and after treatment,and we calculated the incidence of adverse reactions for both groups.RESULTS The total effective rate was 97.50%in the observation group and 72.5%in the control group.After 8 weeks of treatment,the main and secondary symptom scores remarkably decreased,especially in the observation group(P<0.05),and there was a significant reduction in the serum levels of hyaluronic acid(HA),laminin(LN),human rocollagen III(PC III),and collagen type IV(CIV).The levels of HA,LN,PC III,and CIV were significantly lower in the observation group(P<0.05).After 8 weeks,both groups indicated remarkable improvements in liver function and blood lipid levels,with the observation group having even lower levels(P<0.05).Serum levels of interleukin-1β,tumor necrosis factor-α,and interleukin-8 also dropped significantly.The observation group had a lower rate of adverse reactions(5.00%)compared to the control group(22.50%).CONCLUSION Adding Dendrobium nobile Lindl.powder to standard treatment has been found to remarkably improve symptoms and reduce inflammation in patients with mild to moderate fatty liver disease.It also enhances hepatic function and lipid profile,ameliorates liver fibrosis indices,and lowers the risk of side effects.Consequently,this therapeutic protocol shows promise for clinical implementation and dissemination.展开更多

Despite of reported and self experienced good results in favorable fitting preoperative...Background: There are at least 5 very popular methods described for the correction of mild to moderate hallux valgus deformities. Despite of reported and self experienced good results in favorable fitting preoperative anatomical conditions of some of these methods;it seems that no one is really satisfying under “difficult” or non favorable preoperative anatomic conditions. How could an optimum operation method for a satisfying correction of a mild to moderate hallux valgus look like? The well known criteria for an optimally satisfying method to achieve full normalization of important forefoot parameters under all conditions should be a lateralization, plantarization and derotation of the Ist metatarsal head and also a normalization of the distal metatarsal articular angle (DMAA) in combination with a soft tissue correction. Methods: In order to achieve this outcome without the disadvantages of shortening or the need for unloading, we developed a new operation technique and new fixation devices. We performed a simple transverse lazy-L subcapital osteotomy and after an additional soft tissue release the Ist metatarsal head was restored to its preplaned optimum position in terms of narrowing the IM angle, plantarization, correction of the hallux valgus angle, derotation of a pronation l malrotation and improvement of the DMAA. Fixation was achieved with an intramedullary angel-stable transfixed 30 mm titanium plate. The rigidity of the implant rendered interfragmental compression or solid bone contact unnecessary. The goal was only an optimal correction. All patients were allowed to ambulate with full weightbearing immediately after the operation. Results: We retrospectively reviewed 346 patients (433 feet) who underwent subcapital osteotomy between May 2007 and December 2011;308 were women and 38 men;their mean age was 65 years. The follow-up investigation was performed on average after 6.5 years (78 months);range 5.5 to 10 years (66 to 120 months). The AOFAS Score improved from 61.18 to 96.82 (t = 55.13, p ° to 4.21° (t = 89.70, p ° to 7.75° (t = 51.68, p ° to 6.61° (t = 29.34, p < 0.001). Minimal shortening of the first metatarsal (0.33 mm) and no recurrence of the deformity was observed. Conclusion: A new access to hallux valgus surgery is presented. Our results show that interfragmental compression between the osteotomy partners or good interfragmental contact is not necessary when a rigid intramedullary fixation device is used. The goal of this approach to hallux valgus surgery was to achieve excellent correction and a very rigid intramedullary angle-stable locked implant. The procedure yielded excellent results and was associated with no recurrence of hallux valgus.展开更多

Methods: The patients with hypertensive cerebral hemorrhage who received hematoma aspiration in our hospital between August 2014 and November 2017 were chosen and randomly divided into the observation group who received mild hypothermia irrigation fluid and the control group who received room temperature irrigation fluid. Before treatment and 72 h after treatment, the contents of nerve injury markers, cytokines and oxidative stress mediators were measured after peripheral blood extraction and serum separation. Results: Compared with those of control group, serum neuron-specific enolase (NSE), visinin-like protein-1 (VILIP-1), glial fibrillary acidic protein (GFAP), neuropeptide Y (NPY), platelet-derived growth factor (PDGF), vascular endothelial growth factor (VEGF), high mobility group protein B1 (HMGB1), macrophage migration inhibitory factor (MIF), interleukin-1β (IL-1β), interleukin-10 (IL-10), malondialdehyde (MDA), 8-OHdG and nitric oxide (NO) contents of observation group were significantly lower whereas serum brain-derived neurotrophic factor (BDNF), insulin-like growth factor-1 (IGF-1) and total antioxidant capacity (TAOC) contents were significantly higher after treatment (P<0.05). Conclusion: The application of mild hypothermia irrigation fluid during intracranial hematoma irrigation can alleviate the nerve function injury, improve the neurotrophic status and inhibit the oxidative stress response.展开更多

Memory function were assessed at...Forty patients with multi-infarct dementia (MID) were randomly assigned to the treatment group (25 cases) treated with Jian Nao Ning (健脑宁JNN) and the duxil control group (15 cases). Memory function were assessed at baseline and endpoint using memory subscales of a battery of New Psychometric Tests (Chinese version) including mini-mental state examination (MMSE), verbal memory, and non-verbal memory, etc. After treatment, the mean scores of verbal memory in the Hopkins Verbal Learning Test (P<0.05) and total memory scores of memory items (P<0.001) in JNN group increased significantly; and improvement in episodic memory function including story recall (immediate and delayed), delayed word recall, verbal learning and verbal recognition and visual recognition in the JNN group was better than that in the duxil control group, suggesting that JNN can obviously improve memory function for the patients with mild or moderate multi-infarct dementia.展开更多

M...Objective: To explore the effects of mild hypothermia combined with minimally invasive surgery on the degree of nerve injury and the generation of toxic metabolites in patients with hypertensive cerebral hemorrhage. Methods: A total of 138 patients with hypertensive cerebral hemorrhage who received treatment in the hospital between December 2015 and March 2017 were collected and divided into control group (n=69) and mild hypothermia group (n=69) by random number table method. Control group received conventional minimally invasive surgery, and mild hypothermia group received mild hypothermia combined with minimally invasive surgery. The differences in the serum contents of nerve injury indexes, excitatory amino acids and oxidative metabolites were compared between two groups of patients before treatment and after 1 week of treatment. Results: There was no statistically significant difference in serum levels of nerve injury indexes, excitatory amino acids and oxidative metabolites between the two groups. 1 week after treatment, serum pro-BNP, h-FABP, MBP and NSE levels of mild hypothermia group were lower than those of control group;serum excitatory amino acids Glu and Asp levels were lower than those of control group;oxidative metabolites ROS, MDA, AOPPs and LHP levels were lower than those of control group. Conclusion: Mild hypothermia combined with minimally invasive surgery can effectively relieve the nerve injury and reduce the secretion of excitatory amino acids, oxidative metabolites and so on in patients with hypertensive cerebral hemorrhage.展开更多
Background: COVID-19 is currently one of the most infectious diseases worldwide. In this study, we focused on the mild and moderate cases of COVID-19 that can present with mild respiratory symptoms or non-respiratory ...Background: COVID-19 is currently one of the most infectious diseases worldwide. In this study, we focused on the mild and moderate cases of COVID-19 that can present with mild respiratory symptoms or non-respiratory symptoms. Many of that cases got miss diagnoses. We aim to help emergency physicians in reaching a proper and faster diagnosis of COVID-19 cases. Method: In this retrospective cross-sectional qualitative study, we collected 100 confirmed cases of COVID-19 that were presented in April 2020 in Al Wakra Hospital, Qatar. All that cases were mild-moderate cases without severe respiratory symptoms. We reviewed the electronic files on patient presentation, emergency department physician’s note, temperature data, and chest X-ray findings. Result: Our result showed about 49% of the total COVID-19 confirmed cases had respiratory symptoms, while the remaining 51% had no respiratory symptoms. The respiratory symptoms, such as cough and sore throat, and non-respiratory symptoms like headache, vomiting, abdominal pain, and skin rash. Regarding fever presentation, we found that 66% of cases had a fever, while 34% had no fever complaints. The most frequently observed body temperature of patients was 37+ °C, followed by 38+ °C, 36+ °C, and 39+ °C. About 41% of cases had non-significant X-ray findings, and 40% cases had significant X-ray findings. The remaining 19% of cases did not undergo any X-ray examination due to mild and stable presentation. Conclusion: The presentations and symptoms of a mild-moderate case of COVID-19 are not respiratory only, there are extra-pulmonary symptoms and presentations should be considered. The most common presentation for mild-moderate COVID-19 was found to be fever. Chest X-ray may be performed depending on the patient’s condition, red flags, and abnormal findings in clinical examination, and should not be routine in cases with the mild presentation of COVID-19 suspicion in the emergency department.展开更多

Results: Compared with those of control group, serum neuron-specific enolase (NSE), visinin-like protein-1 (VILIP-1), glial fibrillary acidic protein (GFAP), neuropeptide Y (NPY), platelet-derived growth factor (PDGF), vascular endothelial growth factor (VEGF), high mobility group protein B1 (HMGB1), macrophage migration inhibitory factor (MIF), interleukin-1β (IL-1β), interleukin-10 (IL-10), malondialdehyde (MDA), 8-OHdG and nitric oxide (NO) contents of observation group were significantly lower whereas serum brain-derived neurotrophic factor (BDNF), insulin-like growth factor-1 (IGF-1) and total antioxidant capacity (TAOC) contents were significantly higher after treatment (P<0.05). Conclusion: The application of mild hypothermia irrigation fluid during intracranial hematoma irrigation can alleviate the nerve function injury, improve the neurotrophic status and inhibit the oxidative stress response.
文摘Forty patients with multi-infarct dementia (MID) were randomly assigned to the treatment group (25 cases) treated with Jian Nao Ning (健脑宁JNN) and the duxil control group (15 cases). Memory function were assessed at baseline and endpoint using memory subscales of a battery of New Psychometric Tests (Chinese version) including mini-mental state examination (MMSE), verbal memory, and non-verbal memory, etc. After treatment, the mean scores of verbal memory in the Hopkins Verbal Learning Test (P<0.05) and total memory scores of memory items (P<0.001) in JNN group increased significantly; and improvement in episodic memory function including story recall (immediate and delayed), delayed word recall, verbal learning and verbal recognition and visual recognition in the JNN group was better than that in the duxil control group, suggesting that JNN can obviously improve memory function for the patients with mild or moderate multi-infarct dementia.
文摘Objective: To explore the effects of mild hypothermia combined with minimally invasive surgery on the degree of nerve injury and the generation of toxic metabolites in patients with hypertensive cerebral hemorrhage. Methods: A total of 138 patients with hypertensive cerebral hemorrhage who received treatment in the hospital between December 2015 and March 2017 were collected and divided into control group (n=69) and mild hypothermia group (n=69) by random number table method. Control group received conventional minimally invasive surgery, and mild hypothermia group received mild hypothermia combined with minimally invasive surgery. The differences in the serum contents of nerve injury indexes, excitatory amino acids and oxidative metabolites were compared between two groups of patients before treatment and after 1 week of treatment. Results: There was no statistically significant difference in serum levels of nerve injury indexes, excitatory amino acids and oxidative metabolites between the two groups. 1 week after treatment, serum pro-BNP, h-FABP, MBP and NSE levels of mild hypothermia group were lower than those of control group;serum excitatory amino acids Glu and Asp levels were lower than those of control group;oxidative metabolites ROS, MDA, AOPPs and LHP levels were lower than those of control group. Conclusion: Mild hypothermia combined with minimally invasive surgery can effectively relieve the nerve injury and reduce the secretion of excitatory amino acids, oxidative metabolites and so on in patients with hypertensive cerebral hemorrhage.
文摘Background: COVID-19 is currently one of the most infectious diseases worldwide. In this study, we focused on the mild and moderate cases of COVID-19 that can present with mild respiratory symptoms or non-respiratory symptoms. Many of that cases got miss diagnoses. We aim to help emergency physicians in reaching a proper and faster diagnosis of COVID-19 cases. Method: In this retrospective cross-sectional qualitative study, we collected 100 confirmed cases of COVID-19 that were presented in April 2020 in Al Wakra Hospital, Qatar. All that cases were mild-moderate cases without severe respiratory symptoms. We reviewed the electronic files on patient presentation, emergency department physician’s note, temperature data, and chest X-ray findings. Result: Our result showed about 49% of the total COVID-19 confirmed cases had respiratory symptoms, while the remaining 51% had no respiratory symptoms. The respiratory symptoms, such as cough and sore throat, and non-respiratory symptoms like headache, vomiting, abdominal pain, and skin rash. Regarding fever presentation, we found that 66% of cases had a fever, while 34% had no fever complaints. The most frequently observed body temperature of patients was 37+ °C, followed by 38+ °C, 36+ °C, and 39+ °C. About 41% of cases had non-significant X-ray findings, and 40% cases had significant X-ray findings. The remaining 19% of cases did not undergo any X-ray examination due to mild and stable presentation. Conclusion: The presentations and symptoms of a mild-moderate case of COVID-19 are not respiratory only, there are extra-pulmonary symptoms and presentations should be considered. The most common presentation for mild-moderate COVID-19 was found to be fever. Chest X-ray may be performed depending on the patient’s condition, red flags, and abnormal findings in clinical examination, and should not be routine in cases with the mild presentation of COVID-19 suspicion in the emergency department.
