Test of incremental respiratory endurance as home-based, standalone therapy in chronic obstructive pulmonary disease: A case report

BACKGROUND The prevalence of chronic obstructive pulmonary disease(COPD)is increasing worldwide,and at the same time it is associated with increased mortality and reduced quality of life.Efforts to build sustainable rehabilitation approaches to COPD treatment and prevention are crucial.The system of long-term pulmonary rehabilitation care is insufficient.The main reasons for the absence of these outpatient programs are the lack of experience,the lack of interest of insurance companies in secondary prevention programs,and the lack of healthcare facilities in large geographical areas.The possibility of at-home pulmonary rehabilitation models(telemonitoring and telecoaching)could solve this problem.CASE SUMMARY A 71-year-old man with severe COPD,Global Initiative for Obstructive Lung Diseases stage 3 underwent an 8-wk remotely monitored inspiratory muscle training with a device based on the test of incremental respiratory endurance method.Spirometry,body plethysmography,test of incremental respiratory endurance examination,6-min walking test,body mass index,airflow obstruction,dyspnea,exercise capacity index,and subjective perception of dyspnea were performed as part of the initial and final examination.The patient performed training at home,and the physiotherapist monitored the patient remotely through a web application that allowed the physiotherapist to evaluate all training parameters in real-time and respond to any problems.After 8 wk of home training,there was a significant increase in all monitored values:maximal inspiratory pressure,a novel parameter sustained maximal inspiratory pressure,forced expiratory volume in 1 s,total lung capacity,forced vital capacity,peak expiratory flow,and inspiratory capacity.There was also an improvement in the perception of dyspnea according to the COPD Assessment Test and a modified Medical Research Council Breathlessness Scale,an increase in exercise tolerance according to the 6-min walking test,and a decrease in the exercise capacity index as a predictor of prognosis.CONCLUSION Respiratory telerehabilitation was greatly beneficial in a cooperative patient with COPD and may represent an alternative therapeutic approach to the increasing incidence of all lung diseases. 展开更多

The design of structural components in low-cycle fatigue field often requires the knowledge of the cyclic properties of the material, which are commonly described by the classical relation of Ramberg-Osgood. In order to obtain the cyclic curve using experimental data from incremental step tests, four methodologies are described and critically discussed. Three methods differ in the procedure of evaluation of the elastic modulus, while in the last one the experimental data are interpolated with a single non-linear regression. The various techniques were applied to data obtained from tests carried out on stainless steel specimens, and the resulting differences were analysed and quantified. An average behaviour was evaluated considering the total set of data obtained from experimental tests. The choice of the most suitable method is related to both the strain range of interest and the goal for which the results are used. 展开更多

Validity and Reproducibility of the Garmin Vector Power Meter When Compared to the SRM Device

Mobile power meters allow for cyclists to monitor power output (PO) during training and competition. The Garrnin Vector power meter (VPM) measures PO at the pedal compared to the crank and has been tested in only a few limited studies. The purpose of this study was to determine the validity and reproducibility of the VPM by comparing it to the SRM. The VPM validity was tested by (1) a submaximal incremental test, (2) submaximal constant power test, (3) sprint test, and (4) a field test. The reliability of the VPM was tested by repeating the laboratory tests 10 times over a 6 week span. Significant differences (P = 0.046) were found between the mean POSRM (178 ± 1.8 W) and POVPM (163.5 ± 14.7 W) for the submaximal constant-power test. No significant differences were found between the POMAX SRM and the POMAx VPM. The reproducibility of the VPM was lower than the SRM (CV = 8.52 ±4.0 vs 3.48 ± 1.9, 10.66% vs 5.50%, and 67.7% vs 55.3% for the submaximal incremental test, submaximal constant-power test, and field test respectively). The POVPM appears to underestimate the POSRM and is less valid and reliable across various cycling efforts. 展开更多
