BACKGROUND In endemic areas,vertical transmission of hepatitis B virus(HBV)remains a major source of the global reservoir of infected people.Eliminating mother-to-child transmission(MTCT)of HBV is at the heart of World Health Organization’s goal of reducing the incidence of HBV in children to less than 0.1%by 2030.Universal screening for hepatitis B during pregnancy and neonatal vaccination are the main preventive measures.AIM To evaluate the efficacy of HBV vaccination combined with one dose of immunoglobulin in children born to hepatitis B surface antigen(HBsAg)-positive mothers in Djibouti city.METHODS We conducted a study in a prospective cohort of HBsAg-positive pregnant women and their infants.The study ran from January 2021 to May 2022,and infants were followed up to 7 mo of age.HBV serological markers and viral load in pregnant women were measured using aVidas microparticle enzyme-linked immunosorbent assay(Biomérieux,Paris,France)and the automated Amplix platform(Biosynex,Strasbourg,France).All infants received hepatitis B immunoglobulin and were vaccinated against HBV at birth.These infants were closely monitored to assess their seroprotective response and for failure of immunoprophylaxis.Simple logistic regression was also used to identify risk factors associated with immunoprophylaxis failure and poor vaccine response.All statistical analyses were performed with version 4.0.1 of the R software.RESULTS Of the 50 pregnant women recruited,the median age was 31 years,ranging from 18 years to 41 years.The MTCT rate in this cohort was 4%(2/50)in HBsAg-positive women and 67%(2/3)in hepatitis B e antigen-positive women with a viral load>200000 IU/mL.Of the 48 infants who did not fail immunoprophylaxis,8(16%)became poor responders(anti-HB<100 mIU/mL)after HBV vaccination and hepatitis B immunoglobulin,while 40(84%)infants achieved a good level of seroprotection(anti-HB>100 mIU/mL).Factors associated with this failure of immunoprophylaxis were maternal HBV DNA levels(>200000 IU/mL)and hepatitis B e antigen-positive status(odds ratio=158,95%confidence interval:5.05-4958,P<0.01).Birth weight<2500 g was associated with a poor immune response to vaccination(odds ratio=34,95%confidence interval:3.01-383.86,P<0.01).CONCLUSION Despite a failure rate of immunoprophylaxis higher than the World Health Organization target,this study showed that the combination of immunoglobulin and HBV vaccine was effective in preventing MTCT of HBV.Therefore,further studies are needed to better understand the challenges associated with immunoprophylaxis failure in infants in Djibouti city. 展开更多

Diabetes mellitus(DM)is a systemic chronic metabolic disorder characterized by increased insulin resistance and/orβ-cell defects.It affects all ages from the foetal life,neonates,childhood to late adulthood.Gestational diabetes is a critical risk factor for congenital heart diseases(CHDs).Moreover,the risk increases with low maternal education,high body mass index at conception,undiagnosed pregestational diabetes,inadequate antenatal care,improper diabetes control,and maternal smoking during pregnancy.Maternal DM significantly affects the foetal heart and foetal-placental circulation in both structure and function.Cardiac defects,myocardial hypertrophy are three times more prevalent in infants of diabetic mothers(IDMs).Antenatal evaluation of the cardiac function and structures can be performed with foetal electrocardiography and echocardiography.Postnatal cardiac evaluation can be performed with natal and postnatal electrocardiography and echocardiography,detection of early atherosclerotic changes by measuring aortic intima-media thickness,and retinal vascular changes by retinal photography.Ameliorating the effects of diabetes during pregnancy on the offspring depends mainly on pregestational and gestational diabetes prevention.However,other measures to reduce the risk,such as using medications,nutritional supplements,or probiotics,still need more research.This review discusses the mechanism of foetal sequels and the risk factors that increase the prevalence of CHDs in gestational DM,the various cardiac outcomes of gestational DM on the foetus and offspring,cardiac evaluation of foetuses and IDMs,and how to alleviate the consequences of gestational DM on the offspring. 展开更多

Predictors of readiness for discharge in mothers of preterm infants: The role of stress, self-efficacy and perceived social support 被引量:1

Objective:To determine the predictive role of stress,self-efficacy,and perceived social support on readiness for discharge in mothers of preterm infants.Methods:The present cross-sectional,descriptive-analytical study was conducted on 120 mothers of preterm infants admitted to hospitals affiliated to Lorestan University of Medical Sciences,Iran in 2019.Participants were selected by a convenience sampling method and based on inclusion criteria.Data collection tools included the demographic questionnaire of mothers and infants,parent perceptions of their child's hospital discharge,parental stressor scale:neonatal intensive care unit,perceived maternal parenting,and multidimensional scale of perceived social support.Data were analyzed using Pearson correlation and stepwise regression at the significance level of 0.05.Results:Infant behavior and appearance,situational belief,and family support achieved the highest mean score from parents'stress,self-efficacy,and perceived social support dimensions,respectively.There was a significant relationship between stress,self-efficacy,and perceived social support with readiness for discharge in mothers of preterm infants(P<0.001).The score of mothers'readiness for discharge decreased by 0.07 per 1-point increase in stress score,and the score of readiness for discharge in mothers of preterm infants rose by 0.35 and 0.43,respectively,for a unit increase in the scores of self-efficacy and perceived social support.Conclusions:Stress,self-efficacy,and perceived social support can be considered as predictors of readiness for discharge in mothers of preterm infants.It is suggested that nurses in neonatal intensive care units provide a better platform for the readiness for discharge in mothers of preterm infants by reducing stressors and increasing maternal self-efficacy and social support. 展开更多

AIM: To evaluate a low cost Indian recombinant hepatitis B vaccine GeneVac-B for its immunogenicity and safety in comparison to Engerix B and Shanvac B vaccine in high risk newborn infants born to (hepatitis B surface antigen) HBsAg positive mothers.METHODS: A total of 158 infants were enrolled in the study. Fifty eight infants were enrolled in the GeneVac-B group while 50 each were included for Engerix B and Shanvac B groups. A three-dose regimen of vaccination; at birth (within 24 h of birth), 1st mo and 6 too. were adopted with 10 μg dosage administered uniformly in all the three groups. Clinical and immunological parameters were assessed for safety and immunogenicity of the vaccines, in all the enrolled infants.RESULTS: Successful follow up until seven months of age was achieved in 83% (48/58) for GeneVac-B, 76% (38/50) and 64% (32/50) for Engerix B and Shanvac B groups respectively. 100% seroconversion and seroprotection was achieved in all the three groups of infants. The geometric mean titers of anti-HBs one month after the completion of three dose of vaccination were 90.5, 80.9 and 72.5 mTU/mL in GeneVac-B, Engerix B and Shanvac B vaccine group respectively. Furthermore the level of anti-HBs increases with age of babies who were born to HBsAg positive mothers. The GMT values of anti-HBs were 226.7, 193.9 and 173.6 mIU/mL respectively in GeneVac-B, Engerix B and Shanvac B groups one year after the completion of the three doses of vaccine. No systemic reactions were reported in infants during the entire vaccination process of GeneVac-B and the other two vaccines. Clinical safety parameters remained within the normal limits throughout the study period.CONCLUSION: The study concludes that there is no significant difference between the three recombinant hepatitis B vaccines. Administration of these vaccines within 24 h of birth to babies, born to HBsAg positive mothers will reduce the incidence of HBV infection. 展开更多

BACKGROUND Breast milk is the best and principal nutritional source for neonates and infants.It may protect infants against many metabolic diseases,predominantly obesity and type 2 diabetes.Diabetes mellitus(DM)is a chronic metabolic and microvascular disease that affects all the body systems and all ages from intrauterine life to late adulthood.Breastfeeding protects against infant mortality and diseases,such as necrotizing enterocolitis,diarrhoea,respiratory infections,viral and bacterial infection,eczema,allergic rhinitis,asthma,food allergies,malocclusion,dental caries,Crohn's disease,and ulcerative colitis.It also protects against obesity and insulin resistance and increases intelligence and mental development.Gestational diabetes has short and long-term impacts on infants of diabetic mothers(IDM).Breast milk composition changes in mothers with gestational diabetes.AIM To investigate the beneficial or detrimental effects of breastfeeding on the cardiometabolic health of IDM and their mothers.METHODS We performed a database search on different engines and a thorough literature review and included 121 research published in English between January 2000 and December 15,2022,in this review.RESULTS Most of the literature agreed on the beneficial effects of breast milk for both the mother and the infant in the short and long terms.Breastfeeding protects mothers with gestational diabetes against obesity and type 2 DM.Despite some evidence of the protective effects of breastfeeding on IDM in the short and long term,the evidence is not strong enough due to the presence of many confounding factors and a lack of sufficient studies.CONCLUSION We need more comprehensive research to prove these effects.Despite many obstacles that may enface mothers with gestational diabetes to start and maintain breastfeeding,every effort should be made to encourage them to breastfeed. 展开更多

The mothers of preterm infants often feel hard to relate their infants because they have separated since their first days after delivery. Long term separation and less attractive, less responsive appearance of preterm infants also make it difficult to build mother-child relationships. In addition, the mothers of preterm infants are likely to have mood disorders such as depression, anxiety, and stress-related disorders. The mothers’ psychiatric illnesses affect the psychosocial development of preterm infants and are often regarded as a risk factor for child abuse and maltreatment in later life. Child abuse and maltreatment are also prevalent among preterm infants than the full term infants. Intervention from the early period of preterm birth is an important issue for both preterm infants and their mothers. Medical and co-medical professionals should pay attention to developmental outcome of preterm as well as psychosocial conditions of their mothers for the improvement of their mental health. 展开更多

Poor knowledge among mothers about the care requirements of a preterm neonate is immediate cause for post-discharge medical problems in premature and neonates readmission to NICU. Hence, this study aims to evaluate mothers’ knowledge of caring for premature infants post-discharge from Neonatal Intensive Care Units in the Gaza strip. Methods: A Quantitative-based cross-sectional designs study was used to survey 120 mothers of preterm neonates at the time of preterm neonates discharge by face-to-face interview at Al-Shifa medical complex and Nasser hospital between February and June 2018. Results: The results showed that only about 58.4% of mothers of premature babies had good knowledge about health care needed for premature infants after discharge from NICU. Furthermore, there was no statistically significant difference between the level of knowledge and mother’s sociodemographic characteristics (P-values > 0.05). Conclusion: Mothers’ knowledge of premature infants care was not at the optimal level, which might put the newborns at risk. Therefore, the study emphasizes the necessity of thoughtful exchange of health information between team members and mothers and establishing pre- and post-discharge plans with mothers to start their healthy transition of preterm neonate to home and to ameliorate family concerns. 展开更多

Gusii region is number 5 among counties that contribute to 65% of new HIV (Human Immunodeficiency Virus) in infections in Kenya. The study was to determine the prevalence of new HIV infections among infants (3 - 18 months) born of HIV positive mothers on PMTCT follow up at Kisii Teaching and Referral Hospital (KTRH). Method: This was a descriptive cross sectional study involving 96 infants born of HIV positive mothers on PMTCT programme at Kisii Teaching and Referral Hospital (KTRH) was conducted. The infants were between 3 to 18 months of age. The infants were investigated for their HIV status and factors associated with prevalence. The data collection was done during postnatal checkups, antiretroviral treatment follow up and immunization schedule. Data was analyzed using Statistical Package for Social Sciences (SPSS) version 20. Results: 13.5% (95% C.I = 10.1% to 16.9%) were found to be positive despite their mothers being on PMTCT program. There was no significant difference between gender and status of the infant (χ2 = 0.758, df = 1, p-value = 0.0384). Conclusion: The study confirms that there are still new HIV infections despite being followed up in the PMTCT program. The new HIV infections were associated with mixed feeding. Recommendation: Counseling on Infant feeding, use of ant-retroviral treatment and support from partners should be strengthen in the PMTCT program to ensure reduction of new HIV infections. 展开更多

Objective:To investigate predictors of caring behaviors of mothers of premature infants based on the health belief model.Methods:This cross-sectional study was conducted by using the structural equation modeling on 168 mothers of premature infants,who were selected by convenience sampling method from October 2017 to February 2018 in Iran.Data were collected by using a standard scale.Validity and reliability of all data collection tools were approved.Data were analyzed by using SPSS V.16 and Mplus6 software.Results:The structural equation modeling of the initial health belief model did not have a good fit,but the fitness of model 2 obtaining from the modified initial model was confirmed by changes in locations of constructs.None of constructs of model 2 had a significant positive association with the caring behavior of mothers of premature infants and only 2.8%of variance of caring behaviors in mothers could be predicted by the sum of variables of demographic characteristics and the modified health belief model constructs.Conclusions:Given that the findings do not approve the use of the health belief model in predicting determinants of caring behavior of mothers of premature infants,it is suggested to apply this model to investigate the effect of educational intervention based on the health belief model on the caring behavior of mothers. 展开更多

Aim: To assess the knowledge and oral health practices of a selected population of mothers in order to develop appropriate oral health promotion program for children in the area. Method: A cross sectional study was carried out among a convenient sample of mothers who attended two well-baby clinics—a tertiary and a primary health care centre in Lagos, Nigeria. A questionnaire requesting personal information, previous exposure to and source of oral health education (OHE), knowledge of oral health, diet and oral hygiene practices was administered. Results: There were 104 participants, aged 21 - 46 years (mean 32.01 ± 4.85 years);58.7% had received OHE, 23.1% from electronic media, 22.1% dentist and 9.6%, doctor or nurse. Only 44 (42.3%) had attended the dentist, 5 (4.8%) in <1 year and 10 (9.6%) within 1 - 2 years. Most younger mothers, compared with older mothers had knowledge of fluoride (x2 = 8.51, p = 0.014). Only 44 (42.3%) and 20 (19.2%) respectively believed the type of baby food and supplement affect the child’s teeth. Conclusion: Participants in this study showed inadequate knowledge of preventive oral health care. Regular OHE is recommended for mothers at the well baby and immunization clinics where they can be easily reached by health professionals. 展开更多

The Influence of Individual Determinants on Adherence to Feeding Guidelines for Infants Born to HIV Positive Mothers in Rakai District

The district has continued to experience high HIV rates at 12% compared to the national average of 7.3% with relatively higher vertical transmission rates. Objective: The purpose of this study was to determine the individual determinants of adherence to feeding guidelines for infants born to HIV positive mothers in Rakai District, Uganda. Methodology: A descriptive cross-sectional study design was used with a sample size of 138 respondents and employing quantitative method of enquiry. A non-probability sampling strategy called consecutive sampling was used to sample the mothers with HIV. Data were analyzed using SPSS to generate descriptive and bivariate analysis data. Results: Individual determinants found to have a statistically significant influence to feeding guidelines for infants born to mothers living with HIV were: Age (X2 = 45.967, P-value = 0.000), Number of children (X2 = 93.069, P-value = 0.000), Number of children given birth to since diagnosis (X2 = 18.604, P-value = 0.000), income per month (X2 = 50.477, P-value = 0.000), ever had breast complication (X2 = 15.039, P-value = 0.000), disclosure of HIV status (X2 = 10.740, P-value = 0.001), quantity of breast milk produced (X2 = 10.637, P-value = 0.0005) and whether mothers attended antenatal care services (X2 = 38.613, P-value = 0.000). Conclusion: Age, Number of Children, Number of Children given birth to since diagnosis, breast complications, disclosure of HIV status, quantity of breast milk produced and utilization of antenatal care services were found to have an association with the adherence to feeding guidelines among HIV positive mother and this was statistically significant. Recommendation: General improvement of socio-economic status of the mothers should be done and be given priority so that in turn it will enable the women to adequately focus on maintaining optimum and the health of their children. 展开更多

However, the relationship between infants’ characteristics and mothers’ attitudes toward breastfeeding is still uncertain. The aim of this study was to assess infants’ characteristics and their impact on Jordanian mothers’ attitudes toward breastfeeding. Methods: A descriptive cross-sectional design was used. A sample of 130 mothers was selected conveniently from mothers attending four major governmental maternal-child health centers at Zarqa city in Jordan. The Iowa Infant Feeding Attitude Scale along with infants’ characteristics data sheet were used for data collection. Results: A more positive attitude toward breastfeeding was manifest (mean 63.5, SD: 4.67). Positive attitudes toward breastfeeding were higher among women who delivered normally than women who delivered by cesarean section (p = 0.040). In addition, women who had healthy infants were more likely to have positive attitudes toward breastfeeding compared to women with ill infants (p = 0.021). However, women with preterm deliveries were less likely to have positive attitudes compared to women with full-term deliveries (p = 0.013). Likewise, women whose infants admitted to neonatal intensive care unit recorded less positive attitudes toward breastfeeding than women with healthy neonates (p = 0.043). Gestational age, admission to neonatal intensive care unit, infant’s health status, and type of delivery, were factors influencing mothers’ attitudes towards breastfeeding. Conclusions: Jordanian mothers have positive attitudes toward breastfeeding. However, positive attitudes are subject to different infants’ characteristics that should be considered while undertaking heath education programs for promoting breastfeeding. More focusing is recommended for women who delivered preterm infants, women who have ill infants, and women who delivered by cesarean section. 展开更多

Descriptive statistical analysis was performed for each category surveyed. Inferential statistics were used to compare the dangerous incidents experienced with regard to differences between primi/multipara and the presence/absence of guidance concerning such incidents. This study was approved by the research ethics committees of the affiliated university. <strong>Results:</strong> 60.0% of mothers and families experienced dangers while washing their infant in a baby tub or similar apparatus (ablution), and 64.9% did while bathing their infant in the normal bath. For ablution, the most common dangers were, in order, nearly dropping the infant in the water and nearly getting soap suds in the mouth. For bathing, these were nearly getting soap suds in the mouth, near submersion of the face, and nearly dropping the infant in the water. The percentage of mothers and families who did not receive instruction regarding potential dangers and their prevention was 55.9% for ablution and 81.8% for bathing. <strong>Conclusion:</strong> This study revealed the dangers experienced by mothers and families of infants aged 3 - 4 months in Japan during ablution and bathing. Most of the mothers experienced relaxation, communicated with each other, resolved physical problems, reduced childcare concerns, and developed parenting friends. <strong>Conclusions:</strong> In addition, feasibility was assured as they were very satisfied with the content of programs such as baby massage and aroma treatment. 展开更多

Background: Over 90% of infection in children is acquired through mother to child transmission (MTCT). Nevirapine’s efficacy, safety and affordable cost make it the most preferred non-nucleoside reverse transcriptase inhibitor (NNRTI) for paediatric prophylactic use in resource limited settings. There are very few studies on pediatric adherence to antiretroviral prophylaxis, especially in very young infants. The objective of the study was to evaluate the adherence to nevirapine for prevention of mother to child transmission (PMTCT) amongst infants of HIV positive mothers in the first 6 weeks of life. Methods: This was a questionnaire based prospective cross-sectional multicenter study initiated in July 2015 and concluded in December 2017. The study was carried out at the Aids Prevention in Nigeria (APIN/HIV) human immunodeficiency paediatric clinics of the Lagos University Teaching Hospital (LUTH), Idi-Araba, Lagos State, 68 Nigerian Army Reference Hospital Yaba (68NARHY) Lagos, Nigeria and the Holy Family Catholic Hospital, Festac Town, Amuwo Odofin, Lagos State. Nevirapine adherence, in infants of 0 - 6 weeks was measured by maternal verbal reports. Results: Eighty-one (81) infants receiving nevirapine within the age of 0 - 6 weeks completed the study. Majority of the infants started treatment at birth (32%) and after 4 weeks (31%). 54% of the 81 respondents do not miss or skip their doses while 46% of respondents skip doses. Majority of the respondents 18 (49%) missed their doses for 24 - 48 hours while 25% missed their doses because they were away from home and 23% ran out of pills. 85.2% took ≥95% of the prescribed medications (good or high adherence ), 8.2% took 80% - 95% of doses (moderate adherence) while 6.2% took 0.05). Conclusions: The adherence rates found in this study are comparable to other studies in developing countries. But these rates are still low for good clinical outcomes. Different levels of adherence were documented with the majority having good or high adherence. Not all of the infants started treatment at birth and Institutional delivery should be encouraged to improve early initiation of prophylaxis. The finding of patients with low adherence rates among the study population calls for intensification of counselling on adherence and should continue to be an integral part of prevention of mother to child transmission. Improved access to antiretroviral agents and improved care delivery systems are important. 展开更多

Objective:To assess whether infants of diabetic mothers [pre pregnancy diabetics(PPD)and gestational diabetics mellitus(GDM)] have compromised arachidonic and docosahexaenoic acids in their plasma and the relationship with deficiency of the same compounds in their mothers.Methods:This study was conducted on 30 diabetic mothers(both PPD and GDM)and their infants.Twenty healthy infants and their mothers with age and sex matched were included as controls.All infant(of diabetic and non diabetic mothers)were subjected to assessment of APGAR Scoring,thorough history taking and anthropometric measures.Lipid profile components as well as polyunsaturated fatty acids(PUFA)were assessed in diabetic GDM and PPD and non diabetic mothers as well as in their babies.Results:High-density lipoproteir(HDL)level was found to be significantly lower in diabetic mothers(specially those with PPD)compared to non diabetic ones,whereas no significant difference was found between babies of the two groups.Also,the current study revealed that diabetic mothers(GDM and PPD)and their babies had significantly higher levels of PUFA precursors linoleic acids(LA)and alpha linoleic acids(ALA).PUFA arachidonic acid(AA)and docosahexaenoic acids(DHA)were found to be significantly lower in diabetic mothers(GDM and PPD)compared to non diabetic mothers,and same results were found in the babies of the two groups.Conclusion:Neonates with diabetic mothers(both GDM and PPD)have highly compromised plasma levels of AA and DHA PUFA,which affects the child well being by far,and produces hazardous multi-system complications on the long run. 展开更多

The objective of this pilot study was to compare two different methods of educating prenatal women regarding breast-feeding. Comparisons were made between traditional and innovative methods to determine which was more effective in increasing breastfeeding duration. Over a 32 month period, 197 prenatal women were assigned to either a control (C, n = 139) or an experimental (E, n = 51) group. The C group received standard breastfeeding education, while the E group received standard education in addition to information about infant hunger cues. Cox Regression and Kap-lan- Meier analysis were performed. Estimated mean number of weeks for C and E groups to continue breastfeeding was 14.3 + 17.4 weeks and 18.5 + 17.1 weeks, respectively. At 26 weeks, duration of breastfeeding approached significance (chi square = 2.907, df = 1, p = 0.088), indicating probability of continuing to breastfeed was about 28% better for those in E group when compared to C group. Duration of breastfeeding may increase when prenatal women are taught to identify infant behavior such as hunger cues. 展开更多

OBJECTIVE: To study the relationship between the different replication status of hepatitis B virus (HBV) and mutations in the core promoter (CP) in mother and her child infected by mother-to-infant transmission. METHODS: The core promoter was amplified by PCR and cloned into pGEM-T vector with the T-A choning technique. The recombinant plasmid pGEM-CP was confirmed by digestion with restriction enzyme Apa I and Sac I. Two clones were selected to be sequenced in each patient. RESULTS: Every pair of mother and child had same serotype and genotype and the homology of nucleotides encoding 'a' determinant was 98%-100%. The number of mutations in the core promoter of patients with a high replication status was less than that in those with a low replication status. Mutations were mainly distributed in basia core promoter (BCP) and the inbibitor region of Kunitz-type serine protease. This difference was not associated with mother or child. CONCLUSION: The different replication status of HBV is caused by mutations in the core promoter in mother and child infected hy mother-to-infant transmission and appears to be not associated with the status of development of the infection. 展开更多
