Lead-Lag Connection of the Atlantic Multidecadal Oscillation (AMO) with East Asian Surface Air Temperatures in Instrumental Records 被引量:10

The authors analyzed the lead-lag connection of the Atlantic Multidecadal Oscillation (AMO) with East Asian surface air temperatures (EATs) using instrumental records, and compared the results with the Pacific Decadal Oscillation (PDO). The maximum correlation was found when EATs led the AMO by five to seven years (with a correlation coefficient of 0.72, whereas the correlation coefficient was 0.91 when the AMO led EATs by 24-28 years). This is different from the PDO, which mostly correlated with EATs when the PDO led EATs by 13-15 years (with a correlation coefficient of 0.67, whereas the correlation coefficient was 0.76 when EATs led the PDO by 24-26 years). The PDO led the AMO by 19-21 years (with a correlation coefficient of 0.71, whereas the correlation coefficient was 0.84 when the AMO led the PDO by 16-18 years). These results support a previous understanding that EATs positively correlate with the AMO, and imply that the observed East Asian warming trend may have been slowing down since the early 2010s. 展开更多

Foods wherein the starch is slowly digested contribute to good health by reducing the tendency to,and for the maintenance of,diabetes,obesity and colo-rectal cancers.While foods with high amylose content have this desirable property,they usually do not have high sensory appeal and consumers are reluctant to eat them.While sensory evaluation by trained human panelists is the best way to obtain consumer preferences,these tests are expensive,time-consuming and require considerable effort and care.Instrumental measurements are easier,cheaper and invaluable for screening,but only useful if they correlate with human data for the type of food being considered.Here,we test this using cooked rice with a wide range of amylose contents.Functional properties were correlated against quantitative descriptive panelist analysis.Swelling power and breakdown viscosity correlated with all panelist sensory attributes,but no other correlations with gelatinization properties were observed.There was a strong correlation between hardness and stickiness measured by texture profile analysis(TPA)and by panelists,suggesting that TPA can be used to measure hardness and stickiness of cooked rice.We also showed that breakdown viscosity is a reliable instrumental means to provide indicative measurements of hardness and rice preference,and swelling power is a predictor of rice stickiness. 展开更多

Objective:The objective of this article is to investigate the competence of Central Sterile Supply Department(CSSD)staff in insulation testing on electrosurgical instruments,to analyze the needs of training for CSSD staff,and also to strengthen the effectiveness of monitoring the insulation testing on electrosurgical instruments for CSSD staff to provide a reference for the nursing safety training.Materials and Methods:A total of 180 CSSD staff from 36 hospitals were enrolled as the research objects and investigated using“self‑assessment questionnaire of the competence in insulation testing on electrosurgical instruments and the needs of training".Results:The overall self‑assessment score of competence in insulation testing on electrosurgical instruments for CSSD staff is 3.10±0.42,including four dimensions which are attitude,system,cognition,and skills with the scores 3.31±0.55,2.80±0.63,2.74±0.68,and 3.03±0.36,respectively.Based on the survey of training needs for CSSD staff,the top three ranked training needs are trace management record of insulation testing on electrosurgical instruments(93.89%),disposal procedures for insulation damage of electrosurgical instruments(92.78%),and adverse events caused by leakage of electrosurgical instruments(80.55%).Conclusion:The competence of insulation testing for CSSD staff is at a low level by overall.The competence of CSSD staff could be improved by analyzing the needs of training,establishing a systematic training system,and taking effective training action,thus ensuring the safety of patients and medical personnel. 展开更多

The standalone Global Positioning System (GPS) does not meet the higher accuracy requirements needed for approach and landing phase of an aircraft. To meet the Category-I Precision Approach (CAT-I PA) requirements of civil aviation, satellite based augmentation system (SBAS) has been planned by various countries including USA, Europe, Japan and India. The Indian SBAS is named as GPS Aided Geo Augmented Navigation (GAGAN). The GAGAN network consists of several dual frequency GPS receivers located at various airports around the Indian subcontinent. The ionospheric delay, which is a function of the total electron content (TEC), is one of the main sources of error affecting GPS/SBAS accuracy. A dual frequency GPS receiver can be used to estimate the TEC. However, line-of-sight TEC derived from dual frequency GPS data is corrupted by the instrumental biases of the GPS receiver and satellites. The estimation of receiver instrumental bias is particularly important for obtaining accurate estimates of ionospheric delay. In this paper, two prominent techniques based on Kalman filter and Self-Calibration Of pseudo Range Error (SCORE) algorithm are used for estimation of instrumental biases. The estimated instrumental bias and TEC results for the GPS Aided Geo Augmented Navigation (GAGAN) station at Hyderabad (78.47°E, 17.45°N), India are presented. 展开更多

The effect of the policy on quality of care has received a lot of attention in the literature. Some of the earlier papers have ignored the endogeneity of length of stay in the readmission equation, an approach that could lead to inconsistent estimation. This study develops a statistical technique for the consistent estimation of the effect of the early discharge policy. An instrument that can be used extensively across different diagnostic groups is provided, hence solving the difficult problem of finding an instrument for length of stay. The exogeneity test in Gorgger (1990), the test for weak instruments in Staiger and Stock (1997) as well as the Hensen (1982) for over identification confirmed respectively that length of stay is endogenous the instrument is strong and the valid. 展开更多

However, traditional instrumental variable methods can only handle single variable which has limited its capability. This paper presents an extended instrumental variable method (EIVM) which is based on multiple variables. This provides the capability of taking into account of exogenous variables and reflects better the economic activities. Five-in-one: a novel, cost-effective yet simple use of micro needle holder

METHODS: In this study, the novel multifunctional use of micro needle holders was performed between 2018 and 2019 at the Department of Ophthalmology in the 4 hospitals in Shaanxi Province. In this innovation, the micro needle holders were initially used as micro forceps to remove sutures, as eye spud to safety extract foreign body from cornea, as ciliary forceps to remove trichiasis, as well as punctal dilator to dilate most small puncta. RESULTS: Using this technique, the medical costs of both procurement and sterilization were cut off in the selected 4 hospitals. The purchase cost has dropped by roughly 50%. The sterilization cost was decreased by about 30%. CONCLUSION: The innovation in the five-in-one multifunctional use of micro needle holders saves the medical costs. 展开更多

Based on the ground motion parameters from these records,the spatial distribution of seismic intensity can be quickly determined,and the degree of damage in different areas can be estimated. This information provides the technical basis for the emergency response,so as to ensure that rescue teams can reach extreme earthquake areas and carry out the search and rescue operation in an accurate and timely manner to reduce casualties and property loss. In this paper,we introduced 7 intensity algorithms and compared the results with the records of the Jiuzhaigou M_S7. 0 earthquake. We found that the differences between the instrumental intensities calculated by each method and the macro intensities were within a 1-degree range,which suggested good practicality of these different methods. The results calculated by the industrial standards-based calculation method and the integrated test showed good consistency. 展开更多

BACKGROUND Magnetically controlled growing rods(MCGR) are a novel treatment option for early onset scoliosis(EOS). Although the complication profile with MCGR use has been reviewed, these reviews do not take into account important implants modifications, termed iterations, that were made due to early on postoperative complications is not well reported or understood.AIM To assess the effect of MCGR implant iterations on post-operative complications in EOS.METHODS A systematic review was performed to identify studies investigating MCGR specifically for the treatment of EOS, refined to those reporting the implant iteration, specifically the incorporation of the keeper plate to the implant design.Articles with mixed implant iteration usage were excluded. Complications following surgery were recorded as well as potential risk factors and compared between implant cohorts.RESULTS Although 20 articles were identified for inclusion, 5 included mixed implant iteration leaving a total of 271 patients identified through 15 clinical studies thatmet inclusion criteria. The average follow-up was 25.4-mo. Pre-keeper plate implants were utilized in 3 studies with a total of 49 patients. Overall, 115(42.4%)post-operative complications were identified, with 87% defined as major. The addition of the keeper plate significantly decreased the rate of post-operative complications per study(35.7% vs 80.6%, P = 0.036), and the rate of distraction failure(8.1% vs 40.8%, P = 0.02). Unplanned reoperation occurred in 69(26.7%)patients but was not different between implant iteration cohorts(25.5% without keeper plate vs 27.1% with keeper plate, P = 0.92).CONCLUSION MCGR for EOS has a cumulative complication rate of 42.4% but this is significantly reduced to 35.7% when reviewing only keeper-plate enabled implants. However, 25% of published articles included mixed implant iterations.Future studies should discern between implants iterations when reporting on the usage of MCGR for EOS. 展开更多

Determination of a Cut-Off Point of a Scale for Achievement Motive in Geriatrics That Predicts the Frequency of Undertaking Instrumental Activities of Daily Living in Community-Dwelling Elderly People

Background: Among elderly people, rehabilitation is important for reducing the risk of falls and hospitalization and to maintain an independent life for longer. Motivation is a factor for elderly people to lead an active daily life and leave home more frequently. Purpose: The purposes of this study were to determine the optimal cut-off point in the Scale for Achievement Motive in Geriatrics (SAMG) that could identify individuals as inactive or active, based on the Frenchay Activities Index (FAI), and to compare characteristics between active and inactive groups classified by the cut-off point. Methods: A cross-sectional study of 285 community-dwelling elderly people measured SAMG and FAI and physical function. Receiver operating characteristic (ROC) analysis was used to calculate the area under the curve (AUC) that was the optimal cut-off point for the SAMG total score, with the associated sensitivity and 1-specificity when compared with the FAI results. Demographic parameters and physical function were compared between two groups defined by cut-off point. Results: The AUC was 0.78, the optimal cut-off for SAMG total score for indicating inactivity was ≤48, the sensitivity was 77.98% and the 1-specificity was 75.00%. The group with SAMG score >48 was characterized by a significantly higher proportion of women, fewer people sharing a house, and participation in more hobbies. Discussion: A cut-off point of 48 in the SAMG could be a predictive index of motivation toward goals for individuals with inactive daily activities. Objectives: The aim of this study was to show an overview of situation of instrumental extraction at the maternity of Cocody University Hospital by determining availability and assessing the maternal and fetal prognosis. Patients and method: it was a retrospective study with a descriptive purpose on 6 months from January 1 to June 31, 2015. It concerned all patients admitted in the expulsive phase of labor delivery room and with an indication of fetal instrumental extraction and the state of their newborns at birth. Results: We saved 2288 vaginal deliveries;including 28 instrumental deliveries on 104 indications of instrumental extraction is an implementation rate of 26.9% (16 by vacuum extractor and 12 forceps). Among patients with indication but without instrumental extraction (n = 76), there is 42.3% vaginal delivery (n = 44) and 30.8% of cesarean section (n = 32). 44 have given birth vaginally (42.3%) and 32 by caesarean section (30.8%). It’s young patient (28 years), nulliparous (42.3%). Average time between instrumental extraction indication and the delivery of the baby was 58 min in the case of instrumental extraction and 1 hour 41 minutes in the case of spontaneous delivery in anticipation of the c-section. Motherhood had 3 instrumental extractors (2 vacuum extractors and 1 forceps) recycled after each use. The Apgar score was good in 85.7% in children born by instrumental extraction and bad in 54.5% in children born vaginally without instrumental extraction. We found 20 stillborn in intra partum occurred only in children born vaginally without instrumental extraction. Twelve (12) cases of bleeding of the issue by uterine atony (27.3%) were recorded in patients pregnant without instrumental extraction. No maternal deaths were observed. Conclusion: The realization of instrumental extraction rate remains low at the maternity of the UH-C. In the event, the fetal prognosis was better. 展开更多
