Clinical efficacy of integral theory–guided laparoscopic integral pelvic floor/ligament repair in the treatment of internal rectal prolapse in females 被引量:3

BACKGROUND Internal rectal prolapse(IRP)is one of the most common causes of obstructive constipation.The incidence of IRP in women is approximately three times that in men.IRP is mainly treated by surgery,which can be divided into two categories:Abdominal procedures and perineal procedures.This study offers a better procedure for the treatment of IRP.AIM To compare the clinical efficacy of laparoscopic integral pelvic floor/ligament repair(IPFLR)combined with a procedure for prolapse and hemorrhoids(PPH)and the laparoscopic IPFLR alone in the treatment of IRP in women.METHODS This study collected the clinical data of 130 female patients with IRP who underwent surgery from January 2012 to October 2014.The patients were divided into groups A and B.Group A had 63 patients who underwent laparoscopic IPFLR alone,and group B had 67 patients who underwent the laparoscopic IPFLR combined with PPH.The degree of internal rectal prolapse(DIRP),Wexner constipation scale(WCS)score,Wexner incontinence scale(WIS)score,and Gastrointestinal Quality of Life Index(GIQLI)score were compared between groups and within groups before surgery and 6 mo and 2 years after surgery.RESULTS All laparoscopic surgeries were successful.The general information,number of bowel movements before surgery,DIRP,GIQLI score,WIS score,and WCS score before surgery were not significantly different between the two groups(all P>0.05).The WCS score,WIS score,GIQLI score,and DIRP in each group 6 mo,and 2 years after surgery were significantly better than before surgery(P<0.001).In group A,the DIRP and WCS score gradually improved from 6 mo to 2 years after surgery(P<0.001),and the GIQLI score progressively improved from 6 mo to 2 years after surgery(P<0.05).In group B,the DIRP,WCS score and WIS score significantly improved from 6 mo to 2 years after surgery(P<0.05),and the GIQLI score 2 years after surgery was significantly higher than that 6 mo after surgery(P<0.05).The WCS score,WIS score,GIQLI score,and DIRP of group B were significantly better than those of group A 6 mo and 2 years after surgery(all P<0.001,Bonferroni)except DIRP at 2 years after surgery.There was a significant difference in the recurrence rate of IRP between the two groups 6 mo after surgery(P=0.011).There was no significant difference in postoperative grade I-III complications between the two groups(P=0.822).CONCLUSION Integral theory–guided laparoscopic IPFLR combined with PPH has a higher cure rate and a better clinical efficacy than laparoscopic IPFLR alone. 展开更多

Secondly, by using cyclic integral or energy integral reduces the order of the equation and obtains generalized Routh equation and Whittaker equation respectively. Thirdly, derives canonical equation and variation equation and by using the first integral constructs integral invariant. The integration of additive manufacturing and topology optimization makes it possible to fabricate complex configurations,especially for microscale structures,which can guarantee the realization of high-performance structural designs.However,topology results often contain microstructures(several multicellular scales)similar to the characteristic length of local macrostructures,leading to errors in structural performance analysis based on classical theories.Therefore,it is necessary to consider the size effect in topology optimization.In this paper,we establish a novel topology optimization model utilizing the integral nonlocal theory to account for the size effect.The approach consists of an integral constitutive model that incorporates a kernel function,enabling the description of stress at a specific point in relation to strain in a distant field.Topology optimization structures based on nonlocal theory are presented for some benchmark examples,and the results are compared with those based on classical medium theory.The material layout exhibits significant differences between the two approaches,highlighting the necessity of topology optimization based on nonlocal theory and the effectiveness of the proposed method. 展开更多

This study explores the capabilities of ChatGPT, specifically in relation to consciousness and its performance in the Turing Test. The article begins by examining the diverse perspectives among both the cognitive and AI researchers regarding ChatGPT’s ability to pass the Turing Test. It introduces a hierarchical categorization of the test versions, suggesting that ChatGPT approaches success in the test, albeit primarily with na?ve users. Expert users, conversely, can easily identify its limitations. The paper presents various theories of consciousness, with a particular focus on the Integrated Information Theory proposed by Tononi. This theory serves as the framework for assessing ChatGPT’s level of consciousness. Through an evaluation based on the five axioms and theorems of IIT, the study finds that ChatGPT surpasses previous AI systems in certain aspects;however, ChatGPT significantly falls short of achieving a level of consciousness, particularly when compared to biological sentient beings. The paper concludes by emphasizing the importance of recognizing ChatGPT and similar generative AI models as highly advanced and intelligent tools, yet distinctly lacking the consciousness attributes found in advanced living organisms. 展开更多

This work deals with incompressible two-dimensional viscous flow over a semi-infinite plate ac-cording to the approximations resulting from Prandtl boundary layer theory. The governing non-linear coupled partial differential equations describing laminar flow are converted to a self-simi- lar type third order ordinary differential equation known as the Falkner-Skan equation. For the purposes of a numerical solution, the Falkner-Skan equation is converted to a system of first order ordinary differential equations. These are numerically addressed by the conventional shooting and bisection methods coupled with the Runge-Kutta technique. However the accompanying energy equation lends itself to a hybrid numerical finite element-boundary integral application. An appropriate complementary differential equation as well as the Green second identity paves the way for the integral representation of the energy equation. This is followed by a finite element-type discretization of the problem domain. Based on the quality of the results obtained herein, a strong case is made for a hybrid numerical scheme as a useful approach for the numerical resolution of boundary layer flows and species transport. Thanks to the sparsity of the resulting coefficient matrix, the solution profiles not only agree with those of similar problems in literature but also are in consonance with the physics they represent. 展开更多

The Conceptual Integration Theory was first formally put forward in 1997 by Fauconnier and Turner. According to it, there is a conceptual blending network comprised of four mental spaces: Input space Ⅰ, Input space Ⅱ, generic space, and blended space. In the process of blending, common information or structures from input spaces are projected to the "generic space". Meanwhile, through partially cross-space mapping, those structures are selectively projected to the "blended space". By means of composition, completion, and elaboration, consequently "emergent structure" comes into being from the development of blending. This theory instantly became a fresh power in cognitive research field. With the rapid development of network technology and the popularization of the internet, network language makes tremendous progresses and spreads quickly, which reflects the social and cultural development. The uniqueness and effectiveness of network language creation, to a great extent, relies on various rhetorical devices, among which parody is frequently used and plays an important role. In recent years, studies about network language somehow concentrate a lot on the construction, word transformation, and features of network vocabulary, and cognitive analysis on the mechanism of parody in network language is rather limited and requires further exploration. It is concluded that whether the second-order temporal interference pattern can or cannot be retrieved via two-photon coincidence counting rate is dependent on the resolution time of the detection system and the frequency difference between these two lasers. Two identical and tunable single-mode continuous-wave diode lasers are employed to verify the predictions. The second-order temporal interference of classical and nonclassical light at an asymmetrical beam splitter is discussed based on two-photon interference in Feynman's path integral theory. The visibility of the second-order interference pattern is determined by the properties of the superposed light beams, the ratio between the intensities of these two light beams, and the reflectivity of the asymmetrical beam splitter. Some requirements about the asymmetrical beam splitter have to be satisfied in order to ensure that the visibility of the second-order interference pattern of nonclassical light beams exceeds the classical limit. The visibility of the second-order interference pattern of photons emitted by two independent single-photon sources is independent of the ratio between the intensities. These conclusions are important for the researches and applications in quantum optics and quantum information when an asymmetrical beam splitter is employed. 展开更多

The polymer reference interaction site model (PRISM) integral equation theory was used to describe the structure and thermodynamic properties of atactic polystyrene (aPS) melt, in which the monomer of aPS is represented with an eight-site model to characterize its microstructure. The intramolecular structure factors needed in the PRISM calculations were obtained from single chain MD simulations. The calculated results indicate that the results by the integral equation method agrees well with experiments, and can reflect the fine microscopic structure of real aPS melt. This work shows that the PRISM theory is a powerful tool for investigating the structure and properties of complex polymers. 展开更多
