目的探讨经骨隧道微创复位联合Jail螺钉技术固定治疗孤立性胫骨平台后外侧塌陷骨折的临床疗效。方法2016年10月~2020年10月我科对25例孤立性胫骨平台后外侧塌陷骨折应用Mimics20.0软件行术前规划,术中经胫骨骨隧道空心顶棒锤击复位胫骨...目的探讨经骨隧道微创复位联合Jail螺钉技术固定治疗孤立性胫骨平台后外侧塌陷骨折的临床疗效。方法2016年10月~2020年10月我科对25例孤立性胫骨平台后外侧塌陷骨折应用Mimics20.0软件行术前规划,术中经胫骨骨隧道空心顶棒锤击复位胫骨平台后外侧塌陷骨折,Jail螺钉固定。比较术前、术后第2天及术后1年胫骨平台关节面塌陷距离、胫骨平台后倾角(posterior slope angle,PSA)及Rasmussen解剖评分,术后第2天、术后1年采用美国特种外科医院(Hospital for Special Surgery,HSS)评分评价膝关节功能。结果手术时间35~60 min,(55.6±4.4)min。术中出血量10~50 ml,(20.6±9.2)ml。骨折均Ⅰ期临床愈合,愈合时间8~12周,(10.5±1.3)周。25例随访12~36个月,(16.5±4.1)月。胫骨平台关节面塌陷距离由术前(9.2±2.2)mm恢复至术后第2天及术后1年(1.0±0.4)、(1.1±0.1)mm(均P=0.000);PSA由术前21.2°±2.1°恢复至术后第2天8.9°±0.9°分和术后1年9.2°±0.6°(均P=0.000);Rasmussen解剖评分由术前(7.8±1.8)分恢复至术后第2天(17.1±1.0)分和术后1年(16.3±0.7)分(均P=0.000)。与术后第2天比较,术后1年胫骨平台关节面塌陷距离、胫骨平台PSA及Rasmussen解剖评分差异无统计学意义(均P>0.05)。术后1年HSS评分(94.8±3.2)分,明显高于术后第2天(76.8±3.5)分(t=18.713,P=0.000)。结论经胫骨隧道复位联合Jail螺钉技术固定治疗孤立性胫骨平台后外侧塌陷骨折临床疗效满意,可避免继发性的关节面塌陷发生。展开更多
This study comprehensively characterizes the boundary values of generalized permeability jail in tight reservoirs through relative-permeability curve analysis,numerical simulation,and economic evaluation.A total numbe...This study comprehensively characterizes the boundary values of generalized permeability jail in tight reservoirs through relative-permeability curve analysis,numerical simulation,and economic evaluation.A total number of 108 relative-permeability curves of rock samples from tight reservoirs were obtained,and the characteristics of relative-permeability curves were analyzed.The irreducible water saturation(Swi)mainly ranges from 20% to 70%,and the residual gas saturation(Sgr)ranges from 5% to 15% for 55% of the samples.The relative-permeability curves are categorized into six types(Category-Ⅰ to Ⅵ)by analyzing the following characteristics:The relative permeability of gas at Swi,the relative permeability of water at Sgr,and the relative permeability corresponding to the isotonic point.The relative permeability curves were normalized to facilitate numerical simulation and evaluate the impact of different types of curves on production performance.The results of simulation show significant difference in production performance for different types of relative-permeability curves:Category-Ⅰ corresponds to the case with best well performance,whereas Categories-Ⅴ and Ⅵ correspond to the cases with least production volume.The results of economic evaluation show a generalized permeability jail for Categories-Ⅳ,Ⅴ,and Ⅵ,and the permeability jail develops when the relative permeability of gas and water is below 0.06.This study further quantifies the range of micro-pore parameters corresponding to the generalized permeability jail for a tight sandstone reservoir.展开更多
AIM To study the relationship of jailed polymer jacketed guide wires(PGW) with procedural myocardial infarction(PMI) after bifurcation coronary interventions.METHODS Consecutive bifurcation interventions performed fro...AIM To study the relationship of jailed polymer jacketed guide wires(PGW) with procedural myocardial infarction(PMI) after bifurcation coronary interventions.METHODS Consecutive bifurcation interventions performed from January 2010 to October 2014 were included in the study. Chart review was performed to obtain demographic, clinical and procedural data. PMI was defined as Creatine Kinase MB > 3 × upper reference limit of normal. Multivariate logistic regression was used to ascertain relationship of PGW use with PMI.RESULTS Two hundred and ninety-three patients(age 63.5 ± 12.3 years; 33.8% diabetic) were included in the study. Eighty point two percent(n = 235) were true bifurcation lesions use of PGW was associated with PMI on univariate analysis(OR = 4.1; P = 0.002). This association remained significant after adjusting for other possible risk factors(OR = 3.5; P = 0.02).CONCLUSION Our results suggest that PGW use for side branch protection may be associated with PMI. Randomized studies are needed to validate these findings.展开更多

Often, legal claims of medical negligence and §1983 based on deliberate indifference by the correctional officials follow. Unique stable and dynamic factors and circumstances and their interaction explain most suicides. A systematic suicide screening using a well-designed suicide screening questionnaire that captures known risk factors will identify most inmates at risk if adequately used. Identification and subsequent mental health assessment set the stage for preventive intervention, monitoring, and treatment of the inmates. A multidimensional suicide prevention program saves not only lives but also the best defense against a liability lawsuit.展开更多

In the present study, ...Aim Ulcerative colitis （UC） is an increasingly common condition particularly in developed countries. The lack of satisfactory treatment fuels the search for alternative therapeutic strategies. In the present study, we as- sessed the preclinical activity of berberine for the treatment of dextran sodium sulphate （DSS）-induced chronic re- lapsing colitis in C57BL/6 mice. Methods Colitis of mice was induced by three cycles of 2.0% DSS. From day13 onward, colitis mice were orally administered with 20 mg/kg berberine for 30 days. The disease severity was de- termined by daily monitoring the body weight, stool consistency, and stool bleeding of mice. At the end of treat- ment, colons were collected and subjected to histological, RT-qPCR, Western blot, and LC-MS analyses. Lympho- eytes were isolated from spleens and cultured for assessment of eytokine secretion. Results Berberine significantly ameliorated disease severity, colon shortening, histological injuries of colitis mice. Further, berberine treatment consistently and notably regulated DSS-assoeiated increase in mRNAs levels of Thl7-related eytokines （inhibition of IL-17 and ROR-γt） in the colon out of all tested eytokines. Moreover, the increases of TNF-α, IL-6 and IL-23 mRNA, and the phosphorylated STAT3 in colons of DSS - treated mice were significantly reversed by berberine. In addition, berberine directly inhibited TNF-α and IL-17 secretion from cultured lymphoeytes upon PMA/ionomyein -1 re-stimulation. In the meanwhile, a six-time amount of berberine in colon tissue （4.26 ± 2.62 ng · g^-1 colon） was measured when compared that in serum （0.76 ± 0.23 ng · m1^-1） and no detected histological changes was found in major organs of colitis mice. Conclusion We demonstrate for the first time that berberine exerts immuno- modulatory effect in alleviating DSS-indueed chronic relapsing colitis via inhibition of the JAK/STAT signalling acti- vation in the inflamed colon. The demonstration of activity in this model supports the possibility of clinical efficacy of berberine in treating chronic UC.展开更多
