Mid-term results of sub-trochanteric valgus osteotomy for symptomatic late stages Legg-Calvé-Perthes disease

BACKGROUND The treatment of late stages of Legg-Calvé-Perthes disease(LCPD)is controversial.Although the concept of femoral head containment is a well-established technique of treatment,its use remains debatable in the late stages of the disease,as it does not improve symptoms in terms of limb length discrepancy and gait.AIM To assess the results of subtrochanteric valgus osteotomy in symptomatic patients with late-stage Perthes disease.METHODS From 2000 to 2007,36 symptomatic patients with late stage of Perthes disease were surgically treated with subtrochanteric valgus osteotomy and followed-up for 8 to 11 years using the IOWA score and range of motion(ROM)variables.The Mose classification was also assessed at the last follow-up to reflect possible remodeling.The patients were 8 years old or older at the time of surgery,in the post-fragmentation stage,and complaining of pain,limited ROM,Trendelenburg gait,and/or abductor weakness.RESULTS The preoperative IOWA score(average:53.3)markedly improved at the 1-year post follow-up period(average:85.41)and then slightly improved at the last follow-up(average:89.4)(P value<0.05).ROM improved,with internal rotation increased on average by 22°(from 10°preoperatively to 32°postoperatively)and abduction increased on average by 15.9°(from 25°preoperatively to 41°postoperatively).The mean Mose deviation of femoral heads was 4.1 mm at the end of the follow-up period.The tests used were the paired t-test and Pearson correlation test,where the level of significance was a P value less than 0.05.CONCLUSION Subtrochanteric valgus osteotomy can be a good option for symptomatic relief in patients with late-stage of LCPD. 展开更多

Test results and comprehensive evaluation analysis demonstrated that 72% Bt·monosultap WP and 8%lambda-cyhalothrin·emamectin benzoate SA by manual spraying and unmanned aerial vehicle spraying had good prevention and control effect on strains and internodes damaged by Tryporyza intacta Snellen in middle and late stage, which were ideal new biological formulations with high effi-ciency and low risk for prevention and control of T. intacta in middle and late stage, and could be popularized in sugarcane area. 72% Bt·monosultap WP at the dose of 3 000 g/hectare and 8% lambda-cyhalothrin·emamectin benzoate SA at the dose of 750 mL/hectare could be sprayed in mid September at the peak occurrence period of the forth and fifth generations of T. intacta and Proceras venosatus. Agents were diluted with 900 kg water per hectare, and manually sprayed with electric knapsack sprayer; or agents were diluted with 15 kg of Haoyang aerial control special addi-tives and water per hectare, and sprayed with unmanned aerial vehicle. The control effect against borer-damaged strain rate was above 81.3% and that against borer-damaged internode rate was above 88.6%. 展开更多

Trajectory of Walking Function in Late-Stage Older Individuals Managed with a Regular Exercise Program: A 5-Year Longitudinal Tracking with an IoT Gait Analysis System Using Accelerometers

Purpose: This study focused on maintaining and improving the walking function of late-stage older individuals while longitudinally tracking the effects of regular exercise programs in a day-care service specialized for preventive care over 5 years, using detailed gait function measurements with an accelerometer-based system. Methods: Seventy individuals (17 male and 53 female) of a daycare service in Tokyo participated in a weekly exercise program, meeting 1 - 2 times. The average age of the participants at the start of the program was 81.4 years. Gait function, including gait speed, stride length, root mean square (RMS) of acceleration, gait cycle time and its standard deviation, and left-right difference in stance time, was evaluated every 6 months. Results: Gait speed and stride length improved considerably within six months of starting the exercise program, confirming an initial improvement in gait function. This suggests that regular exercise programs can maintain or improve gait function even age groups that predictably have a gradual decline in gait ability due to enhanced age. In the long term, many indicators tended to approach baseline values. However, the exercise program seemingly counteracts age-related changes in gait function and maintains a certain level of function. Conclusions: While a decline in gait ability with aging is inevitable, establishing appropriate exercise habits in late-stage older individuals may contribute to long-term maintenance of gait function. 展开更多

Purpose: This study verified the effects of transcutaneous electrical nerve stimulation (TENS), which can be worn during walking and exercise, in elderly individuals with late-stage knee pain who exercise regularly. Methods: Thirty-two late-stage elderly individuals were evaluated for knee pain during rest, walking, and program exercises, with and without TENS. Gait analysis was performed using an IoT-based gait analysis device to examine the effects of TENS-induced analgesia on gait. Results: TENS significantly reduced knee pain during rest, walking, and programmed exercises, with the greatest analgesic effect observed during walking. The greater the knee pain without TENS, the more significant the analgesic effect of TENS. A comparison of gait parameters revealed a significant difference only in the gait cycle time, with a trend towards faster walking with TENS;however, the effect was limited. Conclusion: TENS effectively relieves knee pain in late-stage elderly individuals and can be safely applied during exercise. Pain management using TENS provides important insights into the implementation of exercise therapy in this age group. 展开更多

Apatite Fission Track (AFT) data from the Songliao Basin indicates that the late stage tectonic movements in the Songliao Basin have zoning in space and episodes in time. The late stage tectonic movements started from the east part of the basin and migrated westward. AFT ages in the east part of the basin are older than those in the west part of the basin, suggesting that the uplift occurred earlier in the east than in the west. The denudation thickness in the east part of the basin is significantly greater than that in the centre and west. The thermal history evolved two episodes of rapid cooling and subsequent slow cooling processes. Age-depth relationship derived from the AFT data indicates a four-episode denudation history. Further Monte Carlo random simulation of the AFT data reveals the four changing points of the thermal evolution at 65 Ma, 43.5 Ma, 28 Ma and 15 Ma, respectively. The uplifting and denudation rates from different episodes of evolution are proportional to the plate convergence rate. Based on the above analyses and the regional geologic background, it is concluded that the late stage thermal events in the Songliao Basin are the far field response to the subduction of the Pacific Plate under the Eurasian Plate. The first episode of the rapid cooling probably started at the end of the Nenjiang Formation, climaxed at the end of the Cretaceous and ceased at the Late Eocene. The subsequent slow cooling lasts another 15 Ma. The first episode of the evolution is the far field response to the major episode of the Yanshan Movement and subsequent series of the tectonic reorganization, especially the directional change of the Pacific Movement and also the subduction of the Indian Plate underneath the Eurasian Plate. While the second episode of the evolution is the far field response to the extension and closure of the Sea of Japan. Extension led to the migration and converging of the mantle heat flow to the Sea of Japan and resulted in the rapid cooling of the Songliao Basin. 展开更多

Complex superimposed basins exhibit multi-stage tectonic events and multi-stage reservoir formation; hydrocarbon reservoirs formed in the early stage have generally late-stage genesis characteristics after undergoing adjustment, reconstruction and destruction of later-stage multiple tectonic events. In this paper, this phenomenon is called the late-stage reservoir formation effect. The late-stage reservoir formation effect is a basic feature of oil and gas-forming reservoirs in complex superimposed basins, revealing not only multi-stage character, relevance and complexity of oil and gas- forming reservoirs in superimposed basins but also the importance of late-stage reservoir formation. Late-stage reservoir formation is not a basic feature of oil and gas forming reservoir in superimposed basins. Multi-stage reservoir formation only characterizes one aspect of oil and gas-forming reservoir in superimposed basins and does not represent fully the complexity of oil and gas-forming reservoir in superimposed basins. We suggest using "late-stage reservoir formation effect" to replace the "late-stage reservoir formation" concept to guide the exploration of complex reservoirs in superimposed basins. Under current geologic conditions, the late-stage reservoir formation effect is represented mainly by four basic forms: phase transformation, scale reconstruction, component variation and trap adjustment. The late-stage reservoir formation effect is produced by two kinds of geologic processes: first, the oil and gas retention function of various geologic thresholds (hydrocarbon expulsion threshold, hydrocarbon migration threshold, and hydrocarbon accumulating threshold) causes the actual time of oil and gas reservoir formation to be later than the time of generation of large amounts of hydrocarbon in a conventional sense, producing the late-stage reservoir formation effect; second, multiple types of tectonic events (continuously strong reconstruction, early-stage strong reconstruction, middle-stage strong reconstruction, late-stage strong reconstruction and long-term stable sedimentation) after oil and gas reservoir formation lead to adjustment, reconstruction and destruction of reservoirs formed earlier, and form new secondary hydrocarbon reservoirs due to the late-stage reservoir formation effect. 展开更多

Neotectonism occurred intensively in the Bozhong depression in the Bohai Bay Basin, which was reflected vertically by dramatic subsidence and a number of uplifts and laterally by notable fault movements. This particularity has resulted in the special petroleum geological conditions of the Bozhong depression which are different from those of adjacent lands. For example, the source rocks of the Shahejie Formation were overpressured and hydrocarbon generation occurred in the late stage; the Dongying Formation was deeply buried below the hydrocarbon-generating threshold, therefore there were sufficient oil sources. The rapid subsidence led to starved sedimentation of the Guantao Formation fine sandstone and the regional Minghuazhen Formation lacustrine shale, which formed the Neogene regional reservoir-caprock association. The active faults formed in the neotectonism became passages for oil to migrate from the Paleogene to Neogene. The traps formed by late fault activity and accompanied anticlines provided spaces for the formation of reservoirs. All the above factors match well with one another in the Bozhong depression, providing favorable conditions for the formation of a series of large oilfields in the region 展开更多

Ammonia volatilized from aboveground parts of winter wheat was collected with an enclosure growth chamber and measured from jointing to maturing stage. The results showed that ammonia released from unfertilized plants grown in high and low fertility soils remained at low rates of 2.3 and 0. 9μg NH3 40 plant-1 h-1 respectively at late filling stage. However, fertilized plants rapidly increased the rates to 43. 4 and 52. 2μg NH3, 40 plant-1 h-1 in the high and low fertility soils, respectively, at the same period. The released a-mount was different in different parts of plants. At filling stage, lower senescing stems and leaves volatilized more ammonia than upper parts, i. e. , ears and flag leaves that grew normally, with an average of 1. 4 and 0.7μg NH3 20 plant-1 h-1 respectively, strongly suggesting that it was the senile organs that released large amounts of ammonia. At the grain filling stage, shortage of water supply (drought stress) reduced ammonia volatilization. The average rate of ammonia released under water stress was 0. 9μg NH3 40 plant-1 h-1, but 1.2μg NH3 40 plant-1 h-1 with moderate water supply. Application of N together with P fertilizer resulted in a higher ammonia volatilization than N fertilization alone at the maturing stage. The average rate released was 135.3 μg NH3 40 plant-1 h-1 when 0.4 g N and 0.13 g P had been added to per kg soil, while 33. 7μg when 0. 4 g N added alone. Ammonia volatilization from plants was closely related with plant biomass and N uptake;P fertilization increased plant biomass and N uptake and therefore increased its release. 展开更多

Based on the study of palaeo-environmental evolution in the shelves of the Eastern China Seas, the concept of "shelf desertization" in the late stage of Upper Pleistocene is defined; the environmental background and evolutionary process of shelf desertization are analysed. Study on the records of subbottom profiling and the data of core samples from shelf areas revealed that during low sea-level stages, the sedimentary environment in the exposed shelf plains was dominated by aeolian depositional process under cold and dry climatic conditions, i.e. under the action of strong winter-monsoon winds. Parts of the exposed marine strata were disintegrated, and aeolian sand dunes were formed on the disintegrated marine deposits, from which the finer sediment grains were blown away by wind and deposited in the downwind areas to form the derivative loess deposits. Thus a desertization environmental system was formed in the exposed shelf plains of the Eastern China Seas. 展开更多

Oceanic plateaus are large igneous provinces in the oceans,created by massive underwater eruptions,but their late-stage volcanism is poorly understood.With the addition of recent high-quality bathymetric data to existing data,286 secondary cones were discovered over Shatsky Rise oceanic plateau.These cones with steeper flank slopes(mean 6.1°±4.4°)and smaller sizes(102–1923 m in height)are morphologically distinct from the plateau,and they are thought to have formed after the main volcanic episodes.Cone height and characteristic height(420 m)are close to seamounts in the Pacific Ocean,whereas greater than those in the Atlantic Ocean.Mean flatness of Shatsky Rise’s cones(0.25±0.20)are similar to that of seamounts in both Pacific and Atlantic Oceans,but notably density in cone distribution(0.56 km^-3)and their mean slope are significantly lower than those of seamounts in the two oceans.Lower slopes of secondary cones within Shatsky Rise may be explained by higher effusion rates of remaining magma.Although cone formation was expected to have a link to rifting by seafloor spreading,weak relationship between cone orientation and magnetic anomaly pattern implies that the expectation is negative.Moreover,weak correlation between the cone height and depth indicates it is not true that volcanic cones grow taller when they occur closer to the massif summits with thicker oceanic crust,which was suggested as the increase in hydraulic pressure.Cone height and flatness are also not strongly related,implying that remaining magma supply was too limited to foster the cones to critical height. 展开更多

Objectives: To evaluate the concentration differences of sulforaphene and sulforaphane at various ages and in different parts of Raphanus sativus L. var. caudatus with respect to their potential cancer preventive effect on HCT116 colon cancer cells.Methods: FTIR–ATR and GC–MS were used to characterize the isothiocyanates in the plant extracts followed by HPLC for quantification. Antiproliferation and apoptosis induction were determined by using MTT assay and flow cytometry, respectively.Results: The respective rank of anticancer activity of Raphanus sativus were as follows:vegetative(3 week) < older rosette(4 week) < early-bolting(5 week) < senescence(7 week) < late-bolting(6 week). The low to high concentration of sulforaphene and sulforaphane occurred in the same stage order.Conclusions: The reproductive parts(flower, pod, and dry seed) of Raphanus sativus have the greatest isothiocyanate concentration, evidenced by a sulforaphene concentration higher than the sulforaphane. This result should inform the selection of the most appropriate harvesting stage and plant part for use as a potential chemopreventive agent. 展开更多

New structural and petrological data unveil a very complicated ductile deformation history of the Xiongdian-Suhe HP metamorphic unit, north-western Dabie Shan, central China. The fine-grained symplectic amphibolite-facies assemblage and coronal structure enveloping eclogite-facies garnet, omphacite and phengite etc., representing strain-free decompression and retrogressive metamorphism, are considered as the main criteria to distinguish between the early-stage deformation under HP metamorphic conditions related to the continental deep subduction and collision, and the late-stage deformation under amphibolite to greenschist-facies conditions occurred in the post-eclogite exhumation processes. Two late-stages of widely developed, sequential ductile deformations D 3 and D 4, are recognized on the basis of penetrative fabrics and mineral aggregates in the Xiongdian-Suhe HP metamorphic unit, which shows clear, regionally, consistent overprinting relationships. D 3 fabrics are best preserved in the Suhe tract of low post-D 3 deformation intensity and characterized by steeply dipping layered mylonitic amphibolites associated with doubly vergent folds. They are attributed to a phase of tectonism linked to the initial exhumation of the HP rocks and involved crustal shortening with the development of upright structures and the widespread emplacement of garnet-bearing granites and felsic dikes. D 4 structures are attributed to the main episode of ductile extension (D 1 4) with a gently dipping foliation to the north and common intrafolial, recumbent folds in the Xiongdian tract, followed by normal sense top-to-the north ductile shearing (D 2 4) along an important tectonic boundary, the so-called Majiawa-Hexiwan fault (MHF), the westward continuation of the Balifan-Mozitan-Xiaotian fault (BMXF) of the northern Dabie Shan. It is indicated that the two stages of ductile deformation observed in the Xiongdian-Suhe HP metamorphic unit, reflecting the post-eclogite compressional or extrusion wedge formation, the subhorizontal ductile extension and crustal thinning as well as the top-to-the north shearing along the high-angle ductile shear zones responsible for exhumation of the HP unit as a coherent slab, are consistent with those recognized in the Dabie-Sulu UHP and HP metamorphic belts, suggesting that they were closely associated in time and space. The Xiongdian-Suhe HP metamorphic unit thus forms part of the Triassic (250-230 Ma) collision orogenic belt, and can not connect with the South Altun-North Qaidam-North Qinling UHP metamorphic belt formed during the Early Paleozoic (500-400 Ma). 展开更多

According to our analysis of variations in China's industrial growth rate, industrial demand, industrial structure, regional structure and the performance of industrial firms, China's industrial economy is moving towards a "new normal" of slower growth and structural improvement. This process coincides with the late stage of China's industrialization, which international experience suggests is often fraught with twists and challenges. Among these challenges, great attention must be paid to overcapacity, industrial restructuring and upgrades, and the "reindustrialization" of developed countries. As China enters the late stage of its industrialization, particularly during the 13th Five-Year Plan period from 2016 to 2020, the promotion of industrial development is of strategic significance for China to successfully complete industrialization and its economy to adapt to the "new normal". In the face of these new challenges regarding industrial development, policymakers must strive to increase the dynamism of growth for industrial economy. In this new era, the momentum of industrial growth derives from the integration of the supply impetus of industrialization and the pull of demand caused by urbanization, while the comprehensive deepening of reform provides the key source of momentum. 展开更多

The results showed that frs(luency of allelic deletion of chromosome 17short arms (FADC17P) in Dukes’ D stage (95 % ) was higher than those in Dukes’ A, B. C stages (54% 68% ); Fisher’s exact test P <0. 01; FADC17p in colorectal carcinomas with distant metastasis (95 % ) washigher than those without distant metastasis, Fisher’s exact test P <0. 01 However, there were no significant differences in FADC 1 7p between the colorectal carcinomas with lymph node metastasis and those without lymph node metastasis (P >0. 05). Therefore, it is considered that ADC17p is an important diagnosticmarker of late stage colorectal carcinomas, and indicates a poor prognosis. 展开更多

目的研究安罗替尼联合化疗一线治疗晚期非小细胞肺癌(NSCLC)的价值。方法回顾分析2018年3月至2022年5月郑州大学第五附属医院收治的90例晚期NSCLC患者资料,其中45例接受单纯一线化疗的患者纳入对照组,45例联合安罗替尼治疗的患者纳入观... 目的研究安罗替尼联合化疗一线治疗晚期非小细胞肺癌(NSCLC)的价值。方法回顾分析2018年3月至2022年5月郑州大学第五附属医院收治的90例晚期NSCLC患者资料,其中45例接受单纯一线化疗的患者纳入对照组,45例联合安罗替尼治疗的患者纳入观察组。两组均连续治疗4个化疗周期。记录两组治疗效果、肿瘤标志物水平[细胞角蛋白19片段抗原21-1(CYFRA21-1)、癌胚抗原(CEA)和神经元特异性烯醇化酶(NSE)]、生存情况、不良反应发生情况。结果治疗后,观察组治疗总有效率高于对照组(P<0.05)。治疗前,两组CYFRA21-1、NSE、CEA差异无统计学意义(P>0.05);治疗后,两组CYFRA21-1、NSE、CEA均降低,且观察组低于对照组,差异有统计学上的意义(P<0.05)。观察组生存时间长于对照组,生存率高于对照组,差异有统计学上的意义(P<0.05)。治疗期间,两组不良反应发生率差异无统计学意义(P>0.05)。结论安罗替尼联合化疗一线治疗晚期NSCLC可以进一步提高效果,降低肿瘤标志物水平,有助于延长患者生存时间,且不会增加不良反应,安全性较好。 展开更多
