Objective:This study aimed to identify the challenges of community health nurses(CHNs)in delivering effective community health care to achieve universal health coverage(UHC)in Myanmar.Methods:A total of 30 CHNs from t...Objective:This study aimed to identify the challenges of community health nurses(CHNs)in delivering effective community health care to achieve universal health coverage(UHC)in Myanmar.Methods:A total of 30 CHNs from township health centers in the northeastern,southern,and western parts of Myanmar were purposefully recruited for quantitative and qualitative interviews.Quantitative data were processed using Microsoft Excel software,and qualitative data were analyzed using thematic analysis.This study is registered with researchregistry6201.Results:Around the country,30 CHNs uncovered their hardships in implementing primary health care to achieve UHC.Over 90%of the participants agreed to the problem of inadequate health infrastructure,while half of them felt unmotivated when they encountered role conflicts among various cadres of healthcare providers and poor opportunities for career promotion.Major problems arose from the lack of standard professional education at the entry point to community settings because most CHNs did not achieve specialized training in providing public health services.Complications are incapable of evaluating health services for policy-making and the inability to conduct health research to develop evidencebased practices.Insecure work and living conditions,unsupportive community relationships,and undereducation in professional practices were supportive major themes explored by CHNs to achieve a deeper understanding of the barriers to UHC.Not only the health system itself but also the population and other geographical factors have contributed to many challenges to CHNs.Conclusion:Myanmar’s CHNs face many challenges in achieving UHC.These challenges are not confined to the health sector.Some situations,such as geographical barriers and transportation problems,remain persistent challenges for healthcare providers.This study highlights the fact that current health systems should be strengthened by qualified healthcare providers and sufficient infrastructure.Meanwhile,public empowerment plays a critical role in promoting health development.展开更多
Objectives: To explore patient’s perspectives of the role of the community pharmacist in view of other health care professionals (specialist physician, general practitioner, nurse and others (e.g. herbalist)) in Jord...Objectives: To explore patient’s perspectives of the role of the community pharmacist in view of other health care professionals (specialist physician, general practitioner, nurse and others (e.g. herbalist)) in Jordan, UAE and Iraq. The study also investigated patient’s willingness to pay for specialized pharmaceutical care services. Methods: This study followed a single phase cross sectional survey methodology, conducted in three Middle Eastern countries. Data were collected over a period of 6 months on two consecutive years (March to May 2009 and 2010). A questionnaire was designed and validated, then completed by patients walking into community pharmacies. The source of advice (specialist physician, general practitioner, pharmacist, nurse, or herbalist) regarding patient’s medication use and medical management were investigated. Key findings: Patients visiting community pharmacies in Jordan (n = 1000), UAE (n = 1000) and Iraq (n = 968) were recruited into the study (mean age 35.9 ± 13.1, 50.6% males). Significant difference between the three countries was shown, as more patients chose the pharmacist as their primary source of advice on medication use vs. the specialist physician in Jordan (50.8% vs. 37.3%) and Iraq (41.9% vs. 36.7%) compared to UAE (38.0% vs. 40.1%), P < 0.001, Chi-square test. Few agreed to receive paid pharmaceutical care services (Jordan 19.5%;UAE 24.7%;Iraq 2.3%). Conclusion: Current situation resulting in the lower socioeconomic status in Jordan and Iraq seems to lead to patients’ higher reliance on the pharmacist, as compared to the UAE. Majority of patients, regardless of the country of origin, are not willing to pay for specialized pharmaceutical care services. Tropical forests provide some of the richest biodiversity amid and adjacent to the world’s poorest people as they lose access to resources when forests are designated for strict preservation.Integrated Conservation D...Tropical forests provide some of the richest biodiversity amid and adjacent to the world’s poorest people as they lose access to resources when forests are designated for strict preservation.Integrated Conservation Development Projects(ICDPs)aimed at conservation and compensated communities adjacent to conserved resources through revenue sharing from the resultant economic activities.However,Community-Based Natural Resource Management(CBNRM)approaches seek to reconcile biodiversity conservation with community livelihoods strategies to reduce poverty.Nkuringo Conservation and Community Development Foundation on the Southern Fringes of Bwindi National Park in Kisoro District adopted the Community-Based Natural Resource Management model where communities benefit both financially and socially from conservation.The model provides a“win-win”situation where both human beings and wildlife benefit.The objective of this paper is to profile Nkuringo Conservation and Community Development Foundation(NCCDF)adoption of the model by analyzing the model,actors involved,the legal framework,benefits accrued,challenges,and strategies to ameliorate them.The paper adopted a case study research design and collected both qualitative and quantitative data using desk top research methods,observation,oral interviews,and focus group discussions(FGDs).The communities of Nteko and Rubuguri parishes participate in the project which has conserved the mountain gorillas and forests for tourism and diversified community livelihood through tourism,sale of handcrafts,an eco-lodge(Clouds Lodge)and commercial agriculture which contribute to poverty reduction.展开更多

To achieve it, we perform a mini-review of health development policies and pl...The objective of this work is to analyze the reasons why community health interventions in Africa do not consider multisectoral approaches. To achieve it, we perform a mini-review of health development policies and plans available online in seven countries from four regions of Sub-Saharan Africa. Thus, two main reasons have been highlighted. First, national strategic plans and policies for health development, in their formulation, neither sufficiently emphasize multisectoral approaches, nor sufficiently make these approaches operational in strategies and activities. Second, the mindset of health professionals due to their initial training orientation based on the biomedical approach, stands that disease is only a result of a physiological imbalance in the body;therefore, to restore health, such an imbalance only needs sophisticated procedures and interventions to be overcome. Such an orientation completely ignores the social, cultural and economic context in which the individual lives, which has an irretrievable influence on the health imbalance. However, health, influenced by the conditions in which people are conceived, born, grow, live, work and aged, cannot be effectively improved in a sustainable way without taking into account all these conditions. Whence the importance of approaches based on every sector of human activity that influences the living conditions.展开更多
