In this editorial review,we comment on the article published in the recent issue of the World Journal of Gastrointestinal Surgery.Carcinoembryonic antigen(CEA)is a fetal glycoprotein and can be secreted in very small amounts from healthy adults after birth.CEA is widely used not only for diagnostic tumor markers but also importantly for the management of some gastrointestinal tumors.The most common clinical use is surveillance for the monitoring of colorectal carcinoma.However,CEA can become elevated in several malign or benign characterized pathologies.Serum CEA level may vary depending on the location of the lesion,whether it metastasizes or not,and its histopathological characteristics.It has been determined that cases with high preoperative CEA have a more aggressive course and the risk of metastasis to the lymph tissue and liver increases.In this editorial review,we focused on evaluating the role of CEA in clinical practice with a holistic approach,including the diagnostic and prognostic significance of CEA in patients with focal liver lesions,the role of CEA in follow-up after definitive surgery,and also hepatic resection for metastasis,and the management of all patients with raised CEA. 展开更多

In patients with liver tumors,the histopathology examination can assist in diagnosis,staging,prognosis,and therapeutic management strategy.Endoscopic ultrasound(EUS)-guided tissue acquisition using fine needle aspiration(FNA)or more newly fine needle biopsy(FNB)is a well-developed technique in order to evaluate and differentiate the liver masses.The goal of the EUS-FNA or EUS-FNB is to provide an accurate sample for a histopathology examination.Therefore,malignant tumors such as hepatocarcinoma,cholangiocarcinoma and liver metastasis or benign tumors such as liver adenoma,focal hyperplastic nodular tumors and cystic lesions can be accurately diagnosed using EUS-guided tissue acquisition.EUS-FNB using 19 or 22 Ga needle provide longer samples and a higher diagnostic accuracy in patients with liver masses when compared with EUS-FNA.Few data are available on the diagnostic accuracy of EUS-FNB when compared with percutaneously,ultrasound,computer tomography or transjugulary-guided liver biopsies.This review will discuss the EUS-guided tissue acquisition options in patients with liver tumors and its efficacy and safety in providing accurate samples.The results of the last studies comparing EUS-guided liver biopsy with other conventional techniques are presented.The EUS-guided tissue acquisition using FNB can be a suitable technique in suspected liver lesions in order to provide an accurate histopathology diagnosis,especially for those who require endoscopy. 展开更多

BACKGROUND:Contrast agents help to improve visibility in magnetic resonance(MR)imaging.However,owing to the large interstitial spaces of the liver,there is a reduction in the natural contrast gradient between lesions and healthy tissue.This study was undertaken to evaluate the efficacy and safety of the liverspecific MR imaging contrast agent gadoxetate disodium(GdEOB-DTPA)in Chinese patients.METHODS:This was a single-arm,open-label,multicenter study in patients with known or suspected focal liver lesions referred for contrast-enhanced MR imaging.MR imaging was performed in 234 patients before and after a single intravenous bolus of Gd-EOB-DTPA(0.025 mmol/kg body weight).Images were evaluated by clinical study investigators and three independent,blinded radiologists.The primary efficacy endpoint was sensitivity in lesion detection.RESULTS:Gd-EOB-DTPA improved sensitivity in lesion detection by 9.46%compared with pre-contrast imaging for the average of the three blinded readers(94.78%vs 85.32%for Gd-EOB-DTPA vs pre-contrast,respectively).Improvements in detection were more pronounced in lesions less than 1cm.Gd-EOB-DTPA improved diagnostic accuracy in lesion classification.CONCLUSIONS:This open-label study demonstrated that Gd-EOB-DTPA improves diagnostic sensitivity in liver lesions,particularly in those smaller than 1 cm.Gd-EOB-DTPA also significantly improves the diagnostic accuracy in lesion classification,and furthermore,Gd-EOB-DTPA is safe in Chinese patients with liver lesions. 展开更多

BACKGROUND: Contrast-enhanced ultrasonography (CEUS) is increasingly accepted in clinical settings for diagnostic imaging of focal liver lesions (FLLs). This study aimed to assess the efficacy of CEUS in the characterization of FLLs in comparison with final diagnosis based on gold standard assessment. METHODS: The study was approved by the local ethics committee and participating patients provided written informed consent. A total of 148 patients with 164 FLLs were studied. Unenhanced ultrasonography (US) and CEUS were performed using fundamental and harmonic imaging, respectively. Contrast enhancement was assessed during the arterial, portal and late vascular phases after intravenous administration of contrast (SonoVue (R), Bracco, Italy). Sensitivity, specificity and diagnostic accuracy of US and CEUS were compared in identifying the lesion as benign, malignant or indeterminate and its actual tumor type. Final diagnosis was established by biopsy (129/164), MR imaging (11/164) or medical history (24/164). RESULTS: When compared to the gold standard, the number of indeterminate diagnoses was reduced from 56.7% (93/164) as assessed by fundamental imaging to 6.1% (10/164) after SonoVue (R) enhanced US examination. Sensitivity and specificity improved from 49% and 25% at baseline US to 93% and 75% with CEUS, respectively (P<0.01). Diagnostic accuracy of CEUS was 88% in contrast to 41% of baseline US. CONCLUSION: SonoVue (R) enhanced US improves the characterization of FLLs and may limit the need for further investigations. 展开更多

BACKGROUND R2^(*)estimation reflects the paramagnetism of the tumor tissue,which may be used to differentiate between benign and malignant liver lesions when contrast agents are contraindicated.AIM To investigate whether R2^(*)derived from multi-echo Dixon imaging can aid differentiating benign from malignant focal liver lesions(FLLs)and the impact of 2D region of interest(2D-ROI)and volume of interest(VOI)on the outcomes.METHODS We retrospectively enrolled 73 patients with 108 benign or malignant FLLs.All patients underwent conventional abdominal magnetic resonance imaging and multi-echo Dixon imaging.Two radiologists independently measured the mean R2^(*)values of lesions using 2D-ROI and VOI approaches.The Bland-Altman plot was used to determine the interobserver agreement between R2^(*)measurements.Intraclass correlation coefficient(ICC)was used to determine the reliability between the two readers.Mean R2^(*)values were compared between benign and malignant FFLs using the nonparametric Mann-Whitney test.Receiver operating characteristic curve analysis was used to determine the diagnostic performance of R2^(*)in differentiation between benign and malignant FFLs.We compared the diagnostic performance of R2^(*)measured by 2D-ROI and VOI approaches.RESULTS This study included 30 benign and 78 malignant FLLs.The interobserver reproducibility of R2^(*)measurements was excellent for the 2D-ROI(ICC=0.994)and VOI(ICC=0.998)methods.Bland-Altman analysis also demonstrated excellent agreement.Mean R2^(*)was significantly higher for malignant than benign FFLs as measured by 2D-ROI(P<0.001)and VOI(P<0.001).The area under the curve(AUC)of R2^(*)measured by 2D-ROI was 0.884 at a cut-off of 25.2/s,with a sensitivity of 84.6%and specificity of 80.0%for differentiating benign from malignant FFLs.R2^(*)measured by VOI yielded an AUC of 0.875 at a cut-off of 26.7/s in distinguishing benign from malignant FFLs,with a sensitivity of 85.9%and specificity of 76.7%.The AUCs of R2^(*)were not significantly different between the 2D-ROI and VOI methods.CONCLUSION R2^(*)derived from multi-echo Dixon imaging whether by 2D-ROI or VOI can aid in differentiation between benign and malignant FLLs. 展开更多

BACKGROUND The accurate classification of focal liver lesions(FLLs)is essential to properly guide treatment options and predict prognosis.Dynamic contrast-enhanced computed tomography(DCE-CT)is still the cornerstone in the exact classification of FLLs due to its noninvasive nature,high scanning speed,and high-density resolution.Since their recent development,convolutional neural network-based deep learning techniques has been recognized to have high potential for image recognition tasks.AIM To develop and evaluate an automated multiphase convolutional dense network(MP-CDN)to classify FLLs on multiphase CT.METHODS A total of 517 FLLs scanned on a 320-detector CT scanner using a four-phase DCECT imaging protocol(including precontrast phase,arterial phase,portal venous phase,and delayed phase)from 2012 to 2017 were retrospectively enrolled.FLLs were classified into four categories:Category A,hepatocellular carcinoma(HCC);category B,liver metastases;category C,benign non-inflammatory FLLs including hemangiomas,focal nodular hyperplasias and adenomas;and category D,hepatic abscesses.Each category was split into a training set and test set in an approximate 8:2 ratio.An MP-CDN classifier with a sequential input of the fourphase CT images was developed to automatically classify FLLs.The classification performance of the model was evaluated on the test set;the accuracy and specificity were calculated from the confusion matrix,and the area under the receiver operating characteristic curve(AUC)was calculated from the SoftMax probability outputted from the last layer of the MP-CDN.RESULTS A total of 410 FLLs were used for training and 107 FLLs were used for testing.The mean classification accuracy of the test set was 81.3%(87/107).The accuracy/specificity of distinguishing each category from the others were 0.916/0.964,0.925/0.905,0.860/0.918,and 0.925/0.963 for HCC,metastases,benign non-inflammatory FLLs,and abscesses on the test set,respectively.The AUC(95%confidence interval)for differentiating each category from the others was 0.92(0.837-0.992),0.99(0.967-1.00),0.88(0.795-0.955)and 0.96(0.914-0.996)for HCC,metastases,benign non-inflammatory FLLs,and abscesses on the test set,respectively.CONCLUSION MP-CDN accurately classified FLLs detected on four-phase CT as HCC,metastases,benign non-inflammatory FLLs and hepatic abscesses and may assist radiologists in identifying the different types of FLLs. 展开更多

To explore the clinical value of contrast-enhanced ultrasound (CEUS) in differentiating benign and malignant focal liver lesions (FLLs) with SonoVue, CEUS was used to examine 113 patients with focal liver lesions (FLLs) in our hospital during July 2005 to December 2006. All the patients underwent contrast-enhanced CT (CECT) or contrast-enhanced MRI (CEMRI). Except for patients with focal fatty sparings (n=18) and with hemangiomas (n=8), all the patients were confirmed by operation or ultrasonic-guided liver puncture biopsy. A sulfur hexafluoride gas-based contrast agent was used with a MI of 0.15 to 0.17. Forty-eight cases of malignant FLLs, including 30 hepatocellular carcinomas (HCCs), 2 cholangiocarcinomas and 16 metastatic tumors, were detected. Seventy-eight cases of benign FLLs, including 33 hemangiomas, 9 focal nodular hyperplasias (FNHs), 19 focal fatty sparings, 5 abscesses, 7 regenerative nodules and 2 inflammatory pseudo-tumor, were involved. The contrast pattern of benign and malignant FLLs was quite different. CEUS has higher specificity and sensitivity than conventional ultrasound in differentiating benign and malignant FLLs. 展开更多

BACKGROUND The clinical management and prognosis differ between benign and malignant solid focal liver lesions(FLLs),as well as among different pathological types of malignant FLLs.Accurate diagnosis of the possible types of solid FLLs is important.Our previous study confirmed the value of shear wave elastography(SWE)using maximal elasticity(Emax)as the parameter in the differential diagnosis between benign and malignant FLLs.However,the value of SWE in the differential diagnosis among different pathological types of malignant FLLs has not been proved.AIM To explore the value of two-dimensional SWE(2D-SWE)using Emax in the differential diagnosis of FLLs,especially among different pathological types of malignant FLLs.METHODS All the patients enrolled in this study were diagnosed as benign,malignant or undetermined FLLs by conventional ultrasound.Emax of FLLs and the periphery of FLLs was measured using 2D-SWE and compared between benign and malignant FLLs or among different pathological types of malignant FLLs.RESULTS The study included 32 benign FLLs in 31 patients and 100 malignant FLLs in 96 patients,including 16 cholangiocellular carcinomas(CCCs),72 hepatocellular carcinomas(HCCs)and 12 liver metastases.Thirty-five FLLs were diagnosed as undetermined by conventional ultrasound.There were significant differences between Emax of malignant(2.21±0.57 m/s)and benign(1.59±0.37 m/s)FLLs(P=0.000),and between Emax of the periphery of malignant(1.52±0.39 m/s)and benign(1.36±0.44 m/s)FLLs(P=0.040).Emax of liver metastases(2.73±0.99 m/s)was significantly higher than that of CCCs(2.14±0.34 m/s)and HCCs(2.14±0.46 m/s)(P=0.002).The sensitivity,specificity and accuracy were 71.00%,84.38%and 74.24%respectively,using Emax>1.905 m/s(AUC 0.843)to diagnose as malignant and 23 of 35(65.74%)FLLs with undetermined diagnosis by conventional ultrasound were diagnosed correctly.CONCLUSION Malignant FLLs were stiffer than benign ones and liver metastases were stiffer than primary liver carcinomas.2D-SWE with Emax was a useful complement to conventional ultrasound for the differential diagnosis of FLLs. 展开更多

Diagnostic and therapeutic role of endoscopic ultrasound in liver diseases:A systematic review and meta-analysis

BACKGROUND In hepatology,the clinical use of endoscopic ultrasound(EUS)has experienced a notable increase in recent times.These applications range from the diagnosis to the treatment of various liver diseases.Therefore,this systematic review summarizes the evidence for the diagnostic and therapeutic roles of EUS in liver diseases.AIM To examine and summarize the current available evidence of the possible roles of the EUS in making a suitable diagnosis in liver diseases as well as the therapeutic accuracy and efficacy.METHODS PubMed,Medline,Cochrane Library,Web of Science,and Google Scholar databases were extensively searched until October 2023.The methodological quality of the eligible articles was assessed using the Newcastle-Ottawa scale or Cochrane Risk of Bias tool.In addition,statistical analyses were performed using the Comprehensive Meta-Analysis software.RESULTS Overall,45 articles on EUS were included(28 on diagnostic role and 17 on therapeutic role).Pooled analysis demonstrated that EUS diagnostic tests had an accuracy of 92.4%for focal liver lesions(FLL)and 96.6%for parenchymal liver diseases.EUS-guided liver biopsies with either fine needle aspiration or fine needle biopsy had low complication rates when sampling FLL and parenchymal liver diseases(3.1%and 8.7%,respectively).Analysis of data from four studies showed that EUS-guided liver abscess had high clinical(90.7%)and technical success(90.7%)without significant complications.Similarly,EUS-guided interventions for the treatment of gastric varices(GV)have high technical success(98%)and GV obliteration rate(84%)with few complications(15%)and rebleeding events(17%).CONCLUSION EUS in liver diseases is a promising technique with the potential to be considered a first-line therapeutic and diagnostic option in selected cases. 展开更多

BACKGROUND Contrast-enhanced ultrasound(CEUS)can be used to diagnose focal liver lesions(FLLs)in children.The America College of Radiology developed the CEUS liver imaging reporting and data system(LI-RADS)for standardizing CEUS diagnosis of FLLs in adult patients.Until now,no similar consensus or guidelines have existed for pediatric patients to improve imaging interpretation as adults.AIM To evaluate the performance of CEUS LI-RADS combined with alpha-fetoprotein(AFP)in differentiating benign and malignant FLLs in pediatric patients.METHODS Between January 2011 and January 2021,patients≤18 years old who underwent CEUS for FLLs were retrospectively evaluated.The following criteria for diagnosing malignancy were proposed:Criterion I considered LR-4,LR-5,or LRM lesions as malignancies;criterion II regarded LR-4,LR-5 or LR-M lesions with simultaneously elevated AFP(≥20 ng/mL)as malignancies;criterion III took LR-4 Lesions with elevated AFP or LR-5 or LR-M lesions as malignancies.The sensitivity,specificity,accuracy and area under the receiver operating characteristic curve(AUC)were calculated to determine the diagnostic value of the aforementioned criteria.RESULTS The study included 63 nodules in 60 patients(mean age,11.0±5.2 years;26 male).There were no statistically significant differences between the specificity,accuracy,or AUC of criterion II and criterion III(95.1%vs 80.5%,84.1%vs 87.3%,and 0.794 vs 0.902;all P>0.017).Notably,criterion III showed a higher diagnostic sensitivity than criterion II(100%vs 63.6%;P<0.017).However,both the specificity and accuracy of criterion I was inferior to those of criterion II and criterion III(all P<0.017).For pediatric patients more than 5 years old,the performance of the three criteria was overall similar when patients were subcategorized by age when compared to all patients in aggregate.CONCLUSION CEUS LI-RADS combined with AFP may be a powerful diagnostic tool in pediatric patients.LR-4 with elevated AFP,LR-5 or LR-M lesions is highly suggestive of malignant tumors. 展开更多

Contrast-enhanced ultrasound(CEUS)represents a great innovation for the evaluation of focal liver lesions(FLLs).The main advantage of CEUS is the realtime imaging examination and the very low toxicity in patients with renal failure.Liver cirrhosis has been recognized as a major risk factor for the onset of hepatocellular carcinoma(HCC)and intrahepatic cholangiocarcinoma(ICC).HCC in liver cirrhosis develops as the last step of a complex that leads to the gradual transformation from regenerative nodule through dysplastic nodule to HCC.In patients with liver cirrhosis,a surveillance program is recommended consisting of ultrasound(US)for detecting small focal lesions.A wide spectrum of benign and malignant lesions other than HCC may be found in the cirrhotic liver and their differentiation is important to avoid errors in staging diseases that may preclude potentially curative therapies.Several published studies have explored the value of CEUS in liver cirrhosis and they have been shown to have excellent diagnostic and prognostic performances for the evaluation of non-invasive and efficient diagnosis of FLLs in patients at high risk for liver malignancies.The purpose of this article is to describe and discuss CEUS imaging findings of FLLs including HCC and ICC,all of which occur in cirrhotic livers with varying prevalence. 展开更多

BACKGROUND: Acoustic radiation force impulse (ARFI) imaging is a new elastography method for the evaluation of tissue stiffness. This study aims to evaluate the performance of ARFI in noninvasive assessment of the tissue stiffness of focal liver lesion (FLL) and to explore its potential value in the differential diagnosis of FLL. METHODS: ARFI was performed in 140 patients with 154 FLLs, which included 28 hemangiomas (ANGIs), 14 focal nodular hyperplasias (FNHs), 61 hepatocellular carcinomas (HCCs), 39 metastases and 12 cholangiocellular carcinomas (CCCs). Virtual touch tissue quantification (VTTQ) values were obtained, analyzed and compared. The area under the receiver operating characteristic curve (AUROC) and optimal cut-off values were obtained using a receiver operating characteristic (ROC) curve analysis to assess diagnostic performance. All cases were definitively diagnosed using histopathology, CT, MRI or contrast-enhanced ultrasound. RESULTS: The VTTQ median values of ANGI, FNH, HCC metastasis and CCC were 1.30, 1.80, 2.52, 3.08 and 3.89 m/s respectively. A significant increase in the VTTQ values of different lesions was observed: ANGI【FNH【HCC【metastasis 【CCC (P【0.001). The AUROC (95% CI) of VTTQ values was 0.94 (0.90-0.98) for ANGI, 0.91 (0.87-0.96) for malignant lesions and 0.87 (0.79-0.94) for CCC. The sensitivity and specificity for ANGI (86.5%, 89.3%, respectively), malignancy (81.3% 92.9%, respectively), and CCC (91.7%, 72.5%, respectively) were associated with VTTQ cut-off values of 1.76, 2.22 and 3.00 m/s respectively.CONCLUSIONS: ARFI can accurately and objectively assess the elasticity of lesions by obtaining the shear wave elastic value of FLL with VTTQ. Therefore, ARFI is a novel, simple, noninvasive and useful diagnostic method for the characterization of FLL. 展开更多

Factors such as the ease of acquisition and ability to obtain functional information in the absence of intravenous contrast, especially in patients with abnormal renal function, have contributed to the growing interest in exploring clinical applications of DWI. In the liver, DWI demonstrates a gamut of clinical applications ranging from detecting focal liver lesions to monitoring response in patients undergoing serial follow-up after loco-regional and systemic therapies. DWI is also being applied in the evaluation of diffuse liver diseases such as non-alcoholic fatty liver disease, hepatic fibrosis and cirrhosis. In this review, we intend to review the basic principles, technique, current clinical applications and future trends of DW-MRI in the liver. 展开更多

BACKGROUND Focal liver lesions(FLLs)are abnormal masses that are distinguishable from the surrounding liver parenchyma,solid or cystic and may be benign or malignant.They are usually detected incidentally on abdominal examinations.The classification of FLLs is very important as it directly determines the diagnosis and treatment of patients.CASE SUMMARY A 46-year-old male patient was admitted into the hospital with tarry stool,during the investigation of this issue an incidental FLL was detected.Upon further investigation of this“incidentaloma”computerized tomography and magnetic resonance imaging reached contradictory conclusions.The lesion was then further investigated using contrast-enhanced ultrasound(CEUS)with an initial diagnosis of idiopathic FLL was acquired and observation of the FLL over time need for final diagnosis,however in the follow up the FLL disappeared spontaneously.CONCLUSION CEUSs value for characterization of FLLs is undeniable,especially when other methods produce inconsistent results,is undeniable but with its limitations.Why and how the FLL disappeared is not known,and can be only hypothesized it was a pseudolesion. 展开更多

Background and Aims:This study aimed to evaluate the diagnostic performance of high frame rate contrast-en-hanced ultrasound(H-CEUS)of focal liver lesions(FLLs).Methods:From July 2017 to June 2019,conventional con-trast-enhanced ultrasound(C-CEUS)and H-CEUS were per-formed in 78 patients with 78 nodules.The characteristics of C-CEUS and H-CEUS in malignant and benign groups and the differences between different lesion sizes(1-3 cm,3-5 cm,or>5 cm)of C-CEUS and H-CEUS were examined.The diagnostic performance of C-CEUS and H-CEUS was ana-lyzed.The chi-square test or Fisher’s exact test was used to assess inter-group differences.The receiver operating characteristic curve was plotted to determine the diagnostic performance of C-CEUS and H-CEUS.Results:There were significant differences in the enhancement area,fill-in direc-tion and vascular architecture between C-CEUS and H-CEUS for both benign and malignant lesions(all p=0.000-0.008),but there were no significant differences in washout results(p=0.566 and p=0.684,respectively).For lesions 1-3 cm in size,the enhancement area,fill-in direction,and vascular architecture on C-CEUS and H-CEUS were significantly dif-ferent(all p=0.000),unlike for lesions 3-5 cm or>5 cm in size.For differentiation of malignant from benign FLLs in the 1-3 cm group,H-CEUS showed sensitivity,specific-ity,accuracy,and positive and negative predictive values of 92.86%,95.0%,96.3%,90.48%and 93.75%,respectively,which were higher than those for C-CEUS(75.0%,70.0%,77.78%,66.67%and 72.91%,respectively).Conclusions:H-CEUS provided more vascular information which could help differentiate malignant from benign FLLs,especially for lesions 1-3 cm in size. 展开更多

With the development of second-generation contrast agents and advancement in contrast harmonic imaging,contrast-enhanced ultrasonography(CEUS)now has the capacity to sensitively and accurately show tumor vascularity.Therefore,marked improvements have been achieved in the diagnosis of focal liver lesions(FLLs),including hepatocellular carcinoma(HCC),by US.In contrast to other agents,Kupffer cells in liver sinusoids take up Sonazoid.Two contrast enhancement phases occur in CEUS with Sonazoid:a vascular phase and Kupffer phase.Images obtained in the Kupffer phase have higher diagnostic sensitivity for hepatic malignancies because the majority of these malignancies do not contain Kupffer cells.Dynamic images obtained in the vascular phase markedly narrow the clinical differential diagnoses of FLLs.The sustainable detection of inconspicuous HCC,adequate guidance of ablation therapy,and accurate assessment of treatment responses in HCC are all facilitated by Sonazoid.The principles,clinical applications,and techniques of CEUS with Sonazoid in the diagnosis of HCC will be reviewed herein. 展开更多
