Humeral shafts non-union either from late presentation after initial treatment by traditional bone setters or failed non-operative orthodox care is a major problem in this part of the world. This non-union is a major treatment challenge with increased cost of care and morbidity in this part of the world. Humeral shaft non-union can be treated with locked intra-medullary nailing (LIMN) or dynamic compression plating (DCP). Study on comparison of these methods of fixation in this part of the world is scarce in literature search, hence the reason for this study. Objective: The objectives of this study are: (1) to compare early clinical outcome following fixation of humeral shaft fracture nonunion with DCP versus LIMN;(2) to compare the time of radiologic fracture union of DCP with LIMN;(3) to compare complications following fixation of humeral shaft fracture nonunion with DCP versus LIMN. Patients and Methods: This was a randomized control study done for 2 years in which fifty adult patients with humeral shaft non-union were recruited. The patients were grouped into 2 (P = DCP & N = LIMN). Forty five of the patients completed the follow up periods of the study and then analyzed. The P group had ORIF with DCP while the N group had ORIF with LIMN. Both groups had grafting with cancellous bones. Each patient was followed up for a period of 6 months at the time which radiographic union is expected. Any patient without clinical and/or radiographic evidence of union after six months of surgery was diagnosed as having recurrent non-union. The data generated was analyzed using SPSS Version 23. The results were presented in charts and tables. The paired t-test was used while considering p value Result: Forty five patients completed follow up. There was a male preponderance (4:1), right humerus predominated (3:2). Motor vehicular accidents were the commonest cause of the fractures (62%). Most non-union fractures occurred at the level of the middle 3rd of the humeral shaft (60%). Failed TBS treatment was the commonest indication for the osteosynthesis (71%). More patients had plating (53%) compared to 47% who had LIMN. Most patients (93.4%) had union between 3 to 6 months irrespective of fixation type with no significant statistical difference between the union rate of DCP and LIMN (p value 0.06) with similar functional outcome and complication rates irrespective of the type of fixation. Conclusion: This study showed that the success rates in term of fracture union, outcome functional grades and complication rates were not directly dependent on the types of the fixation: plating or locked intra-medullary nailing.展开更多

We measured the distance of the distal locking screws to the joint line immediately postoperatively and then measured radial shortening after six to eight weeks using the change in ulnar variance.RESULTS Multivariate linear regression analysis showed that there was a significant linear association between the distance of the screws from the joint line and radial shortening. No other patient, injury, or treatment-related characteristic significantly influenced radial shortening in multivariate analysis.CONCLUSION Distal locking screws should be placed as close as possible to the subchondral joint line to prevent postoperative loss of reduction.展开更多

Cesium levels 6S_(1/2), 6P_(3/2), and the n D_(5/2) state, compose a cascade three-level system, where a coupling laser drives Rydberg transition, and probe laser detects the EIT signal. The error signal, obtained by demodulating the EIT signal, is used to lock the coupling laser frequency to Rydberg transition. The laser frequency fluctuation, ~0.7 MHz, is obtained after locking on, with the minimum Allan variance to be 8.9 × 10^(-11).This kind of locking method can be used to stabilize the laser frequency to the excited transition.展开更多

The results show that the seasonal variation of the zonal wind anomaly over the equatorial Pacific associated with the seasonal variation of the ITCZ is the mechanism of the locking in the model. From January to March of the E1 Nino year, the western wind anomaly over the western equatorial Pacific can excite the downwelling Kelvin wave that propagates eastward to the eastern and middle Pacific by April to June. From April to December of the year before the E1 Nifio year, the eastern wind anomaly over the equatorial Pacific forces the downwelling Rossby waves that modulate the ENSO cycle. The modulation and the reflection at the western boundary modulate the time of the transition from the cool to the warm phase to September of the year before the E1 Nifio year and cause the strongest downwelling Kelvin wave from the reflected Rossby waves at the western boundary to arrive in the middle and eastern equatorial Pacific by April to June of the E1 Nino year. The superposition of these two kinds of downwelling Kelvin waves causes the El Nino event to tend to occur from April to June.展开更多

There also tend to be warm anomalies in the boreal summer or fall during La Nina development years and cold anomalies in winter or spring following the La Nina events. The seasonal phase-locking of SST change in the EIO associated with E1 Nino/Southern Oscillation is linked to the variability of convection over the maritime continent, which induces an atmospheric Rossby wave over the EIO. Local air-sea interaction exerts different effects on SST anomalies, depending on the relationship between the Rossby wave and the mean flow related to the seasonal migration of the buffer zone, which shifts across the equator between summer and winter. The summer cold events start with cooling in the Timor Sea, together with increasing easterly flow along the equator. Negative SST anomalies develop near Sumatra, through the interaction between the atmospheric Rossby wave and the underneath sea surface. These SST anomalies are also contributed to by the increased upwelling of the mixed layer and the equatorward temperature advection in the boreal fall. As the buffer zone shifts across the equator towards boreal winter, the anomalous easterly flow tends to weaken the mean flow near the equator, and the EIO SST increases due to the reduction of latent heat flux from the sea surface. As a result, wintertime SST anomalies appear with a uniform and nearly basin-wide pattern beneath the easterly anomalies. These SST anomalies are also caused by the increase in solar radiation associated with the anticyclonic atmospheric Rossby wave over the EIO. Similarly, the physical processes of the summer warm events, which are followed by wintertime cold SST anomalies, can be explained by the changes in atmospheric and oceanic fields with opposite signs to those anomalies described above.展开更多

Moreover, a locking angle model of the MBWK fabric is established for the first time according to its unique construction. Two kinds of locking angles are considered under different restraint conditions: the locking angle θ1 controlled by the inserting yarns and the locking angle θ2 controlled by the stitch yarns. It is concluded that the ultimate value of the locking angle θ is the larger one of the two angles.展开更多

Flushing with 0.9% sodium chloride becomes an alternative to heparinized solution. As flushing and locking solutions are still controversial, this study was conducted to compare efficacy of heparinized solution versus normal saline solution for locking in ports TIVAP. Patients and Methods: Prospective Cohort study performed in teaching hospital Sulaymaniyah-University of Kurdistan, Iraq, including 384 TIVAP implanted in cancer and non-cancer patients. The study reports the TIVAP outcome in 2 groups of patients where 2 different solutions used for maintaining catheter’s patency by heparinized solution in group (A), versus normal saline for group (B). Results: In group A, the rate of complications was 8.2% (n = 16) while in group B complications rate was 7.9% (n = 15). Thrombosis in group A occurred in 1.03% of the cases and in group B was 1.57%. There were no significant differences between the two groups regarding the causes for unwanted removals of the TIVAP. Conclusions: The results of our study suggest that heparin has no role in preventing the early or late complications of TIVAP and we do not recommend using it as a locking solution.展开更多

文摘Coherent beam combining(CBC)is an efficient way to scale the brightness of laser arrays. We demonstrate the active phase locking CBC of two slab laser amplifiers based on the multi-dithering technique. The experimental investigation on the 102 W coherent beam combining of two slab amplifiers shows that the whole system in a closed loop performs well over a long time observation. The contrast of the coherent combined beam profile is about 87%and the combining efficiency is nearly 85%.In addition, the CBC of two green lasers is realized based on the second-harmonic generation of the phase locking pump lasers. To the best of our knowledge, this is the first report about second-harmonic active phase locking, which indicates further potential applications of CBC.
文摘AIM To determine factors correlated with postoperative radial shortening in patients with distal radius fractures treated with volar locking distal radius plates.METHODS A total of 250 patients with a distal radius fracture stabilised with volar locking plates between January 2010 and December 2014 were included in a multicentre retrospective cohort study. No other patient, injury, or treatment-related characteristic significantly influenced radial shortening in multivariate analysis.CONCLUSION Distal locking screws should be placed as close as possible to the subchondral joint line to prevent postoperative loss of reduction.

文摘We present a laser frequency locking to Rydberg transition with electromagnetically induced transparency(EIT)spectra in a room-temperature cesium vapor cell. The laser frequency fluctuation, ~0.7 MHz, is obtained after locking on, with the minimum Allan variance to be 8.9 × 10^(-11).This kind of locking method can be used to stabilize the laser frequency to the excited transition.

文摘65 km Semangko Fault is part of southern segments of Sumatran Fault Zone(SFZ)which is complex corresponds to the transition from the strike-slip regime of the SFZ to the normal faulting tectonics of the Sunda Strait. The recent publication showed branches of Semangko Fault:West Semangko Fault(WSF)and East Semangko Fault(ESF).This study estimated geodetic slip rate and locking depth of ESF using Global Positioning System(GPS)time series data from 2007 to 2019 from all available GPS sites.GPS velocities refer to Sundaland Plate were used to estimate the fault parameters of the WSF and ESF simultaneously.Non-uniformity of velocity direction shows the complexities of Semangko Fault possibly caused by the contribution of all faults around ESF.An ESF geodetic slip rate, which is 12.5±2 mm/yr was lower than WSF,which is 16.5±2 mm/yr.It is consistent with the rigid block nature of the SFZ system as northern segment slip rates have similar value.Small slip rates are possibly leading to lower generated seismic moment than the major segment of SFZ.

文摘The mechanism of the locking of the E1 Nino event onset phase to boreal spring (from April to June) in an intermediate coupled ocean-atmosphere model is investigated. The superposition of these two kinds of downwelling Kelvin waves causes the El Nino event to tend to occur from April to June.
文摘The sea surface temperature (SST) anomaly of the eastern Indian Ocean (EIO) exhibits cold anomalies in the boreal summer or fall during E1 Nino development years and warm anomalies in winter or spring following the E1 Nino events. Similarly, the physical processes of the summer warm events, which are followed by wintertime cold SST anomalies, can be explained by the changes in atmospheric and oceanic fields with opposite signs to those anomalies described above.
文摘This paper introduces the construction of the multi-layered biaxial weft knitted fabric (MBWK fabric) and studies the locking angle of this kind of fabric. Moreover, a locking angle model of the MBWK fabric is established for the first time according to its unique construction. Two kinds of locking angles are considered under different restraint conditions: the locking angle θ1 controlled by the inserting yarns and the locking angle θ2 controlled by the stitch yarns. It is concluded that the ultimate value of the locking angle θ is the larger one of the two angles.
文摘Introduction: Totally implantable venous access port (TIVAP) is essential prerequisite for most of chemotherapy protocols. Conclusions: The results of our study suggest that heparin has no role in preventing the early or late complications of TIVAP and we do not recommend using it as a locking solution.
