This editorial comments on an article published in a recent issue of World Journal of Gastroenterology, entitled“Association of low muscle strength with metabolic dysfunction-associated fatty liver disease:A nationwide study”.We focused on the association between muscle strength and the incidence of non-alcoholic fatty liver disease(NAFLD)and metabolic-associated fatty liver disease(MAFLD),as well as the mechanisms underlying the correlation and related clinical applications.NAFLD,which is now redefined as MAFLD,is one of the most common chronic liver diseases globally with an increasing prevalence and is characterized by malnutrition, which may contribute to decreased muscle strength.Reduction of muscle strength reportedly has a pathogenesis similar to that of NAFLD/MAFLD,including insulin resistance, inflammation, sedentary behavior,as well as insufficient vitamin D.Multiple studies have focused on the relationship between sarcopenia or muscle strength and NAFLD.However, studies investigating the relationship between muscle strength and MAFLD are limited.Owing to the shortage of specific medications for NAFLD/MAFLD treatment, early detection is essential.Furthermore, the relationship between muscle strength and NAFLD/MAFLD suggests that improvements in muscle strength may have an impact on disease prevention and may provide novel insights into treatments including dietary therapy,as well as tailored physical activity. 展开更多

The diagnosis of non-alcoholic fatty liver disease(NAFLD)and metabolic dysfunction-associated fatty liver disease only on the basis of laboratory parameter score such as Hepatic Steatosis Index which includes liver enzymes, gender, basal metabolic index, and presence of diabetic mellitus is not sufficient to exclude other causes of deranged liver enzymes especially medications and autoimmune related liver diseases. As the guideline suggests ultrasound is the preferred first-line diagnostic procedure for imaging of NAFLD,as it provides additional diagnostic information and the combination of biomarkers/scores and transient elastography might confer additional diagnostic accuracy and evident from previous similar studies too. 展开更多

Background:Technological advancements in modern military and acrobatic jet planes have resulted in extraordinary psychophysiological loads being exerted upon flying personnel, including inducing neck and back pain. The purpose of this study was to examine the effects of 12 weeks of functional strength training on 1)the volume and strength of the neck and shoulder muscles and 2)muscular activity upon exposure to helmets of different masses and elevated Gz forces in a long-arm centrifuge in high-performance aircraft personnel. Methods:Eighteen participants underwent 12 weeks of functional strength training(n=12)or the control protocol(n=6)without additional strength training.Pre-and post-intervention tests included evaluations of isometric strength of the head extensor muscles, flexion, and lateral flexion and rotation,as well as magnetic resonance imaging(MRI)to measure the volume of the m.sternocleidomastoideus,m.trapezius,and deep neck muscles.Furthermore, during a long-arm centrifuge(+1.4 and+3Gz)protocol,the muscular activity levels of the m.sternocleidomastoideus,m.trapezius and m.erector spinae muscles were assessed without a flight helmet, with a helmet, and with a helmet and night vision goggles. Each participant’s perception of muscular strain was noted immediately after the long-arm centrifuge protocol. Results:The maximal isometric strength in all exercises and muscle volumes increased in the training group but not the control group(P<0.05).Relative muscle activity(%MVC)with a helmet decreased after the intervention in the training but not the control group(P=0.01).Relative muscle activity while wearing a helmet and night vision goggles was higher after intervention in the control group than in the training group(P<0.01).The perceived muscular strain of the neck muscles induced by the long-arm centrifuge did not differ between the groups. Conclusions:Twelve weeks of functional strength training improves the maximal isometric strength and volume of neck and shoulder muscles and leads to lower relative muscle activation upon exposure to elevated Gz forces in a long-arm centrifuge. 展开更多

BACKGROUND There is limited evidence regarding the association between muscle strength and metabolic dysfunction-associated fatty liver disease(MAFLD).AIM To investigate the association between muscle strength and MAFLD in the general population in Korea.METHODS This nationwide representative cross-sectional study included 31649 individuals aged≥19 years who participated in the Korea National Health and Nutrition Examination Survey between 2015 and 2018.Odds ratios(ORs)and 95%confidence intervals(95%CIs)for MAFLD according to sex-specific quartiles of muscle strength, defined by relative handgrip strength, were calculated using multivariable logistic regression analysis.Additionally, multivariable logistic regression analysis was used to assess the association between muscle strength and probable liver fibrosis in patients with MAFLD.RESULTS Of all the participants,29.3%had MAFLD.The prevalence of MAFLD was significantly higher in the lower muscle strength quartile groups for all participants, sexes, and age groups(P<0.001).A 1.92-fold(OR=1.92,95%CI:1.70–2.16)and 3.12-fold(OR=3.12,95%CI:2.64–3.69)higher risk of MAFLD was observed in the lowest quartile(Q1)group than in the other groups(Q2–Q4)and the highest quartile(Q4)group,respectively.The ORs of MAFLD were significantly increased in the lower muscle strength quartile groups in a dose-dependent manner(P for trend<0.001).These associations persisted in both sexes. An inverse association between muscle strength and the risk of MAFLD was observed in all subgroups according to age, obesity, and diabetes mellitus. In patients with MAFLD,the odds of severe liver fibrosis were higher in Q1(OR=1.83,95%CI:1.25–2.69)than in other groups(Q2–Q4).CONCLUSION Among Korean adults, low muscle strength was associated with an increased risk of MAFLD and liver fibrosis in patients with MAFLD. 展开更多

Background:This prospective study explored the effects of endurance running(ER) in minimal versus standard running shoes on the foot’s superficial layer intrinsic muscles and the function of the longitudinal arch. Our hypothesis was that running in minimal shoes would cause hypertrophy in these muscles and lead to higher, stronger, stiffer arches. Methods:The hypothesis was tested using a sample of 33 healthy runners randomized into two groups,a control group shod in traditional running footwear and an experimental group shod in minimal support footwear, whose feet were scanned in an MRI before and after a 12-week training regime.Running kinematics as well as arch stiffness and height were also assessed before and after the treatment period. Results:Analysis of anatomical cross-sectional areas and muscle volumes indicate that the flexor digitomm brevis muscle became larger in both groups by 11%and 21%,respectively,but only the minimally shod runners had significant areal and volumetric increases of the abductor digiti minimi of 18%and 22%,respectively,and significantly increased longitudinal arch stiffness(60%).Conclusion:These results suggest that endurance running in minimal support footwear with 4 mm offset or less makes greater use of the springlike function of the longitudinal arch, thus leading to greater demands on the intrinsic muscles that support the arch, thereby strengthening the foot. 展开更多

AIM:To investigate the prevalence of chronic dyspnea and its relationship to respiratory muscle function in end-stage liver disease.METHODS:Sixty-eight consecutive, ambulatory,Caucasian patients with end-stage liver disease, candidates for liver transplantation, were referred for preoperative respiratory function assessment.Forty of these(29 men) were included in this preliminary study after applying strict inclusion and exclusion criteria.Seventeen of 40 patients(42%) had ascites, but none of them was cachectic.Fifteen of 40 patients(38%)had a history of hepatic encephalopathy, though none of them was symptomatic at study time. All patients with a known history and/or presence of co-morbidities were excluded.Chronic dyspnea was rated according to the modified medical research council(mMRC) 6-point scale.Liver disease severity was assessed according to the Model for end-stage liver disease(MELD).Routine lung function tests, maximum static expiratory(Pemax) and inspiratory(Pimax) mouth pressures were measured.Respiratory muscle strength(RMS) was calculated from Pimax and Pemax values. In addition, arterial blood gases and pattern of breathing(VE:minute ventilation;VT:tidal volume;VT/TI:mean inspiratory flow;TI:duration of inspiration) were measured.RESULTS:Thirty-five(88%) of 40 patients aged(mean ± SD) 52 ± 10 years reported various degrees of chronic dyspnea(mMRC),ranging from 0 to 4,with a mean value of 2.0 ± 1.2.MELD score was 14 ± 6.Pemax,percent of predicted(%pred) was 105 ± 35,Pimax,%pred was 90 ± 29,and RMS,%pred was 97 ± 30.These pressures were below the normal limits in 12(30%),15(38%),and 14(35%) patients, respectively.Furthermore, comparing the subgroups of ascites to non-ascites patients, all respiratory muscle indices measured were found significantly decreased in ascites patients.Patients with ascites also had a significantly worse MELD score compared to non-ascites ones(P = 0.006).Significant correlations were found between chronic dyspnea and respiratory muscle function indices in all patients.Specifically,mMRC score was significantly correlated with Pemax,Pimax, and RMS(r =-0.53,P < 0.001;r =-0.42,P < 0.01;r =-0.51,P < 0.001,respectively).These correlations were substantially closer in the non-ascites subgroup(r =-0.82,P < 0.0001;r =-0.61,P < 0.01;r =-0.79,P < 0.0001,respectively) compared to all patients.Similar results were found for the relationship between mMRC vs MELD score, and MELD score vs respiratory muscle strength indices. In all patients the sole predictor of mMRC score was RMS(r =-0.51,P < 0.001).In the subgroup of patients without ascites this relationship becomes closer(r =-0.79,P < 0.001),whilst this relationship breaks down in the subgroup of patients with ascites. The disappearance of such a correlation may be due to the fact that ascites acts as a "confounding" factor.PaCO2(4.4 ± 0.5 kPa) was increased, whereas pH(7.49 ± 0.04) was decreased in 26(65%) and 34(85%) patients,respectively.PaO2(12.3 ± 0.04 kPa) was within normal limits.VE(11.5 ± 3.5 L/min),VT(0.735 ± 0.287 L),and VT/TI(0.449±0.129 L/s) were increased signifying hyperventilation in both subgroups of patients.VT/TI was significantly higher in patients with ascites than without ascites.Significant correlations, albeit weak, were found for PaCO2 with VE and VT/TI(r =-0.44,P < 0.01;r =-0.41,P < 0.01,respectively).CONCLUSION:The prevalence of chronic dyspnea is 88% in end-stage liver disease. The mMRC score closely correlates with respiratory muscle strength. 展开更多

BACKGROUND Shoulder is the most injured part in table tennis players, and it takes multiple roles in transmitting power and striking the center of the ball during the stroke.Proprioception is strongly correlated with high level of athletic performance. It is customary to assume that there is a correlation between proprioception and muscle strength and therefore proprioceptive assessment and rehabilitation is often neglected.AIM To investigate the correlation between isokinetic muscle strength and proprioception in the internal and external rotation muscle groups of elite Chinese male table tennis players,to provide reference for physical training and rehabilitation of elite table tennis players.METHODS A total of 19 national elite table tennis players from the Chinese National Table Tennis Team were recruited in this research. All of them had more than 10 years training experience and had participated major competitions such as the National Games and World Youth Championships.IsoMed 2000 was used to test the peak torque of internal and external rotation isokinetic concentric contraction of the athletes'bilateral shoulder joints at low speed(60°/s)and high speed(180°/s)respectively;IsoMed 2000 was used to conduct the Joint Position Reproduction test to evaluate the athletes'proprioceptive ability capacity at low speed(60°/s)and high speed(180°/s)respectively.If the data satisfied the normal distribution, the correlation between the differences in peak torque s and angles in different directions was analyzed using a Pearson simple linear model;otherwise,Spearman correlation analysis was used. The comparison of proprioceptive ability between the table tennis racket-holding hand and non-racket-holding hands was performed using independent samples t-test if the data satisfied a normal distribution;otherwise, the Mann-Whitney U test was used.RESULTS There was no direct linear correlation between the strength and proprioceptive correlation analysis at slow speed(60°/s)and fast speed(180°/s)in the racket-holding hand;At the slow speed(60°/s)and fast speed(180°/s),there was no correlation between muscle strength and proprioception in the non-racket-holding hand except for the internal rotation variable error(VE)and external rotation relative peak torque, which showed a moderate positive correlation(r=0.477,P<0.05),(r=0.554,P<0.05).The internal rotation’s constant error(CE)and VE were 1.06±3.99 and 2.94±2.16,respectively,for the racket-holding hand,and-3.36±2.39 and 1.22±0.93,respectively,for the non-racket-holding hand;the internal rotation’s CE,VE of the racket-holding hand was lower than that of the non-racket-holding hand, and there was a highly significant difference(P<0.01).CONCLUSION There was no correlation between muscle strength and proprioceptive function in the internal and external rotation of the racket-holding hand’s shoulder in elite Chinese male table tennis players. These results may be useful for interventions for shoulder injuries and for the inclusion of proprioceptive training in rehabilitation programs. 展开更多

OBJECTIVE To assess the effects of polyprenols(isolated from Picea abies L.spruce needles)on muscle strength/tone and coordination, and to investigate whether polyprenols may protect atorvastatin-mediated muscle strength/tone weakness in female Wistar rats.METHODS Polyprenols at doses of 1,10 and 20mg·kg-1,atorvastatin 80mg·kg-1 or their combination were administered once daily per os for 16 consecutive days in Wistar female rats(n=9-10 per group).Assessment of muscle strength was performed by grip strength test(on day 15)and wire hang test(on day 16).Rotarod test was used to measured locomotor coordination and muscle tone(on day 16).General locomotor activity was evaluated in open field test(on day 15).Assessment of plasma cholesterol level and creatine kinase activity was done on day 17.RESULTS Atorvastatin-treated rats exhibited a marked decrease in grasping strength and hanging time.PP(20mg·kg-1)significantly protected against atorvastatin-induced muscle weakness in grip strength test, and restored it to control values. At all doses, polyprenols prolonged hanging time which was decreased by atorvastatin in wire hang test.Polyprenols per se at 1 and 10mg·kg-1 did not show difference compared to control group animals, while only at 20mg·kg-1 h anging time was prolonged vs.control in wire hang test.No changes between control and tested groups were observed in rotarod and open field tests.Blood cholesterol level was not changed in any of tested groups in female Wistar rats.Polyprenols(20mg·kg-1)significantly(by 25%)increased plasma creatine kinase activity but it was not affected by the combined treatment.CONCLUSION Since polyprenols acted as protectors of atorvastatin-induced muscle weakness, the combination of polyprenols with atorvastatin may be helpful for reducing muscle-related side effects in patients receiving a long-term atorvastatin therapy. 展开更多

Maximum Voluntary Ventilation (MVV), one of the components of Pulmonary Function Testing (PFT), has multiple uses. Various factors including the inspiratory muscle strength (IMS) influence its magnitude. Our aim was to quantify the IMS indirectly using an economical and non invasive bedside assessment tool, determine its association with MVV and then develop a predictive equation for MVV. 41 healthy non-athletic physical therapy students participated in the study. IMS measurement was performed with a sphygmomanometer. Average of the three net deflections in sphygmomanometer following deepest possible breaths was taken as indirect measurement of IMS in mm of Hg. MVV was measured according to ATS guidelines using a spirometer. Results from the data analysis revealed a significant correlation between IMS and MVV(r = 0.83, p < 0.001) and the coefficient of determination = 0.68. So, we developed a regression equation: Y = 1.9669(X) + 49.838 with SEE: 13.02L/min and ANOVA for the equation was (F=68.9, p < 0.001). Hence, it can be concluded that a strong correlation between the indirect IMS and MVV was established and a predictive equation to estimate MVV was developed. This equation proved to have a high predictive value with a small error of estimation. This indicates that the value of the indirect IMS measurement obtained using the sphygmomanometer can be used to estimate MVV in normal healthy individuals without the use of a conventional spirometer. 展开更多

Background: The lower extremity muscle strength has been shown to be weaker in patients with diabetes mellitus, primarily due to the effects of diabetic polyneuropathy, a common diabetic complication. With respect to assessments of the lower extremity muscle strength, reference values for the knee extension force in healthy Japanese subjects have been reported. The clinical significance of applying knee extension force in the assessment of efficacy of exercise therapy and patient education has also been demonstrated, and the use of an individualized approach in physical exercise based on the quantitative assessment of the lower extremity muscle strength is becoming one of the most important strategies for promoting effective rehabilitation. Methods: The study is a multicenter, cross-sectional investigation. We will measure the knee extension force and evaluate the diabetic polyneuropathy in patients with type 2 diabetes using uniform equipment and methodology at all centers. We will compare the knee extension force between the subjects with and without diabetic polyneuropathy and will investigate the associations between the knee extension force and both glycemic control and exercise habits. The trial registration is UMIN CTR (UMIN000002810). Discussion: The lower extremity muscle strength has been insufficiently investigated in Japanese patients with diabetes. This study was designed to establish reference values for the knee extension force in Japanese diabetic patients with and without diabetic polyneuropathy. 展开更多

A total of 30 elderly and 45 young women aged ≥ 65 and 19 - 22 years, respectively, participated in this study. The participants underwent body composition measurement via bioelectrical impedance analysis and examinations of handgrip and leg muscle strength. The age-related body composition changes varied among age groups. Compared with young women, fat-free mass (FFM) in the yE group did not decrease significantly;however, fat mass (FM) and waist-hip ratio (WHR) were significantly greater. Compared with the yE group, decreases in FFM, FM, and WHR in the oE group were significant;furthermore, the decrease in FM measurements was attributed to the loss of FM in the trunk and limbs (upper and lower). The measurement results suggested that the greater FM decrease in the oE group was characterized by decreases in both visceral and subcutaneous fat. In the yE group, the muscle mass was comparable to that in young women;however, there was a remarkable reduction in the lower-limb muscle mass (9% - 10%). In the oE group, muscle mass was reduced in all body parts, including upper and lower limbs and trunk. In young women, significant positive correlations between muscle mass and muscle strength in the upper and lower limbs were observed. No such correlations in the lower limbs were found in elderly women, indicating that muscle mass is not proportionally reflected in muscle strength. In conclusion, for improving and maintaining the health of elderly women, especially those above the age of 75 years, it is important to maintain muscle mass, including muscle strength, and prevent the loss of muscle quality. 展开更多

Isokinetic and isometric force measurements involving a dynamometer setup are widely used in training and in muscle assessment. For further understanding of the method, we investigated the activation of key functional muscles during isokinetic and isometric movements. During force measurements in an isokinetic Biodex System 3 ProTM, acoustic myography (AMG) was applied. Ten healthy subjects (5 men/5 women) in each decade from 20 to 69 years of age participated in the study. Measurements were carried out during extension and flexion of the ankle, knee and elbow joints. Muscle fibre use was measured by efficiency (E-score) and fibre recruitment (temporal (T-score) and spatial (S-score) summation). AMG measurements showed good reproducibility, and the recruitment pattern of muscle fibres did not change with gender or age. Overall, a significantly higher E-score (P < 0.05) was found at the lower angular velocities than at the higher ones, indicating a lower level of muscle efficiency at higher velocities. Muscles used for knee movement exhibited higher scores than muscles associated with the ankle and elbow joints, most likely related to the greater degree of force production at this joint compared to the ankle and elbow. The ability to activate and inactivate muscle fibres during periods of isokinetic activity becomes increasingly more difficult as the velocity increases. When assessing training effects in sports or rehabilitation, AMG in parallel with isokinetic measurements adds important additional information by giving a measure of possible improvements in efficiency and fibre use. 展开更多

Objective: The purpose of this study was to clarify the relationship between respiratory muscle strength and skeletal muscle mass (trunk, upper limbs, and lower limbs) in older people who were certified as requiring nursing or supportive care. Methods: Thirty-five older people (65 years or older) who were certified as requiring nursing care or support were included in the study. The subjects were divided into a non-sarcopenic group (n = 12) and a sarcopenic group (n = 23) according to the sarcopenia diagnostic criteria proposed by the Asian Working Group for Sarcopenia. Maximum inspiratory pressure, maximum expiratory pressure, skeletal muscle mass (trunk, upper and lower limbs), and hand grip strength were measured. Pearson’s correlation coefficient and multiple regression analysis were used for statistical processing. Results: In the non-sarcopenic group, both expiratory muscle strength and hand grip strength were correlated with skeletal muscle mass. In the sarcopenia group, expiratory muscle strength was not correlated with skeletal muscle mass, and only hand grip strength was correlated with upper limb muscle mass. Multiple regression analysis revealed that, in the non-sarcopenic group, trunk muscle mass was the primary factor in expiratory muscle strength and upper limb muscle mass was the primary factor in hand grip strength. In the sarcopenia group, upper limb muscle mass was found to be the main factor in hand grip strength. Conclusion: Our results highlight the importance of assessing expiratory muscle strength and trunk muscle mass before sarcopenia develops in older people who require support and nursing care. 展开更多

Aims: Diabetes Mellitus (DM) is a metabolic disorder which affects whole systems of human body. This study aimed to compare the strength of foot muscles, dynamic balance, and peak plantar pressure between diabetic patients before developing polyneuropathy and healthy peers. Methods: 21 people, 11 diabetic patients and 10 age-matched healthy peers, were included in the study. A manual muscle tester (model 01163 Lafayette) was used to assess muscle strength. Pedobarography was the device to determine the distribution of plantar pressure into nine regions of foot. Dynamic balance was also measured by using a mobile platform (Techno-body, PK 200 WL, Italy). Results: Diabetic and control groups had similar muscle strength and dynamic balance (p > 0.05). Most of the plantar pressure findings were also similar (p > 0.05). There were significant differences in only two regions of foot between two groups (p < 0.05). Conclusion: Diabetes Mellitus is not a factor influencing balance and muscle strength before polyneuropathy. However, it is possible to state that it may negatively affect the distribution of plantar pressure so clinians should assess and treat this distribution in the patients with DM. 展开更多
