Update and latest advances in mechanisms and management of colitis-associated colorectal cancer

Colitis-associated colorectal cancer(CAC)is defined as a specific cluster of colorectal cancers that develop as a result of prolonged colitis in patients with inflammatory bowel disease(IBD).Patients with IBD,including ulcerative colitis and Crohn’s disease,are known to have an increased risk of developing CAC.Although the incidence of CAC has significantly decreased over the past few decades,individuals with CAC have increased mortality compared to individuals with sporadic colorectal cancer,and the incidence of CAC increases with duration.Chronic inflammation is generally recognized as a major contributor to the pathogenesis of CAC.CAC has been shown to progress from colitis to dysplasia and finally to carcinoma.Accumulating evidence suggests that multiple immune-mediated pathways,DNA damage pathways,and pathogens are involved in the pathogenesis of CAC.Over the past decade,there has been an increasing effort to develop clinical approaches that could help improve outcomes for CAC patients.Colonoscopic surveillance plays an important role in reducing the risk of advanced and interval cancers.It is generally recommended that CAC patients undergo endoscopic removal or colectomy.This review summarizes the current understanding of CAC,particularly its epidemiology,mechanisms,and management.It focuses on the mechanisms that contribute to the development of CAC,covering advances in genomics,immunology,and the microbiome;presents evidence for management strategies,including endoscopy and colectomy;and discusses new strategies to interfere with the process and development of CAC.These scientific findings will pave the way for the management of CAC in the near future.

Management System Activities of the Operation and Utilization of GHARR-1

The Ghana Research Reactor-1 (GHARR-1) is a 34 kW low enriched uranium (LEU) Miniature Neutron Source Reactor (MNSR), tank-in-pool type and cooled by natural circulation under atmospheric pressure operating conditions. GHARR-1 is owned by Ghana Atomic Energy Commission (GAEC) and operated by National Nuclear Research Institute (NNRI), one of the institutes of GAEC. GHARR-1 is housed by Nuclear Reactors Research Centre (NRRC), one of the Centres of NNRI. Management/Administration, Radiation protection, Reactor operation and maintenance, Reactor utilization and Physical protection are the various systems/units that integrate to manage the activities of operation and utilization of GHARR-1 in addition to the quality assurance and quality control management system of the research reactor facility. The GHARR-1 which is currently in operation follows a robust maintenance culture adopted by the management system and this has made it possible to keep the reactor in operation with minimal interruption. The management system activities adopted at the Centre to ensure safety of the workers, public and the research reactor facility include authorization of the operation of the reactor for any experiments/modifications;providing material and financial resources for maintaining the research reactor facility;following standard procedures while carrying out Neutron Activation Analysis;participation in IAEA proficiency test;irradiation sites/positions characterization;following standard procedures while carrying out reactor operation and maintenance including reactor and pool water purification and other related activities;monitoring radiation levels in the controlled, supervised and uncontrolled areas of the research reactor facility as well as during reactor operation and maintenance;controlling the physical entry of the workers and public into the research reactor facility;and ensuring that the security structures provided to protect the reactor facility are functioning properly. The thorough knowledge on the functions of the various components that make up the electrical/electronic and control systems of the reactor has been observed to be important for continuous successful maintenance of the research reactor to keep the reactor in operation. This work provides some management system activities adopted to monitor the activities of the research reactor operation and utilization to guarantee safety of workers, public and the environment as well as to safeguard a continuous operation of the research reactor. These management system activities adopted among others, are in the form of Monitoring Forms provided for monitoring the activities of the research reactor operation and utilization in order to ensure standard procedures and specifications are followed and quality services are rendered to the public.

Application of Specialized Group Management in the Quality Control of Perioperative Nursing

Objective:To explore the role of specialized group management in the quality control of perioperative nursing.Methods:45 surgical nurses from our hospital were selected as the research subjects.Traditional operating room management was adopted from July 2019 to June 2020,and specialized group management was adopted from July 2020 to June 2021.The surgeon’s satisfaction,surgical nurses’core professional competence,and surgical patients’satisfaction were obtained through surveys and the results were analyzed.Results:Surgeon satisfaction before the implementation of specialized group management was significantly lower than after its implementation(P<0.05).Besides,surgical nurses’core professional competency scores before the implementation of specialized group management were significantly lower than after its implementation(P<0.05).Lastly,surgical patients’satisfaction before the implementation of specialized group management was significantly lower than after its implementation(P<0.05).Conclusion:Specialized group management helps to improve the quality of perioperative care and should be applied in clinical practice.

For Mega-Constellations: Edge Computing and Safety Management Based on Blockchain Technology

In mega-constellation Communication Systems, efficient routing algorithms and data transmission technologies are employed to ensure fast and reliable data transfer. However, the limited computational resources of satellites necessitate the use of edge computing to enhance secure communication.While edge computing reduces the burden on cloud computing, it introduces security and reliability challenges in open satellite communication channels. To address these challenges, we propose a blockchain architecture specifically designed for edge computing in mega-constellation communication systems. This architecture narrows down the consensus scope of the blockchain to meet the requirements of edge computing while ensuring comprehensive log storage across the network. Additionally, we introduce a reputation management mechanism for nodes within the blockchain, evaluating their trustworthiness, workload, and efficiency. Nodes with higher reputation scores are selected to participate in tasks and are appropriately incentivized. Simulation results demonstrate that our approach achieves a task result reliability of 95% while improving computational speed.

Multiscale Characteristics and Connection Mechanisms of Attraction Networks:A Trajectory Data Mining Approach Leveraging Geotagged Data

Urban tourism is considered a complex system,and multiscale exploration of the organizational patterns of attraction networks has become a topical issue in urban tourism,so exploring the multiscale characteristics and connection mechanisms of attraction networks is important for understanding the linkages between attractions and even the future destination planning.This paper uses geotagging data to compare the links between attractions in Beijing,China during four different periods:the pre-Olympic period(2004–2007),the Olympic Games and subsequent‘heat period’(2008–2013),the post-Olympic period(2014–2019),and the COVID-19(Corona Virus Disease 2019)pandemic period(2020–2021).The aim is to better understand the evolution and patterns of attraction networks at different scales in Beijing and to provide insights for tourism planning in the destination.The results show that the macro,meso-,and microscales network characteristics of attraction networks have inherent logical relationships that can explain the commonalities and differences in the development process of tourism networks.The macroscale attraction network degree Matthew effect is significant in the four different periods and exhibits a morphological monocentric structure,suggesting that new entrants are more likely to be associated with attractions that already have high value.The mesoscale links attractions according to the common purpose of tourists,and the results of the community segmentation of the attraction networks in the four different periods suggest that the functional polycentric structure describes their clustering effect,and the weak links between clusters result from attractions bound by incomplete information and distance,and the functional polycentric structure with a generally more efficient network of clusters.The pattern structure at the microscale reveals the topological transformation relationship of the regional collaboration pattern,and the attraction network structure in the four different periods has a very similar importance profile structure suggesting that the attraction network has the same construction rules and evolution mechanism,which aids in understanding the attraction network pattern at both macro and micro scales.Important approaches and practical implications for planners and managers are presented.

Impact Damage Testing Study of Shanxi-Beijing Natural Gas Pipeline Based on Decision Tree Rotary Tiller Operation

The North China Plain and the agricultural region are crossed by the Shanxi-Beijing natural gas pipeline.Resi-dents in the area use rototillers for planting and harvesting;however,the depth of the rototillers into the ground is greater than the depth of the pipeline,posing a significant threat to the safe operation of the pipeline.Therefore,it is of great significance to study the dynamic response of rotary tillers impacting pipelines to ensure the safe opera-tion of pipelines.This article focuses on the Shanxi-Beijing natural gas pipeline,utilizingfinite element simulation software to establish afinite element model for the interaction among the machinery,pipeline,and soil,and ana-lyzing the dynamic response of the pipeline.At the same time,a decision tree model is introduced to classify the damage of pipelines under different working conditions,and the boundary value and importance of each influen-cing factor on pipeline damage are derived.Considering the actual conditions in the hemp yam planting area,targeted management measures have been proposed to ensure the operational safety of the Shanxi-Beijing natural gas pipeline in this region.

Effects of personalized oral hygiene management on oral health status of pregnant women

BACKGROUND The Cariostat caries activity test(CAT)was used to evaluate the effectiveness of personalized oral hygiene management combining oral health education and professional mechanical tooth cleaning on the oral health status of pregnant women.AIM To investigate whether personalized oral hygiene management enhances the oral health status of pregnant women.METHODS A total of 114 pregnant women who were examined at Dalian Women’s and Children’s Medical Center were divided into four groups:High-risk experimental group(n=29;CAT score≥2;received personalized oral hygiene management training),low-risk experimental group(n=29;CAT score≤1;received oral health education),high-risk control group(n=28;CAT score≥2),and low-risk control group(n=28;CAT score≤1).No hygiene intervention was provided to control groups.CAT scores at different times were compared using independent samples t-test and least significant difference t-test.RESULTS No significant difference in baseline CAT scores was observed between the experimental and control groups,either in the high-risk or low-risk groups.CAT scores were reduced significantly after 3(1.74±0.47 vs 2.50±0.38,P<0.0001)and 6 months(0.53±0.50 vs 2.45±0.42,P<0.0001)of personalized oral hygiene management intervention but not after oral health education alone(0.43±0.39 vs 0.46±0.33,P>0.05 and 0.45±0.36 vs 0.57±0.32,P>0.05,respectively).Within groups,the decrease in CAT scores was significant(2.43±0.44 vs 1.74±0.47 vs 0.53±0.50,P<0.0001)for only the high-risk experimental group.CONCLUSION Personalized oral hygiene management is effective in improving the oral health of pregnant women and can improve pregnancy outcomes and the oral health of the general population.
