Intelligence refers to learning, adap- tation,...We advance here a novel methodology for robust intelligent biometric information management with inferences and predictions made using randomness and complexity concepts. Intelligence refers to learning, adap- tation, and functionality, and robustness refers to the ability to handle incomplete and/or corrupt adversarial information, on one side, and image and or device variability, on the other side. The proposed methodology is model-free and non-parametric. It draws support from discriminative methods using likelihood ratios to link at the conceptual level biometrics and forensics. It further links, at the modeling and implementation level, the Bayesian framework, statistical learning theory (SLT) using transduction and semi-supervised lea- rning, and Information Theory (IY) using mutual information. The key concepts supporting the proposed methodology are a) local estimation to facilitate learning and prediction using both labeled and unlabeled data;b) similarity metrics using regularity of patterns, randomness deficiency, and Kolmogorov complexity (similar to MDL) using strangeness/typicality and ranking p-values;and c) the Cover – Hart theorem on the asymptotical performance of k-nearest neighbors approaching the optimal Bayes error. Several topics on biometric inference and prediction related to 1) multi-level and multi-layer data fusion including quality and multi-modal biometrics;2) score normalization and revision theory;3) face selection and tracking;and 4) identity management, are described here using an integrated approach that includes transduction and boosting for ranking and sequential fusion/aggregation, respectively, on one side, and active learning and change/ outlier/intrusion detection realized using information gain and martingale, respectively, on the other side. The methodology proposed can be mapped to additional types of information beyond biometrics.展开更多
The imminent danger of the Covid-19 pandemic has accelerated research in pharmaceuticals that either target the viral Spike proteins fusion with ACE2 receptors,or the infectious RNA replication that often overwhelms i...The imminent danger of the Covid-19 pandemic has accelerated research in pharmaceuticals that either target the viral Spike proteins fusion with ACE2 receptors,or the infectious RNA replication that often overwhelms immune defences.The scope of this review was to elucidate the main human vulnerabilities to Covid-19,including the accumulation of ACE2 receptors in testes,adipose tissue,thyroid,heart and kidneys that escalate viral affinity in males,the aged,and certain medical conditions,including diabetes,CVD,and pulmonary diseases.Pre-existing inflammation inherent in obesity may exacerbate the“cytokine storm,”a rampaging immune reaction during the course of Covid-19 that is deleterious to the host.We examined the molecular dynamics illustrating the action of new therapeutics necessary for Covid-19 patients;the estradiol advantage hypothesis;alternative therapies including hormone replacement procedures and mesenchymal stem cells;plus preventive and protective interventions.The current perspective also explored the primary components of dysregulated health predisposing individuals to Covid-19,including hormonal imbalance,increased lipids and lipoproteins,thyroid dysfunction,degraded fitness,and age-related testosterone decline accompanied by cortisol increase that provokes stress eating behaviours and weight accumulation.Obesity increases the probability of Covid-19 infection due to its abundance of ACE2 receptors;while physical activity may decrease Covid-19 vulnerability,by reducing fat and increasing muscle mass that manifests a relatively inhibited ACE2 expression.Several weight management solutions feature lasers and radiofrequency which diminish subcutaneous adiposity but do not enhance fitness.A data metanalysis of seven recently published clinical studies on 95 obese individuals,73 males and 22 females with an average BMI of 30.9,demonstrated visceral fat reduction combined with increased skeletal muscle mass.It also revealed a statistically significant decrease in BMI,lipids,lipoproteins,inflammation and toxicity as measured by CRP,Creatinine and Bilirubin respectively,juxtaposed by optimally healthier levels of Cortisol,Testosterone,Free T3,IGF-1,Insulin,and the appetite controlling hormones Leptin and Ghrelin.展开更多

The question of whether Alzheimer’s disease is an adaptability disorder is raised, given the patient blocked out her memory of her childhood experience. The importance of “happiness” as a treatment goal raises issues of advocacy and Guardianship as well as capacity, which is addressed by actual case reference and court action in defence of the patient’s rights to have their wishes respected and observed. Functional mental capacity assessment, using the Functional Mental State Measure (FMSM) gives a greater indication of neuronal reserve than standard cognitive testing, as it helps to unravel the dilemma associated with pure cognitive assessment in Alzheimer’s Disease as well as vascular dementia patients and patients who, despite retained and intact functional capacity and ability to express their wishes, i.e. “best interest”, are “wrongly” placed under Guardianship. In this editorial,we proceed to comment on the article by Chua et al,addressing the management of metastatic lateral pelvic lymph nodes(mLLN)in stage II/III rectal cancer patients below the peritoneal reflection.The t...In this editorial,we proceed to comment on the article by Chua et al,addressing the management of metastatic lateral pelvic lymph nodes(mLLN)in stage II/III rectal cancer patients below the peritoneal reflection.The treatment of this nodal area sparks significant controversy due to the strategic differences followed by Eastern and Western physicians,albeit with a higher degree of convergence in recent years.The dissection of lateral pelvic lymph nodes without neoadjuvant therapy is a standard practice in Eastern countries.In contrast,in the West,preference leans towards opting for neoadjuvant therapy with chemoradiotherapy or radiotherapy,that would cover the treatment of this area without the need to add the dissection of these nodes to the total mesorectal excision.In the presence of high-risk nodal characteristics for mLLN related to radiological imaging and lack of response to neoadjuvant therapy,the risk of lateral local recurrence increases,suggesting the appropriate selection of strategies to reduce the risk of recurrence in each patient profile.Despite the heterogeneous and retrospective nature of studies addressing this area,an international consensus is necessary to approach this clinical scenario uniformly.展开更多

Agriculture plays a crucial role in the economy,and there is an increasing global emphasis on automating agri-cultural processes.With the tremendous increase in population,the demand for food and employment has also i...Agriculture plays a crucial role in the economy,and there is an increasing global emphasis on automating agri-cultural processes.With the tremendous increase in population,the demand for food and employment has also increased significantly.Agricultural methods traditionally used to meet these requirements are no longer ade-quate,requiring solutions to issues such as excessive herbicide use and the use of chemical fertilizers.Integration of technologies such as the Internet of Things,wireless communication,machine learning,artificial intelligence(AI),and deep learning shows promise in addressing these challenges.However,there is a lack of comprehensive documentation on the application and potential of AI in improving agricultural input efficiency.To address this gap,a desk research approach was used by utilizing peer-reviewed electronic databases like PubMed,Scopus,Goo-gle Scholar,Web of Science,and Science Direct for relevant articles.Out of 327 initially identified articles,180 were deemed pertinent,focusing primarily on AI’s potential in enhancing yield through better management of nutrients,water,and weeds.Taking into account researchfindings worldwide,we found that AI technologies could assist farmers by providing recommendations on the optimal nutrients to enhance soil quality and deter-mine the best time for irrigation or herbicide application.The present status of AI-driven automation in agricul-ture holds significant promise for optimizing agricultural input utilization and reducing resource waste,particularly in the context of three pillars of crop management,i.e.,nutrient,irrigation,and weed management.展开更多
BACKGROUND The emergency department(ED)plays a critical role in establishing artificial airways and implementing mechanical ventilation.Managing airbags in the ED presents a prime opportunity to mitigate the risk of v...BACKGROUND The emergency department(ED)plays a critical role in establishing artificial airways and implementing mechanical ventilation.Managing airbags in the ED presents a prime opportunity to mitigate the risk of ventilator-associated pneumonia.Nonetheless,existing research has largely overlooked the understanding,beliefs,and practical dimensions of airway airbag management among ED nurses,with a predominant focus on intensive care unit nurses.AIM To investigate the current status of ED nurses'knowledge,beliefs,and practical behaviors in airway airbag management and their influencing factors.METHODS A survey was conducted from July 10th to August 10th,2023,using convenience sampling on 520 ED nurses from 15 tertiary hospitals and 5 sary hospitals in Shanghai.Pathway analysis was utilized to analyze the influencing factors.RESULTS The scores for ED nurses'airway airbag management knowledge were 60.26±23.00,belief was 88.65±13.36,and behavior was 75.10±19.84.The main influencing factors of airbag management knowledge included participation in specialized nurse or mechanical ventilation training,department,and work experience in the department.Influencing factors of airbag management belief comprised knowledge,department,and participation in specialized nurse or mechanical ventilation training.Primary influencing factors of airbag management behavior included knowledge,belief,department,participation in specialized nurse or mechanical ventilation training,and professional title.The belief in airbag management among ED nurses acted as a partial mediator between knowledge and behavior,with a total effect value of 0.513,and an indirect effect of 0.085,constituting 16.6%of the total effect.CONCLUSION ED nurses exhibit a positive attitude toward airbag management with relatively standardized practices,yet there remains room for improvement in their knowledge levels.Nursing managers should implement interventions tailored to the characteristics of ED nurses'airbag management knowledge,beliefs,and practices to enhance their airbag management proficiency.展开更多

Due to the pipe ruptures caused by wall thinning in Surry unit 2 of USA in 1986 and in Mihama unit 3 of Japan in 1994, the pipe wall thinning management has emerged as one of the most important issues in nuclear power plants. To manage the pipe wall thinning in the secondary system, Korea has used a foreign program since 1996. As using the foreign country’s program for long term, it was necessary to improve from the perspective of the users. Accordingly, KEPCO-E & C has started to develop the 3D-based pipe wall thinning management program (ToSPACE, Total Solution for Piping And Component Engineering management) from eight years ago, and the development was successful. This paper describes the major functions included in ToSPACE program, such as 3D-based DB (Database) buildup, development of FAC and erosion evaluation theories, UT (Ultra-sonic Test) data reliability analysis, field connection with 3D, automatic establishment of long-term inspection plan, etc. ToSPACE program was developed to allow site engineers performing the selection of inspection quantity at each refueling outage, UT data reliability analysis, UT evaluation, determination of next inspection timing, identification of the inspecting and replacing components in 3D drawings, etc., to access easily.展开更多
BACKGROUND Colorectal signet-ring cell carcinoma(CSRCC)is a rare clinical entity which accounts for approximately 1%of all colorectal cancers.Although multiple studies concerning this specific topic have been publishe...BACKGROUND Colorectal signet-ring cell carcinoma(CSRCC)is a rare clinical entity which accounts for approximately 1%of all colorectal cancers.Although multiple studies concerning this specific topic have been published in the past decades,the pathogenesis,associated risk factors,and potential implications on treatment are still poorly understood.Besides the low incidence,historically confusing histological criteria have resulted in confusing data.Nevertheless,the rising incidence of CSRCC along with relatively young age at presentation and associated dismal prognosis,highlight the actual interest to synthesize the known literature regarding CSRCC.AIM To provide an updated overview of risk factors,prognosis,and management of CSRCC.METHODS A literature search in the MEDLINE/PubMed database was conducted with the following search terms used:‘Signet ring cell carcinoma’and‘colorectal’.Studies in English language,published after January 1980,were included.Studies included in the qualitative synthesis were evaluated for content concerning epidemiology,risk factors,and clinical,diagnostic,histological,and molecular features,as well as metastatic pattern and therapeutic management.If possible,presented data was extracted in order to present a more detailed overview of the literature.RESULTS In total,67 articles were included for qualitative analysis,of which 54 were eligible for detailed data extraction.CSRCC has a reported incidence between 0.1%-2.4%and frequently presents with advanced disease stage at the time of diagnosis.CSRCC is associated with an impaired overall survival(5-year OS:0%-46%)and a worse stagecorrected outcome compared to mucinous and not otherwise specified adenocarcinoma.The systematic use of exploratory laparoscopy to determine the presence of peritoneal metastases has been advised.Surgery is the mainstay of treatment,although the rates of curative resection in CSRCC(21%-82%)are lower compared to those in other histological types.In case of peritoneal metastasis,cytoreductive surgery with hyperthermic intraperitoneal chemotherapy should only be proposed in selected patients.CONCLUSION CSRCC is a rare clinical entity most often characterized by young age and advanced disease at presentation.As such,diagnostic modalities and therapeutic approach should be tailored accordingly.展开更多

METHODS A search was conducted in MEDLINE, EMBASE, AMED, CINAHL and Cochrane ...AIM To discuss type 2 diabetes mellitus(T2DM) medication changes required during the popular 5:2 intermittent energy restriction(IER) diet. METHODS A search was conducted in MEDLINE, EMBASE, AMED, CINAHL and Cochrane library for original research articles investigating the use of very low calorie diets(VLCD) in people with T2 DM. The search terms used included "VLCD" or "very low energy diet" or "very low energy restriction" or "IER" or "intermittent fasting" or "calorie restriction" or "diabetes mellitus type 2" and "type 2 diabetes". Reference lists of selected articles were also screened for relevant publications. Only research articles written in English, which also included an explanation of medication changes were included. A recent pilot trial using the 5:2 IER method, conducted by our research group, will also be summarized.RESULTS A total of 8 studies were found that investigated the use of VLCD in T2 DM and discussed medication management. Overall these studies indicate that the use of a VLCD for people with T2 DM usually require the cessation of medication to prevent hypoglycemia. Therefore, the 5:2 IER method will also require medication changes, but as seen in our pilot trial, may not require total cessation of medication, rather a cessation on the 2 IER days only. CONCLUSION Guidelines outlined here can be used in the initial stages of a 2-d IER diet, but extensive blood glucose monitoring is still required to make the necessary individual reductions to medications in response to weight loss.展开更多

Although information security and cyber security are sometimes used synonymously, this study contends that they are not the same. The concept of cyber security is explored, which goes beyond protecting information resources to include a wider variety of assets, including people [1]. Protecting information assets is the main goal of traditional information security, with consideration to the human element and how people fit into the security process. On the other hand, cyber security adds a new level of complexity, as people might unintentionally contribute to or become targets of cyberattacks. This aspect presents moral questions since it is becoming more widely accepted that society has a duty to protect weaker members of society, including children [1]. The study emphasizes how important cyber security is on a larger scale, with many countries creating plans and laws to counteract cyberattacks. Nevertheless, a lot of these sources frequently neglect to define the differences or the relationship between information security and cyber security [1]. Indeed, the curre...The fight against insalubrity in large urban and peri-urban agglomerations is a major challenge in developing countries. This problem is compounded by that of sustainable waste management mechanisms. Indeed, the current waste collection system in Guinea has proved inadequate, as moving garbage from point “A” to point “B” is tantamount to “moving the problem”. The aim of this experimental work is to demonstrate the cost-effectiveness and benefits of sustainable waste management. As part of this drive to valorize biodegradable waste, the Waste Management Research Center has undertaken a series of activities ranging from composting organic waste to testing compost on certain crop varieties. An experimental field of 8024 m2 was laid out and treated with 1500 Kg of fine compost in doses ranging from 2.5 to 5 T/ha. Two crop varieties, eggplant and chili, were tested. Compost application increased production yields: 15 to 21 tonnes of eggplant and 10.4 to 11.1 tonnes of chili per hectare. Growth rates compared with usual yields varied from 50% to 64% and from 11% to 17% for eggplant and chili, respectively. This study resulted in an optimum compost dose of 2.5 T/ha for this phase.展开更多
文摘We advance here a novel methodology for robust intelligent biometric information management with inferences and predictions made using randomness and complexity concepts. Intelligence refers to learning, adap- tation, and functionality, and robustness refers to the ability to handle incomplete and/or corrupt adversarial information, on one side, and image and or device variability, on the other side. The proposed methodology is model-free and non-parametric. It draws support from discriminative methods using likelihood ratios to link at the conceptual level biometrics and forensics. It further links, at the modeling and implementation level, the Bayesian framework, statistical learning theory (SLT) using transduction and semi-supervised lea- rning, and Information Theory (IY) using mutual information. The key concepts supporting the proposed methodology are a) local estimation to facilitate learning and prediction using both labeled and unlabeled data;b) similarity metrics using regularity of patterns, randomness deficiency, and Kolmogorov complexity (similar to MDL) using strangeness/typicality and ranking p-values;and c) the Cover – Hart theorem on the asymptotical performance of k-nearest neighbors approaching the optimal Bayes error. Several topics on biometric inference and prediction related to 1) multi-level and multi-layer data fusion including quality and multi-modal biometrics;2) score normalization and revision theory;3) face selection and tracking;and 4) identity management, are described here using an integrated approach that includes transduction and boosting for ranking and sequential fusion/aggregation, respectively, on one side, and active learning and change/ outlier/intrusion detection realized using information gain and martingale, respectively, on the other side. The methodology proposed can be mapped to additional types of information beyond biometrics.
文摘The imminent danger of the Covid-19 pandemic has accelerated research in pharmaceuticals that either target the viral Spike proteins fusion with ACE2 receptors,or the infectious RNA replication that often overwhelms immune defences.The scope of this review was to elucidate the main human vulnerabilities to Covid-19,including the accumulation of ACE2 receptors in testes,adipose tissue,thyroid,heart and kidneys that escalate viral affinity in males,the aged,and certain medical conditions,including diabetes,CVD,and pulmonary diseases.Pre-existing inflammation inherent in obesity may exacerbate the“cytokine storm,”a rampaging immune reaction during the course of Covid-19 that is deleterious to the host.We examined the molecular dynamics illustrating the action of new therapeutics necessary for Covid-19 patients;the estradiol advantage hypothesis;alternative therapies including hormone replacement procedures and mesenchymal stem cells;plus preventive and protective interventions.The current perspective also explored the primary components of dysregulated health predisposing individuals to Covid-19,including hormonal imbalance,increased lipids and lipoproteins,thyroid dysfunction,degraded fitness,and age-related testosterone decline accompanied by cortisol increase that provokes stress eating behaviours and weight accumulation.Obesity increases the probability of Covid-19 infection due to its abundance of ACE2 receptors;while physical activity may decrease Covid-19 vulnerability,by reducing fat and increasing muscle mass that manifests a relatively inhibited ACE2 expression.Several weight management solutions feature lasers and radiofrequency which diminish subcutaneous adiposity but do not enhance fitness.A data metanalysis of seven recently published clinical studies on 95 obese individuals,73 males and 22 females with an average BMI of 30.9,demonstrated visceral fat reduction combined with increased skeletal muscle mass.It also revealed a statistically significant decrease in BMI,lipids,lipoproteins,inflammation and toxicity as measured by CRP,Creatinine and Bilirubin respectively,juxtaposed by optimally healthier levels of Cortisol,Testosterone,Free T3,IGF-1,Insulin,and the appetite controlling hormones Leptin and Ghrelin.
文摘A case presentation indicating the importance of “happiness” in childhood causing memory block until the patent presented with probable mixed vascular and neurodegenerative memory loss at 60 years of age is presented to highlight the role of emotional factors in causing the disease. The question of whether Alzheimer’s disease is an adaptability disorder is raised, given the patient blocked out her memory of her childhood experience. The importance of “happiness” as a treatment goal raises issues of advocacy and Guardianship as well as capacity, which is addressed by actual case reference and court action in defence of the patient’s rights to have their wishes respected and observed. Functional mental capacity assessment, using the Functional Mental State Measure (FMSM) gives a greater indication of neuronal reserve than standard cognitive testing, as it helps to unravel the dilemma associated with pure cognitive assessment in Alzheimer’s Disease as well as vascular dementia patients and patients who, despite retained and intact functional capacity and ability to express their wishes, i.e. “best interest”, are “wrongly” placed under Guardianship. Maladaptive responses, to control the change in external environment that are sensed or perceived, and which lead to disorder or to susceptibility to disease, exemplify a General Systems Theory approach, in which appropriate and adequate responses to environmental change, in behavioural terms, by a person, whether independently, or as the recipient or giver, or both, leads to functional interaction and happiness.

文摘Software Defined Network(SDN)and Network Function Virtualization(NFV)technology promote several benefits to network operators,including reduced maintenance costs,increased network operational performance,simplified network lifecycle,and policies management.Network vulnerabilities try to modify services provided by Network Function Virtualization MANagement and Orchestration(NFV MANO),and malicious attacks in different scenarios disrupt the NFV Orchestrator(NFVO)and Virtualized Infrastructure Manager(VIM)lifecycle management related to network services or individual Virtualized Network Function(VNF).This paper proposes an anomaly detection mechanism that monitors threats in NFV MANO and manages promptly and adaptively to implement and handle security functions in order to enhance the quality of experience for end users.An anomaly detector investigates these identified risks and provides secure network services.It enables virtual network security functions and identifies anomalies in Kubernetes(a cloud-based platform).For training and testing purpose of the proposed approach,an intrusion-containing dataset is used that hold multiple malicious activities like a Smurf,Neptune,Teardrop,Pod,Land,IPsweep,etc.,categorized as Probing(Prob),Denial of Service(DoS),User to Root(U2R),and Remote to User(R2L)attacks.An anomaly detector is anticipated with the capabilities of a Machine Learning(ML)technique,making use of supervised learning techniques like Logistic Regression(LR),Support Vector Machine(SVM),Random Forest(RF),Naïve Bayes(NB),and Extreme Gradient Boosting(XGBoost).The proposed framework has been evaluated by deploying the identified ML algorithm on a Jupyter notebook in Kubeflow to simulate Kubernetes for validation purposes.RF classifier has shown better outcomes(99.90%accuracy)than other classifiers in detecting anomalies/intrusions in the containerized environment.
文摘In this editorial,we proceed to comment on the article by Chua et al,addressing the management of metastatic lateral pelvic lymph nodes(mLLN)in stage II/III rectal cancer patients below the peritoneal reflection.The treatment of this nodal area sparks significant controversy due to the strategic differences followed by Eastern and Western physicians,albeit with a higher degree of convergence in recent years.The dissection of lateral pelvic lymph nodes without neoadjuvant therapy is a standard practice in Eastern countries.In contrast,in the West,preference leans towards opting for neoadjuvant therapy with chemoradiotherapy or radiotherapy,that would cover the treatment of this area without the need to add the dissection of these nodes to the total mesorectal excision.In the presence of high-risk nodal characteristics for mLLN related to radiological imaging and lack of response to neoadjuvant therapy,the risk of lateral local recurrence increases,suggesting the appropriate selection of strategies to reduce the risk of recurrence in each patient profile.Despite the heterogeneous and retrospective nature of studies addressing this area,an international consensus is necessary to approach this clinical scenario uniformly.
文摘The world is steadily moving to the post-liberal order with the urgent need of novel organizational and security approaches,also new levels of international cooperation,in order to support its stability and prosperity.The developed high-level Spatial Grasp Technology(SGT)and its Spatial Grasp Language(SGL)are briefed which may be particularly useful for solving numerous conflicts and crises problems emerging in different areas during this transitional period,in both local and global scale.SGT employs unlimited spatial scenario mobility and parallel holistic matching of distributed systems,with numerous communicating SGL interpreters potentially installed worldwide.Basic network creation and management operations are described in SGL which may operate on top of existing communication systems or serve individually as high level network protocols in case of non-local crises and disasters.Different operations on social networks are presented in SGL including finding strongest and weakest components with resultant changing of network topologies,also determining distances between different communities for preventing and predicting social conflicts.Fully distributed analysis,and tracing and simulation of multiple mobile objects in distributed spaces with complex routes are shown in SGL related to cruise missiles,defence objects and debris in outer space,as well as massively moving refugees through international borders.The proposed technology had trial implementations and applications in different countries,and its latest version can be readily installed by agreement on any platforms needed.
文摘Agriculture plays a crucial role in the economy,and there is an increasing global emphasis on automating agri-cultural processes.With the tremendous increase in population,the demand for food and employment has also increased significantly.Agricultural methods traditionally used to meet these requirements are no longer ade-quate,requiring solutions to issues such as excessive herbicide use and the use of chemical fertilizers.Integration of technologies such as the Internet of Things,wireless communication,machine learning,artificial intelligence(AI),and deep learning shows promise in addressing these challenges.However,there is a lack of comprehensive documentation on the application and potential of AI in improving agricultural input efficiency.To address this gap,a desk research approach was used by utilizing peer-reviewed electronic databases like PubMed,Scopus,Goo-gle Scholar,Web of Science,and Science Direct for relevant articles.Out of 327 initially identified articles,180 were deemed pertinent,focusing primarily on AI’s potential in enhancing yield through better management of nutrients,water,and weeds.Taking into account researchfindings worldwide,we found that AI technologies could assist farmers by providing recommendations on the optimal nutrients to enhance soil quality and deter-mine the best time for irrigation or herbicide application.The present status of AI-driven automation in agricul-ture holds significant promise for optimizing agricultural input utilization and reducing resource waste,particularly in the context of three pillars of crop management,i.e.,nutrient,irrigation,and weed management.
文摘BACKGROUND The emergency department(ED)plays a critical role in establishing artificial airways and implementing mechanical ventilation.Managing airbags in the ED presents a prime opportunity to mitigate the risk of ventilator-associated pneumonia.Nonetheless,existing research has largely overlooked the understanding,beliefs,and practical dimensions of airway airbag management among ED nurses,with a predominant focus on intensive care unit nurses.AIM To investigate the current status of ED nurses'knowledge,beliefs,and practical behaviors in airway airbag management and their influencing factors.METHODS A survey was conducted from July 10th to August 10th,2023,using convenience sampling on 520 ED nurses from 15 tertiary hospitals and 5 sary hospitals in Shanghai.Pathway analysis was utilized to analyze the influencing factors.RESULTS The scores for ED nurses'airway airbag management knowledge were 60.26±23.00,belief was 88.65±13.36,and behavior was 75.10±19.84.The main influencing factors of airbag management knowledge included participation in specialized nurse or mechanical ventilation training,department,and work experience in the department.Influencing factors of airbag management belief comprised knowledge,department,and participation in specialized nurse or mechanical ventilation training.Primary influencing factors of airbag management behavior included knowledge,belief,department,participation in specialized nurse or mechanical ventilation training,and professional title.The belief in airbag management among ED nurses acted as a partial mediator between knowledge and behavior,with a total effect value of 0.513,and an indirect effect of 0.085,constituting 16.6%of the total effect.CONCLUSION ED nurses exhibit a positive attitude toward airbag management with relatively standardized practices,yet there remains room for improvement in their knowledge levels.Nursing managers should implement interventions tailored to the characteristics of ED nurses'airbag management knowledge,beliefs,and practices to enhance their airbag management proficiency.
文摘COVID-19 pandemic restrictions limited all social activities to curtail the spread of the virus.The foremost and most prime sector among those affected were schools,colleges,and universities.The education system of entire nations had shifted to online education during this time.Many shortcomings of Learning Management Systems(LMSs)were detected to support education in an online mode that spawned the research in Artificial Intelligence(AI)based tools that are being developed by the research community to improve the effectiveness of LMSs.This paper presents a detailed survey of the different enhancements to LMSs,which are led by key advances in the area of AI to enhance the real-time and non-real-time user experience.The AI-based enhancements proposed to the LMSs start from the Application layer and Presentation layer in the form of flipped classroom models for the efficient learning environment and appropriately designed UI/UX for efficient utilization of LMS utilities and resources,including AI-based chatbots.Session layer enhancements are also required,such as AI-based online proctoring and user authentication using Biometrics.These extend to the Transport layer to support real-time and rate adaptive encrypted video transmission for user security/privacy and satisfactory working of AI-algorithms.It also needs the support of the Networking layer for IP-based geolocation features,the Virtual Private Network(VPN)feature,and the support of Software-Defined Networks(SDN)for optimum Quality of Service(QoS).Finally,in addition to these,non-real-time user experience is enhanced by other AI-based enhancements such as Plagiarism detection algorithms and Data Analytics.
文摘Moyamoya disease(MMD),characterized by progressive internal carotid artery stenosis and collateral vessel formation,prompts cerebral perfusion complications and is stratified into idiopathic and Moyamoya syndrome subtypes.A multifa-ceted approach toward MMD management addresses cerebral infarctions through revascularization surgery and adjunctive medical therapy,while also navigating risks such as intracranial hemorrhage and cerebral infarction resulting from arte-rial stenosis and fragile collateral vessels.Addressing antithrombotic management reveals a potential role for treatments like antiplatelet agents and anticoagulants,despite the ambiguous contribution of thrombosis to MMD-related infarctions and the critical balance between preventing ischemic events and averting hemo-rrhagic complications.Transcranial doppler has proven useful in thromboembolic detection,despite persisting challenges concerning the efficacy and safety of an-tithrombotic treatments.Furthermore,antihypertensive interventions aim to ma-nage blood pressure meticulously,especially during intracerebral hemorrhage,with recommendations and protocols varying based on the patient’s hypertension status.Additionally,lipid-lowering therapeutic strategies,particularly employing statins,are appraised for their possible beneficial role in MMD management,even as comprehensive data from disease-specific clinical trials remains elusive.Com-prehensive guidelines and protocols to navigate the multifaceted therapeutic ave-nues for MMD,while maintaining a delicate balance between efficacy and safety,warrant further meticulous research and development.This protocol manuscript seeks to elucidate the various aspects and challenges imbued in managing and navigating through the complex landscape of MMD treatment.
文摘Osteoporosis is a systemic skeletal disease characterized by low bone mineral density (BMD) and deterioration of bone architecture, resulting in reduced bone strength and, consequently, increased susceptibility to fractures which poses a significant public health concern worldwide, particularly in aging populations [1]. The health-economic impact of vertebral and hip fractures has been extensively explored and it is well known that these fractures are associated with morbidity/disability and increased mortality;they also account for a substantial portion of the direct fracture costs. This review aims to provide a comprehensive overview of osteoporosis, including its pathophysiology, risk factors, diagnostic approaches, and management strategies. By elucidating the multifaceted nature of this condition, healthcare providers can better identify individuals at risk, implement preventive measures, and optimize treatment to reduce the burden of osteoporotic fractures.

文摘Traffic congestion problem is one of the major problems that face many transportation decision makers for urban areas. The problem has many impacts on social, economical and development aspects of urban areas. Hence the solution to this problem is not straight forward. It requires a lot of effort, expertise, time and cost that sometime are not available. Most of the existing transportation planning software, specially the most advanced ones, requires personnel with lots practical transportation planning experience and with high level of education and training. In this paper we propose a comprehensive framework for an Intelligent Decision Support System (IDSS) for Traffic Congestion Management System that utilizes a state of the art transportation network equilibrium modeling and providing an easy to use GIS-based interaction environment. The developed IDSS reduces the dependability on the expertise and level of education of the transportation planners, transportation engineers, or any transportation decision makers.
文摘A number of piping components in the secondary system of nuclear power plants are exposed to aging mechanisms such as FAC (Flow-Accelerated Corrosion), cavitation, flashing, SPE (Solid Particle Erosion), LDIE (Liquid Droplet Impingement Erosion), etc. Those mechanisms may lead to thinning, leak, or rupture of the components. Due to the pipe ruptures caused by wall thinning in Surry unit 2 of USA in 1986 and in Mihama unit 3 of Japan in 1994, the pipe wall thinning management has emerged as one of the most important issues in nuclear power plants. To manage the pipe wall thinning in the secondary system, Korea has used a foreign program since 1996. As using the foreign country’s program for long term, it was necessary to improve from the perspective of the users. Accordingly, KEPCO-E & C has started to develop the 3D-based pipe wall thinning management program (ToSPACE, Total Solution for Piping And Component Engineering management) from eight years ago, and the development was successful. This paper describes the major functions included in ToSPACE program, such as 3D-based DB (Database) buildup, development of FAC and erosion evaluation theories, UT (Ultra-sonic Test) data reliability analysis, field connection with 3D, automatic establishment of long-term inspection plan, etc. ToSPACE program was developed to allow site engineers performing the selection of inspection quantity at each refueling outage, UT data reliability analysis, UT evaluation, determination of next inspection timing, identification of the inspecting and replacing components in 3D drawings, etc., to access easily.
文摘BACKGROUND Colorectal signet-ring cell carcinoma(CSRCC)is a rare clinical entity which accounts for approximately 1%of all colorectal cancers.Although multiple studies concerning this specific topic have been published in the past decades,the pathogenesis,associated risk factors,and potential implications on treatment are still poorly understood.Besides the low incidence,historically confusing histological criteria have resulted in confusing data.Nevertheless,the rising incidence of CSRCC along with relatively young age at presentation and associated dismal prognosis,highlight the actual interest to synthesize the known literature regarding CSRCC.AIM To provide an updated overview of risk factors,prognosis,and management of CSRCC.METHODS A literature search in the MEDLINE/PubMed database was conducted with the following search terms used:‘Signet ring cell carcinoma’and‘colorectal’.Studies in English language,published after January 1980,were included.Studies included in the qualitative synthesis were evaluated for content concerning epidemiology,risk factors,and clinical,diagnostic,histological,and molecular features,as well as metastatic pattern and therapeutic management.If possible,presented data was extracted in order to present a more detailed overview of the literature.RESULTS In total,67 articles were included for qualitative analysis,of which 54 were eligible for detailed data extraction.CSRCC has a reported incidence between 0.1%-2.4%and frequently presents with advanced disease stage at the time of diagnosis.CSRCC is associated with an impaired overall survival(5-year OS:0%-46%)and a worse stagecorrected outcome compared to mucinous and not otherwise specified adenocarcinoma.The systematic use of exploratory laparoscopy to determine the presence of peritoneal metastases has been advised.Surgery is the mainstay of treatment,although the rates of curative resection in CSRCC(21%-82%)are lower compared to those in other histological types.In case of peritoneal metastasis,cytoreductive surgery with hyperthermic intraperitoneal chemotherapy should only be proposed in selected patients.CONCLUSION CSRCC is a rare clinical entity most often characterized by young age and advanced disease at presentation.As such,diagnostic modalities and therapeutic approach should be tailored accordingly.
文摘AIM To discuss type 2 diabetes mellitus(T2DM) medication changes required during the popular 5:2 intermittent energy restriction(IER) diet. METHODS A search was conducted in MEDLINE, EMBASE, AMED, CINAHL and Cochrane library for original research articles investigating the use of very low calorie diets(VLCD) in people with T2 DM. The search terms used included "VLCD" or "very low energy diet" or "very low energy restriction" or "IER" or "intermittent fasting" or "calorie restriction" or "diabetes mellitus type 2" and "type 2 diabetes". Reference lists of selected articles were also screened for relevant publications. Only research articles written in English, which also included an explanation of medication changes were included. A recent pilot trial using the 5:2 IER method, conducted by our research group, will also be summarized.RESULTS A total of 8 studies were found that investigated the use of VLCD in T2 DM and discussed medication management. Overall these studies indicate that the use of a VLCD for people with T2 DM usually require the cessation of medication to prevent hypoglycemia. Therefore, the 5:2 IER method will also require medication changes, but as seen in our pilot trial, may not require total cessation of medication, rather a cessation on the 2 IER days only. CONCLUSION Guidelines outlined here can be used in the initial stages of a 2-d IER diet, but extensive blood glucose monitoring is still required to make the necessary individual reductions to medications in response to weight loss.
文摘This editorial explores the impact of non-steroidal anti-inflammatory drugs(NSAIDs)on postoperative recovery in hand fracture patients,amidst shifting pain management strategies away from opioids due to their adverse effects.With hand fractures being significantly common and postoperative pain management crucial for recovery,the potential of NSAIDs offers a non-addictive pain control alternative.However,the controversy over NSAIDs'effects on bone healing—stemming from their Cyclooxygenase-2 inhibition and associated risks of fracture non-union or delayed union—necessitates further investigation.Despite a comprehensive literature search,the study finds a lack of specific research on NSAIDs in postoperative hand fracture management,highlighting an urgent need for future studies to balance their benefits against possible risks.

文摘The fight against insalubrity in large urban and peri-urban agglomerations is a major challenge in developing countries. This problem is compounded by that of sustainable waste management mechanisms. Indeed, the current waste collection system in Guinea has proved inadequate, as moving garbage from point “A” to point “B” is tantamount to “moving the problem”. The aim of this experimental work is to demonstrate the cost-effectiveness and benefits of sustainable waste management. As part of this drive to valorize biodegradable waste, the Waste Management Research Center has undertaken a series of activities ranging from composting organic waste to testing compost on certain crop varieties. An experimental field of 8024 m2 was laid out and treated with 1500 Kg of fine compost in doses ranging from 2.5 to 5 T/ha. Two crop varieties, eggplant and chili, were tested. Compost application increased production yields: 15 to 21 tonnes of eggplant and 10.4 to 11.1 tonnes of chili per hectare. Growth rates compared with usual yields varied from 50% to 64% and from 11% to 17% for eggplant and chili, respectively. This study resulted in an optimum compost dose of 2.5 T/ha for this phase.
