Various scaffold materials have been used in...Replacing urethral tissue with functional scaffolds has been one of the challenging problems in the field of urethra reconstruction or repair over the last several decades. Various scaffold materials have been used in animal studies, but clinical studies on use of scaffolds for urethral repair are scarce. The aim of this study was to review recent animal and clinical studies on the use of different scaffolds for urethral repair, and to evaluate these scaffolds based on the evidence from these studies. Pub Med and OVID databases were searched to identify relevant studies, in conjunction with further manual search. Studies that met the inclusion criteria were systematically evaluated. Of 555 identified studies, 38 were included for analysis. It was found that in both animal and clinical studies, scaffolds seeded with cells were used for repair of large segmental defects of the urethra, such as in tubular urethroplasty. When the defect area was small, cell-free scaffolds were more likely to be applied. A lot of pre-clinical and limited clinical evidence showed that natural or artificial materials could be used as scaffolds for urethral repair. Urinary tissue engineering is still in the immature stage, and the safety, efficacy, cost-effectiveness of the scaffolds are needed for further study.展开更多
OBJECTIVE: To provide survival estimates of people living with human immunodeficiency virus(PLHIV) after treatment with Traditional Chinese Medicine(TCM) in rural China, to identify the prognostic factors at enrollmen...OBJECTIVE: To provide survival estimates of people living with human immunodeficiency virus(PLHIV) after treatment with Traditional Chinese Medicine(TCM) in rural China, to identify the prognostic factors at enrollment, and to explore the effectiveness of TCM intreating PLHIV.METHODS: PLHIV who enrolled in national TCM HIV treatment trial program in October 2004 were analyzed in this study and followed up to October 2010. Survival time was estimated by the Kaplan-Meier curve and hazard ratios, and identifying prognostic factors were computed through Cox proportional hazard models.RESULTS:A total of 1666 PLHIV were included with 102 591 person-months of follow-up. Overall, 312(18.7%) patients died. The total mortality rate over the study period was 3.6 per 100 person-years,which was lower than the worldwide rate. The cumulative survival rate was 95.9% at 1 year[95% confidence interval(CI)(94.8-96.8)] and 80.4% at 6years [95% CI(78.4-82.3)]. Elevated death risks emerged among males, older individuals, and thosewithlowerCD4+T-cellcounts.CONCLUSION: TCM could increase survival and lengthen the life span of PLHIV in Henan province of China, as shown by our retrospective cohort study. Factors such as sex, age, education, and CD4+ T-cell counts correlated to survival. However,retrospective cohorts bias the data, so more prospective studies should be performed to confirm our primary results.展开更多

Five studies showed positive results in terms of the effectiveness of acupuncture/Chine...Ten acupuncture-related articles were published in The Journal of the American Medical Association(JAMA) between 1998 and 2017. Five studies showed positive results in terms of the effectiveness of acupuncture/Chinese medicine(CM); five studies showed negative results. This article summarizes the acupuncturerelated clinical trials published over the last 20 years in JAMA, and addresses what seems to be a fundamental ambivalence in Western medical journals regarding the scientific validity of acupuncture/CM. As yet there has been no consensus on the role of acupuncture in healthcare in Western countries. This is hardly surprising, considering the conflicting evidence found in published studies. Skepticism regarding acupuncture/CM is largely grounded in the fact that an accurate model for assessing the true clinical effects of acupuncture has yet to be created. This article discusses some of the pitfalls which result from applying Western-based scientific principles to CM, and suggests that in many cases, "negative" studies have been misinterpreted. The clinical experience of acupuncture practitioners is often in direct conflict with many of the negative conclusions published in journals. We are in need of an accurate model for sham and placebo treatments, and must analyze all published studies for design flaws and faulty conclusions.展开更多

However, in Iranian traditional medicine (ITM) there are numerous plants with h...Hemorrhoidal disease is a prevalent anorectal condition which is generally not managed well with cur- rent pharmacologic interventions. However, in Iranian traditional medicine (ITM) there are numerous plants with hemorrhoid-healing properties. The present research assembled plants with hemorrhoid- healing properties in ITM; their related pharmacological effects, phytochemical constituents and mech- anisms of action in the modern medicine were also gathered. For this purpose, leading ITM textbooks were searched for plants with hemorrhoid-healing effects. Further, in vitro, in vivo and clinical studies on the most cited species were considered using scientific databases. Studying ITM textbooks revealed 37 medicinal plants with hemorrhoid-healing effects. Among the mentioned herbal medicines, six species, including Allium ampeloprasum, Phyllanthus emblica, Aloe vera, Terminalia chebula, Vitis vinifera and Comrniphora mukul, had the largest number of related pharmacological effects documented in scientific databases. These herbs from ITM should be considered as important resources for producing novel drugs for hemorrhoid treatment.展开更多
Objective To decompose and reformulate Banxia Baizhu Tianma Decoction(BBT) in order to optimize its ratio for vasodilatory effect.The fingerprints of these six component herbs were also studied via Tri-step FTIR ana...Objective To decompose and reformulate Banxia Baizhu Tianma Decoction(BBT) in order to optimize its ratio for vasodilatory effect.The fingerprints of these six component herbs were also studied via Tri-step FTIR analysis.Methods The herbal components of BBT were extracted separately with different solvents,such as 95%ethanol,50%ethanol,and distilled water.The vasodilatory effect of the extracts was studied using the in vitro pre-contracted aortic ring model.Among these extracts,the water extracts of Pinellia ternata[PT(W)]and Citri reticulatae[CR(W)],50%ethanol extracts of Castrodia elata[GE(50)]and Clycyrrhiza uralensis[GU(50)],and 95%ethanol extracts of Atractylodes macrocephala[AM(E)],and Poria cocos[PC(E)]were selected for the orthogonal stimulus-response compatibility group study due to their high effectiveness in vasodilation.Results The best combination ratio(G2) was GE(50),GU(50),AM(E),PC(E),CR(W),and PT(W) at EC0,EC5,EC5,EC5,EC5,and EC5,with EC50 of(0.06 ± 0.01) mg/mL.Comparison between the vasodilatory effects of extracts derived from G2 and BBT were also studied.From the results,the 50%ethanol extract following G2 ratio was found to be most potent extract with the highest vasodilatory effect compared with other extracts derived from G2 or BBT,with EC50 of(0.05 ±0.005) mg/mL.Conclusion The optimum ratio and appropriate extraction method for the new BBT formulae are obtained.G2 is determined as the best formulae with the highest potency in producing the vasodilatory effect on isolated rat aortic rings.The results obtained suggest that 50%ethanol is found to be the best solvent to extract the vasoactive compounds of G2 in order to exhibit the highest efficacy of vasodilatory effect.展开更多

The present research assembled plants with hemorrhoid- healing properties in ITM; their related pharmacological effects, phytochemical constituents and mech- anisms of action in the modern medicine were also gathered. For this purpose, leading ITM textbooks were searched for plants with hemorrhoid-healing effects. Further, in vitro, in vivo and clinical studies on the most cited species were considered using scientific databases. Studying ITM textbooks revealed 37 medicinal plants with hemorrhoid-healing effects. Among the mentioned herbal medicines, six species, including Allium ampeloprasum, Phyllanthus emblica, Aloe vera, Terminalia chebula, Vitis vinifera and Comrniphora mukul, had the largest number of related pharmacological effects documented in scientific databases. These herbs from ITM should be considered as important resources for producing novel drugs for hemorrhoid treatment.

文摘Objective To decompose and reformulate Banxia Baizhu Tianma Decoction(BBT) in order to optimize its ratio for vasodilatory effect.The fingerprints of these six component herbs were also studied via Tri-step FTIR analysis.Methods The herbal components of BBT were extracted separately with different solvents,such as 95%ethanol,50%ethanol,and distilled water.The vasodilatory effect of the extracts was studied using the in vitro pre-contracted aortic ring model.Among these extracts,the water extracts of Pinellia ternata[PT(W)]and Citri reticulatae[CR(W)],50%ethanol extracts of Castrodia elata[GE(50)]and Clycyrrhiza uralensis[GU(50)],and 95%ethanol extracts of Atractylodes macrocephala[AM(E)],and Poria cocos[PC(E)]were selected for the orthogonal stimulus-response compatibility group study due to their high effectiveness in vasodilation.Results The best combination ratio(G2) was GE(50),GU(50),AM(E),PC(E),CR(W),and PT(W) at EC0,EC5,EC5,EC5,EC5,and EC5,with EC50 of(0.06 ± 0.01) mg/mL.Comparison between the vasodilatory effects of extracts derived from G2 and BBT were also studied.From the results,the 50%ethanol extract following G2 ratio was found to be most potent extract with the highest vasodilatory effect compared with other extracts derived from G2 or BBT,with EC50 of(0.05 ±0.005) mg/mL.Conclusion The optimum ratio and appropriate extraction method for the new BBT formulae are obtained.G2 is determined as the best formulae with the highest potency in producing the vasodilatory effect on isolated rat aortic rings.The results obtained suggest that 50%ethanol is found to be the best solvent to extract the vasoactive compounds of G2 in order to exhibit the highest efficacy of vasodilatory effect.
