Surgical Resident Attrition and the Menninger Morale Curve

A number of studies speculat... BACKGROUND: Attrition in surgical residency programs continues to be a significant challenge. Ap-proximately 20% of residents who begin a categorical surgery residency fail to complete it. A number of studies speculated reasons for this including work hours, life style, family pressures, and resident feelings of inadequacy including fear of termination. To date no research has been conducted investigating the relation-ship between resident morale and attrition. This study sought to determine if this linkage exists in surgery residents. METHODS: The Morale Assessment in General Practice Index (MAGPI) was administered to 21 PGY 1, 2, 3, and 5 surgical residents to assess level of morale. Non-parametric methods were carried out to assess if there were differences in morale among the four PGY groups. Additionally, analyses of the four factors comprising the MAGPI were also conducted. RESULTS: Although differences did not reach statis-tical significance, analysis of the data reveals that residents demonstrate different trends in their levels of morale based on the amount of time they spend in a residency and in a way that approximates the morale curve described by W. Walter Menninger, M.D. Additionally, two of the four factors comprising the MAGPI also indicate trends similar to that described by the Menninger morale curve. CONCLUSIONS: Although no statistically significant results were achieved, the data reveal trends that approximate shifts in morale similar to those described by the Menninger morale curve, with residents at the PGY 2 and 3 levels present-ing lower morale levels than at the PGY 1 and 5 levels. This may be due in part to the size of the population studied. Future research should be continued in this area with a larger sample size. 展开更多

Objective:The aim of this study was to examine the relationship between moral distress that may affect patient safety,and the clinical practice model,assessing ethical decision-making skills of certified registered nurse anesthetists(CRNAs).Methods:A survey using the Ethical Stress Scale(ESS)and the Ethical Assessment Skills Survey(EASS)was conducted with 134 CRNAs.Results:Results indicated no significant effect of practice model on level of moral distress or perceived ethical assessment skill knowledge[Wilks's lambda=0.952,F(6,256)=1.068,P=0.382,n^(2)=0.02].A statistically significant positive correlation existed between importance and skill(r=0.275,P=0.001).CRNAs felt skilled to manage the actions or activities they deemed important.Conclusion:CRNAs who perceived a higher skill level in addressing ethical issues experienced lower levels of moral distress.Findings indicate content-specific curricula for the CRNAs need to be evaluated for ethical decision-making skill assessment content. 展开更多

This paper makes a comparative study of the theory of heavenly principles and human desires in the Neo-Confucianism of the Song and Ming Dynasties and the theory of personality structure in psychoanalytic theory.From the perspective of the construction of the subject,in the debate between the heavenly principle and the human desire,the person is built into the moral subject.The id,ego,and super-ego in the psychoanalytic theory constitute the unconscious structural subject.In the framework of these two theories,the“maintain the heavenly principles and eradicate human desires”and“the heavenly principles exist in desire”in the theory of heavenly principles and human desires express the unity of opposites between heavenly principles and human desire.The core of personality structure theory is the relationship of obedience and control among the id,ego,and super-ego.As far as the value orientation is concerned,the debate between the heavenly principle and the human desire provides a theoretical basis for the moral practice of Confucianism of“aim at absolute perfection”,while the free will of the id in the unconscious subject is the most essential ethical aspect of psychoanalysis. 展开更多

交往合理性概念是哈贝马斯为解决现代性危机而提出的。由于哈贝马斯的理论与韦伯理论的关系及他本人后期工作重心的转移,这一概念在现代合理性图谱中的定位似乎一直是悬而未决的。要解决这一问题,关键在于理解交往合理性与道德-实践合... 交往合理性概念是哈贝马斯为解决现代性危机而提出的。由于哈贝马斯的理论与韦伯理论的关系及他本人后期工作重心的转移,这一概念在现代合理性图谱中的定位似乎一直是悬而未决的。要解决这一问题,关键在于理解交往合理性与道德-实践合理性的关系。哈贝马斯先后在认识兴趣理论和交往行为理论中,从理性兴趣和行为领域这两个层面对这两种合理性做出过区分。这只是二者关系的一个方面。在哈贝马斯的理论中,交往合理性的任务是,面对认知-工具合理性的过度膨胀,既要承认现代社会已经分化的基本合理性之间相互独立的事实,又要在此前提下实现它们的有限统一。据此,面对自身合法领地遭到侵蚀的状况,道德-实践合理性应当依据交往合理性所提供的有效性要求、原则与程序,实现自身重建,具体包括道德、法律和宗教三个具体领域。 展开更多

马克思主义关于“正义”“公平”等的论述既具有描述性特征,也具有规范性特征。当在描述性语境中阐述其道德观念时,马克思和恩格斯试图说明道德与物质生产方式的关系、道德的历史性、道德的阶级性、道德与阶级利益的关系等内容。而在规... 马克思主义关于“正义”“公平”等的论述既具有描述性特征,也具有规范性特征。当在描述性语境中阐述其道德观念时,马克思和恩格斯试图说明道德与物质生产方式的关系、道德的历史性、道德的阶级性、道德与阶级利益的关系等内容。而在规范性语境中,他们也确实对资本主义作出了道德评价,认为其是不正义的。马克思和恩格斯是站在无产阶级的立场,作为无产阶级利益的代言人来抨击资本主义社会对无产阶级的迫害。通过对历史的科学研究,他们认为无产阶级利益的实现符合历史发展趋势,无产阶级道德观比资产阶级道德观更为合理。他们比较各阶级道德观之合理性的标准不在历史之外,而是由历史本身来提供的,即何者所代表的阶级利益的实现符合历史发展必然性,何者就具有合理性。 展开更多

在生活中,我们会出于各种各样的原因责备别人。道德责备和职业责备是最常见的责备,但是除此之外还有一种独特的责备——认知责备。就认知责备的内涵来看,学界有三种竞争理论。基于情感的认知责备观对于认知责备施加了过强的情感限制;基... 在生活中,我们会出于各种各样的原因责备别人。道德责备和职业责备是最常见的责备,但是除此之外还有一种独特的责备——认知责备。就认知责备的内涵来看,学界有三种竞争理论。基于情感的认知责备观对于认知责备施加了过强的情感限制;基于关系修正的认知责备观在刻画规范的理想状态时采用的客观性视角限制了理论的解释力;基于“信念—欲望”的认知责备观正确识别出责备产生的统一根源。该理论指出,认知责备来源于认知层面规范性期待的挫败。在该理论的基础上,认知责备背后隐藏着的“真之承诺”这一根基性的态度也得以浮现。认知责备具有独特价值。对于受责者来说,认知责备可以贡献于认知规范的内化,有助于塑造受责者在认知层面的能动性。对于责备者来说,认知责备展现了责备者对真理的看重和捍卫。 展开更多
