The risk factors for type 2 diabetes mellitus(T2DM)have been increasingly researched,but the lack of systematic identification and categorization makes it difficult for clinicians to quickly and accurately access and understand all the risk factors,which are categorized in this paper into five categories:Social determinants,lifestyle,checkable/testable risk factors,history of illness and medication,and other factors,which are discussed in a narrative review.Meanwhile,this paper points out the problems of the current research,helps to improve the systematic categorisation and practicality of T2DM risk factors,and provides a professional research basis for clinical practice and industry decision-making.

Exploring Narrations in Drama： An Evolving Framework

There has been a gradual change of the relation in which narration stands to drama. In this process, narration in drama has undergone a series of transformations in terms of significance, appeal and functionality. It has turned, from a suppressed intrusion into dramatic verisimilitude, to a subordinate attendant on plays of reason and passion and then to an epic element in a modernistic turn, and has contemporarily come onto central stage and assumed a particular theatricality of its own. Such an evolving framework corresponds to philosophical and aesthetical developments over time and integrates into an ever-renewing semiotics of drama. Recent developments in Narratology of the theatre provide an inspiring context for understanding this evolution, as they shed light on the generic nature, communi- cational facets and discursive power of dramatic narratives.

人工智能生成内容(AI Generated Content,简称AIGC)电影包括内涵丰富的多种维度,从编剧角度可以分为AI生成故事(AI Generated Storyline,简称AIGSL)与AI生成剧本(AI Generated Screenplay,简称AIGSP)。AIGSL研究始于20世纪60年代初,按照创作模式可以分为基于结构模型的故事生成、基于规则的故事生成和基于机器学习的故事生成三类程序。2022年底以来,以Chat GPT为代表的大语言模型将AIGC的内容从简单的AIGSL拓展到了AIGSP领域,同时催生了基于大语言模型的专用故事与剧本生成工具。大语言模型的故事和剧本生产存在事实错误、陈词滥调以及主观偏见等天然缺陷,但AIGSL与AIGSP已经开始实现产业化。目前的人工智能编剧创作绝大多数并不完全由AI创作,属于弱人工智能领域。中国电影工作者与软件工程师有必要利用机器学习等AI技术,学习中国传统叙事文本并总结其叙事风格,为具有中国叙事特色的编剧创作提供智能助力。

Nursing effect of narrative nursing intervention on postoperative patients with severe lung cancer

BACKGROUND Lung cancer is a common disease with high mortality,and psychological support is very important in the diagnosis and treatment of postoperative patients with cancer pain.AIM To explore the application effect of the narrative nursing method in postoperative lung cancer patients in the intensive care unit.METHODS A total of 120 patients diagnosed with lung cancer and experiencing cancer-related pain were randomly allocated into two groups:an observation group and a control group,each consisting of 60 cases.The control group was given routine analgesic and psychological care,while the research group applied the five-step narrative nursing method based on routine care,comparing the visual analogue scale scores,sleep status,anxiety and depression status,and quality of life of the two groups of patients before and after the intervention.RESULTS The pain scores,anxiety scores,and depression scores of the study group were lower than those of the control group after the intervention using the narrative nursing method,and the difference was statistically significant(P<0.05).CONCLUSION Using narrative nursing methods to intervene in patients with lung cancer combined with cancerous pain can help patients to correctly recognize their disease,adjust their mentality,establish confidence,alleviate patients'subjective pain feelings,and improve their adverse emotions.

Narrative Thinking of Healthcare Providers in Medical Cases and Its Implications for Modern Medical Education

In the realm of Chinese narrative medicine, narrative thinking holds equal significance to evidence-based thinking within clinical practice, making substantial contributions to patient safety and medical quality. Ancient Chinese medical theories advocate forging a narrative bond with patients, urging healthcare practitioners to utilize Dao Shu Jie He(道术结合 integration of Dao and techniques) and Yi Dao Yu Shu(以道驭术 guiding techniques with Dao) during diagnosis, treatment, and rehabilitation. In this context, Dao(道) in traditional Chinese medicine(TCM) corresponds to narrative thinking(叙事思维) while Shu(术) aligns with scientific thinking(科学思维) in evidence-based medicine. This study adopts “narrative Chinese medicine” within the “discourse system of Chinese narrative medicine” as its framework, centering on medical stories that exemplify the application of narrative thinking in ancient Chinese diagnosis and treatment. It delves into the narrative thinking encapsulated in Chinese medicine, such as Guan Wu Qu Xiang(观物取象 observing things and extracting information), Can He Er Xing(参合而行 final examination and decision by integrating the four diagnoses), Xing Shen Bing Zhi(形神并治 body-spirit treatment), Xin Shen Jian Li(心身兼理 mind-body balance) and Bian Zheng Lun Zhi(辨证论治 treatment based on pattern differentiation). The study endeavors to inspire contemporary medical educators and clinicians to acknowledge the value of narrative thinking in TCM and fully incorporate it into daily medical practice.

Health Democracy and Storytelling:A Synthesis of Knowledge

Aims:Health democracy requires tools and methodologies to involve non-scientific actors in the development and implementation of health policies.Storytelling could be one of the tools to make health democracy effective.Our aim is to describe how storytelling is used in relation to health democracy,the aims of its use,the methodology adopted and the results obtained.Procedure:We conducted a narrative review of the literature.Our search equation was composed by the keyword“narration”and its variations“récit de vie”,“histoire de vie”and“medécine narrative”and by the keyword“démocratie en santé”/“démocratie sanitaire”.Results:We obtained 135 results and included six articles,published between 2015 and 2022.The interest of storytelling in a health democracy approach concerns six main themes:a)the plurality of experiences;b)empowerment;c)the recognition of different types of knowledge;d)the involvement of all care actors in care;storytelling as a lever for change;e)in the care pathway;f)in health policies.Conclusion:The limited number of articles included in this literature review raises questions about the diffusion of the concept of health democracy associated with that of storytelling.Storytelling can contribute to the dissemination and effectiveness of health democracy:it is a complementary tool to quantitative tools for understanding the experiences of users of the health care system,and its use is particularly relevant to chronic diseases such as cancer.

This article employs the logical discourse framework of Chinese narrative medicine to illustrate the experiences of various patients exhibiting typical instances of narrative foreclosure in traditional Chinese medicine(TCM) cases, such as Bei Gong She Ying(杯弓蛇影 to see a bow reflected in a cup as a snake), Wu Qiu Zhi Chong(吴球治虫 Wu Qiu treats worms), and Fa Jing Qu She(法靖驱蛇 Fajing drives away snakes). It explores how TCM practitioners leverage narrative wisdom to facilitate patients in dispelling their internal doubts through the concept of Bu Yao Wei Yao(不药为药 healing diseases without medicine). Through these medical cases, the article advocates for the significance of narrative competencies among medical practitioners in diagnosing and treating patients, alongside the use of pharmaceuticals and surgical interventions. Additionally, it outlines the prospects for the future development of narrative approaches in TCM.

Depression and anxiety disorders in chronic obstructive pulmonary disease patients:Prevalence,disease impact,treatment

Chronic obstructive pulmonary disease(COPD)is a common respiratory disorder that often co-occurs with depression and anxiety,worsening disease progression and reducing quality of life.A thorough review of the existing literature was conducted,including searches in PubMed,Embase,PsycINFO,and Cochrane Library databases up to 2024.This review encompasses a critical analysis of studies reporting on the prevalence,impact,and management of depression and anxiety in COPD patients.We found a high prevalence of psychological comorbidities in COPD patients,which were associated with worse disease outcomes,including increased exacerbations,hospitalizations,and reduced health-related quality of life.Diagnosing and managing these conditions is complex due to overlapping symptoms,necessitating a comprehensive patient care approach.While there has been progress in understanding COPD comorbidities,there is a need for more personalized and integrated treatments.This review emphasizes the need for increased awareness,tailored treatment plans,and further research for effective interventions.

Cultural symbols,a manifestation of cities’cultural resources,are not only signs that frame concepts but also forms that express meanings.Exploring the international communication of cities from the perspective of symbols,this paper analyzes in depth how cities create their cultural symbols in the dynamic process of international communication in an era of symbol-based digital media,and how they develop their narratives and explain meanings through the dissemination of symbols when telling their stories to international audiences,thus enhancing the efficiency and effectiveness of their international communication efforts.

The Development of Doctor-nurse-patient Co-construction through Evidence-based Narrative Exploration of Traditional Chinese Medicine

Traditional Chinese medicine(TCM)is the wisdom crystallization and cultural treasure of the Chinese nation.Establishing a positive relationship among doctors,nurses,and patients is essential for promoting the sustainable development of TCM culture.As an important branch of evidence-based narrative medicine,evidence-based narrative of TCM significantly contributes to the co-construction of doctors,nurses and patients.This paper discusses the necessity and clinical practice value of promoting co-construction of doctors,nurses and patients based on the principles of evidence-based narrative TCM.By working together to select optimal diagnosis and treatment options,we aim to enhance patient compliance,alleviate tensions among the parties involved,and establish equitable collaborative relationships.Creating a harmonious medical environment is crucial for promoting the continuous development of this collaborative model in clinical practice,thereby improving patients'overall health,increasing public recognition of TCM,and supporting its sustainable growth.

Doctor-patient Narrative Re-discovered from Overseas Traditional Chinese Medicine Practices

The doctor-patient narrative has been revisited and appreciated in both the West and the East due to the negative impact of biochemical medicine in the past two centuries on healthcare.Biochemical medicine system simply marginalized the roles of doctors and patients.More research and practice of“doctor's benevolence”and“humanistic medicine”have called for the return of the doctor-patient narrative.This paper draws on interviews with several non-Chinese overseas traditional Chinese medicine(TCM)practitioners,whose clients are also non-Chinese.We adopted discourse analysis to explore our data.We found that they actively engaged in doctor-patient narrative with localized interpretation of TCM.We believe such a return to basic doctor-patient narrative is caused by fundamental needs for doctor-patient narrative coinciding with loose control of TCM practices in the studied countries.This discovery may inspire further study on re-establishing doctor-patient narratives in healthcare institutions by re-positioning biochemical medicine.

Narrative nursing for negative emotions in patients with acute pancreatitis:Based on model construction and application

BACKGROUND Acute pancreatitis(AP),as a common acute abdomen disease,has a high incidence rate worldwide and is often accompanied by severe complications.Negative emotions lead to increased secretion of stress hormones,elevated blood sugar levels,and enhanced insulin resistance,which in turn increases the risk of AP and significantly affects the patient's quality of life.Therefore,exploring the intervention effects of narrative nursing programs on the negative emotions of patients with AP is not only helpful in alleviating psychological stress and improving quality of life but also has significant implications for improving disease outcomes and prognosis.AIM To construct a narrative nursing model for negative emotions in patients with AP and verify its efficacy in application.METHODS Through Delphi expert consultation,a narrative nursing model for negative emotions in patients with AP was constructed.A non-randomized quasi-experimental study design was used in this study.A total of 92 patients with AP with negative emotions admitted to a tertiary hospital in Nantong City of Jiangsu Province,China from September 2022 to August 2023 were recruited by convenience sampling,among whom 46 patients admitted from September 2022 to February 2023 were included in the observation group,and 46 patients from March to August 2023 were selected as control group.The observation group received narrative nursing plan,while the control group was given with routine nursing.Self-rating anxiety scale(SAS),self-rating depression scale(SDS),positive and negative affect scale(PANAS),caring behavior scale,patient satisfaction scale and 36-item short form health survey questionnaire(SF-36)were used to evaluate their emotions,satisfaction and caring behaviors in the two groups on the day of discharge,1-and 3-month following discharge.RESULTS According to the inclusion and exclusion criteria,a total of 45 cases in the intervention group and 44 cases in the control group eventually recruited and completed in the study.On the day of discharge,the intervention group showed significantly lower scores of SAS,SDS and negative emotion(28.57±4.52 vs 17.4±4.44,P<0.001),whereas evidently higher outcomes in the positive emotion score,Caring behavior scale score and satisfaction score compared to the control group(P<0.05).Repeated measurement analysis of variance showed that significant between-group differences were found in time effect,inter-group effect and interaction effect of SAS and PANAS scores as well as in time effect and inter-group effect of SF-36 scores(P<0.05);the SF-36 scores of two groups at 3 months after discharge were higher than those at 1 month after discharge(P<0.05).CONCLUSION The application of narrative nursing protocols has demonstrated significant effectiveness in alleviating anxiety,ameliorating negative emotions,and enhancing satisfaction among patients with AP.

Zhu You Shu(祝由术a form of traditional Chinese spiritual healing)originated and was extensively utilized in China.This paper illustrates the origin,concept and techniques of Zhu You Shu in traditional Chinese medicine(TCM)and explores the correlation between Zhu You Shu and the three focuses and three elements of narrative medicine.The elements of Zhu You Shu,such as Zhu Shuo Bing You(祝说病由explaining the original cause of the disease)and Qing Zhi Xiang Sheng(情志相胜generation and restraint of emotions)share fundamental similarities with narrative medicine.Based on the principles and the interoperability of Zhu You Shu and narrative medicine,the doctor engages in active listening and communication with the patient to establish a trusting relationship.Utilizing the Qing Zhi Xiang Sheng technique,the doctor seeks to understand and regulate the patient's psychological processes.By integrating psychological cues,the aim is to restore emotional balance and enhance the patient's emotional and mental well-being while fostering a lasting emotional bond and providing spiritual support.Zhu You Shu has the potential to enhance the clinical pathway of narrative medicine in TCM.Simultaneously,the progress of narrative medicine will offer more sophisticated contemporary medical theories to assist the advancement of Zhu You Shu.

Teaching Life Writing: Theory and Textual Practice

In this research about teaching life-writing,the authors would like to focus on different ways of demonstrating narratives about personal lives.Some established research articles and academic writings on autobiography and biography can be read as textual practices of methods for teaching narrating lives in the classroom.It is significant to read the literary texts as practices of narratives about different lives.The relations among the narrator(first person in terms of autobiography,third person in terms of biography),the text,and the theory/the method of writing,in a way,all come to show the readers the richness of different lives and how distinguish these narratives are.

The Impact of Storyline Complexity on L2 Learners’Written Performance

Task-based Language Teaching(TBLT)research has provided ample evidence that cognitive complexity is an important aspect of task design that influences learner’s performance in terms of fluency,accuracy,and syntactic complexity.Despite the substantial number of empirical investigations into task complexity in journal articles,storyline complexity,one of the features of it,is scarcely investigated.Previous research mainly focused on the impact of storyline complexity on learners’oral performance,but the impact on learners’written performance is less investigated.Thus,this study aims at investigating the effects of narrative complexity of storyline on senior high school students’written performance,as displayed by its complexity,fluency,and accuracy.The present study has important pedagogical implications.That is,task design and assessment should make a distinction between different types of narrative tasks.For example,the task with single or dual storyline.Results on task complexity may contribute to informing the pedagogical choices made by teachers when prioritizing work with a specific linguistic dimension.
