The disease-causing mutation is an expansion of a GAG trinucleotide repeat (〉 36 repeats) encoding a polygluta- mine stretch in the N-terminal region ofhuntingtin (Htt) (Zuc- cato et al., 2010), a ubiquitous protein whose function is still unclear (Zuccato et al., 2010). Expansion of the polyQ stretch endows mutant Htt (mHtt) with toxic properties, and results in the development of a broad array of undesirable effects in both neuronal and non-neuronal cells (Zuccato et al., 2010). Among all cellular dysfunctions and biochemical imbalances classically associated with HD, perturbed metabolism of (glyco) sphingolipids appears to play a crucial role in the pathogenesis of the disease. She had been treated with topical adapalene once daily, and closed and open comedo subsided without inflammatory lesions one month after the treatment without any side effects induced by adapalen. The recurrence of comedo had not been observed afterwards. In the Japanese Dermatological Association (JDA) guidelines for the treatment of acne vulgaris, comedonal phase has not been stated. Then we propose the tentative new terminology of a comedonal phase and emphasize its importance in the early initial treatment for comedo to prevent the development of the acute inflammatory phase such as red papules and pustules and formation of acne scars. 展开更多

Despite a decrease in incidence over past decades,gastric cancer remains a major global health problem. In the more recent period, survival has shown only minor improvement, despite significant advances in diagnostic techniques, surgical and chemotherapeutic approaches, the development of novel therapeutic agents and treatment by multidisciplinary teams. Because multiple genetic mutations, epigenetic alterations, and aberrant molecular signalling pathways are involved in the development of gastric cancers, recent research has attempted to determine the molecular heterogeneity responsible for the processes of carcinogenesis, spread and metastasis. Currently, some novel agents targeting a part of these dysfunctional molecular signalling pathways have already been integrated into the standard treatment of gastric cancer, whereas others remain in phases of investigation within clinical trials. It is essential to identify the unique molecular patterns of tumours and specific biomarkers to develop treatments targeted to the individual tumour behaviour. This review analyses the global impact of gastric cancer, as well as the role of Helicobacter pylori infection and the efficacy of bacterial eradication in preventing gastric cancer development. Furthermore, the paper discusses the currently available targeted treatments and future directions of research using promising novel classes of molecular agents for advanced tumours. 展开更多

Ongoing Mutations in Polytreated Metastatic Cancer Patients May Create a New Chance of Treatment with Unexpected Drugs

Objectives: In this study molecular/genomic characteristics were done on new tissue biopsies taken from Egyptian patients with refractory metastatic solid tumors aiming for two end points: To figure out a personalized treatment and to find the percent of discrepancy between the elaborated drugs of potential benefit and that stated in the guidelines. Methods: 22 eligible patients joined the study. (breast = 5, colon = 3, liver = 2, kidney = 2, ovary = 2, sarcoma = 2, metastasis of unknown origin = 2, Tongue = 1, Adrenal cancer = 1, gastric = 1 and lung cancer = 1). Biopsies were subjected to one or more of the following tests;Immunohistochemistry, Chromogenic/Fluorescence in situ Hybridization, Next Generation Sequencing, Sanger Sequencing. Results: Biomarkers and their corresponding drugs with associated potential benefits were detected as following;TUBB3, PGP and TLE3 (indicating potentiality of paclitaxel) in 22% of cases, TS (Antifolates) 18%, TOPO1 (Irinotecan) 14%, RRM1 (Gemcitabine) 13%, MGMT (Temozolomide) 7%, TOPO2 (Doxorubcin) 7%, ERCC1 (Platinum) 6%, BRAF (Vemurafenib) 2%, KRAS and NRAS (anti EGFR) 2%, C-KIT (TKIs potentiality) 1%, hormonal receptors in 5% of cases (Antihormonal potentiality), monoSPARK and polySPARK in 3% of cases indicating nabpaclitaxel potentiality. Potentiality of some drugs (Based on their corresponding biomarkers) was unexpectedly detected as following;Pemetrexed, irinotecan, dacarbazine and temozolomide in breast cancer patients, platinums and taxanes in liver, Taxanes, gemcitabine, fluoropyramidines, pemetrexed, dacarbazine and temozolomide in kidney cancer, Taxanes, gemcitabine, pemetrexed, dacarbazine and temozolomide in cancer colon, irinotecan in cancer tongue, Pemetrexed and irinotecan in adrenal gland cancer. The percentage of drugs of potential benefit that is not stated in the guidelines case by case was as following: Breast (12%, 15%, 23%, 31%, 21%), Colon (38.1%, 26.5%, 27%), Liver (33.5%, 25%), Kidney (15%, 29%), Ovary (1%, 2%) Sarcoma (17%, 53.5%) tongue 35%, adrenal 73.2%, Gastric 27.8% and lung 36%. Conclusion: Studying molecular/genomic characteristics of new tissue biopsies from polytreated fit metastatic cancer patients may detect unexpected drugs with potential benefits. 展开更多

目的:分析简版国家早期预警评分(NEWS)识别急诊科抢救室老年病人入院30 d内死亡风险的效能。方法:选择2019年1月—2020年12月深圳市某三级甲等医院急诊科抢救室收治的1184例老年病人作为研究对象。收集急诊科抢救室收治的老年病人入急... 目的:分析简版国家早期预警评分(NEWS)识别急诊科抢救室老年病人入院30 d内死亡风险的效能。方法:选择2019年1月—2020年12月深圳市某三级甲等医院急诊科抢救室收治的1184例老年病人作为研究对象。收集急诊科抢救室收治的老年病人入急诊科抢救室时的体温、心率、呼吸频率、血压、意识状态、血氧饱和度数据及入急诊科抢救室的主要诊断、入院30 d内的预后结局,通过模型区分度、校准度分析简版NEWS在识别急诊科抢救室老年人入院30 d内死亡的效能。结果:简版NEWS、NEWS及改良早期预警评分(MEWS)预测急诊科抢救室老年人入院30 d内死亡的受试者工作特征曲线下面积(AUC)分别为0.847,0.842及0.793,Brier评分分别为0.0450,0.0455及0.0478。当简版NEWS得分>3.5分时,病人30 d内死亡风险为13.7%;当简版NEWS得分<3.5分,病人30 d内死亡风险为1.5%。结论:简版NEWS的操作更简捷,在急诊科抢救室老年病人入院30 d内死亡风险评估中有良好的区分度及校准度。 展开更多

The plan of heat-treatment process for 2014Al alloy is designed using orthogonal method, the heat-treatment experiments are made and the mechanical properties are tested according to the designed plan. The effect of solid solution temperature, ageing temperature, ageing time on microscopic mechanism of the mechanical properties of the 2014Al alloy is studied using microscope, transmission electron microscope. The best heat treatment process of the 2014Al alloy is developed. The experimental results indicate that the strength σ<sub>b</sub>, yield stress σ<sub>0.2</sub>, percentage elongation δ of the alloy reach separately 490~500 MPa, 450~490 MPa, 10~12% adopting the new heat treatment process. Compared with GB, the strength increases 20~30%, the percentage elongation increases 30~40%. The mechanism of the new heat-treatment process is also discussed. 展开更多

急诊老年危重症患者NEWS评分、APACHEⅡ评分和乳酸水平的变化及其临床意义 被引量:1

目的探讨急诊老年危重症患者国家早期预警(NEWS)评分、急性生理与慢性健康状况Ⅱ(APACHEⅡ)评分、乳酸(Lac)水平的变化及其临床意义。方法回顾性分析2019年2月至2021年12月西安国际医学中心医院急诊科收治的100例急诊老年危重症患者临... 目的探讨急诊老年危重症患者国家早期预警(NEWS)评分、急性生理与慢性健康状况Ⅱ(APACHEⅡ)评分、乳酸(Lac)水平的变化及其临床意义。方法回顾性分析2019年2月至2021年12月西安国际医学中心医院急诊科收治的100例急诊老年危重症患者临床资料,依据28d生存状况分为死亡组(n=36)和存活组(n=64),比较两组患者的NEWS评分、APACHEⅡ评分及Lac水平的差异,采用受试者工作特征曲线(ROC)分析上述各项指标单独检测及联合检测在判断、评估老年危重症预后状况的临床价值。结果存活组患者的NEWS评分、APACHEⅡ评分、Lac水平分别为(5.28±0.81)分、(18.93±2.97)分、(4.82±0.48)mmol/L,明显低于死亡组的(6.09±1.02)分、(21.16±3.26)分、(5.12±0.55)mmol/L,差异均有统计学意义(P<0.05);经ROC分析结果显示,NEWS评分、APACHEⅡ评分及Lac水平单独预测老年危重症死亡的曲线下面积(AUC)分别为0.740、0.684、0.651,均明显低于联合检测的0.816,差异均有统计学意义(P<0.05)。结论老年危重症患者的NEWS评分、APACHEⅡ评分、Lac水平能够在一定程度上反映其预后,联合上述指标能够较为准确地对其预后进行预测和评估,密切观察上述指标变化对于老年危重症患者的临床诊治具有一定的指导意义。 展开更多

The paper firstly put forward the severe problem. Then the possible main reasons were analyzed which might attribute to both the inefficient removal of wastewater treatment plants with conventional technology and ignorance of the monitor and control of new-emerging pollutants in the effluents. Also, the complexity and extreme high costs may also make the organizations sidestep the problem. Finally, possible strategies to deal with the problems were proposed. 骨质疏松症是目前临床上最常见的慢性代谢性骨病,在老年人中尤其高发。骨质疏松症的诱因是患者机体内的骨吸收和骨形成之间存在不平衡。年龄增长、经期结束、营养失调,以及药物使用等均会诱发骨质疏松症。骨质疏松症患者常伴有骨折、全... 骨质疏松症是目前临床上最常见的慢性代谢性骨病,在老年人中尤其高发。骨质疏松症的诱因是患者机体内的骨吸收和骨形成之间存在不平衡。年龄增长、经期结束、营养失调,以及药物使用等均会诱发骨质疏松症。骨质疏松症患者常伴有骨折、全身疼痛,行动能力受损,机能损伤,易感染等一系列症状。因其高发病率及其引发的高死亡率,骨质疏松症已经成为我国乃至全世界一个极为严重的健康问题。老年骨质疏松症患者的临床诊断标准尚存在一定争议。包括X线计算机断层成像、高分辨率周边定量X线计算机断层成像和定量超声在内的新型诊断技术已经逐步应用于临床。目前,针对骨质疏松症的临床药物选择主要是抗再吸收和合成代谢药物。另外,联合用药以及新型药物亦在稳步推进中。针对该疾病的药物种类繁多,新旧不一,优缺互现,患者甄别存在很大的困难。本文基于目前老年骨质疏松症研究的最新进展,着重就其临床诊断和药物治疗等几个方面做一简要综述。 展开更多

首先对新型Al-4.8Cu-1.8Zn-1Ti-0.6Mg合金进行了不同时间的530℃固溶处理及不同时间的165℃人工时效处理,随后进行不同时间的自然时效处理。采用电子万能试验机和扫描电镜研究了不同工艺处理后合金的力学性能和微观组织。结果表明:新型A... 首先对新型Al-4.8Cu-1.8Zn-1Ti-0.6Mg合金进行了不同时间的530℃固溶处理及不同时间的165℃人工时效处理,随后进行不同时间的自然时效处理。采用电子万能试验机和扫描电镜研究了不同工艺处理后合金的力学性能和微观组织。结果表明:新型Al-4.8Cu-1.8Zn-1Ti-0.6Mg合金经530℃固溶处理8 h及165℃人工时效3 h后,其抗拉强度可达387.72 MPa,屈服强度达245.54 MPa,断后伸长率为9.8%;经15、75和135天自然时效后,合金的抗拉强度分别为422.97、390.09和387.72 MPa,断后伸长率分别为5.03%、8.4%和9.8%。可以看出随着自然时效时间增加合金的抗拉强度降低、断后伸长率升高,最终趋于稳定;热处理后合金内存在3种第二相组织,其中亮银色θ(Al_(2)Cu)相在晶界处粗大析出会导致合金的抗拉强度下降,暗灰色Al_(3)Ti相易聚集,聚集后的粗大Al_(3)Ti相会导致合金的变形能力下降。 展开更多

AIM:To survey ophthalmologists(who have participated previously in clinical research) and ophthalmic industry professionals(who have been involved in ocular research and development) to indicate perceived needs for new pharmaceuticals in various ophthalmic subspecialties.METHODS:A prospective,industry-based survey was sent to ophthalmologists and ophthalmic industry professionals about the perceived needs for new pharmaceutical products.RESULTS:This survey was sent to 559 ophthalmic pharma professionals and ophthalmologists.We received 82(15%) responses.The results showed that the most commonly perceived need for new pharmaceuticals were dry and wet age-related macular degeneration,glaucoma,diabetic macular edema and dry eye.There was a statistical difference found between response groups(P〈0.0001).Respondents indicated they would express their commitment to a new product they perceived as needed by recommending to colleagues(63%),prescribing(60%),participating as principle investigator in a related clinical trial(52%),advising the company(52%),lecturing on behalf of the product(43%),investing in the product(38%),taking no action(7%) or obtain a position in the company(1%).CONCLUSION:Ophthalmic pharma professionals and ophthalmologists perceive the greatest need for new medicines in ophthalmology to be in dry and wet agerelated macular degeneration,glaucoma,diabetic macular edema and dry eye. 展开更多

近年来微塑料污染对环境的影响逐渐引起人们的重视,在不同空间尺度、环境介质以及与常见环境污染物质的作用机理等相关研究也逐渐增多,通过查阅微塑料环境危害以及处理的诸多文献,对其污染及去除工艺的进展进行研究。结果显示,微塑料流... 近年来微塑料污染对环境的影响逐渐引起人们的重视,在不同空间尺度、环境介质以及与常见环境污染物质的作用机理等相关研究也逐渐增多,通过查阅微塑料环境危害以及处理的诸多文献,对其污染及去除工艺的进展进行研究。结果显示,微塑料流入环境会造成生态系统功能的破坏、富集有机物流入食物链最终危害人体健康等危害,并且对不同水体造成了不同程度的危害,现我国水处理工艺中没有特定去除微塑料的工艺,对微塑料的去除仅依赖于传统工艺,虽然有资料显示去除效率可以达到90%,但是污水量巨大,仍然有大量的微塑料流入水环境。从国内不同水处理厂、不同水处理工艺对微塑料的去除效率,从而引出当前国内外对微塑料处理的技术,为我国水处理厂对微塑料去除提供参考。 展开更多
