Because of the effect of unbalance excitation and nonlinear magnetic force, the large vibration of the rotor supported by active magnetic bearing(AMB) will go beyond the radial gap of the bearing, even causing mechanical touch-rubbing when the system works at an operational speed closer to the critical speed. In order to investigate this problem, the linear model and nonlinear model of the single mass symmetric rigid rotor system supported by AMB are established respectively and the corresponding transfer functions of close-loop system are given. To pass through the numerical calculation by using MATLAB/Simulink, the effect of both the unbalance response and threshold speed of touch-rubbing of the system subjected to nonlinear magnetic forces and nonlinear output current of power amplifier are studied. Furthermore, threshold speed of touch-rubbing of the rotor-bearing system is defined and the results of numerical simulation are presented. Finally, based on above studies, two methods of increasing the touch-rubbing threshold speed are discussed. 展开更多

Background: Accurate temperature measurement is a critical step in evaluating health or disease especially in children and immmunocompromised subjects;inaccurate measurement may lead to improper diagnosis, wrong treatment or inappropriate intervention. Several methods of temperature measurements exist and comparing these gives room for choosing a near ideal method in terms of speed, safety and accuracy. The study aimed to compare the forehead non touch infra-red thermometer with the axilllary mercury-in-glass method of temperature measurement in the Paediatric age-group. Methods: Study was given ethical approval as part of a larger study. Four hundred and thirty seven children aged 1 to 24 months were studied at the well-baby/immunizationclinic of the University of Ilorin Teaching Hospital over a 6-months period. Both non-touch infrared and theregular mercury-in-glass thermometers were used to take the body temperatures. Data were analysed with SPSS version 21. Pearson correlation was used to determine the relationship between the two methods of temperature measurements, while Bland-Altman method was used to test for level of agreement between them. Results: The mean age and SD was 5.81 ± 4.04 months. Pearson correlation showed a positive correlation between the axillary mercury-in-glass and forehead non-touch infra-red thermometry readings (r = 0.281, p < 0.001). Also, Bland-Altman method revealed a good agreement between both methods of thermometry as 96% of the readings were within the limits of agreement. Mean difference was 0.09°C (95% confidence interval 0.05 - 0.13). Conclusion: Axillary mercury-in-glass thermometer and forehead non touch infra-red thermometers have a good agreement and can be used interchangeably. 展开更多

Methods: we performed a systematicsearch in the electronic databases such as Cochrane Library, Pubmed, Embase, Chinese Journal Full-text Database,Chinese Biomedical Literature Database, Chinese Science and Technology Periodical Full-text Database andWanfang Database up to 30 January 2017. Randomized controlled trials (RCT) of Shenqi Fuzheng Injectioncombined with GP chemotherapy in the treatment of advanced NSCLC were searched, and all the RCTs wereconducted on methodological quality assessment. Data extraction and data analysis were according to standards ofCochrane systematic review. Results: Eight trials were included including a total of 701 patients. Meta-analysisresults: Shenqi Fuzheng injection combined with GP chemotherapy could significantly improve the functionalstatus of patients with NSCLC (OR = 3.44, 95% CI [2.26, 5.25], P 〈 0.0001) and clinical treatment efficacy (OR =(OR = 0.31, 95%CI [0.20, 0.47], P 〈 0.0001. The rate of leukopenia (OR = .31, 95%CI [0.20,0.47], P 〈 0.0001),thrombocytopenia (OR = 0.58, 95%CI [0.37, 0.91], P = 0.020), hemoglobin decline ((OR = 0.31, 95%CI [0.16,0.59], P = 0.0004) and incidence of gastrointestinal reactions (OR = 0.58,P 〈 0.05) could be reduced. Conclusion:Shenqi Fuzheng injection combined with GP chemotherapy in the treatment of advanced NSCLC obtainedsignificantly clinical efficacy. CYFRA 21-1 as an early predictor of first line chemotherapy response in advanced non small cell lung cancer

Objective: In an era of ever evolving, promising new therapies for advanced non small cell lung cancer (NSCLC), early predictors of response to therapy, are needed. We evaluated early variations in CYFRA 21-1 serum levels of patients with advanced NSCLC receiving first line chemotherapy and correlated the results with objective tumor response. Methods: 29 consecutive, previously untreated, patients of advanced non small cell lung cancer, with measurable disease on CT scan were evaluated. All patients were treated with conventional systemic chemotherapy, although the choice of chemotherapy was left to the discretion of the treating physicians. Serum samples were obtained immediately before the start of 1st and 2nd cycles of chemotherapy. CYFRA 21-1 was measured with an electrochemiluminescense immunoassay on an automatic analyzer (Elecsys 2000; Roche Diagnostics). Response was evaluated using Response evaluation criteria in solid tumors (RECIST) criteria. Results: 10 patients had partial response, 9 patients had stable disease and 9 had progressive disease. None of the patients had complete response. 21/29 (72%) patients had an elevated baseline value of CYFRA 21-1.62% patients (18/29) had a decrease in CYFRA 21-1 after 1 cycle of chemotherapy. The average reduction in the 2nd reading was irrespective of whether baseline value was normal or not. The average reduction was statistically significant (P = 0.002; 95% CI, from 0.8369 to 3.49464; t test). 8 out of 10 (80%) patients with partial response had a reduction in their 2nd reading of. CYFRA (P = 0.019; 95% CI, from 0.81965 to 7.20035; t test) which was significant. We also observed that 6/9 (66%) patients whose disease remains stable also had a decrease in their subsequent reading (P = 0.0106; 95% CI, from -0.44942 to 3.82720; t test), though it was not significant statistically. Although 5 out of 9 (55%) patients, who had an increase in their CYFRA 21-1 level, had progressive disease, but it was not statistically significant (P = 0.537; 95% CI, from -1.20021 to 2.13354; ttest). 14 out of 19 (73%) who either had partial response or had stable disease, had a reduction in their 2nd value of CYFRA 21-1 and was significant statistically (P = 0.004; 95% CI, from 0.74792 to 3.50208; t test). We also observed that except for 1 patient, all patients who had a decrease of 42% or more in their subsequent CYFRA 21-1 level, were those who had either responded to chemotherapy or had stable disease (P = 0.001), which was statistically significant. Conclusion: We can conclude that monitoring of serum marker CYFRA 21-1, early dudng first-line chemotherapy may be a useful prognostic tool for evaluation of early tumor response in patients with advanced NSCLC. 展开更多

Increasing evidence indicates that aberrant expressions of some microRNAs are associated with cancer progression.However,the roles and biological mechanisms of miRNA-16-5p in human non-small cell lung cancer(NSCLC)are not to be well studied.Here,we validated that the expression of miR-16-5p was decreased significantly in NSCLC samples and cell lines.The correlation between the clinicopathological features of NSCLC and the miR-16-5p expression showed that the expression of miR-16-5p in non-small cell lung cancer was linked with the advanced TNM stage,positive lymph node metastasis,with short overall survival(OS).Also,a negative correlation between miR-16-5p and Fermitin family member 2(FERMT2)was observed,implying there may be a potential link about their regulation.The hypothesis was further confirmed by in-silico analysis and dual-luciferase reporter assay.Moreover,we demonstrated that the transfections of miR-16-5p mimics could alter some biological characteristics of NSCLC cells remarkably accomplished by the expression variance of FERMT2 in vitro and in vivo assays.Summarily,this study demonstrated that miR-16-5p,as a tumor suppression factor in NSCLC by targeting FERMT2,could serve as one promising biomarker in the prediction for NSCLC patients. 展开更多

The maximum bending moment or curvature in the neighborhood of the touch down point (TDP) and the maximum tension at the top are two key parameters to be controlled during deepwater J-lay installation in order to ensure the safety of the pipe-laying operation and the normal operation of the pipelines. In this paper, the non-linear governing differential equation for getting the two parameters during J-lay installation is proposed and solved by use of singular perturbation technique, from which the asymptotic expression of stiffened catenary is obtained and the theoretical expression of its static geometric configuration as well as axial tension and bending moment is derived. Finite element results are applied to verify this method. Parametric investigation is conducted to analyze the influences of the seabed slope, unit weight, flexural stiffness, water depth, and the pipe-laying tower angle on the maximum tension and moment of pipeline by this method, and the results show how to control the installation process by changing individual parameters. 展开更多

And I would agree with writers such as Gardner and Horwitz et al. (1991)that anxiety may impede learner’s ability to perform successfully in all aspects of learning a foreign language. 目的探讨声触诊弹性成像(sound touch elastography,STE)诊断非酒精性脂肪性肝病(nonalcoholic fatty liver disease,NAFLD)合并慢性乙型肝炎(chronic hepatitis B,CHB)的价值,明确STE分期诊断NAFLD合并CHB患者肝纤维化程度的效能。方... 目的探讨声触诊弹性成像(sound touch elastography,STE)诊断非酒精性脂肪性肝病(nonalcoholic fatty liver disease,NAFLD)合并慢性乙型肝炎(chronic hepatitis B,CHB)的价值,明确STE分期诊断NAFLD合并CHB患者肝纤维化程度的效能。方法选取2018年1月至2023年1月在深圳市第三人民医院肝病科就诊的NAFLD患者74例和NAFLD合并CHB患者142例,分别纳入NAFLD组、NAFLD合并CHB组。选取健康志愿者61例,纳入对照组。NAFLD合并CHB组患者根据METAVIR肝纤维化分级标准分为S1期、S2期、S3期、S4期。所有受试者均接受肝STE、肝声触诊定量(sound touch quantify,STQ)检查,比较三组间肝弹性测值有无差异,绘制受试者操作特征曲线(receiver operator characteristic curve,ROC曲线)。结果三组间肝硬度值(liver stiffness measurement,LSM)、肝STE、肝STQ有统计学差异(P<0.001)。肝STE、肝STQ区分对照组与NAFLD合并CHB组的ROC曲线下面积(under the curve,AUC)为0.834、0.805;区分对照组与NAFLD组的AUC为0.628、0.617;区分NAFLD合并CHB组与NAFLD组的AUC为0.741、0.715。肝STE诊断NAFLD合并CHB组≥S1、≥S2、≥S3期肝纤维化和S4期肝硬化的AUC分别为0.834、0.889、0.940、0.950。结论NAFLD合并CHB时,肝脏STE、STQ值较NAFLD患者及正常人群升高。肝STE在分期诊断NAFLD合并CHB患者肝纤维化程度方面具备显著价值。 展开更多

目的:探讨体位支持、非营养性吸吮(NNS)配合轻柔抚触对新生儿股静脉穿刺采血疼痛的影响。方法:选取2021年6月-2022年6月本院需行股静脉穿刺采血的186例新生儿,随机数字表法分为观察组(93例,行体位支持、NNS配合轻柔抚触方法)和对照组(93... 目的:探讨体位支持、非营养性吸吮(NNS)配合轻柔抚触对新生儿股静脉穿刺采血疼痛的影响。方法:选取2021年6月-2022年6月本院需行股静脉穿刺采血的186例新生儿,随机数字表法分为观察组(93例,行体位支持、NNS配合轻柔抚触方法)和对照组(93例,行常规体位和安抚方法)。比较两组操作前后心率、呼吸频率,采血后1min、5min新生儿疼痛评分量表(NIPS)评分,首次穿刺成功率及采血操作时间,以及不良事件发生情况。结果:观察组新生儿静脉穿刺采血后心率(122.37±15.30次/min)、呼吸频率(44.53±5.57次/min)低于对照组(143.31±17.91次/min、56.29±7.04次/min),静脉穿刺采集后1min、5min的NIPS评分(4.29±0.89分、1.67±0.25分)均低于对照组(5.53±1.13分、3.09±0.71分),静脉穿刺采集首次成功率(95.7%)高于对照组(85.0%),采血时间(1.13±0.17 min)短于对照组(1.32±0.22 min),采血后不良事件发生率(12.9%)低于对照组(28.0%)(均P<0.05)。结论:体位支持、NNS配合轻柔抚触应用于新生儿静脉穿刺采血,可减轻新生儿疼痛程度,提高首次采血成功率,缩短采血时间。 展开更多

目的:分析非营养性吸吮联合腹部抚触对极低出生体质量儿喂养不耐受的影响。方法:选取2021年10月—2023年4月泰安市妇幼保健院收治的极低出生体质量儿76例作为研究对象,随机分为观察组和对照组,各38例。对照组给予常规喂养护理,观察组给... 目的:分析非营养性吸吮联合腹部抚触对极低出生体质量儿喂养不耐受的影响。方法:选取2021年10月—2023年4月泰安市妇幼保健院收治的极低出生体质量儿76例作为研究对象,随机分为观察组和对照组,各38例。对照组给予常规喂养护理,观察组给予非营养性吮吸联合腹部抚触护理。比较两组极低出生体质量儿喂养状况、喂养不耐受症状改善时间、并发症发生率和体格发育情况。结果:观察组每日摄奶量大于对照组,每日残余奶量小于对照组,每日睡眠时间长于对照组,鼻胃管留置时间、恢复出生体质量时间、达全肠道喂养时间、住院时间短于对照组,观察组呕吐、腹胀、胃潴留等症状改善时间均短于对照组,差异有统计学意义(P<0.001)。观察组坏死性小肠结肠炎(NEC)、动力性肠梗阻、肠黏膜萎缩、胆汁淤积等并发症发生率低于对照组,差异有统计学意义(P<0.05)。干预前,两组患儿体格发育情况比较,差异无统计学意义(P>0.05);干预第3周后,观察组体质量、身长、头围增长量均大于对照组,差异有统计学意义(P<0.05)。结论:非营养性吸吮联合腹部抚触可显著改善极低出生体质量儿新生儿的喂养不耐受情况,改善其喂养效果,降低其NEC、动力性肠梗阻、肠黏膜萎缩、胆汁淤积等并发症发生率,促进其体格生长。 展开更多

目的探讨非营养性吸吮联合抚触干预对足跟采血新生儿的影响。方法方便选取2022年3月—2023年7月浙江省人民医院毕节医院收治的116例新生儿为研究对象,根据不同方法分为对照组和观察组,每组58例。对照组实施抚触干预,观察组在对照组的基... 目的探讨非营养性吸吮联合抚触干预对足跟采血新生儿的影响。方法方便选取2022年3月—2023年7月浙江省人民医院毕节医院收治的116例新生儿为研究对象,根据不同方法分为对照组和观察组,每组58例。对照组实施抚触干预,观察组在对照组的基础上实施非营养性吸吮干预。比较两组新生儿采血相关指标、疼痛程度、家属护理满意度。结果观察组采血时间(81.46±12.77)s、哭闹时间(84.33±14.78)s均短于对照组的(94.11±13.28)s、(135.79±20.46)s,差异有统计学意义(P均<0.05)。观察组的疼痛评分为(2.46±1.14)分,低于对照组的(4.03±1.88)分,差异有统计学意义(P<0.05)。观察组家属护理总满意度为96.55%(56/58),高于对照组的82.76%(48/58),差异有统计学意义(χ^(2)=5.949,P<0.05)。结论在足跟采血新生儿中实施非营养性吸吮联合抚触干预效果显著,可明显缩短采血时间,减轻新生儿疼痛程度,有助于提升新生儿家属护理满意度。 展开更多

Background:Non–small cell lung cancer(NSCLC)is a common malignant tumor with an increasing incidence.PIEZO-type mechanosensitive ion channel component 1(PIEZO1)is a mechanosensitive ion channel whose expression has been implicated in various cancers.However,its expression patterns,prognostic implications,and specific molecular mechanisms of action in NSCLC remain un-clear.This study aimed to characterize the expression profile of PIEZO1 in NSCLC in vitro and using bioinformatics analyses.Methods:To determine the expression profile of PIEZO1 in normal and cancerous human tissues and the mRNA expression of PIEZO1 in NSCLC,we analyzed data from The Cancer Genome Atlas.Various bioinformatics analyses were performed for correlation analyses,construction of survival curves,and identification of upstream mRNA targets and genes coexpressed with PIEZO1.Furthermore,the coexpressed genes were functionally annotated with Gene Ontology and subjected to pathway enrichment analyses.Additionally,we an-alyzed and compared PIEZO1 expression in normal and cancerous human tissue samples in vitro.Results:NSCLC tissue samples had lower PIEZO1 expression than adjacent tissues.PIEZO1 overexpression inhibited NSCLC progres-sion and cell migration and correlated with improved survival outcomes,as revealed by bioinformatics analyses.Additionally,analysis using the OncomiR database revealed that miR-942-5p upregulation contributed to the downregulation of PIEZO1 in NSCLC.Mechanistic in-vestigations suggested that PIEZO1 modulates NSCLC by regulating the mitogen-activated protein kinase pathway,particularly by targeting FLNA,RRAS,and MAP3K6.Bioinformatics analyses highlighted the potential of PIEZO1 as a therapeutic target and prognostic indicator of NSCLC.Conclusions:These novel findings reveal the expression profile of PIEZO1 in NSCLC and highlight the potential of PIEZO1 and miR-942-5p as diagnostic and therapeutic biomarkers of NSCLC and other cancer types. 展开更多
