Nowadays, more and more patients with VLU accept wound care in community setting. Clinical nurses generally take care of the patients based on their own experiences. Healing in VLU is an incredibly complex process, which puzzles even experts. The majority of general nurses do not have this level of expertise, especially those nurses serving in community. Function is the basis of nursing activities. Patients always show different clinical manifestations and selfcare abilities due to various function states, which cannot be reflected completely by the existing nursing practice. How to describe nursing practice standardized in order to demonstrate the effectiveness of interventions and facilitate interdisciplinary communication is another urgent problem. Therefore, the aim of this project is to develop an accurate nursing program based on function in chronic venous leg ulcers, which can both satisfy the needs of patients and promote nursing revolution. Methods: This study will use International Classification of Functioning Disability and Health as a framework to choose suitable functions and to filter function classification standards of chronic venous leg ulcers through evidence-based systematic research.Nursing interventions related to VLU are selected based on Nursing Interventions Classification, adding other nursing activities by methods of evidence-based systematic review and clinical observation. Then, nursing interventions and function status are matched through steering committee. Finally, the Delphi survey method is adopted to make nursing program native and scientific. Conclusions: This study is expected to be very significant and meaningful in using standardized nursing terminology. The nursing program established could better meet the needs of both patients with chronic venous leg ulcers and clinical nurses, promoting the development of wound specialist and standardized nursing language.展开更多

This study was carried out from December 2017 to March 2018. We applied the theme of the dissipation of the dissociation between nursing skills learned in basic nursing education and nursing skills required in practice. A questionnaire survey was conducted at four facilities among new nursing staff who had acquired their nursing license in the past year. The questionnaire was composed of questions about their experiences of oral care for intubated patients, when they learned these skills, and what they wanted to learn. Answers were obtained from 118 new nurses. Learning about oral care was important for approximately 20% of the nurses, although it was not considered mandatory by all of the nurses. As infection control measures have revealed the acute needs of care for intubated patients, it is important for nursing students to learn about oral care and attitudes towards intubated patients, along with methods to make intubated patients more comfortable.展开更多

Current UC therapy relies on controlling acute episodes and preventing relapse. To predict modifications in the natural course of UC, mucosal healing(MH) has emerged as a major treatment goal. Endoscopic evaluation is considered the gold standard for assessing MH, which can be achieved by conventional drugs and biologics in many, but not all, patients. Consequently, interest is focusing on the development of new substances for UC therapy, and new oral agents are in the pipeline. This review will focus on the ability of newly developed oral drugs to induce and maintain MH in UC patients.展开更多

文摘Background:Pedunculoside(PE)is a pentacyclic triterpenoid saponin found in the dried barks of the Ilex rotunda Thunb. In traditional Chinese medicine, the juice extracted from various Ilex species is commonly used for treating inflammatory diseases. Objective:This study aims to investigate the potential of PE in promoting the healing of oral ulcers in mice and to explore its possible mechanism.Materials and Methods:A mouse model of oral ulcer was established to evaluate the healing promotion effects of PE,and an in-vitro inflammatory cell model was established by using primary fibroblasts isolated from mouse oral mucosal tissue.Gene transcription and protein expression related to cell proliferation, migration, and collagen synthesis were measured. Results:PE showed the highest cure rate of 85.7%for oral ulcers in mice within 7-days treatment. It significantly promoted cell proliferation and collagen secretion in fibroblast in-vitro model.PE also upregulated the gene transcription of Smad3,Col1a1 and Col3a1.Furthermore,PE antagonized the inhibitory effect of WP1066 on STAT3activation,but did not affect cell migration. Conclusion:This study demonstrates that PE has potential therapeutic effects in promoting oral ulcer healing in mice. The improvement was achieved by the increasing collagen synthesis through the upregulation of STAT3 and Smad3 in mucosa fibroblasts. These results provide a preliminary pharmacological work for developing PE as a potential treatment agent for oral ulcers.

文摘Our aim was to understand the actual conditions of oral care for intubated patients by new nurses. As infection control measures have revealed the acute needs of care for intubated patients, it is important for nursing students to learn about oral care and attitudes towards intubated patients, along with methods to make intubated patients more comfortable.

文摘This paper reports the treatment of eight cases with toe ulcers. The eight patients received nursing treatments, including debridement,dressings,decompression,anti-fungal management, and education from specialist nurses. The patients were also given drugs to control glucose, nourish the nerve, and improve blood circulation of the lower extremities. The toe ulcers of the eight diabetic patients healed after 42e168 days[average 85±36 days].Thus far,few studies have focused on diabetic toe ulcers.Timely identification of risk factors and regular toe care could decrease the prevalence of toe ulcers and shorten the recovery time in patients with diabetic mellitus.Hence, these patients must be diagnosis and treated early. Results suggest that feet, particularly toes, should be inspected and cared for regularly in diabetic patients.
文摘Objective:To evaluate the improvement of dysphagia and the daily amounts of oral fluids served among people who had recently experienced a cerebrovascular stroke(CVS)after applying a nursing intervention protocol. Methods:A quasi-experimental design was used to examine 60 stroke patients who were randomly and alternatively divided equally into a study group and a control group.Gugging Swallowing Screen(GUSS)and fluid balance chart were used.Within the first 24 h of a patient’s admission, expert nurses were trained in nursing intervention protocol to manage dysphagia and daily oral fluids.Results:Post-nursing intervention, the severity of dysphagia decreased among the study group more than in the control group.Additionally, the study group began taking greater amounts of fluids by the oral route than before the nursing intervention. Conclusions:A standardized nursing intervention protocol is needed to decrease the severity of dysphagia after CVS and increase the amount of daily oral fluids. The presence of a structured theory-based nursing intervention protocol for dysphagia management will greatly contribute to decreasing the consequences of dysphagia after CVS,and can also be expected to attenuate the effects of aspiration pneumonia, dehydration, and malnutrition;further,it also increases adherence to the protocol by both nurses and patients.
文摘In recent years, many foreign and domestic scholars have made a further study on the risk factors, high-risk groups, prevention and nursing of pressure ulcers. For all reasons such as clinical nursing workers and managers, new progresses can' t be well applied, and simultaneously pressure ulcers are misunderstood to some extent. These conditions are not conducive to the prevention and nursing of pressure ulcers and the improvement of patients' life quality and nursing service quality. Therefore, it is necessary to keep away from the misunderstanding, know well new progresses and reduce the incidence of pressure ulcers.

文摘Ulcerative colitis(UC), a chronic, relapsing, remitting disease of the colon and rectum, is characterized by inflammatory ulceration of the mucosa. This review will focus on the ability of newly developed oral drugs to induce and maintain MH in UC patients.

文摘Recurrent oral ulcer is a painful oral mucosal disorder that affects 20%of the world’s population. The lack of a radical cure due to its unknown underlying cause calls for innovative symptomatic treatments. This work reports a hyaluronic acid-based dissolvablemicroneedle patch(ROUMNpatch, short for recurrent oral ulcer microneedle)loaded with dexamethasone acetate, vitamin C and tetracaine hydrochloride for the treatment of recurrent oral ulcers. The ROUMN patch shows enhancement in both the anti-inflammatory effect elicited by dexamethasone and the pro-proliferation effect of vitamin C.In vitro experiments show that ROUMN has a higher efficiency in suppressing lipopolysaccharide(LPS)-induced interleukin-6(IL-6)expression than dexamethasone alone.Cell proliferation and migrationwere also significantly promoted byROUMNcompared to vitamin C alone. The healing-promoting effect of ROUMN was also verified in vivo using an acetic acid-cauterized oral ulcer model in rats.ROUMN as a treatment accelerated the healing process of oral ulcers, shortening the total healing time to 5 days compared with the 7 days required by treatment using watermelon frost,a commonly used over-the-counter(OTC)drug for oral ulcers. The rapid dissolution of the hyaluronic acid-based microneedles and the superior healing-promoting effect of the drug combination could lead to a broad application prospect of the ROUMN patch in the treatment of recurrent oral ulcers.
