Association of Body Mass Index with Hypothalamus-Pituitary-Ovarian Axis Hormones in Infertile Women in the Niger Delta Region, Nigeria

Background: Infertility is well-known global health problem that has significant impacts on an individual, families and communities. Many modifiable lifestyle risk factors increase the risk of women to several reproductive disorders. Aim: This study established the relationship between obesity and Hypothalamic-Pituitary-Ovarian (HPO) axis hormones in infertile women in the Niger Delta Region, Nigeria. Methodology: Six hundred and twenty-six (626) women aged 18 - 40 years comprising of 513 obese infertile women and 113 non obese women who served as control were recruited for the study. Anthropometric measurements were taken and Body Mass Index was calculated. A non-fasting venous blood sample was collected from the women and analyzed for serum Estrogen, Luteinizing Hormone (LH), Follicle Stimulating Hormone (FSH), Progesterone, Inhibin B, and Prolactin using Enzyme linked immunosorbent assay method. Results: In the present study, the Body Mass Index of women with primary (1°) infertility is significantly (p < 0.05) higher than secondary (2°) infertility women. Whereas, women with 2° infertility were older and have a higher height than women with 1° infertility. The result revealed that serum estrogen, luteinizing hormone, follicle stimulating hormone and prolactin levels were significantly (p < 0.05) higher in the obese infertile women, while inhibin B and progesterone levels were significantly (p < 0.05) reduced in the obese infertile women compared to the control subjects. However, women with 1° infertility have a significantly higher LH and FSH levels than the 2° infertility women. Furthermore, the study revealed that hyperestrogenism is the most prevalent gonadal disorder in women with primary infertility and secondary infertility. The BMI of infertile women suffering Hyperestrogenism is significantly higher than any other female gonadal disorder. The result also showed that there is statistically significant positive correlation between BMI and Hypogonadism, Hypogonadotropic and Amenorrhoea in obese infertility women. While, no significant correlation between BMI and Hypergonadism and Hypergonadotropic was observed. Furthermore, there was a positive correlation between BMI and Hypothalamus-Pituitary Ovarian hormones, as BMI showed a positive correlation with LH, FSH, Estrogen, progesterone, and prolactin in women with primary and secondary infertility, while Inhibin B showed a negative correlation with BMI. Conclusion: There is a relationship between BMI and Hypothalamus-Pituitary Ovarian hormones, signifying that obesity could affect female reproduction and directly impact ovarian function. Therefore, body weight maintenance should be considered as a first line of management of Hypothalamus-Pituitary Ovarian hormonal related infertility. 展开更多

目的研究神经内镜经鼻蝶窦手术与显微镜下经鼻蝶窦手术治疗垂体腺瘤患者的效果及对血清促肾上腺皮质激素(ACTH)、泌乳素(PRL)、生长激素(GH)水平的影响。方法选取2015年5月至2023年5月单县中心医院收治的80例垂体腺瘤患者进行随机对照... 目的研究神经内镜经鼻蝶窦手术与显微镜下经鼻蝶窦手术治疗垂体腺瘤患者的效果及对血清促肾上腺皮质激素(ACTH)、泌乳素(PRL)、生长激素(GH)水平的影响。方法选取2015年5月至2023年5月单县中心医院收治的80例垂体腺瘤患者进行随机对照试验。采用随机数字表法将其分为研究组和对照组,各40例。研究组男23例,女17例,年龄(50.29±5.25)岁。对照组男20例,女20例,年龄(51.09±5.21)岁。研究组给予神经内镜下经鼻蝶窦入路切除术治疗;对照组给予显微镜下经鼻蝶窦入路切除手术治疗。对比两组患者手术指标(手术用时、术中出血量、住院天数),肿瘤切除率,ACTH、PRL、GH水平,并记录两组并发症发生情况。采用独立样本t检验、χ^(2)检验。结果研究组手术用时、住院天数均短于对照组[(81.67±12.03)min比(114.35±11.68)min、(7.51±1.18)d比(11.07±1.24)d],术中出血量少于对照组[(72.59±8.14)ml比(85.26±8.31)ml],差异均有统计学意义(t=12.327、13.154、6.889,均P<0.01)。研究组完全切除率高于对照组[87.50%(36/40)比70.00%(28/40);χ^(2)=3.660,P=0.056]。术后7 d,研究组血清ACTH、PRL、GH水平均低于对照组[(15.37±4.19)pmol/L比(20.01±4.38)pmol/L、(1.33±0.48)nmol/L比(1.71±0.52)nmol/L、(0.88±0.31)nmol/L比(1.09±0.37)nmol/L],差异均有统计学意义(t=4.841、3.396、2.752,均P<0.05)。研究组并发症发生率为10.00%(4/40),低于对照组的27.50%(11/40),差异有统计学意义(χ^(2)=4.021,P=0.045)。结论神经内镜下入路切除垂体腺瘤较显微镜下入路更有利于促进患者术后激素水平的恢复,减少并发症发生。 展开更多

艾灸治疗排卵障碍性疾病的作用机制研究概述

目的:探讨神经内镜辅助下经鼻蝶窦手术治疗垂体瘤的效果及对嗅觉功能、神经内分泌激素水平的影响。方法:回顾性将我院2018年03月至2022年02月收治的垂体瘤患者58例作为此次的研究对象,其中采用显微镜辅助下进行经鼻蝶窦手术的28例作为... 目的:探讨神经内镜辅助下经鼻蝶窦手术治疗垂体瘤的效果及对嗅觉功能、神经内分泌激素水平的影响。方法:回顾性将我院2018年03月至2022年02月收治的垂体瘤患者58例作为此次的研究对象,其中采用显微镜辅助下进行经鼻蝶窦手术的28例作为对照组,神经内镜辅助下进行经鼻蝶窦手术的30例作为观察组,均观察至患者出院,并进行随访1年。比较两组围术期指标,术后1个月视力下降、头痛及性功能障碍缓解率,术前、术后1个月激素水平、T细胞亚群、生活质量评分、嗅觉功能,研究期间并发症发生情况,随访1年后复发情况。结果:与对照组比较,观察组的出血量相对更少,术后住院时间较短,肿瘤全切率相对更高;术后1个月,与对照组进行比较,观察组视力下降、头痛及性功能障碍缓解率均相对更高;相比术前,两组术后1个月血清泌乳素(PRL)、促甲状腺素(TSH)、生长激素(HGH)、促肾上腺皮质激素(ACTh)水平均降低,观察组低于对照组,外周血CD3^(+)、CD4^(+)水平、CD4^(+)/CD8^(+)值、社会功能、躯体功能、心理功能、物质生活状态各项生活质量评分术后1个月则均升高,观察组高于对照组;与术前比较,两组术后1个月嗅觉功能均升高,但观察组低于对照组;研究期间,观察组并发症总发生率3.33%低于对照组的25.00%;随访1年后,观察组的复发率0.00%同样低于对照组的14.29%(P<0.05)。结论:神经内镜辅助下经鼻蝶窦手术可有效提高垂体瘤患者的生活质量以及视力下降、头痛等临床症状缓解率,缩短住院时间,同时还能够改善患者的免疫功能以及嗅觉功能,稳定患者神经内分泌激素水平,降低并发症发生以及复发率。 展开更多

Intrinsic optical changes that follow infundibular stalk stimulation of the neurointermediate lobe of the mouse pituitary gland exhibit three different phases that reflect three distinct physiological events.The first(E-wave)is the rapid light-scattering increase that is associated with a nerve terminal volume increase(mechanical spike),and that accompanies excitation of the neurohy-pophysial terminals by the invading action potential;the second(S-wave)is the slower light-scattering decrease that is tightly corelated with the secretion of the peptide hormones oxytocin and arginine vasopressin,and the third is the long duration response(R-wave)that reflects cell volume changes in the pars interrmedia.We have studied the E-wave and the S-wave in earlier publications.The R-wave,considered here,is sensitive to chloride replacement as wel as to blockade of chloride channels.By blocking GABAA reeptors(which are ligand gated chloride channels)with pharrmacological agents,and by applying GABA directly into the bathing solution,or evoking its release from GABAergic inputs,we have demonstrated that this long-duration optical response is sensitive to chloride movements and reflects GABA-induced changes in the intrinsic optical propertis of the pars intermedia.The full time course of this optical response takes mimutes and,therefore,has to embody some other process(or processes)related to the restoration of resting physiological chloride concentrations,following the opening and closing of GABA A-reeptor channels.Here we demonstrate that the shape of the R wave,the longlasting light-scattering signal,is indeed affected by the activity of GAT1,one of the sodium-and.chloridedependent GA BA transporters. 展开更多

卡麦角林联合神经内镜辅助下经鼻蝶窦垂体瘤切除术治疗老年垂体瘤对PRL HGH及ACTh水平的影响

目的探讨卡麦角林与神经内镜下经鼻蝶窦垂体瘤切除术联用对老年垂体瘤患者的治疗效果及对泌乳素(PRL)、促肾上腺皮质激素(ACTh)、生长激素(HGH)的影响。方法选取2020-05—2023-05运城市中心医院收治的老年垂体瘤患者82例,随机分为对照... 目的探讨卡麦角林与神经内镜下经鼻蝶窦垂体瘤切除术联用对老年垂体瘤患者的治疗效果及对泌乳素(PRL)、促肾上腺皮质激素(ACTh)、生长激素(HGH)的影响。方法选取2020-05—2023-05运城市中心医院收治的老年垂体瘤患者82例,随机分为对照组41例,研究组41例。对照组采用神经内镜下经鼻蝶入路手术治疗,研究组在此基础上加用卡麦角林。比较2组总有效率、激素水平、炎症因子指标、生活质量评分及治疗安全性。结果研究组总有效率(92.68%)较对照组(75.61%)更高(P<0.05)。2组患者的激素水平均有明显改善,研究组PRL、ACTH、HGH水平显著低于对照组(P<0.05)。2组治疗后炎症因子水平均下降,研究组基质金属蛋白酶2(MMP-2)、Xklp2靶蛋白(TPX2)、P53蛋白水平显著低于对照组(P<0.05)。治疗后2组患者的生活质量均明显改善,研究组生活质量量表(SF-36)评分显著高于对照组(P<0.05)。2组治疗安全性无统计学差异(P>0.05)。结论对老年垂体瘤患者采用神经内镜下经典术式与卡麦角林联合治疗,可有效提高患者的生活质量,减轻炎症反应,效果较为理想,临床应用价值较高。 展开更多

目的对比经鼻蝶入路神经内镜下切除手术和显微镜下切除手术对垂体腺瘤(PA)患者的临床疗效和催乳素(PRL)、生长激素(GH)、促肾上腺皮质激素(ACTH)水平的影响。方法回顾性分析经鼻蝶入路切除手术的60例PA患者,按照手术切除方式的不同分成... 目的对比经鼻蝶入路神经内镜下切除手术和显微镜下切除手术对垂体腺瘤(PA)患者的临床疗效和催乳素(PRL)、生长激素(GH)、促肾上腺皮质激素(ACTH)水平的影响。方法回顾性分析经鼻蝶入路切除手术的60例PA患者,按照手术切除方式的不同分成显微组(28例)和内镜组(32例)。显微组行显微镜下切除手术,内镜组行神经内镜下切除手术。比较2组的肿瘤全切率、围术期指标(手术失血量、手术时长、住院时间)、激素(PRL、GH、ACTH)水平和手术后并发症发生情况等指标的变化。结果内镜组的肿瘤全切率为93.75%,高于显微组的71.43%(P<0.05);内镜组的手术失血量、手术时长、住院时间均低于显微组(P<0.05)。2组手术后的PRL、GH、ACTH水平均低于本组手术前,且术后内镜组的激素水平均低于显微组(P<0.05)。内镜组手术后的并发症发生率为9.38%,低于显微组的35.71%(P<0.05)。结论经鼻蝶入路神经内镜下手术切除垂体腺瘤的疗效较显微镜下切除更加显著,能改善患者围术期的各项指标,降低激素水平,减少术后并发症的发生。 展开更多

促性腺激素释放激素类似物治疗特发性中枢性性早熟女童的疗效分析

目的探讨促性腺激素释放激素类似物(GnRHa)治疗特发性中枢性性早熟女童的疗效及对体质量指数、性腺轴激素水平的影响。方法选取2021年3月至2023年5月在开平市中心医院生长发育专科诊断和治疗的42例特发性中枢性性早熟女童作为研究对象,... 目的探讨促性腺激素释放激素类似物(GnRHa)治疗特发性中枢性性早熟女童的疗效及对体质量指数、性腺轴激素水平的影响。方法选取2021年3月至2023年5月在开平市中心医院生长发育专科诊断和治疗的42例特发性中枢性性早熟女童作为研究对象,年龄(7.5±2.3)岁,病程(10.98±2.75)个月,均予以GnRHa治疗,以注射用醋酸曲普瑞林为主要治疗药物,肌肉注射,每28~32 d注射1次。第1、2、3针剂量均为3.75 mg(最大剂量不超过180μg/kg),个别体质量较高患儿可间隔21 d加强1针;于第3针治疗后第14天行GnRH简易激发试验,根据黄体生成素(LH)峰值结果调整剂量至80~100μg/kg(最大剂量不超过3.75 mg)。测量治疗前、治疗半年后患儿的身高、体质量、骨龄、预测成年终身高(FAH)等生长发育相关指标,比较治疗前后体质量指数的变化以及卵泡刺激素(FSH)、LH、雌二醇(E2)等激素水平的改变,观察治疗前后子宫容积、卵巢容积、卵泡直径的变化,记录不良反应发生情况。采用Wilcoxon符号秩和检验。结果治疗半年后,患儿第二性征明显抑制,其中8例患儿月经撤退,34例患儿乳房分期退缩;患儿的身高、体质量、骨龄、FAH均高于治疗前[131.1(8.7)cm比127.8(8.2)cm、29.5(6.9)kg比27.0(5.5)kg、9.4(1.7)岁比9.0(1.6)岁、157.4(3.7)cm比154.9(2.8)cm](Z=-5.648、-5.600、-5.713、-5.125,均P<0.001);体质量指数标准差分值(BMISDS)无明显变化[16.7(1.6)kg/m^(2)比16.5(1.4)kg/m^(2)](Z=-0.356,P=0.682);子宫容积、卵巢容积、卵泡直径均较治疗前缩小[2.1(1.6)ml比3.1(2.1)ml、1.2(0.9)ml比2.0(2.8)ml、3.0(1.3)mm比8.0(2.3)mm],差异均有统计学意义(Z=-5.646、-5.579、-5.622,均P<0.001)。第3针治疗后,E2水平、FSH激发峰值、LH激发峰值均较治疗前下降[36.5(10.0)pmol/L比80.0(44.8)pmol/L、2.6(3.6)U/L比13.3(9.3)U/L、0.7(0.3)U/L比12.9(11.6)U/L](均P<0.001)。治疗过程中1例患儿出现臀部脂肪纹,其余患儿在治疗过程中并未诉不适,血常规、肝肾功能等相关指标未见异常。结论GnRHa治疗特发性中枢性性早熟女童能改善FAH,对体质量指数无明显影响,能抑制性腺轴激素水平,具有较好的临床疗效和较高的安全性。 展开更多

Immunohistochemistry showed significantly reduced aquaporin 4 expression and adrenocorticotropic hormone expression in the pituitary gland of rabbits with craniocerebral injury. Aquaporin 4 expression was positively correlated with adrenocorticotropic hormone expression. Dopamine agonists effectively reduce the secretion of prolactin (PRL) in the great majority of prolactinomas and reduce the bulk of the adenomas, as well as have partial therapeutic effect on some patients with acromegaly. The inhibitory effect of bromocriptine (BC), a dopamine agonist, on growth hormone (GH) and PRL secretion of dispersed cells from the pituitary adenomas of 16 cases of acromegaly, which secret GH and PRL simultaneously, were evaluated in vitro. The significant inhibitory effects of BC on PRL secretion were found in 12 cases. It was also found that PRL secretion was strongly inhibited when GH was suppressed; on the contrary, when GH secretion was not suppressed, the production of PRL was not or weakly inhibited. The exact mechanism of the effects is nuclear so far. It is necessary to investigate, at molecular level, the etiology of GH-PRL adenomas and its response to therapeutic agents. 展开更多

An increasing amount of evidence demonstrates the anti-aging effect of Heshouwu in pill form. In this study, a subacute aging rat model was established by continuous intraperitoneal injection of D-galactose and treated with Heshouwu decoction (a Chinese herb for tonifying the kidney, comprising Heshouwu pill, Herba Epimedii, Radix Salviae Miltiorrhiae, and Poria). Heshouwu pill treated rats were the positive control group. Radioimmunoassay, immunohistochemical staining, and western blot assay showed hypothalamic gonadotropin-releasing hormone, hypothalamic substance P, and serum gonadotropin levels to be significantly increased in the model rats; the concentrations of hypothalamic ~3-endorphin, and serum levels of insulin-like growth factor I and testosterone were significantly decreased. 1713- and 3[3-hydroxysteroid dehydrogenase expression in testicular tissue was also decreased. Intragastric administration of Heshouwu decoction at high (9.6 g/mL/100 g), medium (4.8 g/mL/100 g), and low (2.4 g/mlJ100 g) doses, Heshouwu decoction pretreatment at a medium dose (4.8 g/mL/100 g), and Heshouwu pill (2.06 g/mL/100 g) significantly reversed these changes. Heshouwu decoction pretreatment and high-dose Heshouwu decoction had the greatest anti-aging effects. These experimental findings indicate that Heshouwu decoction can improve hypothalamic-pituitary-testicular axis secretion in a subacute aging rat model, and prevent and delay gonadal axis aging, with an effect superior to that of Heshouwu pill. 展开更多

However, it is unknown whether this regulatory effect is limited to the hypothalamus. OBJECTIVE: To detect the effect of different dosages of leptin on luteinizing hormone (LH) and follicle stimulating hormone (FSH) secretion from in vitro cultured rat anterior pituitary cells. DESIGN: Contrast study based on cells. SETTING: This study was performed in the Basic Institute of Chengde Medical College, Chengde City, Hebei Province, China from March to June 2007. MATERIALS: Eighteen female Wistar rats of three months of age, weighing 200-220 g, and of clean grade were used. Leptin was provided by Peprotech Company, DMEM culture medium by Invitrogen Company, and the radioimmunological kit by Beijing Zhongshan Jinqiao Biotechnology Co., Ltd. METHODS: Three glandular organs were regarded as one group for culture of anterior pituitary cells. In the control group, saline was added to the culture medium instead of leptin. In the leptin group, leptin was prepared into different concentrations of 1×10^-12, 1×10^-11, 1×10^-9, 1×10^-7, and 1×10^-6 mol/L for stimulation of cultured cells. The culture supernatant was obtained at three hours after additional of saline/leptin. MAIN OUTCOME MEASURES: Contents of LH and FSH were detected by radioimmunology. RESULTS: Following leptin stimulation, LH release increased with increasing concentrations of leptin up to 1×10^-9 mol/L, where LH release peaked. LH release then progressively decreased with increasing leptin concentrations (P 〈 0.01). LH release in the leptin (1×10^-12, 1×10^-11, 1×10^-7, and 1×10^-6 mol/L) groups was significantly higher than that in the control group (P 〈 0.01). FSH content in the leptin (1×10^-11, 1×10^-9, and 1×10^-7 mol/L) groups was significantly higher than that in the control group (P 〈 0.01). CONCLUSION: Leptin can directly affect pituitary tissue to promote the secretion of LH and FSH in a dose-dependent manner. 展开更多

Long-term gonadotropin-releasing hormone agonist(Gn RHa) administration before in vitro fertilization(IVF)/intracytoplasmic sperm injection(ICSI) in infertile women with endometriosis or adenomyosis significantly enhanced the chances of pregnancy in both fresh and frozen embryo transfer cycles. We hypothesized that long-term Gn RHa treatment might also be beneficial for the idiopathic repeated implantation failure(RIF) patients. In the 21 patients receiving Gn RHa and hormone replacement therapy(G-HRT) protocols for frozen embryo transfer, their data were compared with those of the 56 of frozen/fresh cycles they had previously undergone(previous protocols). Comparison showed that the finial results were significantly better with G-HRT protocols than with their previous protocols, with pregnancy rate, clinical pregnancy rate, implantation rate and on-going pregnancy rate being 70%, 60%, 40% and 38% respectively with G-HRT protocols, against 17%, 11%, 6.3% and 5% with previous protocols. The results showed that hormonally controlled endometrial preparation with prior Gn RHa suppression could be used for patients who had experienced repeated failures of IVF treatment despite having morphologically optimal embryos, and the treatment may help increase the receptivity of the endometrium in these patients. 展开更多

The effects of pituitary suppression with one-third depot of long-acting gonadotropin-releasing hormone(Gn RH) agonist in Gn RH agonist long protocol for in vitro fertilization(IVF)/intracytoplasmic sperm injection(ICSI) were investigated. A retrospective cohort study was performed on 3186 cycles undergoing IVF/ICSI with Gn RH agonist long protocol in a university-affiliated infertility center. The pituitary was suppressed with depot triptorelin of 1.25 mg or 1.875 mg. There was no significant difference in live birth rate between 1.25 mg triptorelin group and 1.875 mg triptorelin group(41.2% vs. 43.7%). The mean luteinizing hormone(LH) level on follicle-stimulating hormone(FSH) starting day was significantly higher in 1.25 mg triptorelin group. The mean LH level on the day of human chorionic gonadotrophin(h CG) administration was slightly but statistically higher in 1.25 mg triptorelin group. There was no significant difference in the total FSH dose between the two groups. The number of retrieved oocytes was slightly but statistically less in 1.25 mg triptorelin group than in 1.875 mg triptorelin group(12.90±5.82 vs. 13.52±6.97). There was no significant difference in clinical pregnancy rate between the two groups(50.5% vs. 54.5%). It was suggested that one-third depot triptorelin can achieve satisfactory pituitary suppression and produce good live birth rates in a long protocol for IVF/ICSI. 展开更多

To investigate the difference of biochemical characteristics on gsp positive and gsp negative growth hormone (GH) secreting pituitary tumors, 18 GH secreting pituitary tumors were examined for their clinical characteristics and gsp oncogenes. All patients received the pituitary function combinative stimulating test. It was found that there were no difference in the sex, age, tumor size, course of disease and plasma basal GH levels with gsp positive and gsp negative patients. The plasma levels of PRL were increased in most patients (11/18), and the plasma levels of TSH in gsp positive patients were higher than those in gsp negative patients ( P <0.05). There was no significant difference in the responses to pituitary combinative stimulating test in gsp positive and gsp negative patients. It was concluded that there was little difference in the clinical biochemical characteristics of gsp positive with gsp negative GH secreting pituitary tumors. 展开更多

Pituitary stalk interruption syndrome(PSIS)is a rare congenital abnormality characterized by thinning or disappearance of the pituitary stalk,hypoplasia of the anterior pituitary and an ectopic posterior pituitary.Although the etiology of PSIS is still unclear,gene changes and perinatal adverse events such as breech delivery may play important roles in the pathogenesis of PSIS.PSIS can cause multiple hormone deficiencies,such as growth hormone,which then cause a series of changes in the human body.On the one hand,hormone changes affect growth and development,and on the other hand,they could affect human metabolism and subsequently the liver resulting in nonalcoholic fatty liver disease(NAFLD).Under the synergistic effect of multiple mechanisms,the progression of NAFLD caused by PSIS is faster than that due to other causes.Therefore,in addition to early identification of PSIS,timely hormone replacement therapy and monitoring of relevant hormone levels,clinicians should routinely assess the liver function while managing PSIS. 展开更多

In order to investigate the relationship between abnormal intracellular signal transduction and tumorgenesis of human pituitary somatotrophinomas, the effects of protein kinase A (PKA) dependent growth hormone (GH) releasing hormone (GHRH) and protein kinase C (PKC) dependent GH releasing peptide (GHRP 6) on cAMP production were observed by using cell culture and biochemical methods, and the expression of the gsp oncogene was detected by using PCR and direct sequence assay methods in 11 patients with human pituitary somatotrophinomas. It was found that GHRP 6 exerted significant stimulatory effect on cAMP production by 2 gsp positive tumors and no effect on the gsp negative tumors. GHRP 6 could enhance the stimulation of cAMP production induced by GHRH in tumor without gsp oncogenes. It was suggested that both GHRH and GHRP 6 exert identical effects on human pituitary soamtotrophinomas, which was contributed to the cross talk between the two intracellular signal transduction pathways in pituitary cells. 展开更多
