BACKGROUND Venous variations are uncommon and usually hard to identify,and basilic vein variation is particularly rare.Basilic vein variation usually presents without any clinical symptoms and is often regarded as a benign alteration.This case was a patient with congenital basilic vein variation encountered during surgery for an infusion port.CASE SUMMARY We documented and analyzed an uncommon anatomical variation in the basilic vein encountered during arm port insertion.This peculiarity has hitherto remained undescribed in the literature.We offer remedial strategies for addressing this anomaly in the future and precautionary measures to circumvent its occurrence.We conducted a comprehensive review of analogous cases in the literature,offering pertinent therapeutic recommendations and solutions,with the aim of enhancing the efficacy and safety of future arm port implantations.CONCLUSION Venous variation is rare and requires detailed intraoperative and postoperative examination to ensure accuracy,so as not to affect subsequent treatment. 展开更多

Itshould respect the historical evolution process of river courses, and highlight the ecological service functionand leisure tourism value of river courses from the coordination perspective of urban and rural ecologicalenvironment, economic industries, society and culture in the planning ideas of ecology, production, andlife integration. Impact of coupling coordination development of port and city environments on urban economic competitiveness:Evidence from the coastal port cities in eastern China

Promoting the coupling coordination development of port and its hinterland city environments is an important way to improve urban economic competitiveness.Based on relevant data of 13 coastal port cities in eastern China from 2000 to 2018,this study explores the coupling coordination development of port and city environments and its impact on urban economic competitiveness by constructing the coupling coordination degree model and the panel threshold model.The research results show that:(1)In terms of the coupling coordination development of port and city environments,most coastal ports and their hinterland cities are in a state of moderate or serious disorder.Overall,the degree of coupling coordination of port and city environments needs to be further improved;(2)The coupling coordination degree of port and city environments has a significant impact on urban economic competitiveness,and this effect gradually increases with the development of the ports and the urban economy.Among the variables that impact the urban economic competitiveness,fixed assets investment and foreign trade are significant factors that can enhance urban economic competitiveness.(3)At present,there is a“U-shaped”relationship between the coupling coordination degree of port-city environments and the urban economic competitiveness.This relationship lies on the right side of the inflection point of the“U-shaped”curve.Therefore,following the concept of assigning priority to ecological development,expanding fixed assets investment and actively developing foreign trade can further enhance the urban economic competitiveness. 展开更多

Seroprevalence Survey of HIV and Hepatitis B Virus and Behavioral Characteristics among Heavy Truck Drivers along Port Sudan-Khartoum Highways

The prevalence of human immunodeficiency virus (AIDS) and hepatitis B virus among heavy truck drivers and their assistants has been well documented globally in correlation with their behavioral characteristics. The present study aimed to screen for human immunodeficiency virus (HIV), hepatitis B virus (HBV), and behavioral characteristics among heavy truck drivers in Port Sudan. A cross-sectional study was conducted on 274 heavy truck drivers and their assistants who used the highway Port Sudan-Khartoum in Port Sudan city during 2019-2021. Data on behavioral characteristics and substance use habits were collected using a structured questionnaire, and an ELISA test was used to screen for HIV and HBV infections in the study participants. The chi-square test, odds ratio, and confidence intervals were used to find the association between behavioral characteristics and seropositive HIV/HBV. Of the 274 enrolled participants, the seroprevalence rates of HIV were 2.7% and HBV was 23.7%. Ninety-four (34.3%) of them had a history of high-risk sexual behavior outside of marriage;only two (0.7%) used condoms;14.2% of participants reported alcohol use;and 1.1% reported drug use. Univariate analysis revealed that having a sex history outside of marriage with ≥1 sex partner and never using a condom with a spouse or casual partner were significant risk factors for HIV and HBV among drivers. Fortunately, we found that most of the drivers reported low alcohol and drug use. Concerning this study, the seroprevalence of HIV and HBV is highly associated with a history of having sex outside of marriage and sexual behavior among truck drivers and assistances. Additional studies are needed to further investigate other STIs and behavioral characteristics associated with factors in truck drivers/assistance in different truck stop regions in Sudan. 展开更多

Overcoming the global sustainability challenges of logistics requires applying solutions that minimize the negative effects of logistics activities.The most efficient way of doing so is through intermodal transportation(IT).Current IT systems rely mostly on road,rail,and sea transport,not inland waterway transport.Developing dry port(DP)terminals has been proven as a sustainable means of promoting and utilizing IT in the hinterland of seaport container terminals.Conventional DP systems consolidate container flows from/to seaports and integrate road and rail transportation modes in the hinterland which improves the sustainability of the whole logistics system.In this article,to extend literature on the sustainable development of different categories of IT terminals,especially DPs,and their varying roles,we examine the possibility of developing DP terminals within the framework of inland waterway container terminals(IWCTs).Establishing combined road–rail–inland waterway transport for observed container flows is expected to make the IT systems sustainable.As such,this article is the first to address the modelling of such DP systems.After mathematically formulating the problem of modelling DP systems,which entailed determining the number and location of DP terminals for IWCTs,their capacity,and their allocation of container flows,we solved the problem with a hybrid metaheuristic model based on the Bee Colony Optimisation(BCO)algorithmand themeasurement of alternatives and ranking according to compromise solution(i.e.,MARCOS)multi-criteria decision-making method.The results from our case study of the Danube region suggest that planning and developingDP terminals in the framework of IWCTs can indeed be sustainable,as well as contribute to the development of logistics networks,the regionalisation of river ports,and the geographic expansion of their hinterlands.Thus,the main contributions of this article are in proposing a novel DP concept variant,mathematically formulating the problems of its modelling,and developing an encompassing hybrid metaheuristic approach for treating the complex nature of the problem adequately. 展开更多

Impact of central venous port implantation method and access choice on outcomes

BACKGROUND Although the number of patients who need central venous ports for permanent vascular access is increasing,there is still no“gold standard”for the implantation technique.AIM To identify the implantation technique that should be favored.METHODS Two hundred central venous port-implanted patients in a tertiary hospital were retrospectively evaluated.Patients were assigned into two groups according to the access method.The first group comprised patients whose jugular veins were used,and the second group comprised patients whose subclavian veins were used.Groups were evaluated regarding age,sex,application side,primary diagnosis,active follow-up period in the hospital,chemotherapy agents administered,number of complications,and the Clavien-Dindo severity score.The distribution of the variables was tested with the Kolmogorov-Smirnov test and the Mann-Whitney U test.Theχ^(2) test was used to analyze the variables.RESULTS There was no statistically significant difference between the groups regarding age,sex,side,number of chemotherapy drugs,and duration of port usage(P>0.05).Only 2 patients in group 1 had complications,whereas in group 2 we observed 19 patients with complications(P<0.05).No port occlusion was found in group 1,but the catheters of 4 patients were occluded in group 2.One port was infected in group 1 compared to three infected ports in group 2.Two port ruptures,two pneumothorax,one revision due to a mechanical problem,one tachyarrhythmia during implantation,and four suture line problems were also recorded in group 2 patients.We also showed that it would be sufficient to evaluate and wash ports once every 2 mo.CONCLUSION Our results robustly confirm that the jugular vein route is safer than the subclavian vein approach for central venous port implantation. 展开更多

BACKGROUND Implanted intravenous infusion port(TIAP)is mainly used for patients who need central venous infusion and poor peripheral vascular conditions.With the advantages of easy to carry,long maintenance cycle,few complications and excellent quality of life,it has been widely used in the fields of malignant tumor chemotherapy,parenteral nutrition support and repeated blood collection.Implanted intravenous infusion port(IVAP)dislocation can have significant complications if not recognised and reinstated immediately.CASE SUMMARY A 24-year-old man was treated with adjuvant chemotherapy for osteosarcoma.Severe displacement of IVAP catheter was found by chest X-ray examination.The IVAP cannot be used normally.Therefore,we conducted an emergency procedure to reset the catheter through double pigtail catheters,the operation was successful and the infusion port was restored.CONCLUSION When IVAP catheter displacement cannot be reset by conventional techniques,two pigtail catheters can be successfully used instead. 展开更多

目的:探讨自制Port入路平台经脐单孔与传统多孔腹腔镜手术对异位妊娠患者手术效果、腹壁美观及满意度的影响。方法:将160例在我院行腹腔镜手术治疗的异位妊娠患者根据不同术式分为两组,各80例。传统组给予传统多孔腹腔镜手术,单孔组实... 目的:探讨自制Port入路平台经脐单孔与传统多孔腹腔镜手术对异位妊娠患者手术效果、腹壁美观及满意度的影响。方法:将160例在我院行腹腔镜手术治疗的异位妊娠患者根据不同术式分为两组,各80例。传统组给予传统多孔腹腔镜手术,单孔组实施自制Port入路平台经脐单孔腹腔镜手术。两组术后均随访3个月,比较两组手术完成率、手术情况(手术时间、术中出血量、取出标本时间、肛门排气时间、肠道功能恢复时间)、术后不同时间疼痛程度(术后6h、24h、48h的VAS评分)及腹壁美容效果(术后1周及术后3个月的BIS、CS评分),比较两组患者并发症发生情况及满意度。结果:两组手术完成率及手术时间比较,差异无统计学意义(P>0.05)。单孔组术中出血量少于传统组,取出标本时间、肛门排气时间及肠道功能恢复时间均短于传统组(P均<0.05);单孔组术后6h、24h、48h VAS评分均低于传统组(P<0.05);单孔组术后1周及术后3个月的CS评分、BIS评分均明显高于传统组(P<0.05);单孔组并发症发生率低于传统组,而总满意率高于传统组(P<0.05)。结论:异位妊娠患者采用自制Port入路平台经脐单孔腹腔镜术安全可靠,较传统多孔腹腔镜术可进一步改善手术情况,减轻患者疼痛,降低并发症发生风险,同时可兼顾微创与美学的优点,提高患者腹壁美容效果,患者满意度更高,值得临床应用与推广。 展开更多

While most of the research addresses this issue by highlighting the need for re-positioning and redefining the role of the port within the supply chains, not much attention has been given to the structure, scope and reach as well as to the tools of regulation hat competent authorities could exploit in order to avert potential abusive actions of global powerful actors along the containerized transport chain, that would affect the competitive functioning of distinct segment or the integrity of supply chains. In this context, we propose the reconceptualization and reinforcement of the regulatory governance structures of ports on a national and international level as well as the extension of the scope of ports’ regulations along the supply chain to counterbalance the globalization, consolidation and integration of containerized transport actors. 展开更多

The port railway operation in Germany and Europe is not least due to its long history of technological and regulatory developments characterized by a high number of players and a lack of comprehensive planning and optimization of all relevant processes.Innovative technologies and business processes are therefore useful and needed to achieve key steps on the way to an overall optimization of rail transport within global supply chains.A substantial part of the European rail freight transport has its origin or its destination in an inland or sea port.Considering the railway system,ports play a more important role for urgently needed innovations than the pure interfaces between sea and land transport.Especially ports with their own railway system have an own responsibility in this matter.Insofar the goal of the Bremen Port Railway—which already now has a leading share of railway in modal split in Europe—is to exploit the opportunities offered by digitalization.Significant steps for this are the optimization and gradually automatization of rail operational processes on the last mile including modern and transparent IT systems and the designing of autonomous shunting processes.Together with research partners ISL(Institute of Shipping Economics and Logistics Bremen),BIBA(Bremen Institute for Production and Logistics)and IVE(Institute for Transport,Railway Construction and Operation in Brunswick)and in connection with associated business partners the project Rang-E has been applied for at the Ministry of Transport in the funding initiative IHATEC(Innovated HArbour TEChnologies)—and had won a grant to perform the proposed work.Basic thoughts are outlined in the following. 展开更多

As a consequence, the number of African shipping lines has been severely reduced. International trade of the WCA countries remains weak, limited to 30% of their Gross Domestic Product (GDP). The goal of this paper is to provide a comprehensive overview of the port and shipping convey management in the WCA region, and of course to describe what has been achieved since then and what still have to be done to better its infrastructures and port community. 展开更多

目的探讨经单一切E1腹腔镜手术系统(SILS—Port系统)胸、腹腔镜联合在食管癌根治术中的应用。方法2011年8月~2013年5月我科经SILS—Port系统施行胸、腹镜联合食管癌根治术5例。肩胛听诊三角区第6肋间为观察孔,胸腔镜下两操作孔游离... 目的探讨经单一切E1腹腔镜手术系统(SILS—Port系统)胸、腹腔镜联合在食管癌根治术中的应用。方法2011年8月~2013年5月我科经SILS—Port系统施行胸、腹镜联合食管癌根治术5例。肩胛听诊三角区第6肋间为观察孔,胸腔镜下两操作孔游离胸段食管及清扫淋巴结,通过SILS系统,腹腔镜下两孔游离胃及清扫淋巴结,胃上提至颈部与食管吻合。结果手术过程顺利,围手术期无死亡。腔镜手术时间(296.0±24.1)min,术中出血量(114±40)ml,术后胸、腹腔引流时间均是(1.2±0.4)d。术后第3天疼痛评分(2.2±0.8)分。5例分别清扫淋巴结9、8、6、5、7枚,其中2枚阳性。5例分别随访10、22、8、16、3个月,1例术后16个月肿瘤复发转移,无死亡。结论经SILS-Port系统胸、腹腔镜联合根治性切除早期、无明显外侵的中下段食管癌,创伤小,出血少,安全可靠。 展开更多

SPI是一种高速、全双工、同步的串行通信接口.由于其简单易用的特性,现在越来越多的外围芯片集成了这种通信协议.但在实际应用中有相当一部分MCU因为自身不具备SPI接口而限制了其在SPI总线器件中的使用,出于产品体积、成本和可扩展性等方面的考虑,设计人员往往希望使用不具备SPI接口的MCU去控制具备SPI接口的外围器件.笔者介绍了SPI总线的特征和时序,给出了利用I/O Port软件方式模拟三线SPI通信时序以实现串行通信的方法和程序流程. 展开更多

经脐单孔SILS-Port腹腔镜结肠次全切除术治疗慢传输型便秘10例报告 被引量:17

目的探讨经脐单孔SILS-Port腹腔镜结肠次全切除术治疗慢传输型便秘的安全性与可行性。方法 2014年12月~2015年6月我院采用经脐单孔SILS-Port腹腔镜结肠次全切除术治疗慢传输型便秘10例,记录术后疼痛及腹壁美容满意度评分,采用Wexner便... 目的探讨经脐单孔SILS-Port腹腔镜结肠次全切除术治疗慢传输型便秘的安全性与可行性。方法 2014年12月~2015年6月我院采用经脐单孔SILS-Port腹腔镜结肠次全切除术治疗慢传输型便秘10例,记录术后疼痛及腹壁美容满意度评分,采用Wexner便秘评分、便秘症状严重程度评分和胃肠生活质量指数评价便秘治疗的疗效。结果均顺利完成手术,手术时间(217±35)min,术中出血量(36±11)ml。均无术后出血、吻合口漏、腹腔感染、肠梗阻及肛门失禁等并发症发生。首次排气、排便时间分别为(2.4±0.8)d和(2.9±0.6)d,术后切口疼痛VAS评分(3.3±0.5)分,术后腹壁美容满意度评分(4.3±0.5)分。术后6个月Wexner便秘评分(4.5±1.0)分,显著低于术前(22.0±1.9)分(t=26.087,P=0.000);便秘症状严重程度评分(5.5±0.5)分较术前(23.2±0.9)分,明显降低(t=52.836,P=0.000);胃肠生活质量指数(105.7±3.9)分,较术前(77.4±6.0)分明显提高(t=-12.457,P=0.000)。结论经脐单孔SILS-Port腹腔镜结肠次全切除术治疗慢传输型便秘安全可行,腹壁美容效果好,近期疗效满意。 展开更多
