Practice of Infection Prevention and Control Strategies in Risk Departments during the COVID-19 Epidemic

Objective: To explore the practice and application of infection prevention and control strategies in risk departments during the COVID-19 epidemic, and to formulate the infection prevention and control measures to provide advice and guidance in risk departments. Methods: According to the latest plan of diagnosis and treatment, prevention and control issued by the National Health Commission, expert advice and consensus, combined with the actual situation in our hospital, a series of infection prevention and control measures of COVID-19 in risk department was formulated. Results: During the epidemic period, the prevention and control measures of nine risk departments including emergency operation, anesthesiology, endoscopy center, blood purification center, otolaryngology, stomatology, medical imaging department, medical cosmetology department and pulmonary function room were established from six aspects, including pre-examination and screening, medical technology control, personnel management, personal protection, environmental disinfection, medical waste disposal, etc. Conclusion: During the epidemic period, the infection prevention and control strategy of risk departments is one of the key links to control the spread of the epidemic, and risk departments must pay attention to and strictly implement various infection prevention and control measures.

Research of Risk Identification and Prevention of Underground Pressure Pipelines Damage Caused by External Disturbance

External disturbance is an important cause of underground pressure pipeline damage,which leads to accidents,and it is crucial to study the risk of damage caused by external disturbance and come up with proper prevention and control measures.We reviewed literature on risk identification of underground pressure pipelines damage due to external disturbance was conducted,and a list of risk factors was formed.Based on the list of risk factors,fault tree analysis was carried out on underground pressure pipelines damage caused by external disturbances,and risk prevention and control measures were proposed through the calculation of minimum cut sets,minimum path sets,and structural importance,in hopes of providing reference for the normal operation of underground pressure pipelines.

Risk prevention and control strategies for the severely affected areas of snow disaster in the Three Rivers Source Region(TRSR), China

Historically,frequent and heavy snow disaster(SD)has caused serious livestock death and casualties,resulting in a devastating impact on animal husbandry development in the Three Rivers Source Region(TRSR).From winter in 2018 to spring in 2019,the largest SD occurred in this area over the past 10 years,especially in core zones of the Lancang River Source Region.Field research results show that the main causes of the major SD include weak infrastructure(i.e.,roads,communications,warm sheds,and insufficient forage reserve),low rate of domestic animals for sale before the SD,and low loss settlement rate.SD occurrence could furtherly reduce the ability of disaster prevention,mitigation and relief of disaster loss.In the future,heavily affected SD areas should improve the forecasting ability of snowfall incidents,strengthen infrastructure construction,implement grass and livestock balance strategies,optimize livestock structure,improve loss settlement rate,and develop a modern compound model of animal husbandry development model that combines breeding,slaughtering and deep processing of animal product.

Prevention and Control Measures of Environmental Pollution Problem Caused by Dairy Farming in Inner Mongolia

Environmental pollution problem generated in dairy farming process is the bottleneck restricting development of animal husbandry. By using the investigation data from dairy farmers of Inner Mongolia,environmental pollution problem generated by dairy farming in Inner Mongolia and its cause were analyzed,and prevention and control measures were put forward. The research result showed that weak technology support of environmental treatment and environmental-protection awareness of dairy farmer and bad application effect of pollution control technology caused by lower education degree of dairy farmer were main reasons for environmental pollution problem. We should develop dairy circular economy,advocate a moderate scale,scientifically breed,and enhance training of pollution control technology and propaganda to protect environment.

On this basis,it analyzed the causes of serious rural environmental pollution in China,Finally,it came up with recommendations including establishing a sound rural environmental governance system,making diversified capital investment for nurturing " green farmers".

Clinical Research of Pre-Hospital Emergency Care, Nursing, Infection Prevention and Control for Senile Osteoporotic Vertebral Compression Fracture during Epidemic Period

Background: Novel coronavirus pneumonia (NCP) and osteoporotic vertebral compression fractures (OVCF) are the high incidences of diseases in the elderly. During the epidemic period, if not treated in time, the complications are high and the mortality is high. If we do not pay attention to infection prevention and control in pre-hospital emergency care, it will lead to the first time infection of medical staff and in-hospital cross infection in emergency outpatient receiving area. The correct consideration of both and the establishment of perfect pre-hospital emergency treatment and infection prevention and control synchronous strategy is an important premise to ensure the stable, orderly and safe medical treatment. Objective: To explore the effect of synchronous implementation of pre-hospital emergency care, nursing and infection pre-vention and control for senile OVCF during the epidemic. In order to improve the efficiency of pre-hospital emergency care and prevent the spread of infection. Method: A total of 92 elderly patients with OVCF who received pre-hospital treatment in 18 hospitals in Zhangjiakou City during the epidemic prevention and control period from January 2020 to November 2022 and met the inclusion criteria were selected as research objects, including 24 males and 68 females, aged 65 - 82 (74.2 ± 2.2) years. All patients were associated with concomitant injuries and underlying diseases. All patients in this group underwent predictive pre-hospital rescue and infection prevention and control procedures. Results: All the 92 elderly patients with OVCF received timely pre-hospital treatment during the epidemic period, and no aggravation occurred of the 92 patients, 35 were in the high risk area, 10 were in the medium risk area, and 47 were in the low risk area. Exclude OVCF for NCP Patients were treated according to the conventional diagnosis and treatment principles. Suspected and confirmed cases are transferred to designated surgical hospitals for treatment. All patients were followed up 3 months, 6 months and 12 months after treatment. There was no death rate, high satisfaction of pre-hospital first aid, high diagnostic accuracy, and good curative effect. None of the rescue personnel had any infection rate, and no hospital infection transmission and nosocomial cross infection occurred. Conclusion: It is the first step to safely treat patients and prevent cross infection to establish a perfect synchronous strategy of pre-hospital first aid and infection prevention and control.

In the context of epidemic prevention and control,villages are the weakest part of epidemic prevention and control.Traditional villages,as representatives of villages,are often far away from cities,and medical conditions are relatively backward.Based on this background,epidemic prevention and control are particularly important.The quality of the environment is positively related to the spread of the virus.This article wanted to use CFD technology to simulate the wind environment of the traditional village street space,so as to propose measures to optimize the wind environment of the traditional village street space,and provide references for the future planning of village layout.

Enlightenment from the COVID-19 Pandemic:The Roles of Environmental Factors in Future Public Health Emergency Response

The coronavirus disease 2019(COVID-19)pandemic is challenging the current public health emergency response systems(PHERSs)of many countries.Although environmental factors,such as those influencing the survival of viruses and their transmission between species including humans,play important roles in PHERSs,little attention has been given to these factors.This study describes and elucidates the roles of environmental factors in future PHERSs.To improve countries’capability to respond to public health emergencies associated with viral infections such as the COVID-19 pandemic,a number of environmental factors should be considered before,during,and after the responses to such emergencies.More specifically,to prevent pandemic outbreaks,we should strengthen environmental and wildlife protection,conduct detailed viral surveillance in animals and hotspots,and improve early-warning systems.During the pandemic,we must study the impacts of environmental factors on viral behaviors,develop control measures to minimize secondary environmental risks,and conduct timely assessments of viral risks and secondary environmental effects with a view to reducing the impacts of the pandemic on human health and on ecosystems.After the pandemic,we should further strengthen surveillance for viruses and the prevention of viral spread,maintain control measures for minimizing secondary environmental risks,develop our capability to scientifically predict pandemics and resurgences,and prepare for the next unexpected resurgence.Meanwhile,we should restore the normal life and production of the public based on the“One Health”concept,that views global human and environmental health as inextricably linked.Our recommendations are essential for improving nations’capability to respond to global public health emergencies.

Association between Prenatal Environmental Factors and Child Autism: A Case Control Study in Tianjin,China

Objective To investigate the association between autism and prenatal environmental risk factors.Methods A case-control study was conducted among 193 children with autism from the specialeducational schools and 733 typical development controls matched by age and gender by usingquestionnaire in Tianjin from 2007 to 2012. Statistical analysis included quick unbiased efficientstatistical tree （QUEST） and logistic regression in SPSS 20.0.Results There were four predictors by QUEST and the logistic regression analysis, maternal airconditioner use during pregnancy （OR=0.316, 95% CI： 0.215-0.463） was the single first-level node（χ2=50.994, P=0.000）; newborn complications （OR=4.277, 95% CI： 2.314-7.908） and paternalconsumption of freshwater fish （OR=0.383, 95% CI： 0.256-0.573） were second-layer predictors（χ2=45.248, P=0.000; χ2=24.212, P=0.000）; and maternal depression （OR=4.822, 95% CI： 3.047-7.631）was the single third-level predictor （χ2=23.835, P=0.000）. The prediction accuracy of the tree was 89.2%.Conclusion The air conditioner use during pregnancy and paternal freshwater fish diet might bebeneficial for the prevention of autism, while newborn complications and maternal depression might bethe risk factors.

Interaction of methylenetetrahydrofolate reductase C677T,cytochrome P4502E1 polymorphism and environment factors in esophageal cancer in Kazakh population

AIM： To evaluate the association and interaction of genetic polymorphisms in methylenetetrahydrofolate reductase （MTHER） and cytochrome P4502E1 （CY- P4502E1）, environment risk factors with esophageal cancer （EC） in Kazakh, a high EC incidence area of Xinjiang Uygur Autonomous Region, China. METHODS： A 1：2 matched case-control study was conducted with 120 cases of EC and 240 populationor hospital-based controls. The controls were matched for sex, nationality, area of residence and age within a 5-year difference. MTHER and CYP4502E1 genotypes were identified by PCR-based restriction fragment length polymorphism （RFLP）. A conditional logistic regression model was established to identify risk factors. The strata method was adopted in interaction analysis. RESULTS： Low consumption of green vegetables and fresh fruits, alcohol drinking, and unsafe water （shallow well, or river） were found to be the risk factors for EC. Individuals with the MTHFR677 （C/T ＋ T/T） genotype had a 2.62-fold （95% CI： 1.61-4.28） risk of developing EC compared with those who carried the C/C genotype. Individuals with the CYP4502EIC1/C1 genotype had a 3.00-fold （95% CI： 1.82-4.96） risk compared with those who carried the CYP4502E1 （C1/C2 ＋ C2/C2） genotype. Gene-environment interaction analysis showed that MTHFR677 gene polymorphism was correlated with consumption of green vegetables and fresh fruit, while CYP4502E1 C1/C1 was correlated with alcohol drinking and unsafe drinking water. MTHFR and CYP4502E1 analysis of gene-gene interaction showed that individuals with the MTHFR677 （C/T ＋ T/T） and CYP4502EIC1/ C1 genotypes had a 7.41-fold （95% CI： 3.60-15.25） risk of developing EC compared with those who carried the MTHFR677C/C and CYP4502E1 RsaI C1/C2 ＋ C2/C2 genes, and the interaction rate was higher than that of the two factors alone. CONCLUSION： Low consumption of green vegetables and fresh fruits, alcohol drinking, and unsafe water （shallow well, or river） and polymorphisms in MTHFR and CYP4502E1 genes are important risk factors for EC. There is a synergistic interaction among polymorphisms in MTHFR and CYP4502E1 genes and environment factors. MTHFR and CYP4502E1 genes can be used as biomarkers for prevention of EC in Kazakh, Xinjiang Uygur Autonomous Region, China.

Research on Subcontractor Risk of EPC Hydropower Project in Vietnam

Engineering, procurement and construction (EPC) contracts are becoming the most popular form to undertake construction works on large-scale hydropower projects.?The EPC general contractors in Vietnam have been coped with lots of difficulties during the construction phase of hydropower projects, resulting in significant schedule delays and cost overrun. One of the reasons is poor Subcontractors capacity.?In order to overcome above, the current study attempts?to research on identification and control Subcontractors risks of delay process of EPC hydropower project in Vietnam.?Through summarizing international research achievements of previous researches relating on EPC hydropower projects combining with analysis characteristics of developing construction of EPC hydropower projects in Vietnam, the current study pointed out all kinds of subcontractor risk of delay process of EPC hydropower project in Vietnam;?a questionnaire with 18 risk elements was designed and then delivered to experts in EPC hydropower project management area. Based on the risk model, 11 main risk elements were identified which can be categorized into 3 groups.?Lastly, the current study proposed suggestions to reduce or avoid risks as well as approach to control risks.?Furthermore, the findings of this study can be a good managerial reference for project management teams who are working in other countries that have similar economic and investment environment to Vietnam to get their EPC construction project completed on schedule, thus ensuring the benefits to the government, the investor, the contractor, and the parties involved.

Research on Quality Risk of EPC Hydropower Projects in Vietnam

Engineering, procurement and construction (EPC) contracts are on the way to become the most usual form of contracting applied by the private sector to undertake construction works on large scale Hydropower projects. If EPC contract management is not implemented and controlled properly. Quality is the first important factor for any contractors who involve in the hydropower project execution. The purpose of this study is to investigate and evaluate risks causing not good quality of EPC hydropower projects in Vietnam. In addition, the study also provided the recommendations for project practitioners to mitigate the impacts of those identified risks. The findings of this study can be a good managerial reference for project management teams who are working in other countries that have similar economic and investment environments to Vietnam to get their EPC construction project completed on quality, thus will ensuring the benefits to the government, the investors, the contractors, and the parties involved.
