Preliminary Study on the Construction of Evaluation Index System for Clinical Medicine Postgraduates Based on Job Competency

Objective:To evaluate the practicality of constructing multiple evaluation index systems for postgraduates in clinical medicine professional degree based on job competency.Methods:The theoretical framework of the evaluation index system was initially developed using expert consultation,literature review,and other methods.20 survey experts were selected and consulted using the Delphi method to screen and evaluate the weights of multiple indicators.Data was then entered into a table to evaluate the practicality of the constructed indicators.Results:The questionnaire recovery rate of the first round of expert consultation was 92.50%,and the second round was 100.00%.The comparison between the two groups showed P<0.05.The authority level of the first round was 0.817,and the second round was 0.811.In the first round of coordination,the chi-square value of the first-level indicators was 0.498,and the chi-square value of the second-level indicators was 0.628.In the second round of coordination,the chi-square value of the first-level indicators was 0.573,and the chi-square value of the second-level indicators was 0.634.The comparison between the two rounds showed P<0.05.The evaluation index system included 2 first-level indicators,5 second-level indicators,and 31 third-level indicators.Conclusion:Constructing an evaluation index system based on job competency is highly scientific,with a reasonable construction process and high practical value. 展开更多

Remarkable progress has been made in infection prevention and control(IPC)in many countries,but some gaps emerged in the context of the coronavirus disease 2019(COVID-19)pandemic.Core capabilities such as standard clinical precautions and tracing the source of infection were the focus of IPC in medical institutions during the pandemic.Therefore,the core competences of IPC professionals during the pandemic,and how these contributed to successful prevention and control of the epidemic,should be studied.To investigate,using a systematic review and cluster analysis,fundamental improvements in the competences of infection control and prevention professionals that may be emphasized in light of the COVID-19 pandemic.We searched the PubMed,Embase,Cochrane Library,Web of Science,CNKI,WanFang Data,and CBM databases for original articles exploring core competencies of IPC professionals during the COVID-19 pandemic(from January 1,2020 to February 7,2023).Weiciyun software was used for data extraction and the Donohue formula was followed to distinguish high-frequency technical terms.Cluster analysis was performed using the within-group linkage method and squared Euclidean distance as the metric to determine the priority competencies for development.We identified 46 studies with 29 high-frequency technical terms.The most common term was“infection prevention and control training”(184 times,17.3%),followed by“hand hygiene”(172 times,16.2%).“Infection prevention and control in clinical practice”was the most-reported core competency(367 times,34.5%),followed by“microbiology and surveillance”(292 times,27.5%).Cluster analysis showed two key areas of competence:Category 1(program management and leadership,patient safety and occupational health,education and microbiology and surveillance)and Category 2(IPC in clinical practice).During the COVID-19 pandemic,IPC program management and leadership,microbiology and surveillance,education,patient safety,and occupational health were the most important focus of development and should be given due consideration by IPC professionals. 展开更多

BACKGROUND Over the last few decades,3 pathogenic pandemics have impacted the global population;severe acute respiratory syndrome coronavirus(SARS-CoV),Middle East respiratory syndrome coronavirus(MERS-CoV)and SARS-CoV-2.The global disease burden has attributed to millions of deaths and morbidities,with the majority being attributed to SARS-CoV-2.As such,the evaluation of the mental health(MH)impact across healthcare professionals(HCPs),patients and the general public would be an important facet to evaluate to better understand short,medium and long-term exposures.AIM To identify and report:(1)MH conditions commonly observed across all 3 pandemics;(2)Impact of MH outcomes across HCPs,patients and the general public associated with all 3 pandemics;and(3)The prevalence of the MH impact and clinical epidemiological significance.METHODS A systematic methodology was developed and published on PROSPERO(CRD42021228697).The databases PubMed,EMBASE,ScienceDirect and the Cochrane Central Register of Controlled Trials were used as part of the data extraction process,and publications from January 1,1990 to August 1,2021 were searched.MeSH terms and keywords used included Mood disorders,PTSD,Anxiety,Depression,Psychological stress,Psychosis,Bipolar,Mental Health,Unipolar,Self-harm,BAME,Psychiatry disorders and Psychological distress.The terms were expanded with a‘snowballing’method.Cox-regression and the Monte-Carlo simulation method was used in addition to I2 and Egger’s tests to determine heterogeneity and publication bias.RESULTS In comparison to MERS and SARS-CoV,it is evident SAR-CoV-2 has an ongoing MH impact,with emphasis on depression,anxiety and post-traumatic stress disorder.CONCLUSION It was evident MH studies during MERS and SARS-CoV was limited in comparison to SARS-CoV-2,with much emphasis on reporting symptoms of depression,anxiety,stress and sleep disturbances.The lack of comprehensive studies conducted during previous pandemics have introduced limitations to the“know-how”for clinicians and researchers to better support patients and deliver care with limited healthcare resources. 展开更多

This paper highlights some of those significant advances in the informationization of accounting information systems in China. As China has become a major industrial power in the international economy, further improvements for these information systems are critical to the continued successes of China. To additionally improve these systems, China can draw upon the systems from other world economic leaders. With its fully developed capital markets, the United States offers development experience for the external reporting components of a fully integrated information system. This paper discusses a typical integrated information system in the United States and addresses the regulatory milestones that were instrumental in the development of those external components of accounting information systems. Recommendations are presented for improving informationization of systems in China based on U.S. systems' responses to those milestones. 展开更多

BACKGROUND The coronavirus disease 2019(COVID-19) pandemic produced changes in intensive care units(ICUs) in patient care and health organizations. The pandemic event increased patients’ risk of developing psychological symptoms during and after hospitalisation. These consequences also affected those family members who could not access the hospital. In addition, the initial lack of knowledge about the virus and its management, the climate of fear and uncertainty, the increased workload and the risk of becoming infected and being contagious, had a strong impact on healthcare staff and organizations. This highlighted the importance of interventions aimed at providing psychological support to ICUs, involving patients, their relatives, and the staff;this might involve the reorganisation of the daily routine and rearrangement of ICU staff duties.AIM To conduct a systematic review of psychological issues in ICUs during the COVID-19 pandemic involving patients, their relatives, and ICU staff.METHODS We investigated the PubMed and the ClinicalTrials.gov databases and found 65 eligible articles,upon which we commented.RESULTS Our results point to increased perceived stress and psychological distress in staff, patients and their relatives and increased worry for being infected with severe acute respiratory syndrome coronavirus-2 in patients and relatives. Furthermore, promising results were obtained for some psychological programmes aiming at improving psychological measures in all ICU categories.CONCLUSION As the pandemic limited direct inter-individual interactions, the role of interventions using digital tools and virtual reality is becoming increasingly important. All considered, our results indicate an essential role for psychologists in ICUs. 展开更多

Purpose: The implementation of routine outcome measurement was initiated as a quality improvement initiative in a unit delivering intensive functional rehabilitation for people with lower limb amputation. Two years post-implementation, completion rates remained low which raised the need to gain an indepth understanding of the factors that might impact the systematic use of Outcome Measures (OMs). Method: A qualitative exploratory study embedded in the ongoing quality improvement initiative was designed. Data were gathered through a focus group with members of a multidisciplinary rehabilitation team. A deductive content analysis was performed using Consolidated Framework for Implementation Research (CFIR) as a guide to explore factors that impact routine outcome measurement. Results: Respondents perceived OMs as valid and offering clear advantages in clinical practice. At the organizational level, lack of fit with clinical practice, loss of project leaders and lack of clear management directives had negative repercussions on the use of OMs. Conclusion: Our results suggest that a dedicated project leader throughout the implementation process and effective communication may contribute to bypassing barriers associated to practice changes leading to a more systematic use of OMs among clinicians. 展开更多

Management costs and risks increase, and equipment utilization declines. To address these issues, a professional private cloud solution is introduced to integrate the electric power SCADA system, and conduct experimental study of its applicability, reliability, security, and real time. Occupational Therapy is critically underdeveloped in many developing countries in Asia. There are many internal and external barriers to the growth of the profession, including lack of willpower and lack of professional representation in the government. This article aims to communicate the postulations for its stagnation across developing countries in Asia. An analytical reflection and historical review of the barriers to the growth of occupational therapy in Malaysia was conducted. Leaders in the field who met the inclusion criteria, collaborated to shape the reconstructions using an occupational reconstruction approach. The reconstruction guided key critical reflections on the roots of daily experiences to relive the cooperative action to problem solve the issues encountered by the profession in Malaysia. Medical hegemony was found as the active suppressor of the health professions, and can be seen as being beyond reproach in the Asian’s medical model that created occupational injustices. These occupational injustices hinder the growth of Occupational Therapy, do not serve the patients, science or the overall healthcare system. Autocratic governance has a debilitating impact on health sciences and its workforce especially around badly oppressed Asian countries. A greater systematic approach is needed to examine the extent, effect, problem solve and remediate the magnitude of injustices over the last decade—to make way for an inclusive and affordable care system. 展开更多

The wrong selection decision might lead eventually to reduced profit margins or perhaps financial losses to PSFs (professional service firms). This research proposes a conceptual framework that helps PSFs in the selection of one or more suitable PDSs for their construction operations. The framework uses SWOT (strengths, weaknesses, opportunities and threats) analysis as a tool for assessing each PDS considered in this research. The PDSs included in the framework are design-bid-build, design-build, construction management agency, public-private partnerships and integrated project delivery. Aim: To examine the experiences of health care professional (HCP)-patient interactions in Multiple Sclerosis (MS), identifying factors that can influence these interactions. Methods: A three-stage systematic review and thematic synthesis of qualitative and quantitative research was undertaken. Stage 1: the systematic literature search;Stage 2: methodological appraisal of the qualitative papers;Stage 3: thematic synthesis of all qualitative papers and the integration of quantitative findings into the synthesis. Results: Forty-nine qualitative studies were identified. This included 1014 individuals with MS (244 male, 755 female and 15 unknown) and 106 carers and 86 HCPs. Seventeen quantitative studies were identified which included 7680 (2008 male, 5812 females, and 40 unknown) participants as well as 224 carers. Two themes are discussed: 1) The expectations, experiences and perceptions of interactions with HCPs, and 2) The factors that influenced interactions and relationships. Discussion: There is need for improvement in the content and provision of information to patients with MS from HCPs. Specific strategies are suggested and implications for patients and health care providers are considered. 展开更多
