BACKGROUND Breast cancer(BC)is the second leading cause of tumor-related mortality after lung cancer.Chemotherapy resistance remains a major challenge to progress in BC treatment,warranting further exploration of feas...BACKGROUND Breast cancer(BC)is the second leading cause of tumor-related mortality after lung cancer.Chemotherapy resistance remains a major challenge to progress in BC treatment,warranting further exploration of feasible and effective alternative therapies.AIM To analyzed the quality of life(QoL)and survival of patients with BC treated with integrated traditional Chinese and Western medicine(TCM-WM).METHODS This study included 226 patients with BC admitted to the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine between February 2018 and February 2023,including 100 who received conventional Western medicine treatment(control group)and 126 who received TCM-WM treatment(research group).The total effective rate,side effects(alopecia,nausea and vomiting,hepatorenal toxicity,and myelosuppression),QoL assessed using the European Organization for Research and Treatment of Cancer Core Quality of Life Questionnaire(EORTC QLQ-C30),1-year overall survival(OS),recurrence and metastasis rates,and serum inflammatory factors[interleukin(IL)-6,IL-10,and tumor necrosis factor alpha]were comparatively analyzed.RESULTS The research group showed statistically better overall efficacy,EORTC QoL-C30 scores,and 1-year OS than the control group,with markedly lower side effects and 1-year recurrence and metastasis rates.Moreover,the posttreatment levels of serum inflammatory in the research group were significantly lower than the baseline and those in the control group.CONCLUSION Overall,TCM-WM demonstrated significantly improved therapeutic efficacy while ensuring drug safety in BC,which not only improved patients’QoL and prolonged survival,but also significantly inhibited the inflammatory response.展开更多
[Objectives]This study was conducted to explore the application and development trend of Chinese medicinal material cinnamon in the field of traditional Chinese medicine.[Methods]Articles published from 2008 to 2023 w...[Objectives]This study was conducted to explore the application and development trend of Chinese medicinal material cinnamon in the field of traditional Chinese medicine.[Methods]Articles published from 2008 to 2023 were exported using"cinnamon"as the subject word in the Chinese database of CNKI.Knowledge graphs were drawn by CiteSpace software on the number of articles published,the institutions publishing articles,and keyword clustering,and the data were sorted by Excel.Combined with the extracted information,the application of cinnamon in traditional Chinese medicine and integrated traditional Chinese and Western medicine was analyzed,and its development trend was discussed and prospected,providing further reference for researchers.[Results]The number of articles published showed an overall upward trend and maintained a high number of articles.In the analysis of the journals publishing articles,the journal with the largest number of articles was West China Journal of Pharmaceutical Sciences,which had certain representativeness.In the analysis of institutions publishing articles,the institution with the largest number of articles was Beijing University of Chinese Medicine,and most institutions had little cooperation.Four categories was obtained in keyword clustering,respectively,general research,component identification,production process and product development of cinnamon."Glycyrrhizin"was the keyword with the earliest burst time,and the hot words that have received much attention in recent years are"medication law"and"data mining".[Conclusions]The application of cinnamon in the field of traditional Chinese medicine is mainly to treat diseases and as raw materials for traditional Chinese medicine products.The development trend is"quality control"and"product research and development".Further research and development of cinnamon in traditional Chinese medicine need to promote the participation of more institutions to participate,and cooperation and communication between institutions should be strengthened to promote the deep integration of production,research and academia.展开更多

Methods: In this study, we system...Objective: The objective of this study is to evaluate the impact of various traditional Chinese medicine (TCM)approaches on the quality of life in patients with colorectal cancer. Methods: In this study, we systematically performedthe screening of randomized controlled trials from Cochrane Library, Pubmed, Embase, Medline, and Web of Science.The data were extracted by two reviewers independently, and then risk assessment was performed. All the analyses wereconducted using Review Manager 5.3. Results: A total of 18 eligible studies containing 1312 patients were included.Experimental group were treated with TCM combined with Western medicine or TCM alone (N = 688) and controlgroup were treated with Western medicine treatment alone (N = 624). The results showed that the recent clinicalefficiency between the two groups was not statistically significant (P = 0.06). KPS scores of the experimental group werehigher than those in the control group [P 〈 0.001, WMD = 9.60, 95%CI = (5.62, 13.57)]. The toxicity comparisonshowed that the occurrence of toxicities, such as leucopenia, thrombocytopenia, nausea and vomiting, diarrhea, andneurotoxicity in the experimental group was lower than that in the control group [P 〈 0.001, OR = 0.31, 95%CI = (0.19,0.50)], [P = 0.003, OR = 0.49, 95%CI = (0.31, 0.78)], [P 〈 0.001, OR = 0.30, 95%CI = (0.16, 0.54)], [P 〈 0.001, OR =0.40, 95%CI = (0.27, 0.58)], and [P 〈 0.001, OR = 0.43, 95%CI = (0.30, 0.61)]. The immunological test comparisondemonstrated that the immunological parameters (CD3, CD4, and CD4/CD8) showed higher values in the experimentalgroup than those in the control group [P 〈 0.001, MD = 5.55, 95%CI = (4.83, 6.28)], [P 〈 0.0001, MD = 6.75, 95%CI =(5.25, 8.26)], and [P = 0.001, MD = 0.26, 95%CI = (0.10, 0.41)]. Conclusions: TCM did not show significant recentclinical efficiency. However, treatment with TCM showed increase in KPS scores in patients with colorectal cancer,alleviation of toxicity associated with chemotherapy, regulation of autoimmunity, and improvement in the quality of lifeof patients.展开更多

Objective:To explore and analyze the clinical efficacy of Chinese medicine combined with acupuncture and moxibustion in the treatment of insomnia.Method:80 patients with primary insomnia were selected as the study sam...Objective:To explore and analyze the clinical efficacy of Chinese medicine combined with acupuncture and moxibustion in the treatment of insomnia.Method:80 patients with primary insomnia were selected as the study sample,with 20 cases randomly selected as the control group and the rest as the observation group.The control group was treated with ordinary western medicine,while the observation group patients were treated with traditional Chinese medicine combined with acupuncture and moxibustion based on this syndrome differentiation.The comparison items were treatment effect,sleep quality,mental health,quality of life,specific sleep indicators,cerebral blood flow and neurotransmitters.Result:The treatment effectiveness showed that the observation group had a higher treatment effectiveness rate than the control group(P<0.05);The results of sleep quality showed that after treatment,the scores of the observation group were lower than those of the control group in seven dimensions:sleep quality,sleep time,sleep onset time,sleep efficiency,sleep disorders,hypnotic drugs,and daytime dysfunc‐tion score(P<0.05);The quality of life results showed that after treatment,the observa‐tion group had higher scores than the control group in eight dimensions:physiological function,physiological function,physical pain,health status,energy,social function,emotional function,and mental health(P<0.05);The results of slow wave sleep time and fast eye movement sleep time showed that after treatment,the HAMA and HAMD scores of the physiological mechanism in the observation group were lower than those in the control group(P<0.05);The results of cerebral blood flow and neurotransmitter indica‐tors show that after treatment,glutamateγ-The peak systolic blood flow velocity of ami‐nobutyric acid,vertebral artery,and basilar artery in the observation group was higher than that in the control group(P<0.05).Conclusion:The clinical treatment of Chinese medicine combined with acupuncture and moxibustion in the treatment of insomnia patients can significantly improve the clinical treatment effect,and improve the cerebral hemorrhage and neurotransmitter status of patients,promote the improvement of their physical and mental status,can more significantly improve the quality of sleep,reduce sleep difficulties,reduce the dosage of sleeping pills,which has important clinical application value.展开更多

However, the current quality standard research lacks top-level design and systematic ...An effective quality control system is the key to ensuring the quality, safety and efficacy of traditional Chinese medicines(TCMs). However, the current quality standard research lacks top-level design and systematic design,mostly based on specific technologies or evaluation methods. To resolve the challenges and questions of quality control of TCMs, a brand-new quality standard system, named "iVarious", was proposed. The system comprises eight elements in a modular format. Meaning of every element was specifically illustrated via corresponding research instances. Furthermore, frankincense study was taken as an example for demonstrating standards and research process, based on the "i Various" system. This system highlighted a holistic strategy for effectiveness,security, integrity and systematization of quality and safety control standards of TCMs. The establishment of"i Various" integrates multi-disciplinary technologies and progressive methods, basis elements and key points of standard construction. The successful hyphenation of high-performance liquid chromatography (HPLC) and mass spectrometry (MS) has been applied widely in TCMs and biological samples analysis. Undoubtedly, HPLC/MS technique has facilitated the understanding of the treatment mechanism of TCMs. We reviewed more than 350 published papers within the last 5 years on HPLC/MS in the analysis of TCMs. The present review focused on the applications of HPLC/MS in the component analysis, metabolites analysis, and pharmacokinetics of TCMs etc. 50% of the literature is related to the component analysis of TCMs, which show that this field is the most popular type of research. In the metabolites analysis, HPLC coupled with electrospray ionization quadrupole time-of-flight tandem mass spectrometry has been demonstrated to be the powerful tool for the characterization of structural features and fragmentation behavior patterns. This paper presented a brief overview of the applications of HPLC/MS in the analysis of TCMs. HPLC/MS in the fingerprint analysis is reviewed elsewhere.展开更多
As the most important complementary medication against a variety of diseases,traditional Chinese medicines(TCMs)have been extensively applied over thousands of years.Current quality control of herbal medicines,however...As the most important complementary medication against a variety of diseases,traditional Chinese medicines(TCMs)have been extensively applied over thousands of years.Current quality control of herbal medicines,however,is in great dispute.Unlike chemical drugs,which have clear and validated quality standards,the content of only one(or a few)compounds of many herbs and preparations is currently assessed as an indicator of quality,even though the assessed compound(s)is neither closely associated with the efficacy nor representative of the medicine as a whole.Based on the clinical use,compatibility of multiple component prescriptions,and manufacturing process of TCM,the new concept of a TCM quality marker that was proposed in previous work is discussed further here.In addition,practical technological approaches are described for the qualitative analysis and quantification of TCMs including herbs,processed products,and preparations,which lead to the discovery and identification of specific chemicals as quality markers and new quality control patterns.The progress that has been made in TCM quality control is also addressed.This work provides useful information for the quality control of herbal medicines in the future.展开更多

The Chinese Pharmacopoeia(ChP) is a drug code formulated by the Chinese government, and it includes a special volume for the monographs of TCM, which plays an important role in ensuring the quality of drugs. The use of quality control technology has always been a complex and important factor in TCM. Owing to the chemical diversity of TCM, chromatography technology has been proven to be a comprehensive strategy for the assessment of the overall quality of TCM and has become the main analytical method in the ChP.This article provides an overview of the classical and modern chromatographic technologies applied in the ChP, and summarizes the advantages and disadvantages of each technique in the TCM monographs.In 2020, the new edition of the Ch P(the 2020 edition) has been implemented at the end of 2020. In order to investigate the relationship between the inorganic elemental fingerprint and the geographical origin identification of Meretricis concha, the elemental contents of M. concha from five sampling points in Rushan Bay have been determined by means of inductively coupled plasma optical emission spectrometry(ICP-OES). Based on the contents of 14 inorganic elements(Al, As, Cd, Co, Cr, Cu, Fe, Hg, Mn, Mo, Ni, Pb, Se, and Zn), the inorganic elemental fingerprint which well reflects the elemental characteristics was constructed. All the data from the five sampling points were discriminated with accuracy through hierarchical cluster analysis(HCA) and principle component analysis(PCA), indicating that a four-factor model which could explain approximately 80% of the detection data was established, and the elements Al, As, Cd, Cu, Ni and Pb could be viewed as the characteristic elements. This investigation suggests that the inorganic elemental fingerprint combined with multivariate statistical analysis is a promising method for verifying the geographical origin of M. concha, and this strategy should be valuable for the authenticity discrimination of some marine TCM.展开更多

For that, the inorganic elemental contents ofMeretricis concha from five sampling points in Jiaozhou Bay have been determined by means of inductively coupled plasma optical emission spectrometry, and the comparative investigations based on the contents of 14 inorganic elements (A1, As, Cd, Co, Cr, Cu, Fe, Hg, Mn, Mo, Ni, Pb, Se and Zn) of the samples from Jiaozhou Bay and the previous reported Rushan Bay were performed. It has been found that the samples from the two bays are ap- proximately classified into two kinds using hierarchical cluster analysis, and a four-factor model based on principle component analysis could explain approximately 75% of the detection data, also linear discriminant analysis can be used to develop a prediction model to distinguish the samples from Jiaozhou Bay and Rushan Bay with accuracy of about 93%. The results of the present investi- gation suggested that the inorganic elemental fingerprint based on the combination of the measured elemental content and chemom- etric analysis is a promising approach for verifying the geographical origin ofMeretricis concha, and this strategy should be valuable for the authenticity discrimination of some marine TCM.展开更多
Chromatographic fingerprinting has been perceived as an essential tool for assessing quality and chemical equivalence of traditional Chinese medicine.However,this pattern-oriented approach still has some weak points i...Chromatographic fingerprinting has been perceived as an essential tool for assessing quality and chemical equivalence of traditional Chinese medicine.However,this pattern-oriented approach still has some weak points in terms of chemical coverage and robustness.In this work,we proposed a multiple reaction monitoring(MRM)-based fingerprinting method in which approximately 100 constituents were simultaneously detected for quality assessment.The derivative MRM approach was employed to rapidly design MRM transitions independent of chemical standards,based on which the large-scale fingerprinting method was efficiently established.This approach was exemplified on QiShenYiQi Pill(QSYQ),a traditional Chinese medicine-derived drug product,and its robustness was systematically evaluated by four indices:clustering analysis by principal component analysis,similarity analysis by the congruence coefficient,the number of separated peaks,and the peak area proportion of separated peaks.Compared with conventional ultraviolet-based fingerprints,the MRM fingerprints provided not only better discriminatory capacity for the tested normal/abnormal QSYQ samples,but also higher robustness under different chromatographic conditions(i.e.,flow rate,apparent pH,column temperature,and column).The result also showed for such large-scale fingerprints including a large number of peaks,the angle cosine measure after min-max normalization was more suitable for setting a decision criterion than the unnormalized algorithm.This proof-of-concept application gives evidence that combining MRM technique with proper similarity analysis metrices can provide a highly sensitive,robust and comprehensive analytical approach for quality assessment of traditional Chinese medicine.展开更多

To investigate the principles of differentiation and prescription of traditional Chinese medicine(TCM)treatment for cancerous fever by logistic stepwise regression and literature investigation,the following databases ...To investigate the principles of differentiation and prescription of traditional Chinese medicine(TCM)treatment for cancerous fever by logistic stepwise regression and literature investigation,the following databases were searched(from inception to June 30th 2016):CNKI,CSPD,CBM,PubMed,MEDLINE,EMBASE.According to the inclusion and exclusion criteria,literatures about TCM in the treatment of cancerous fever were screened.The frequency about cancerous fever of differential syndrome types,formulas and herbs were analyzed by logistic multiple regression analysis.The model about syndrome types and formula application of cancerous fever was established.The result showed formula prescriptions used most frequently were Qing-Hao-Bie-Jia decoction,Xiao-Chai-Hu decoction,Xue-Fu-Zhu-Yu decoction,Bai-Hu decoction,San-Ren decoction.The main syndrome types of cancerous fever were as follows:Toxic heat flourishing type,Yin deficiency type,Stagnation of damp-heat type,Qi-Yin deficiency type,Liver meridian depressed-heat type,Blood stasis type and Yang deficiency type.Based on regression equation of the syndrome types,the corresponding herbs of this seven syndrome types were Radix Bupleuri,Radix Paeoniae Alba and Scutellaria Baicalensis Georgi;Radix Bupleuri and Cortex Moutan;Radix Paeoniae Rubra and Radix Angelicae Sinensis;Rhizoma Pinelliae,Cortex Lycii Radicis and Radix Paeoniae Alba;Herba Artemisiae Annuae,Gypsum Fibrosum and Poria Cocos;Rhizoma Anemarrhenae,Scutellaria Baicalensis Georgi and Radix Codonopsis;Radix Bupleuri respectively.The results of Logistic regression analysis and frequency of herbs statistics can provide reference for differentiation and treatment of cancerous fever in clinical.展开更多
Objective:The objective of this study is to explore the current situation,clinical research hot spots,and trends of traditional Chinese medicine(TCM)nursing technology in rheumatoid arthritis(RA),to draw a panorama of...Objective:The objective of this study is to explore the current situation,clinical research hot spots,and trends of traditional Chinese medicine(TCM)nursing technology in rheumatoid arthritis(RA),to draw a panorama of clinical research,to provide the basis and clues for subsequent high-level evidence integration and further in-depth research.Materials and Methods:Seven databases including China National Knowledge Infrastructure,Chinese Scientific Journal Database(VIP),Wanfang Database,Chinese Biomedical Literature Database(Sino-Med),PubMed,Embase,and the Cochrane Library were searched.The bibliometric method and visualization software CiteSpace were used to conduct a multi-dimensional analysis of the included literature.Results:A total of 805 pieces of literature were included(of them,one was written in German and four in English),and the number of published literature showed an increasing trend year by year.There were only 30(3.73%)pieces of literature published in nursing journals.The hot spots of the co-occurrence map were concentrated in:(1)the nursing of fumigation in RA;(2)the efficacy evaluation of TCM nursing technology on pain,joint malformation,and joint dysfunction caused by RA.The included literature themes focused on five TCM technologies:fumigation,moxibustion,acupoint patching,acupoint injection,and Chinese herbal soaking.The beginning year and strength in TCM fumigation were 2012 and 5.2,and those in moxibustion were 2011 and 3.38.Conclusion:The related studies are on the rise.It has entered the field of international readers with clear research hot spots.However,there are still shortcomings such as little literature published in Chinese nursing core journals,few non-Chinese-related documents,and a lack of international exchanges and cooperation.The current research hot spots in this field are TCM fumigation,and the cutting-edge trend of future research may be fumigation and moxibustion technology.It is suggested that further research can focus on evidence integration and original research on the self-optimization of these two techniques.展开更多
Objective Through bibliometrics and visual analysis of the related studies on traditional Chinese medicine(TCM)treatment of immune thrombocytopenia(ITP),this study aims to sort out the overall research progress,hotspo...Objective Through bibliometrics and visual analysis of the related studies on traditional Chinese medicine(TCM)treatment of immune thrombocytopenia(ITP),this study aims to sort out the overall research progress,hotspots,and trends in this field,and provide reference for further research in ITP.Methods The articles on ITP treated by TCM were retrieved from China National Knowledge Infrastructure(CNKI),Wanfang Database,China Science and Technology Journal Database(VIP),Web of Science Core Collection(WOSCC),and PubMed.The retrieval time was from the establishment of the databases to July 31,2022.VOSviewer,CiteSpace,Carrot2,and Note-Express were used for data analysis of the articles in terms of their quantities,types,and journals,and for visualization of research hotspots,authors,institutions,and keywords.Results 1493 Chinese articles and 40 English articles were included.The articles in Chinese mainly focus on clinical trial research and clinical experience summary,while the English articles mainly focus on clinical trial research and animal research.The Chinese articles were published in 317 Chinese journals,while English articles were published in 29 English journals.Research hotspots include the clinical syndrome differentiation of ITP,the therapeutic effect of TCM compounds on ITP,and the mechanism of ITP treatment.Keyword analysis shows that there are many research achievements in integrated traditional Chinese and western medicine treatment,clinical research,famous doctors’experience,TCM treatment,cellular immunity,and humoral immunity.The authors with the most articles in Chinese and English are Professor CHEN Xinyi and Professor MA Rou,respectively,and the research institutions with the most articles are Dongzhimen Hospital of Beijing University of Chinese Medicine and Xiyuan Hospital of China Academy of Chinese Medical Sciences.Chinese herbs often used to treat ITP clinically include Xianhecao(Agrimoniae Herba),Nvzhenzi(Ligustri Lucidi Fructus),Mohanlian(Ecliptae Herba),Zhongjiefeng(Sarcandrae Herba),etc.,and the prescription usually used to treat ITP include Guipi Decoction(归脾汤),Xijiao Dihuang Decoction(犀角地黄汤),Bazhen Decoction(八珍汤),Erzhi Pill(二至丸),and Xiaochaihu De-coction(小柴胡汤).The main development trends toward retrospective study,TCM treatment mechanism,and data mining.展开更多
Background:Coronavirus disease 2019 is still spreading all over the world and without any specific medicine.Traditional Chinese medicine has played an important role in the control of epidemics in China and accumulate...Background:Coronavirus disease 2019 is still spreading all over the world and without any specific medicine.Traditional Chinese medicine has played an important role in the control of epidemics in China and accumulated many experiences.The aim of this study is to summarize the law of traditional Chinese medicine and provide reference for coronavirus disease 2019 traditional Chinese medicine treatment.Methods:We collected the treatment plans of traditional Chinese medicine for coronavirus disease 2019 in 24 provinces of China to evaluate the basic Chinese medicine formulas,and the efficacy types,frequency,“Four Characters”,“Five Tastes”,“Channel Tropism”of herbal medicines.Combination rules between medicines were calculated by Apriori algorithm and Clementine12.0.Results:The formulas for“Relieving Exterior Syndrome”,“Expelling Dampness”,“Clearing Heat”and“Supplementing”have highest proportion.In prevention period and medical observation period,the medicines mainly have effect for“Clearing Heat”,“Relieving Exterior Syndrome”and“Supplementing”.In treatment period,medicines for“Clearing Heat”,“Relieving Superficies”and“Relieving Dampness”had higher frequency.The“Four Characters”of Chinese medicines in all treatment plans were mainly“Cold”and“Warm”.Medicines in the treatment plans always belong to“Pungent Taste”,“Bitter Taste”and“Sweet Taste”.For“Channel Tropism”,most medicines turned over to the“Lung”,“Stoma”and“Spleen”.The common medicine combinations in the recommended formulas during the prevention period and medical observation period were medicines for“Qi-Supplementing”and“Relieving Superficies”.In treatment period(mild,moderate,severe/critical),medicines for“Dampness Expelling”,“Relieving Superficies”and“Qi-Supplementing”were combined in a high frequency.In recovery period,the medicines for“Dispelling Dampness”combined the medicines for“Promoting Blood Circulation”with highest frequency.Conclusions:We found the characteristics of medicine and prescription rules of traditional Chinese medicine for the treatment of coronavirus disease 2019 and provided scientificity and effectiveness of it.More importantly,it provides reference for the development of new clinical prescriptions for coronavirus disease 2019.展开更多
Objective With the increasing publication of clinical practice guidelines(CPG)for Traditional Chinese Medicine(TCM)and Integrated Traditional Chinese and Western Medicine(IM),the standardization and scientifiction of ...Objective With the increasing publication of clinical practice guidelines(CPG)for Traditional Chinese Medicine(TCM)and Integrated Traditional Chinese and Western Medicine(IM),the standardization and scientifiction of its formulation have gradually attracted many people’s attention.To offer an overview of TCM and IM CPGs published over the past decade and analyze their general characteristics and methodological quality.Methods The China National Knowledge Infrastructure(CNKI)and WANFANG databases were searched for clinical practice guidelines and expert consensus papers from January 2010 to June 2021.Two researchers independently completed the literature screening and cross-checking according to the inclusion and exclusion criteria of CPGs and extracted information on general characteristics and methodological quality of CPGs.Results According to the selection criteria,231 CPGs(EB-CPGs=119,CBCPGs=112)were selected and the number of CPGs published in the 11 years showed an overall upward trend.The vast majority of CPGs used the Western naming system for the diseases,and only 11 CPGs were named of TCM diseases or symptoms.TCM treatments were recommended in 223 CPGs.There were 156 ancient Chinese Medicine literature sources cited in 231 CPGs and opinions and experiences of 62 TCM experts cited in 37 CPGs.The methodological quality of EB-CPGs for TCM and IM were significantly better than CB-CPGs in 11 items.Only 60 EB-CPGs and 7 CB-CPGs designated clear criteria for grading quality of evidence and strength of the recommendations and 74 CPGs presented both the level of evidence and the strength of recommendations.We classified all CPGs according to whether or not they used GRADE,and the results showed that the CPGs using GRADE had higher methodological quality and more standardized reports.Conclusion This research has shown that the quantity and quality of CPGs in both TCM and IM have improved over the time span,but the methodological quality,especially evidence citation,and the use of criteria for grading quality of evidence and strength of the recommendations,still needs to further improvement in the future.展开更多
文摘BACKGROUND Breast cancer(BC)is the second leading cause of tumor-related mortality after lung cancer.Chemotherapy resistance remains a major challenge to progress in BC treatment,warranting further exploration of feasible and effective alternative therapies.AIM To analyzed the quality of life(QoL)and survival of patients with BC treated with integrated traditional Chinese and Western medicine(TCM-WM).METHODS This study included 226 patients with BC admitted to the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine between February 2018 and February 2023,including 100 who received conventional Western medicine treatment(control group)and 126 who received TCM-WM treatment(research group).The total effective rate,side effects(alopecia,nausea and vomiting,hepatorenal toxicity,and myelosuppression),QoL assessed using the European Organization for Research and Treatment of Cancer Core Quality of Life Questionnaire(EORTC QLQ-C30),1-year overall survival(OS),recurrence and metastasis rates,and serum inflammatory factors[interleukin(IL)-6,IL-10,and tumor necrosis factor alpha]were comparatively analyzed.RESULTS The research group showed statistically better overall efficacy,EORTC QoL-C30 scores,and 1-year OS than the control group,with markedly lower side effects and 1-year recurrence and metastasis rates.Moreover,the posttreatment levels of serum inflammatory in the research group were significantly lower than the baseline and those in the control group.CONCLUSION Overall,TCM-WM demonstrated significantly improved therapeutic efficacy while ensuring drug safety in BC,which not only improved patients’QoL and prolonged survival,but also significantly inhibited the inflammatory response.

文摘[Objectives]This study was conducted to explore the application and development trend of Chinese medicinal material cinnamon in the field of traditional Chinese medicine.[Methods]Articles published from 2008 to 2023 were exported using"cinnamon"as the subject word in the Chinese database of CNKI.Knowledge graphs were drawn by CiteSpace software on the number of articles published,the institutions publishing articles,and keyword clustering,and the data were sorted by Excel.Combined with the extracted information,the application of cinnamon in traditional Chinese medicine and integrated traditional Chinese and Western medicine was analyzed,and its development trend was discussed and prospected,providing further reference for researchers.[Results]The number of articles published showed an overall upward trend and maintained a high number of articles.In the analysis of the journals publishing articles,the journal with the largest number of articles was West China Journal of Pharmaceutical Sciences,which had certain representativeness.In the analysis of institutions publishing articles,the institution with the largest number of articles was Beijing University of Chinese Medicine,and most institutions had little cooperation.Four categories was obtained in keyword clustering,respectively,general research,component identification,production process and product development of cinnamon."Glycyrrhizin"was the keyword with the earliest burst time,and the hot words that have received much attention in recent years are"medication law"and"data mining".[Conclusions]The application of cinnamon in the field of traditional Chinese medicine is mainly to treat diseases and as raw materials for traditional Chinese medicine products.The development trend is"quality control"and"product research and development".Further research and development of cinnamon in traditional Chinese medicine need to promote the participation of more institutions to participate,and cooperation and communication between institutions should be strengthened to promote the deep integration of production,research and academia.

文摘Objective: The objective of this study is to evaluate the impact of various traditional Chinese medicine (TCM)approaches on the quality of life in patients with colorectal cancer. Methods: In this study, we systematically performedthe screening of randomized controlled trials from Cochrane Library, Pubmed, Embase, Medline, and Web of Science.The data were extracted by two reviewers independently, and then risk assessment was performed. All the analyses wereconducted using Review Manager 5.3. Results: A total of 18 eligible studies containing 1312 patients were included.Experimental group were treated with TCM combined with Western medicine or TCM alone (N = 688) and controlgroup were treated with Western medicine treatment alone (N = 624). The results showed that the recent clinicalefficiency between the two groups was not statistically significant (P = 0.06). KPS scores of the experimental group werehigher than those in the control group [P 〈 0.001, WMD = 9.60, 95%CI = (5.62, 13.57)]. The toxicity comparisonshowed that the occurrence of toxicities, such as leucopenia, thrombocytopenia, nausea and vomiting, diarrhea, andneurotoxicity in the experimental group was lower than that in the control group [P 〈 0.001, OR = 0.31, 95%CI = (0.19,0.50)], [P = 0.003, OR = 0.49, 95%CI = (0.31, 0.78)], [P 〈 0.001, OR = 0.30, 95%CI = (0.16, 0.54)], [P 〈 0.001, OR =0.40, 95%CI = (0.27, 0.58)], and [P 〈 0.001, OR = 0.43, 95%CI = (0.30, 0.61)]. The immunological test comparisondemonstrated that the immunological parameters (CD3, CD4, and CD4/CD8) showed higher values in the experimentalgroup than those in the control group [P 〈 0.001, MD = 5.55, 95%CI = (4.83, 6.28)], [P 〈 0.0001, MD = 6.75, 95%CI =(5.25, 8.26)], and [P = 0.001, MD = 0.26, 95%CI = (0.10, 0.41)]. Conclusions: TCM did not show significant recentclinical efficiency. However, treatment with TCM showed increase in KPS scores in patients with colorectal cancer,alleviation of toxicity associated with chemotherapy, regulation of autoimmunity, and improvement in the quality of lifeof patients.
文摘The objective of this study is to summarize the efficacy and safety of traditional Chinese medicine(TCM)enema in the treatment of ulcerative colitis(UC).The randomized controlled trials on TCM enema intervention in the treatment of UC were searched in seven databases:PubMed,Embase,Web of Science,CBM,CNKI,Wanfang Database,and VIP Database from January 1,2013 to June 6,2022,and the data were analyzed using RevMan 5.3 software.A total of 18 studies involving 1514 UC patients were included.Meta analysis results showed that compared with conventional Western medicine,Chinese medicine enema had a significant effect on UC,and the clinical effective rate of the experimental group using Chinese medicine enema was 4.45 times that of the control group using conventional Western medicine(odds ratio=4.45,95%confidence interval[3.27,6.06]).Therefore,Chinese medicine enema is effective in the treatment of UC,and can significantly reduce related symptoms.

文摘Objective:To explore and analyze the clinical efficacy of Chinese medicine combined with acupuncture and moxibustion in the treatment of insomnia.Method:80 patients with primary insomnia were selected as the study sample,with 20 cases randomly selected as the control group and the rest as the observation group.The control group was treated with ordinary western medicine,while the observation group patients were treated with traditional Chinese medicine combined with acupuncture and moxibustion based on this syndrome differentiation.The comparison items were treatment effect,sleep quality,mental health,quality of life,specific sleep indicators,cerebral blood flow and neurotransmitters.Result:The treatment effectiveness showed that the observation group had a higher treatment effectiveness rate than the control group(P<0.05);The results of sleep quality showed that after treatment,the scores of the observation group were lower than those of the control group in seven dimensions:sleep quality,sleep time,sleep onset time,sleep efficiency,sleep disorders,hypnotic drugs,and daytime dysfunc‐tion score(P<0.05);The quality of life results showed that after treatment,the observa‐tion group had higher scores than the control group in eight dimensions:physiological function,physiological function,physical pain,health status,energy,social function,emotional function,and mental health(P<0.05);The results of slow wave sleep time and fast eye movement sleep time showed that after treatment,the HAMA and HAMD scores of the physiological mechanism in the observation group were lower than those in the control group(P<0.05);The results of cerebral blood flow and neurotransmitter indica‐tors show that after treatment,glutamateγ-The peak systolic blood flow velocity of ami‐nobutyric acid,vertebral artery,and basilar artery in the observation group was higher than that in the control group(P<0.05).Conclusion:The clinical treatment of Chinese medicine combined with acupuncture and moxibustion in the treatment of insomnia patients can significantly improve the clinical treatment effect,and improve the cerebral hemorrhage and neurotransmitter status of patients,promote the improvement of their physical and mental status,can more significantly improve the quality of sleep,reduce sleep difficulties,reduce the dosage of sleeping pills,which has important clinical application value.

文摘An effective quality control system is the key to ensuring the quality, safety and efficacy of traditional Chinese medicines(TCMs). However, the current quality standard research lacks top-level design and systematic design,mostly based on specific technologies or evaluation methods. To resolve the challenges and questions of quality control of TCMs, a brand-new quality standard system, named "iVarious", was proposed. The system comprises eight elements in a modular format. Meaning of every element was specifically illustrated via corresponding research instances. Furthermore, frankincense study was taken as an example for demonstrating standards and research process, based on the "i Various" system. This system highlighted a holistic strategy for effectiveness,security, integrity and systematization of quality and safety control standards of TCMs. The establishment of"i Various" integrates multi-disciplinary technologies and progressive methods, basis elements and key points of standard construction. The system provides a novel idea and technological demonstration for regulation establishment of TCMs quality standards.

文摘In China, traditional Chinese medicines (TCMs) have been used in clinical applications for thousands of years. The successful hyphenation of high-performance liquid chromatography (HPLC) and mass spectrometry (MS) has been applied widely in TCMs and biological samples analysis. Undoubtedly, HPLC/MS technique has facilitated the understanding of the treatment mechanism of TCMs. We reviewed more than 350 published papers within the last 5 years on HPLC/MS in the analysis of TCMs. The present review focused on the applications of HPLC/MS in the component analysis, metabolites analysis, and pharmacokinetics of TCMs etc. 50% of the literature is related to the component analysis of TCMs, which show that this field is the most popular type of research. In the metabolites analysis, HPLC coupled with electrospray ionization quadrupole time-of-flight tandem mass spectrometry has been demonstrated to be the powerful tool for the characterization of structural features and fragmentation behavior patterns. This paper presented a brief overview of the applications of HPLC/MS in the analysis of TCMs. HPLC/MS in the fingerprint analysis is reviewed elsewhere.

文摘As the most important complementary medication against a variety of diseases,traditional Chinese medicines(TCMs)have been extensively applied over thousands of years.Current quality control of herbal medicines,however,is in great dispute.Unlike chemical drugs,which have clear and validated quality standards,the content of only one(or a few)compounds of many herbs and preparations is currently assessed as an indicator of quality,even though the assessed compound(s)is neither closely associated with the efficacy nor representative of the medicine as a whole.Based on the clinical use,compatibility of multiple component prescriptions,and manufacturing process of TCM,the new concept of a TCM quality marker that was proposed in previous work is discussed further here.In addition,practical technological approaches are described for the qualitative analysis and quantification of TCMs including herbs,processed products,and preparations,which lead to the discovery and identification of specific chemicals as quality markers and new quality control patterns.The progress that has been made in TCM quality control is also addressed.This work provides useful information for the quality control of herbal medicines in the future.
文摘As an important branch of medicine, Traditional Chinese Medicine(TCM) has been applied for the treatment of diseases for thousands of years in China and other countries in East Asia. The Chinese Pharmacopoeia(ChP) is a drug code formulated by the Chinese government, and it includes a special volume for the monographs of TCM, which plays an important role in ensuring the quality of drugs. The use of quality control technology has always been a complex and important factor in TCM. Owing to the chemical diversity of TCM, chromatography technology has been proven to be a comprehensive strategy for the assessment of the overall quality of TCM and has become the main analytical method in the ChP.This article provides an overview of the classical and modern chromatographic technologies applied in the ChP, and summarizes the advantages and disadvantages of each technique in the TCM monographs.In 2020, the new edition of the Ch P(the 2020 edition) has been implemented at the end of 2020. This paper also contains a brief introduction about the application of chromatographic technologies in the new edition of the ChP.

文摘Meretricis concha is a kind of marine traditional Chinese medicine(TCM), and has been commonly used for the treatment of asthma and scald burns. In order to investigate the relationship between the inorganic elemental fingerprint and the geographical origin identification of Meretricis concha, the elemental contents of M. concha from five sampling points in Rushan Bay have been determined by means of inductively coupled plasma optical emission spectrometry(ICP-OES). Based on the contents of 14 inorganic elements(Al, As, Cd, Co, Cr, Cu, Fe, Hg, Mn, Mo, Ni, Pb, Se, and Zn), the inorganic elemental fingerprint which well reflects the elemental characteristics was constructed. All the data from the five sampling points were discriminated with accuracy through hierarchical cluster analysis(HCA) and principle component analysis(PCA), indicating that a four-factor model which could explain approximately 80% of the detection data was established, and the elements Al, As, Cd, Cu, Ni and Pb could be viewed as the characteristic elements. This investigation suggests that the inorganic elemental fingerprint combined with multivariate statistical analysis is a promising method for verifying the geographical origin of M. concha, and this strategy should be valuable for the authenticity discrimination of some marine TCM.

文摘The goal of this paper is to explore the relationship between the inorganic elemental fingerprint and the geographical origin identification ofMeretricis concha, which is a commonly used marine traditional Chinese medicine (TCM) for the treatment of asthma and scald bums. For that, the inorganic elemental contents ofMeretricis concha from five sampling points in Jiaozhou Bay have been determined by means of inductively coupled plasma optical emission spectrometry, and the comparative investigations based on the contents of 14 inorganic elements (A1, As, Cd, Co, Cr, Cu, Fe, Hg, Mn, Mo, Ni, Pb, Se and Zn) of the samples from Jiaozhou Bay and the previous reported Rushan Bay were performed. It has been found that the samples from the two bays are ap- proximately classified into two kinds using hierarchical cluster analysis, and a four-factor model based on principle component analysis could explain approximately 75% of the detection data, also linear discriminant analysis can be used to develop a prediction model to distinguish the samples from Jiaozhou Bay and Rushan Bay with accuracy of about 93%. The results of the present investi- gation suggested that the inorganic elemental fingerprint based on the combination of the measured elemental content and chemom- etric analysis is a promising approach for verifying the geographical origin ofMeretricis concha, and this strategy should be valuable for the authenticity discrimination of some marine TCM.

文摘Chromatographic fingerprinting has been perceived as an essential tool for assessing quality and chemical equivalence of traditional Chinese medicine.However,this pattern-oriented approach still has some weak points in terms of chemical coverage and robustness.In this work,we proposed a multiple reaction monitoring(MRM)-based fingerprinting method in which approximately 100 constituents were simultaneously detected for quality assessment.The derivative MRM approach was employed to rapidly design MRM transitions independent of chemical standards,based on which the large-scale fingerprinting method was efficiently established.This approach was exemplified on QiShenYiQi Pill(QSYQ),a traditional Chinese medicine-derived drug product,and its robustness was systematically evaluated by four indices:clustering analysis by principal component analysis,similarity analysis by the congruence coefficient,the number of separated peaks,and the peak area proportion of separated peaks.Compared with conventional ultraviolet-based fingerprints,the MRM fingerprints provided not only better discriminatory capacity for the tested normal/abnormal QSYQ samples,but also higher robustness under different chromatographic conditions(i.e.,flow rate,apparent pH,column temperature,and column).The result also showed for such large-scale fingerprints including a large number of peaks,the angle cosine measure after min-max normalization was more suitable for setting a decision criterion than the unnormalized algorithm.This proof-of-concept application gives evidence that combining MRM technique with proper similarity analysis metrices can provide a highly sensitive,robust and comprehensive analytical approach for quality assessment of traditional Chinese medicine.

文摘With the modernization and internationalization of traditional Chinese medicine(TCM),the requirement for quality control has increased.The quality marker(Q-marker)is an important standard in this field and has been implemented with remarkable success in recent years.However,the establishment of Qmarkers remains fragmented and the process lacks systematicity,resulting in inconsistent quality control and insufficient correlation with clinical efficacy and safety of TCM.This review introduces four multimodal integrated approaches that contribute to the discovery of more comprehensive and accurate Qmarkers,thus aiding in the establishment of new quality control patterns based on the characteristics and principles of TCM.These include the whole-process quality control strategy,chemical-activity-based screening method,efficacy,safety,and consistent combination strategy,and TCM theory-guided approach.Furthermore,methodologies and representative examples of these strategies are described,and important future directions and questions in this field are also proposed.
文摘To investigate the principles of differentiation and prescription of traditional Chinese medicine(TCM)treatment for cancerous fever by logistic stepwise regression and literature investigation,the following databases were searched(from inception to June 30th 2016):CNKI,CSPD,CBM,PubMed,MEDLINE,EMBASE.According to the inclusion and exclusion criteria,literatures about TCM in the treatment of cancerous fever were screened.The frequency about cancerous fever of differential syndrome types,formulas and herbs were analyzed by logistic multiple regression analysis.The model about syndrome types and formula application of cancerous fever was established.The result showed formula prescriptions used most frequently were Qing-Hao-Bie-Jia decoction,Xiao-Chai-Hu decoction,Xue-Fu-Zhu-Yu decoction,Bai-Hu decoction,San-Ren decoction.The main syndrome types of cancerous fever were as follows:Toxic heat flourishing type,Yin deficiency type,Stagnation of damp-heat type,Qi-Yin deficiency type,Liver meridian depressed-heat type,Blood stasis type and Yang deficiency type.Based on regression equation of the syndrome types,the corresponding herbs of this seven syndrome types were Radix Bupleuri,Radix Paeoniae Alba and Scutellaria Baicalensis Georgi;Radix Bupleuri and Cortex Moutan;Radix Paeoniae Rubra and Radix Angelicae Sinensis;Rhizoma Pinelliae,Cortex Lycii Radicis and Radix Paeoniae Alba;Herba Artemisiae Annuae,Gypsum Fibrosum and Poria Cocos;Rhizoma Anemarrhenae,Scutellaria Baicalensis Georgi and Radix Codonopsis;Radix Bupleuri respectively.The results of Logistic regression analysis and frequency of herbs statistics can provide reference for differentiation and treatment of cancerous fever in clinical.

文摘Objective:The objective of this study is to explore the current situation,clinical research hot spots,and trends of traditional Chinese medicine(TCM)nursing technology in rheumatoid arthritis(RA),to draw a panorama of clinical research,to provide the basis and clues for subsequent high-level evidence integration and further in-depth research.Materials and Methods:Seven databases including China National Knowledge Infrastructure,Chinese Scientific Journal Database(VIP),Wanfang Database,Chinese Biomedical Literature Database(Sino-Med),PubMed,Embase,and the Cochrane Library were searched.The bibliometric method and visualization software CiteSpace were used to conduct a multi-dimensional analysis of the included literature.Results:A total of 805 pieces of literature were included(of them,one was written in German and four in English),and the number of published literature showed an increasing trend year by year.There were only 30(3.73%)pieces of literature published in nursing journals.The hot spots of the co-occurrence map were concentrated in:(1)the nursing of fumigation in RA;(2)the efficacy evaluation of TCM nursing technology on pain,joint malformation,and joint dysfunction caused by RA.The included literature themes focused on five TCM technologies:fumigation,moxibustion,acupoint patching,acupoint injection,and Chinese herbal soaking.The beginning year and strength in TCM fumigation were 2012 and 5.2,and those in moxibustion were 2011 and 3.38.Conclusion:The related studies are on the rise.It has entered the field of international readers with clear research hot spots.However,there are still shortcomings such as little literature published in Chinese nursing core journals,few non-Chinese-related documents,and a lack of international exchanges and cooperation.The current research hot spots in this field are TCM fumigation,and the cutting-edge trend of future research may be fumigation and moxibustion technology.It is suggested that further research can focus on evidence integration and original research on the self-optimization of these two techniques.

文摘Objective Through bibliometrics and visual analysis of the related studies on traditional Chinese medicine(TCM)treatment of immune thrombocytopenia(ITP),this study aims to sort out the overall research progress,hotspots,and trends in this field,and provide reference for further research in ITP.Methods The articles on ITP treated by TCM were retrieved from China National Knowledge Infrastructure(CNKI),Wanfang Database,China Science and Technology Journal Database(VIP),Web of Science Core Collection(WOSCC),and PubMed.The retrieval time was from the establishment of the databases to July 31,2022.VOSviewer,CiteSpace,Carrot2,and Note-Express were used for data analysis of the articles in terms of their quantities,types,and journals,and for visualization of research hotspots,authors,institutions,and keywords.Results 1493 Chinese articles and 40 English articles were included.The articles in Chinese mainly focus on clinical trial research and clinical experience summary,while the English articles mainly focus on clinical trial research and animal research.The Chinese articles were published in 317 Chinese journals,while English articles were published in 29 English journals.Research hotspots include the clinical syndrome differentiation of ITP,the therapeutic effect of TCM compounds on ITP,and the mechanism of ITP treatment.Keyword analysis shows that there are many research achievements in integrated traditional Chinese and western medicine treatment,clinical research,famous doctors’experience,TCM treatment,cellular immunity,and humoral immunity.The authors with the most articles in Chinese and English are Professor CHEN Xinyi and Professor MA Rou,respectively,and the research institutions with the most articles are Dongzhimen Hospital of Beijing University of Chinese Medicine and Xiyuan Hospital of China Academy of Chinese Medical Sciences.Chinese herbs often used to treat ITP clinically include Xianhecao(Agrimoniae Herba),Nvzhenzi(Ligustri Lucidi Fructus),Mohanlian(Ecliptae Herba),Zhongjiefeng(Sarcandrae Herba),etc.,and the prescription usually used to treat ITP include Guipi Decoction(归脾汤),Xijiao Dihuang Decoction(犀角地黄汤),Bazhen Decoction(八珍汤),Erzhi Pill(二至丸),and Xiaochaihu De-coction(小柴胡汤).The main development trends toward retrospective study,TCM treatment mechanism,and data mining.

文摘Background:Coronavirus disease 2019 is still spreading all over the world and without any specific medicine.Traditional Chinese medicine has played an important role in the control of epidemics in China and accumulated many experiences.The aim of this study is to summarize the law of traditional Chinese medicine and provide reference for coronavirus disease 2019 traditional Chinese medicine treatment.Methods:We collected the treatment plans of traditional Chinese medicine for coronavirus disease 2019 in 24 provinces of China to evaluate the basic Chinese medicine formulas,and the efficacy types,frequency,“Four Characters”,“Five Tastes”,“Channel Tropism”of herbal medicines.Combination rules between medicines were calculated by Apriori algorithm and Clementine12.0.Results:The formulas for“Relieving Exterior Syndrome”,“Expelling Dampness”,“Clearing Heat”and“Supplementing”have highest proportion.In prevention period and medical observation period,the medicines mainly have effect for“Clearing Heat”,“Relieving Exterior Syndrome”and“Supplementing”.In treatment period,medicines for“Clearing Heat”,“Relieving Superficies”and“Relieving Dampness”had higher frequency.The“Four Characters”of Chinese medicines in all treatment plans were mainly“Cold”and“Warm”.Medicines in the treatment plans always belong to“Pungent Taste”,“Bitter Taste”and“Sweet Taste”.For“Channel Tropism”,most medicines turned over to the“Lung”,“Stoma”and“Spleen”.The common medicine combinations in the recommended formulas during the prevention period and medical observation period were medicines for“Qi-Supplementing”and“Relieving Superficies”.In treatment period(mild,moderate,severe/critical),medicines for“Dampness Expelling”,“Relieving Superficies”and“Qi-Supplementing”were combined in a high frequency.In recovery period,the medicines for“Dispelling Dampness”combined the medicines for“Promoting Blood Circulation”with highest frequency.Conclusions:We found the characteristics of medicine and prescription rules of traditional Chinese medicine for the treatment of coronavirus disease 2019 and provided scientificity and effectiveness of it.More importantly,it provides reference for the development of new clinical prescriptions for coronavirus disease 2019.

文摘Objective With the increasing publication of clinical practice guidelines(CPG)for Traditional Chinese Medicine(TCM)and Integrated Traditional Chinese and Western Medicine(IM),the standardization and scientifiction of its formulation have gradually attracted many people’s attention.To offer an overview of TCM and IM CPGs published over the past decade and analyze their general characteristics and methodological quality.Methods The China National Knowledge Infrastructure(CNKI)and WANFANG databases were searched for clinical practice guidelines and expert consensus papers from January 2010 to June 2021.Two researchers independently completed the literature screening and cross-checking according to the inclusion and exclusion criteria of CPGs and extracted information on general characteristics and methodological quality of CPGs.Results According to the selection criteria,231 CPGs(EB-CPGs=119,CBCPGs=112)were selected and the number of CPGs published in the 11 years showed an overall upward trend.The vast majority of CPGs used the Western naming system for the diseases,and only 11 CPGs were named of TCM diseases or symptoms.TCM treatments were recommended in 223 CPGs.There were 156 ancient Chinese Medicine literature sources cited in 231 CPGs and opinions and experiences of 62 TCM experts cited in 37 CPGs.The methodological quality of EB-CPGs for TCM and IM were significantly better than CB-CPGs in 11 items.Only 60 EB-CPGs and 7 CB-CPGs designated clear criteria for grading quality of evidence and strength of the recommendations and 74 CPGs presented both the level of evidence and the strength of recommendations.We classified all CPGs according to whether or not they used GRADE,and the results showed that the CPGs using GRADE had higher methodological quality and more standardized reports.Conclusion This research has shown that the quantity and quality of CPGs in both TCM and IM have improved over the time span,but the methodological quality,especially evidence citation,and the use of criteria for grading quality of evidence and strength of the recommendations,still needs to further improvement in the future.
