CRPS can be a d...Background: Complex Regional Pain Syndrome (CRPS), previously known as reflex sympathetic dystrophy and causalgia, is a neuropathic pain condition that usually develops after an injury to an extremity. CRPS can be a debilitating condition with high levels of pain and reduced function. Aim: This case report aims to discuss the multimodal approach in the management of a patient who presented with ongoing poorly controlled pain secondary to CRPS from an injury that happened years prior. Case Presentation: A 45-year-old female was involved in a motor vehicle accident where her right leg was injured. She underwent several surgeries and developed CRPS that significantly reduced her mobility and quality of life. She presented to the pain clinic years after her initial injury and a multimodal regimen was started for her resulting in significantly improved function. Conclusion: CRPS can be a severely debilitating condition. While early diagnosis and management are important, ongoing management in the outpatient chronic pain setting is important in maintaining a good level of function.展开更多

Various public health propaganda methods are used, including community loudspeakers and community bulletin boards. In the meantime, WeChat platform account named “Wuhan neighborhood on WeChat” is registered and opened, and a Group Chat on WeChat is created for grid management. A diversity of on-line and off-line countermeasures against the novel coronavirus outbreak are implemented via the WeChat platform: three-early measures (early discovery, early reporting, and early treatment), centralized management, community-based reporting, hierarchical diagnosis, and treatment system, isolation of mild patients at designated facilities, diagnosis and treatment appointment system for patients with fever, patient transferral in special vehicles, terminal disinfection at homes, psychological support and daily caring plan for special populations. This WeChat platform allows for an all-region, all-around, all-time and entire population-coverage control of novel coronavirus outbreak, with the extension and strengthening of health propaganda, guidance for seeking medical assistance, mass prevention and mass treatment, and health self-management among the community residents. Great effect has been achieved in novel coronavirus control so far. The Atlantic Ocean houses three of these: the Northwest, Southwest and Northeast RMUs. The most studied is, so far, the Northwest RMU, but the other two have focused attention of researchers. In contrast, marine turtles from the African Atlantic region (Southeast) belong to a complex but little-known region. What is their role in the Atlantic? Are these RMUs connected? To understand these questions, research was made of reports and publications concerning the Northeast Atlantic (NEA) RMU. The asymmetry in information and available knowledge about NEA is high when compared to other RMUs. This demonstrates that there is still a lack of conservation programs besides Cape Verde, and that there is difficulty in transforming data (nesting ecology, molecular, telemetry, etc.) into publications. However, this issue is evident for other marine turtle species of East African Atlantic. There is a need for better scientific support, to enable local conservation programs to deliver data in reports, and even scientific publications. There are so many aspects of loggerhead’s life cycle that will only be unravelled by more and better supported studies. This is a paper that, by analysing the available information of the NEA loggerhead RMU, reflects the following steps to address in loggerhead conservation for the African Atlantic coast.展开更多

Advancements in both surgery and anesthesia have made it possible to operate more safely on the elderly population and those older patients with multiple severe co-morbidities that were not routinely possible in the recent past. Regional anesthesiologist have proven to be instrumental in this regard as regional anesthetic/analgesic techniques may now permit surgeons to operate on the elderly who were not ideal surgical candidates or unable to tolerate general anesthesia. In addition, regional techniques provide alternatives that may optimize acute pain control and reduce the incidence of devastating side effects during the perioperative period such as: myocardial infarction, pulmonary embolism, pneumonia, and also increases the opportunity to allow for early ambulation and shorter hospital stays. These anesthetic options now provide the elderly patient with better medical care alternatives, but also can show a significant financial impact on health care system resources. Further understanding on aging molecular biology, physiology and pathophysiology, together with technical improvements of regional anesthetic techniques will continue to make it safer and more efficacious to operate on the elderly population with evidence of reduced morbidity and mortality. Although there is only anecdotal evidence that regional anesthesia(RA) improves survival, there is little doubt that RA plays an important role in perioperative optimization of pain control and decreases pain management complications as well as a reduction in healthcare costs. Beyond traditional operating rooms, elderly patients may increasingly benefit from RA and acute pain management in Emergency Rooms, medical clinics and even within a patient's home. The objective of this field experiment was to study the effects of three ...Changes in both soil organic C storage and soil respiration in farmland ecosystems may affect atmospheric CO2 concentration and global C cycle. The objective of this field experiment was to study the effects of three crop field management practices on soil CO2 emission and C balance in a cotton field in an arid region of Northwest China. The three management practices were irrigation methods(drip and flood), stubble managements(stubble-incorporated and stubble-removed) and fertilizer amendments(no fertilizer(CK), chicken manure(OM), inorganic N, P and K fertilizer(NPK), and inorganic fertilizer plus chicken manure(NPK+OM)). The results showed that within the C pool range, soil CO2 emission during the whole growing season was higher in the drip irrigation treatment than in the corresponding flood irrigation treatment, while soil organic C concentration was larger in the flood irrigation treatment than in the corresponding drip irrigation treatment. Furthermore, soil CO2 emission and organic C concentration were all higher in the stubble-incorporated treatment than in the corresponding stubble-removed treatment, and larger in the NPK+OM treatment than in the other three fertilizer amendments within the C pool range. The combination of flood irrigation, stubble incorporation and application of either NPK+OM or OM increased soil organic C concentration in the 0-60 cm soil depth. Calculation of net ecosystem productivity(NEP) under different management practices indicated that the combination of drip irrigation, stubble incorporation and NPK+OM increased the size of the C pool most, followed by the combination of drip irrigation, stubble incorporation and NPK. In conclusion, management practices have significant impacts on soil CO2 emission, organic C concentration and C balance in cotton fields. Consequently, appropriate management practices, such as the combination of drip irrigation, stubble incorporation, and either NPK+OM or NPK could increase soil C storage in cotton fields of Northwest China.展开更多

With the source regions of the Yangtze and the Yellow River as a case study, this paper presents a conceptual framework for an adaptation management of mountain tourism services according to technical, policy, social and economic dimensions. Previous to this study it verified the designation of protected areas as increasing at a faster rate than ever before, comparatively much faster now in China than southern Africa regions. With the aim of evaluating similarities and differences in the use of management model as a management tool for pro- tected areas in China and southern Africa Region, both qualitative and quantitative data were collected. Qualitative data were mainly from policy documents, scientific articles and magazine reports, whereas quantitative (secondary data) statistical data from Interna- tional Union for Conservation of Nature Resources (IUCN) and World Database on Protected Areas (WDPA). In view of the data sources above, the study found that in China all the protected areas are state owned whilst in southern Africa regions there are some protected areas that are privately owned; also that the models or typology of governance applied are successful for the fact that they both combine co-management or collaborative management, community-conserved areas and private protected areas that are subject to greater success and can help design planning and management than those who use exclusively government management. To this, the study concluded that the use of management model is influenced by the type of governance a country applies to its reserved areas.展开更多

Beliefs about what is importa...On the South Island of New Zealand, Anaesthetists and other Medical Professionals, frequently refer their patients with Complex Regional Pain Syndrome (CRPS) for physiotherapy management. Beliefs about what is important for the management of patients with CRPS are lacking across all medical and allied health disciplines. Difficulties are no gold standard for diagnosis and evidence for intervention methods is moderate or can be conflicting. This paper explores what Physiotherapists believe to be important in a clinical setting for their management of CRPS, as well as documenting and evaluating their interventional methods used in everyday clinical practice across the region of the South Island of New Zealand. This has not been recorded before. Eighty-one Physiotherapists replied to questions on their usual treatment interventions for the management of CRPS, their frequency of use of these treatment interventions, and what they believed to be important in the management of pain and improvement of function. The results demonstrated that CRPS is not a common condition seen regularly by Physiotherapists;that there is a high level of variation between the physiotherapy interventions used and that Physiotherapists’ beliefs regarding interventions used for pain management and functional restoration differ. Education was reported as the most frequently used intervention method. Those physiotherapists seeing CRPS patients more frequently are more likely to use evidence based intervention methods like graded motor imagery or sensory motor training.展开更多

Fixed observation results from 1993to 2002 showed that pools covering about 15% of total area could store up 10% of surfacerunoff, keep 78.1% of eroded soil and 65.4% of lost nutrients. The yearly ratio ofinterception and evapotranspiration in land, storage in pools and drainage was 7:2:1,which ensured the resources and nutrients equilibrium and a benign recycle in thecatchment area system, and benefited the aquatic culture and helped to resist seasonaldrought. Moreover, the results showed that soil erosion modulus decreased significantly,equal to or lower than soil loss tolerance (≤500 tkm-2) in reddish yellow soil regions.Soil organic matter, total and available N content in sloping land, dryland and paddyfield increased steadily (>10%); water storage enhanced by more than 20% in sloping landand dryland in drought season; crop production increased by more than 20%; and productionof trees, fruits, tea and fish as well as land productivity increased yearly.展开更多
The paper is based on a study whose objective is to provide an understanding of the extent to which traditional knowledge and indigenous institutions for natural resource governance remain relevant to solving current ...The paper is based on a study whose objective is to provide an understanding of the extent to which traditional knowledge and indigenous institutions for natural resource governance remain relevant to solving current land degradation issues and how they are integrated in formal policy process in Kilimanjaro Region. Data collection for this study combined qualitative and quantitative methods. A total of 221 individuals from households were interviewed using a structured questionnaire;41 in-depth interviews and 24 focus group discussions were held. Findings indicate that the community acknowledges that there is traditional knowledge and indigenous institutions regarding sustainable land management. However, awareness of the traditional knowledge and practices varied between districts. Rural-based districts were found to be more aware and therefore practiced more of traditional knowledge than urban based districts. Variations in landscape features such as proneness to drought, landslides and soil erosion have also attracted variable responses among the communities regarding traditional knowledge and indigenous practices of sustainable land management. In addition, men were found to have more keen interest in conserving the land than women as well as involvement in other traditional practices of sustainable land management. This is due to the fact that, customarily, it is men who inherit and own land. This, among other factors, could have limited the integration of traditional knowledge and indigenous institutions in village by-laws and overall policy process. The paper concludes by recommending that traditional knowledge and indigenous institutions for sustainable land management should be promoted among the younger generations so as to capture their interest, and ensure that successful practices are effectively integrated into the national policies and strategies.展开更多

In Czech Republic, the field of regional development and managing destinations started to develop in the second half of the 1990s. Organizing tourism is in the phase of building and passes through natural development. Fierce competition between individual destinations imposes natural pressures on creating marketing managements of these destinations and such offer of products that perfectly come up to tourists' expectations. Together with increasing requirements, the demands on quality and well-prepared destinations within home tourism revive. In the final consequence, the level of destination management influences the increase in marketability and attendance in the region. The paper outlines the main theoretical basis, principles and tools of destination management, and subsequently it summarizes the results of the analysis, and conducted survey, the objective of which was to prove the functioning of these principles in practice. The practical verification of the functioning of destination management principles in Czech Republic has been conducted in the South-Moravian region (JMK) which is one of the most-often-visited regions in the country. The set targets have been reached not only by analysing available secondary data, but the techniques of depth interviews with experts in a destination management company have also been used, as well as the techniques of an orientation questionnaire survey among travel agencies and offices that should be one of the partners for a well-functioning destination management company.展开更多

The most studied is, so far, the Northwest RMU, but the other two have focused attention of researchers. In contrast, marine turtles from the African Atlantic region (Southeast) belong to a complex but little-known region. What is their role in the Atlantic? Are these RMUs connected? To understand these questions, research was made of reports and publications concerning the Northeast Atlantic (NEA) RMU. The asymmetry in information and available knowledge about NEA is high when compared to other RMUs. This demonstrates that there is still a lack of conservation programs besides Cape Verde, and that there is difficulty in transforming data (nesting ecology, molecular, telemetry, etc.) into publications. However, this issue is evident for other marine turtle species of East African Atlantic. There is a need for better scientific support, to enable local conservation programs to deliver data in reports, and even scientific publications. There are so many aspects of loggerhead’s life cycle that will only be unravelled by more and better supported studies. This is a paper that, by analysing the available information of the NEA loggerhead RMU, reflects the following steps to address in loggerhead conservation for the African Atlantic coast.
文摘Normal aging is a process that involves loss of functional reserve of most organ systems of the human body, most significantly: cardiovascular, pulmonary, renal and nervous systems. Advancements in both surgery and anesthesia have made it possible to operate more safely on the elderly population and those older patients with multiple severe co-morbidities that were not routinely possible in the recent past. Regional anesthesiologist have proven to be instrumental in this regard as regional anesthetic/analgesic techniques may now permit surgeons to operate on the elderly who were not ideal surgical candidates or unable to tolerate general anesthesia. In addition, regional techniques provide alternatives that may optimize acute pain control and reduce the incidence of devastating side effects during the perioperative period such as: myocardial infarction, pulmonary embolism, pneumonia, and also increases the opportunity to allow for early ambulation and shorter hospital stays. These anesthetic options now provide the elderly patient with better medical care alternatives, but also can show a significant financial impact on health care system resources. Further understanding on aging molecular biology, physiology and pathophysiology, together with technical improvements of regional anesthetic techniques will continue to make it safer and more efficacious to operate on the elderly population with evidence of reduced morbidity and mortality. Although there is only anecdotal evidence that regional anesthesia(RA) improves survival, there is little doubt that RA plays an important role in perioperative optimization of pain control and decreases pain management complications as well as a reduction in healthcare costs. Beyond traditional operating rooms, elderly patients may increasingly benefit from RA and acute pain management in Emergency Rooms, medical clinics and even within a patient's home. Therefore, the focus of this review is directed toward geriatric patients and beneficial effects of RA on outcomes in the elderly.

[Methods]International animal epidemics regionalization management experience and specific equine disease-free zone establishment were analysed and discussed based on the State and OIE principles. [Results]The regionalization management of specific equine disease-free zone with county administrative regions level of international recognition was established first in China,combined with the region's geographical barrier and animal health. [Conclusion]The aim is to to provide experience and reference for other areas to establish specific disease-free zone.

文摘China's price fluctuations increasingly exhibit significant structural characteristics,and since 2003,there have been several rounds of significant structural price rise.The degree of structural rise in the prices of industrial and agricultural products in China is not only higher than in the general developed countries and developing countries,but also more prominent than in other transition economies.And the structural rise in the prices of Chinese industrial and agricultural products exhibits significant economic zone differences:the structural fluctuations are the greatest in the central and western regions,significantly higher than in the eastern regions as well as the national average.From the perspective of causes of structural rise in the prices of Chinese industrial and agricultural products,the government must aim to coordinate the industrial and agricultural investments and bridge the gap in the industrial and agricultural technologies and supply capacity.

文摘Changes in both soil organic C storage and soil respiration in farmland ecosystems may affect atmospheric CO2 concentration and global C cycle. The objective of this field experiment was to study the effects of three crop field management practices on soil CO2 emission and C balance in a cotton field in an arid region of Northwest China. The three management practices were irrigation methods(drip and flood), stubble managements(stubble-incorporated and stubble-removed) and fertilizer amendments(no fertilizer(CK), chicken manure(OM), inorganic N, P and K fertilizer(NPK), and inorganic fertilizer plus chicken manure(NPK+OM)). The results showed that within the C pool range, soil CO2 emission during the whole growing season was higher in the drip irrigation treatment than in the corresponding flood irrigation treatment, while soil organic C concentration was larger in the flood irrigation treatment than in the corresponding drip irrigation treatment. Furthermore, soil CO2 emission and organic C concentration were all higher in the stubble-incorporated treatment than in the corresponding stubble-removed treatment, and larger in the NPK+OM treatment than in the other three fertilizer amendments within the C pool range. The combination of flood irrigation, stubble incorporation and application of either NPK+OM or OM increased soil organic C concentration in the 0-60 cm soil depth. Calculation of net ecosystem productivity(NEP) under different management practices indicated that the combination of drip irrigation, stubble incorporation and NPK+OM increased the size of the C pool most, followed by the combination of drip irrigation, stubble incorporation and NPK. In conclusion, management practices have significant impacts on soil CO2 emission, organic C concentration and C balance in cotton fields. Consequently, appropriate management practices, such as the combination of drip irrigation, stubble incorporation, and either NPK+OM or NPK could increase soil C storage in cotton fields of Northwest China.

文摘Mountain areas are often rich in ecological diversity and recreational opportunities. Mountain tourism is thought to be an effective and important means for maintaining and expanding rural economies and, thus, improving the living conditions of rural societies. As mountain tourism service research is a professional field with several disciplines involved, a multi-disciplinary management pIatform is needed and it facilitates participation in sustainable mountain development by diverse stakeholders. With the source regions of the Yangtze and the Yellow River as a case study, this paper presents a conceptual framework for an adaptation management of mountain tourism services according to technical, policy, social and economic dimensions. The framework is based on a vulnerability assessment of mountain ecosystems, and can serve as a reference for the development of tourism service in other mountain areas.
文摘A comparison of the management models of protected areas between China and the African south region allows reading and evaluating the similarities and differences in the use of management model as a management tool for protected areas and specifi- cally some positive and negative features of the management approaches in these two regions. Previous to this study it verified the designation of protected areas as increasing at a faster rate than ever before, comparatively much faster now in China than southern Africa regions. With the aim of evaluating similarities and differences in the use of management model as a management tool for pro- tected areas in China and southern Africa Region, both qualitative and quantitative data were collected. Qualitative data were mainly from policy documents, scientific articles and magazine reports, whereas quantitative (secondary data) statistical data from Interna- tional Union for Conservation of Nature Resources (IUCN) and World Database on Protected Areas (WDPA). In view of the data sources above, the study found that in China all the protected areas are state owned whilst in southern Africa regions there are some protected areas that are privately owned; also that the models or typology of governance applied are successful for the fact that they both combine co-management or collaborative management, community-conserved areas and private protected areas that are subject to greater success and can help design planning and management than those who use exclusively government management. To this, the study concluded that the use of management model is influenced by the type of governance a country applies to its reserved areas.

文摘On the South Island of New Zealand, Anaesthetists and other Medical Professionals, frequently refer their patients with Complex Regional Pain Syndrome (CRPS) for physiotherapy management. Beliefs about what is important for the management of patients with CRPS are lacking across all medical and allied health disciplines. Difficulties are no gold standard for diagnosis and evidence for intervention methods is moderate or can be conflicting. This paper explores what Physiotherapists believe to be important in a clinical setting for their management of CRPS, as well as documenting and evaluating their interventional methods used in everyday clinical practice across the region of the South Island of New Zealand. This has not been recorded before. Eighty-one Physiotherapists replied to questions on their usual treatment interventions for the management of CRPS, their frequency of use of these treatment interventions, and what they believed to be important in the management of pain and improvement of function. The results demonstrated that CRPS is not a common condition seen regularly by Physiotherapists;that there is a high level of variation between the physiotherapy interventions used and that Physiotherapists’ beliefs regarding interventions used for pain management and functional restoration differ. Education was reported as the most frequently used intervention method. Those physiotherapists seeing CRPS patients more frequently are more likely to use evidence based intervention methods like graded motor imagery or sensory motor training.

文摘Taking an example of Majiayu Catchment Area (14.15 ha) in Taoyuan County of HunanProvince, the soil and water resources dynamics, fertility evolution characteristics andland productivity changing situation were studied. Fixed observation results from 1993to 2002 showed that pools covering about 15% of total area could store up 10% of surfacerunoff, keep 78.1% of eroded soil and 65.4% of lost nutrients. The yearly ratio ofinterception and evapotranspiration in land, storage in pools and drainage was 7:2:1,which ensured the resources and nutrients equilibrium and a benign recycle in thecatchment area system, and benefited the aquatic culture and helped to resist seasonaldrought. Moreover, the results showed that soil erosion modulus decreased significantly,equal to or lower than soil loss tolerance (≤500 tkm-2) in reddish yellow soil regions.Soil organic matter, total and available N content in sloping land, dryland and paddyfield increased steadily (>10%); water storage enhanced by more than 20% in sloping landand dryland in drought season; crop production increased by more than 20%; and productionof trees, fruits, tea and fish as well as land productivity increased yearly.
文摘The paper is based on a study whose objective is to provide an understanding of the extent to which traditional knowledge and indigenous institutions for natural resource governance remain relevant to solving current land degradation issues and how they are integrated in formal policy process in Kilimanjaro Region. Data collection for this study combined qualitative and quantitative methods. A total of 221 individuals from households were interviewed using a structured questionnaire;41 in-depth interviews and 24 focus group discussions were held. Findings indicate that the community acknowledges that there is traditional knowledge and indigenous institutions regarding sustainable land management. However, awareness of the traditional knowledge and practices varied between districts. Rural-based districts were found to be more aware and therefore practiced more of traditional knowledge than urban based districts. Variations in landscape features such as proneness to drought, landslides and soil erosion have also attracted variable responses among the communities regarding traditional knowledge and indigenous practices of sustainable land management. In addition, men were found to have more keen interest in conserving the land than women as well as involvement in other traditional practices of sustainable land management. This is due to the fact that, customarily, it is men who inherit and own land. This, among other factors, could have limited the integration of traditional knowledge and indigenous institutions in village by-laws and overall policy process. The paper concludes by recommending that traditional knowledge and indigenous institutions for sustainable land management should be promoted among the younger generations so as to capture their interest, and ensure that successful practices are effectively integrated into the national policies and strategies.
文摘Tourism is not only a significant tool of prosperity in well-known tourist destinations, but it is also an important potential development element in developing rural regions. In Czech Republic, the field of regional development and managing destinations started to develop in the second half of the 1990s. Organizing tourism is in the phase of building and passes through natural development. Fierce competition between individual destinations imposes natural pressures on creating marketing managements of these destinations and such offer of products that perfectly come up to tourists' expectations. Together with increasing requirements, the demands on quality and well-prepared destinations within home tourism revive. In the final consequence, the level of destination management influences the increase in marketability and attendance in the region. The paper outlines the main theoretical basis, principles and tools of destination management, and subsequently it summarizes the results of the analysis, and conducted survey, the objective of which was to prove the functioning of these principles in practice. The practical verification of the functioning of destination management principles in Czech Republic has been conducted in the South-Moravian region (JMK) which is one of the most-often-visited regions in the country. The set targets have been reached not only by analysing available secondary data, but the techniques of depth interviews with experts in a destination management company have also been used, as well as the techniques of an orientation questionnaire survey among travel agencies and offices that should be one of the partners for a well-functioning destination management company.
