Interaction between XRCC1 Polymorphisms and Intake of Long-Term Stored Rice in the Risk of Esophageal Squamous Cell Carcinoma:A Case-Control Study

Objective This study aimed to explore the roles of three common single nucleotide polymorphisms in the X-ray repair cross-complementing group-1 gene （XRCC1） and of life style factors and their possible interactions in the risk of esophageal squamous cell carcinoma （ESCC） in China. Methods A population-based case-control study of 432 cases and 915 controls was conducted in Yangzhong County, Jiangsu Province, China. Subjects were interviewed by trained interviewers using a structured questionnaire that included questions on demographics and life style. XRCC1 genotypes were analyzed using a polymerase chain reaction based restriction fragment length polymorphism （PCR-RFLP） assay. Unconditional logistic regression analysis was used to calculate adjusted odds ratios （aORs） and 95% confidence intervals （CIs） for associations of ESCC with XRCCI polymorphisms and lifestyle-related factors. Results Both the drinking of river water and alcohol intake history were significantly associated with an increased risk of ESCC among men with aORs of 4.20 （95% Ch 2.90-6.07） and 2.03 （95% Ch 1.43-2.89）, respectively. For women, the corresponding odds ratios were 8.37 （95% CI： 5.09-13.75） for river water drinking and 12.78 （95% CI： 2.69-60.69） for long-term stored rice intake. After the XRCC1 G28152A polymorphism was adjusted for potential confounders, subjects with GA and AA genotypes had an increased risk for ESCC （aOR： 1.21, 95% CI： 0.93-1.56）, compared with subjects with a GG genotype, and a positive multiplicative interaction between intake of long-term stored rice and the XRCC1 G28152A polymorphism was observed （P=O.009）.Conclusions Our findings suggest that both lifestyle-related factors, including drinking river water, long-term stored rice and alcohol intake, and the XRCC1 G28152A polymorphism were possible risk factors for ESCC, and that the XRCC1 G28152A polymorphism modified the effect of long-term stored rice intake on the risk of ESCC among Chinese people.

Malaria Prevalence and Associated Risk Factors among Batwa Indigenous People of Kanungu District in Southwestern Uganda: Does “Place” Matter?

Introduction: Malaria is an important disease, causing high morbidity and mortality, especially in Sub-Saharan Africa. Measuring malaria prevalence using malaria rapid diagnostic tests (mRDTs), particularly among a vulnerable population sub-group, is a vital public health step in discovering effective methods of prevention and control. This study set out to examine the association between “place” and other risk factors with malaria prevalence among Batwa Indigenous People (IP). Methods: An analytical cross-sectional study design was implemented. Two surveys January 2014 (n = 572) and April 2014 (n = 541) involving interviews and testing for malaria using mRDTs were conducted in 10 Batwa settlements where a total of 1113 Batwa of all ages were surveyed and tested. The data were first compiled in MS Excel and then imported and analyzed using STATA ver.14. Descriptive statistics, were generated, followed by bivariable and multivariable regression model analysis to establish associations between the predictor and outcome variables with p ≤ 0.05 considered statistically significant. Results: Overall prevalence was 13.94% (n = 146). There is a significant relationship between settlement (place) and malaria prevalence AOR 11.7, 95% CI (1.38 - 98.93), p-value = 0.02. More males 16.97% (n = 84) tested positive compared to females 11.23% (n = 62) but there was no statistically significant association between gender and mRDT (p-value > 0.005). Children less than 5 years registered high prevalence and there was a significant relationship between age and mRDT (p-value ≤ 0.005). Wealth proxy indicators showed no association with prevalence p-value = 0.390. Season had no association with prevalence (p-value = 0.80). However, the proportion of the day spent in the forest/woodlands was significantly associated with malaria prevalence COR 12.83, 95% CI (1.14 - 143.73) p-value = 0.04. Low elevation was significantly associated with malaria prevalence COR 2.42, 95% CI (1.32 - 4.41), p = 0.004 but sleeping under a net and level of education did not show any association with malaria prevalence. Conclusion: This study highlights the importance of place in predicting malaria prevalence among Batwa Indigenous People a marginalized and remortely located sub-population. This study has shown that place matters in determining malaria prevalence. However, other factors like age, elevation and gender also contribute to malaria prevalence. Batwa have higher prevalence than the national and even non-indigenous populations in the same district. We recommend targeting hotspots intervention approach since it has proven reasonable impact on reducing malaria prevalence.

The HIV Infection: Clarification of Its Legal Classification of Transmission and Measures to Protect Societies from Burdening Caused by Social and Medical Care Services for the Coming Decades

This article addresses issues regarding the transmission of HIV;without the combination antiretroviral treatment (cART), HIV causes a fatal outcome of those infected in most cases. First, legal issues: For years, controversial discussions have dealt with the subject of the legal classification of HIV infection, such as “… criminalization of HIV exposure might limit access to and uptake of HIV prevention services…” Based on the rule of law of a constitutional state, we explain the legal principles that serve to protect the legal rights of its citizens. The state has to protect its citizens from harm by other people. The prosecution and conviction of a specific person for a proven HIV infection are legal. Therefore, general decriminalization of HIV infection would undermine the right of thereby harmed citizens to compensation. Second, HIV prevention strategies: Based on the Test and Treatment Strategy (TASP)1, controlled studies were undertaken to find out which framework conditions could improve their benefit. We outline concepts that can help to curb the still ongoing spread of HIV: By providing early HIV diagnosis and ongoing HIV care services as part of updated education and prevention campaigns. Also, concerted, comprehensive campaigns are required to demonstrate further impacts of HIV infection: Both on the quality of life of infected individuals due to the development of non-communicable diseases and the increasing burden to societies as a whole.

Ready or Not: Quezon City Mayor Joy Belmonte and Her Crisis Response Strategy During the COVID-19 Pandemic

Using Situational Crisis Communication Theory(SCCT)by Coombs(2007),this study analyses the case of Quezon City Mayor Joy Belmonte’s CoViD-19 Crisis and her crisis response strategy to address the situation.The study looks at the crisis communication of Quezon City on issues stemming from the Covid-19 pandemic.The SCCT provides a framework for the organization facing a different complex case of crisis to protect the reputational organization’s assets.The organization’s ability to select the most appropriate and beneficial response helps them deal with an emergency and crisis(Coombs,2007).Simultaneously,in this paper,SCCT was used to examine the stakeholder’s perceptions about the organization’s action and response by Mayor Belmonte on the crisis.This paper concludes Mayor Belmonte did not follow the recommendations of SCCT by combining Deny,such as Attacks the Accuser,Deny and Scapegoat tactics,and Diminish strategies such as Excuse and Justice tactics,to respond to the wide variety of stakeholder’s groups.Mayor Belmonte’s crisis response strategies and tactics do not align with the SCCT.The theory recommends corrective action and response such as Rebuild as well as Bolstering strategies are“considered as the most effective communication of crisis response strategy when an organization perceived a strong attribution of crisis responsibility and results in a severe reputational threat”(Coombs,2007).By reviewing the situation that Mayor Belmonte faced,this paper concludes that she was under severe threat.Mayor Belmonte’s CoViD-19 crisis is the preventable cluster based on the overall stakeholder’s perception of the crisis.The preventable cluster considered her primary,secondary,and additional stakeholders.It depicted an organizational misdeed with and without injuries,and the appropriate responses to adapt to re-establish the organization’s reputation is a Rebuild strategy because interventions such as Compensation and Apology should work to improve the organization’s reputation.Conversely,Mayor Belmonte chose to frequently employ Deny strategy(Attacks the Accuser and Scapegoat)and Diminish strategy(Excuse and Justice)to respond to various stakeholders.
