Risk Evaluation in Blood Donation Using Failure Mode and Effective Analysis

The recognition and management of risk in donation process and blood product is critical to ensure donor and patient safety. To achieve this goal, the failure mode and effects analysis (FMEA) is a convenient method;moreover it was used to prevent the occurrence of adverse events and look at what could go strong at each step. This study aimed to utilize FMEA in central blood bank in Khartoum to evaluate the potential risk and adverse event that may occur during the donation process. According to the severity, occurrence and the detection of each failure mode, the risk priority number (RPN) was calculated to determine which of the failures should take priority to find a solution and applying corrective action to reduce the failure risk. The statistical package for social sciences (SPSS) version 11 was used as descriptive and analytical statistics tool. The FMEA technique provides a systematic method for finding vulnerabilities in a process before they result in an error, and in this study a satisfactory outcome was reached. 展开更多

Failuremode and effects analysis(FMEA)is a widely used safety assessmentmethod inmany fields.Z-number was previously applied in FMEA since it can take both possibility and reliability of information into consideration.However,the use of fuzzy weighted mean to integrate Z-valuations may have some drawbacks and is not suitable for some situations.In this paper,an improved method is proposed based on Z-numbers and the graded mean integration representation(GMIR)to deal with the uncertain information in FMEA.First,Z-numbers are constructed based on the evaluations of risk factors O,S,D for each failure mode by different experts.Second,weights of the three risk factors and experts are determined.Third,the integration representations of Z-numbers are obtained by a newmethod based on the GMIRmethod.Finally,risk priorities of the failure modes are derived considering the weights of experts and risk factors.Two examples and a case study are given to show the use of the proposed method and comparison with other methods.The results show that the proposed method is more reasonable,universal and simple in calculation. 展开更多

The present work adopted Reliability Centered Maintenance (RCM) methodology to evaluate marginal oilfield Early Production Facility (EPF) system to properly understand its functional failures and to develop an efficient maintenance strategy for the system. The outcome of the RCM conducted for a typical EPF within the Niger Delta zone of Nigeria provides an indication of equipment whose failure can significantly affect operations at the production facility. These include the steam generation unit and the wellhead choke assembly, using a risk-based failure Criticality Analysis. Failure Mode and Effect Analysis (FMEA) was conducted for the identified critical equipment on a component basis. Each component of the equipment was analyzed to identify the failure modes, causes and the effect of the failure. The outcome of the FMEA analysis aided the development of a robust maintenance management strategy, which is based on an optimized mix of corrective, preventive and condition-based monitoring maintenance for the marginal oilfield EPF. 展开更多
