There is no standard qu...Carpal tunnel syndrome(CTS) is a common peripheral entrapment neuropathy of the median nerve at wrist level, and is thought to be caused by compression of the median nerve in the carpal tunnel. There is no standard quantitative reference for the diagnosis of CTS. Greyscale sonography and sonoelastography(SEL) have been used as diagnostic tools. The most commonly agreed findings in grey-scale sonography for the diagnosis of CTS is enlargement of the median nerve cross-sectional area(CSA). Several authors have assessed additional parameters. "Delta CSA" is the difference between the proximal median nerve CSA at the pronator quadratus and the maximal CSA within the carpal tunnel. The "CSA ratio" is the ratio of CSA in the carpal tunnel to the CSA at the mid forearm. These additional parameters showed better diagnostic accuracy than CSA measurement alone. Recently, a number of studies have investigated the elasticity of the median nerve using SEL, and have shown that this also has diagnostic value, as it was significantly stiffer in CTS patients compared to healthy volunteers. In this review, we summarize the usefulness of grey-scale sonography and SEL in diagnosing CTS.展开更多
Objectives: To examine the cross-sectional associations of the separate subscales of the Perceived Stress Scale (PSS) and tests measuring cognitive domains in older adults. Methods: 897 adults over the age of 70 free ...Objectives: To examine the cross-sectional associations of the separate subscales of the Perceived Stress Scale (PSS) and tests measuring cognitive domains in older adults. Methods: 897 adults over the age of 70 free of amnestic mild cognitive impairment and dementia and enrolled in the Einstein Aging Study made up the study sample. The PSS-14 was used to measure stress. Three cognitive domains (language, episodic memory, and frontal-executive) had previously been found using principle component analysis. Linear regression analyses were used to determine the relationship between the PSS subscales and cognitive domain function. Results: The study sample had a mean age of 79.1 years and 62.8% were female. Bivariate correlations show that the PSS-14 positively worded subscale of the PSS (PSS-PW) was significantly associated with all three cognitive domains (language: r = -0.15, p < 0.001;episodic memory: r = -0.16, p < 0.001;frontal-executive: r = -0.21, p <0.001) while the negatively worded subscale of the PSS (PSS-NW) was not significantly associated with any cognitive domain. In linear regression analyses adjusted for age, white race, gender, years of education, and depressive symptoms, the PSS-PW remained significantly associated with each of the cognitive domains. The PSS-NW was not associated with any cognitive domains in any model. The PSS-14 was significantly associated with language and episodic memory, but not the frontal-executive domain. Conclusion: Worse PSS-PW scores are associated with reduced cognitive function in the executive, memory, and language domains in nondemented older adults. The PSS-PW subscale correlated better with cognitive function than the overall PSS-14. Future research should evaluate the temporality of the association and if stress reduction therapies improve cognitive performance.展开更多

The incremental displacement constraint equations for the nodes on the section between the two models are established based on the plane section premise of classical beam theory. The method to introduce the constraint equations is derived based on the Updated Largrangian method. The location of the micro-model is predicted by the stress field of the beam element, and the length of the collapsed part is adjusted by the plastic energy in the micro model. Several examples are included to illustrate the efficiency and accuracy of this method.展开更多

The optimality criteria (OC) method and mathematical programming (MP) were combined to found the sectional optimization model of frame structures. Different methods were adopted to deal with the different constrai...The optimality criteria (OC) method and mathematical programming (MP) were combined to found the sectional optimization model of frame structures. Different methods were adopted to deal with the different constraints. The stress constraints as local constraints were approached by zero-order approximation and transformed into movable sectional lower limits with the full stress criterion. The displacement constraints as global constraints were transformed into explicit expressions with the unit virtual load method. Thus an approximate explicit model for the sectional optimization of frame structures was built with stress and displacement constraints. To improve the resolution efficiency, the dual-quadratic programming was adopted to transform the original optimization model into a dual problem according to the dual theory and solved iteratively in its dual space. A method called approximate scaling step was adopted to reduce computations and smooth the iterative process. Negative constraints were deleted to reduce the size of the optimization model. With MSC/Nastran software as structural solver and MSC/Patran software as developing platform, the sectional optimization software of frame structures was accomplished, considering stress and displacement constraints. The examples show that the efficiency and accuracy are improved.展开更多
