Objective:To analyze the influence of professional identity and academic efficacy on the professional attitude of higher vocational nursing students.Methods:A total of 656 higher vocational nursing students were investigated with general information questionnaire,academic self-efficacy scale,professional identity scale,and professional attitude scale of nursing students.Results:The score of professional attitude of higher vocational nursing students was(106.34±9.86),which is found to be in the upper middle level.The professional attitude of higher vocational nursing students is positively correlated with academic self-efficacy(P<0.05),and with learning ability self-efficacy(P<0.01).Professional cognition,professional will,and professional values have a positive predictive effect on the professional attitude of higher vocational nursing students(P<0.05).Conclusions:Academic self-efficacy and professional identity are important predictors of professional attitude.Learning behavior self-efficacy,professional cognition,professional will,and professional values have a significant impact on the establishment of positive professional attitude. 展开更多

Job crafting (JC) is a form of proactive behavior and refers to the initiatives employees that could take to change their tasks or other job characteristics to achieve the best person-job fit. JC consists of six dimensions: seeking structural and social job resources, as well as challenges, hindering job demands, optimizing job demands, and delegating tasks. The aim of the present study was to explore the impact of self-efficacy and work engagement on healthcare professionals’ (HP) proactive behavior. A convenience sample of 295 HP working in Primary and Secondary healthcare structures of Northern Greece completed a questionnaire that included: 1) demographic/occupational characteristics, 2) the Job Crafting Scale, 3) the Generalized Self-Efficacy Scale (GSE) and 4) the Utrecht Work Engagement Scale (UWES). Increased age was found to be associated with decreased levels of seeking social job resources (p < 0.01). Education level was positively associated with increased levels of seeking structural job resources (p < 0.01), seeking challenges (p < 0.05), and optimizing job demands (p < 0.05), while job position was found to be negatively related to delegating tasks (p < 0.01). Self-efficacy was positively associated with increased levels of seeking structural resources (p < 0.001), challenges (p < 0.001), and increased levels of optimizing job demands (p < 0.001), while it was positively associated with decreased levels of delegating tasks (p < 0.05). In addition, increased work engagement was found to be associated with increased levels of seeking social job resources (p < 0.05), challenges (p < 0.01), and optimizing job demands (p < 0.01). Findings highlight the importance of self-efficacy, work engagement and individual characteristics in the realization of a resourceful work environment and HP’ adjustment in the organization. Thus, interventions that promote HP’ JC proactive behaviors should be encouraged. 展开更多

Objective:To assess the effectiveness of simulation-based learning regarding the management of post-COVID complications in terms of knowledge,clinical decision-making ability,and self-efficacy among nursing students.Methods:This was a quasi-experimental study conducted among 1152nd-year nursing students.The participants were selected by a simple random sampling technique.The participants were divided into an experimental(n=56)and a comparison group(n=59)by a random table method.Data were analyzed using descriptive and inferential statistics with SPSS version 20.Results:There were significant differences in mean post-test knowledge scores(P=0.03)and mean post-test self-efficacy scores(P=0.001)between the experimental and the comparison groups while the difference in mean post-test clinical decision-making ability scores between the two groups was non-significant(P=0.07).A positive correlation was found between knowledge and clinical decision-making ability in pre-test(P=0.03)and in post-test(P<0.001)and a non-significant correlation was found between pre-test knowledge and self-efficacy score(P=0.52)among the experimental group.Conclusions:Simulation-based learning regarding the management of post-COVID complications is effective among nursing students.Simulation labs should be established in health care settings where simulation training can be provided for updating the knowledge,clinical decision-making ability,and self-efficacy of nursing personnel during program installment and continuous nursing education. 展开更多

Objectives: Nurses' autonomy is a complex and multi-dimensional concept that has often been overlooked.Although many studies have addressed patients' autonomy,there has been no assessment of nurses' experience of professional autonomy.The present study aimed to assess nurses' lived experiences of professional autonomy in Shiraz,Iran.Methods: The present qualitative study was conducted in Shiraz (Iran) from January 2016 to February 2018.The target population was selected among nursing professionals employed by various hospitals affiliated to Shiraz University of Medical Sciences,Shiraz,Iran.The experiences of the participants were assessed through 14 in-depth semi-structured interviews.The response of the participants was analyzed using Van Manen's 6-step approach for interpretive phenomenology.Results: Based on the analysis of the interviews,4 themes,11 categories,and 13 sub-categories were extracted.The themes were: Advocacy for patients and nurses,independence in the workplace,Involvement in professional decision-making,and Professional accountability.Conclusion: Due to the intense interaction between nurses and patients,a better quality of care will be achieved if the professional autonomy of nurses is ensured.Healthcare authorities and hospital managers should provide the framework and permit the nurses to practically exercise full independence in the workplace. 展开更多

Medical professionalism provides the guidelines that govern the patient-physician relationship. This implicit contract requires that patients be informed before making decisions regarding their medical care. Educating patients about diagnostic and treatment decisions is critical to an informed decision-making process. Shared decision-making is a recent paradigm shift in patient education that allows patients to make decisions based both on the counsel of their physicians and according to their own preferences and values. This approach moves away from previous models that focused on physicians or third-party payers as the arbiters of diagnostic and treatment choices. Urologic surgeons have been at the forefront of shared decision-making research and continue to promote this concept in the most recent American Urological Association Guideline on Detection of Prostate Cancer. Unfortunately, the feefor-service financial structure that predominates in the United States' health care system provides a disincentive for shared decision-making. By promoting patient volume rather than time spent with patients, this system rewards physicians who spend less time educating patients about diagnostic and treatment options. Therefore, to promote adherence to the educational responsibility inherent in medical professionalism, werecommend physician payment reform that rewards physicians for time spent with patients rather than the volume of patients seen. 展开更多

Adolescents and young adults(AYA)with a cancer diagnosis or those at risk for cancer due to hereditary cancer syndromes may benefit from genetic counseling and testing not only to manage personal risk but also to address reproductive concerns,especially fertility.The opportunity for genetic counselors to provide important risk information is relevant to both the newly diagnosed as well as to unaffected carriers and survivors.However,genetic counselors may need additional training in reproductive options related to AYA cancer to provide this valuable counsel.This commentary uses hereditary breast and ovarian cancer syndrome as a model to highlight important considerations when discussing preimplanatation genetic diagnosis and prenatal diagnosis,particularly in the context of expanded testing for hereditary cancer risk including multigene panels or whole exome or whole genome sequencing.Other hereditary cancers are also addressed;however,less is known about the psychosocial and fertility concerns in these AYA populations.Additionally,we provide an overview of the concept of"oncofertility"-the linkage between cancer care and reproductive medicine that aims to expand the reproductive opportunities of cancer patients-and offer support for the expansion of guidelines to include genetic counselors in AYA cancer patients’treatment planning related to reproductive health and fertility. 展开更多

Relationship between Digital Literacy and Employment Ability of Vocational Undergraduate Students:The Parallel Mediating Role of Future Time Perspective and Career Decision Self-Efficacy

Purpose:To explore the relationship between digital literacy and the employability of college students,and to investigate the mechanism by which future time perspective and career decision self-efficacy play a role.Method:A survey was conducted on 577 college students using the Digital Literacy Scale,Employability Scale,Career Decision Self-Efficacy Scale,and Future Time Perspective Scale.Results:(1)Digital literacy,future time perspective,career decision self-efficacy,and employability are all significantly positively correlated with each other(r=0.65-0.78,P<0.001).(2)Digital literacy significantly positively predicts future time perspective,career decision self-efficacy,and employability.At the same time,future time perspective and career decision self-efficacy can both positively predict employability significantly.(3)Future time perspective and career decision self-efficacy have a parallel mediating effect between digital literacy and the employability of vocational undergraduate students.Conclusion:Digital literacy can directly affect college students’employability and can also indirectly affect college students’employability through future time perspective and career decision self-efficacy. 展开更多

Objective:This study aims to investigate the self-efficacy of college students during the Coronavirus disease 2019(COVID-19)pandemic and to conduct an empirical analysis on its influence mechanism on decision-making and coping skills to provide reference for decision support to improve the coping behavior of college students during public health emergencies.Methods:The study recruited a total of 1,346 college students in Shaanxi.Data were collected via a questionnaire and analyzed using SPSS 22.0 and AMOS 22.0.Result:The self-efficacy of the college students exerted a significantly positive impact on their coping decisions(β=0.754)and coping skills(β=0.822)during the COVID-19 pandemic.Both passed the significance test at a 99.9%confidence interval(99%CI).Conclusion:During the COVID-19 pandemic,improving the self-efficacy of college students can significantly positively influence their coping behaviors.Therefore,providing effective interventions that target the self-efficacy of college students and encouraging them to adopt positive coping behaviors is necessary. 展开更多

目的 探讨护理本科实习生自我效能和职业认同在转型冲击与职业决策困难间的中介作用。方法 2022年12月—2023年3月采用方便抽样法,采用护理本科实习生转型冲击评价量表、一般自我效能感量表、护生职业认同问卷、职业决策困难问卷对来自... 目的 探讨护理本科实习生自我效能和职业认同在转型冲击与职业决策困难间的中介作用。方法 2022年12月—2023年3月采用方便抽样法,采用护理本科实习生转型冲击评价量表、一般自我效能感量表、护生职业认同问卷、职业决策困难问卷对来自北京市、山东省、河南省13所高校的365名护理本科实习生进行问卷调查。结果 护理本科实习生的职业决策困难得分为(37.85±9.37)分。自我效能、职业认同在转型冲击与职业决策困难之间起中介作用,包括自我效能的独立中介效应(效应值0.145)、职业认同的独立中介效应(效应值0.171)、自我效能—职业认同的链式中介效应(效应值0.048)。结论 护理本科实习生转型冲击通过自我效能和职业认同间接对职业决策困难发挥作用。护理教育者可通过降低护理本科实习生的转型冲击,提高其自我效能和职业认同,从而减轻实习生的职业决策困难。 展开更多

高职护生职业认同的影响因素分析

目的:了解高职护生职业认同现状,分析共情、职业决策自我效能和职业认同的关系,为提升高职护生职业认同提供对策和依据。方法:采用一般资料调查表、护生共情量表、大学生职业决策自我效能感量表和护生职业认同问卷,对1269名高职护生进... 目的:了解高职护生职业认同现状,分析共情、职业决策自我效能和职业认同的关系,为提升高职护生职业认同提供对策和依据。方法:采用一般资料调查表、护生共情量表、大学生职业决策自我效能感量表和护生职业认同问卷,对1269名高职护生进行问卷调查。分析高职护生职业认同的影响因素及高职护生共情、职业决策自我效能和职业认同的相关关系。结果:单因素分析显示,不同性别、年级、家庭氛围、与同学关系、与老师关系、学习状态、学习成绩和护理是否第一专业的高职护生职业认同得分差异均有统计学意义(P<0.01)。Pearson相关分析显示,高职护生共情各维度得分及总分与职业认同总分均呈明显正相关关系(P<0.01),高职护生职业决策自我效能各维度得分及总分与职业认同总分均呈明显正相关关系(P<0.01)。多元线性回归分析显示,高职护生职业认同从大到小的影响因素依次为职业决策自我效能、学习状态、护理是否第一志愿、共情和与老师关系(P<0.05~P<0.01)。结论:高职护生的共情能力越强,职业决策自我效能感越高,职业认同程度越高。高职护生的职业认同有待加强,学校管理者和护理教育者应意识到职业认同的重要性,采取有针对性的干预措施提升高职护生的职业认同感,稳定护理队伍。 展开更多

目的从患者和医护人员的角度归纳分析2型糖尿病患者初始使用胰岛素的决策参与影响因素的质性研究,为促进患者的决策参与提供参考。方法系统检索CINAHL,Cochrane Library,EMBASE,PubMed,Web of Science,PsycINFO,中国知网,万方,维普和Sin... 目的从患者和医护人员的角度归纳分析2型糖尿病患者初始使用胰岛素的决策参与影响因素的质性研究,为促进患者的决策参与提供参考。方法系统检索CINAHL,Cochrane Library,EMBASE,PubMed,Web of Science,PsycINFO,中国知网,万方,维普和SinoMed收录的关于2型糖尿病患者初始胰岛素决策参与影响因素的质性研究,检索日期从建库至2023年9月30日,采用澳大利亚乔安娜布里格斯研究所(Joanna Briggs Institute,JBI)循证卫生保健中心质性研究质量评价工具对纳入文献进行评价,并采用汇集性整合方法对结果进行整合。结果共纳入文献19篇,提炼出20个研究结果,归纳成7个类别(患者决策相关价值观、患者决策参与的角色偏好、患者自身疾病情况、医护人员在患者决策参与中的角色定位、医护人员的专业素质、患者与医护人员的关系、医疗机构支持程度),共4个整合主题(患者个人因素、医护人员因素、患者-医护人员互动因素、医疗机构因素)。结论2型糖尿病患者胰岛素初始使用的决策参与受到患者自身、医护人员和医疗机构的影响,需要多方人员努力,不仅引导患者积极参与决策,医护人员和医疗机构也要提供有效的决策支持。 展开更多

目的:调查山东省血液净化中心护士临床决策能力现状并分析其影响因素,以期为血液净化护士临床决策能力的提高提供科学依据。方法:选取651名血液净化护士作为研究对象,采用一般资料调查问卷、护理临床决策量表和血液净化护士专业胜任力... 目的:调查山东省血液净化中心护士临床决策能力现状并分析其影响因素,以期为血液净化护士临床决策能力的提高提供科学依据。方法:选取651名血液净化护士作为研究对象,采用一般资料调查问卷、护理临床决策量表和血液净化护士专业胜任力问卷作为测评工具进行调查,单因素分析、Spearman相关分析描述血液净化护士临床决策能力现状,并分析其影响因素。结果:血液净化护士临床决策能力总分为122(116,130)分,处于中等及以上水平;血液净化护士胜任力总分为90(66,115)分,处于中等及以上水平;性别、受教育程度、护龄、专业技术职称、人事关系、职务和自我感知护患关系是否平等是血液净化护士临床决策能力的主要因素,血液净化护士胜任力总分和各维度与临床决策能力总分及各维度呈正相关(P<0.05)。结论:为了提高血液净化护士的临床决策能力,除了提升护士的受教育水平、职称和营造良好的护患关系等外,护理管理者和护士应该加强血液净化专业胜任力的培养,提高护理人员的临床决策能力。 展开更多

高职院校博士学位青年教师专业发展路径的决策机制——基于对19名高职院校博士学位青年教师的扎根理论研究

基于对19名高职院校博士学位青年教师的深度访谈,分析高职院校博士学位青年教师专业发展的决策机制并构建“成本—收益”决策模型,结果发现:高职院校博士学位青年教师对成本付出与预期收益的理性权衡是推动他们进行专业发展决策的直接... 基于对19名高职院校博士学位青年教师的深度访谈,分析高职院校博士学位青年教师专业发展的决策机制并构建“成本—收益”决策模型,结果发现:高职院校博士学位青年教师对成本付出与预期收益的理性权衡是推动他们进行专业发展决策的直接作用机制;博士学位青年教师对成本收益的理性权衡受到外部制度环境、专业发展平台、教师协作模式和自身专业发展基础的影响;生涯发展阶段是博士学位青年教师专业发展路径决策的背景性因素,它会影响博士学位青年教师对一些重要影响因素的价值判断进而影响其对成本收益的权衡。为了实现博士学位青年教师专业发展期待与高职院校发展需求的匹配,高职院校应基于学校发展需求审慎招聘博士学位青年教师,以技术学科建设为核心强化应用导向科研机制建设,以科教融汇为方向助力博士学位青年教师转型发展。 展开更多

根据《国家地震应急预案》相关规定,针对近些年我国发生的汶川、玉树、芦山及鲁甸等特别重大地震,均启动了地震应急Ⅰ级响应。但几次地震的伤亡人数及震害影响等都有明显的量级差别,应急救援行动中队伍派遣数量、灾情获取手段、救援装... 根据《国家地震应急预案》相关规定,针对近些年我国发生的汶川、玉树、芦山及鲁甸等特别重大地震,均启动了地震应急Ⅰ级响应。但几次地震的伤亡人数及震害影响等都有明显的量级差别,应急救援行动中队伍派遣数量、灾情获取手段、救援装备配备等都不尽相同。本文对这4次地震开展的专业救援性措施进行了系统的总结和对比分析,更准确地了解政府应对大震巨灾时的需求,为抗震救灾指挥部救援决策提出有针对性的建议。 展开更多
