Sodium homeostasis disorder is one of the most common abnormal symptoms of elderly patients in intensive care unit(ICU),which may lead to physiological disorders of many organs.The current prediction of serum sodium i...Sodium homeostasis disorder is one of the most common abnormal symptoms of elderly patients in intensive care unit(ICU),which may lead to physiological disorders of many organs.The current prediction of serum sodium in ICU is mainly based on the subjective judgment of doctors’experience.This study aims at this problem by studying the clinical retrospective electronic medical record data of ICU to establish a machine learning model to predict the short-term serum sodium value of ICU patients.The data set used in this study is the open-source intensive care medical information set Medical Information Mart for Intensive Care(MIMIC)-IV.The time point of serum sodium detection was selected from the ICU clinical records,and the ICU records of 25risk factors related to serum sodium were extracted from the patients within the first 12 h for statistical analysis.A prediction model of serum sodium value within 48 h was established using a feedforward neural network,and compared with previous methods.Our research results show that the neural network learning model can predict the development of serum sodium in patients using physiological indicators recorded in clinical electronic medical records within 12 h,and has better prediction effect than the serum sodium formula and other machine learning models.展开更多

At the multi-walled carbon nanotube (MWNT) modified electrode, the electrochemical behavior of UPT enhanced greatly. In the presence of bovine serum albumin (BSA), the peak currents of UPT decreased linearly due to the formation of a super-molecular complex. Methods: 80 patients with congestive heart failure were divided into two groups, namely supplying and control group. Serum sodium and chlorine were roeasured in all these patients. All treatments but supplying sodium and chlorine were same between the supplying and control groups. Results: According to NYHA, patients who were in class Ⅳ had lower level of serum sodium and chlorine than those in class Ⅱ ( P 〈0.05). The heart function was improved after the level of serum sodium and chlorine were raised. Conclusions: The concentration of serum sodium and chlorine relates to the severity of heart failure. The therapy of supplying sodium and chlorine is an effective way to decrease death rate.展开更多

Sa(10 mg/kg body weight/day), and powder form of RSL(50 mg/kg body weight/day) were provided as food supplement orallty. Blood indices were measured using commercially available kits through colorimetric methods.Results: It was observed that lactate dehydrogenase, alkaline phosphatase, alanine aminotransferase, and aspartate aminotransferase activities were significantly(P < 0.05)higher in Sa-treated mice than those in the control group. RSL significantly reduced Sainduced elevation of the activities of these enzymes in serum significantly(P < 0.05).Serum butyrylcholinesterase activity and high density lipoproteins cholesterol levels in Satreated mice were significantly(P < 0.05) lower than the control group, and the food supplementation of RSL could significantly(P < 0.05) prevent the reduction of Sa-mediated serum butyryl cholinesterase activity and high density lipoproteins cholesterol levels. RSL could also reduce the Sa-induced elevation of serum urea level significantly(P < 0.05).Conclusions: Results of this study suggest the protective or ameliorating effects of RSL on Sa-induced perturbation of blood indices are related to the hepatic, cardiovascular and kidney dysfunction. Therefore, RSL may be useful to reduce arsenic toxicity in human in the future.展开更多

文摘目的:观察西维来司他钠治疗脓毒症伴急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)的效果。方法:将阳江市人民医院2023年6—12月收治的脓毒症伴ARDS患者总计60例,以随机数字表法分成两组,各30例,其中一组以常规治疗为对照组,另一组以常规治疗配合西维来司他钠治疗为研究组,均治疗7 d。比较两组血清学指标(炎症指标、血小板计数、血气指标),急性生理和慢性健康状况Ⅱ(acute physiology and chronic health evaluationⅡ,APACHEⅡ)评分、肺损伤评分,序贯器官衰竭估计(sequential organ failure assessment,SOFA)评分,机械通气时间及病死率。结果:治疗前,两组炎症指标、血气指标、血小板计数、APACHEⅡ评分、肺损伤评分、SOFA评分相比,差异均无统计学意义(P>0.05);治疗后,两组血清炎症指标、动脉血二氧化碳分压、APACHEⅡ评分、肺损伤评分、SOFA评分均较治疗前显著下降,且研究组均低于对照组,动脉血氧饱和度及血小板计数均较治疗前提升,研究组均高于对照组,差异均有统计学意义(P<0.05)。研究组机械通气时间短于对照组,差异有统计学意义(P<0.05)。两组的14 d病死率相比,差异无统计学意义(P>0.05)。结论:脓毒症伴急性呼吸窘迫综合征患者在接受西维来司他钠治疗,可显著减轻患者的血清炎症指标水平,改善血气指标,减轻肺损伤,缩短患者的机械通气时间,效果理想。
