Delayed assessment and eager adoption of laparoscopic cholecystectomy:Implications for developing surgical technologies 被引量:3

Despite the prevailing emphasis in the medical literature on establishing evidence,many changes in the practice of surgery have not been achieved using proper evidence-based assessment.This paper examines the adoption of laparoscopic cholecystectomy(LC)into regular use for the treatment of cholecystitis and the process of its acceptance,focusing on the limited role of technology assessment in its appraisal.A review of the published medical literature concerning LC was performed.Approximately 3000 studies of LC have been conducted since 1985,and there have been nearly 8500 publications to date.As LC was adopted enthusiastically into practice,the results of outcome studies generally showed that it compared favorably with the traditional,open cholecystectomy with regard to mortality,complications,and length of hospital stay.However,despite the rapid general agreement on surgical technique,efficacy,and appropriateness,there remained lingering doubts about safety,outcomes,and cost of the procedure that suggested that essential research questions were ignored even as the procedure became standard.Using LC as a case study,there are important lessons to be learned about the need for important guidelines for surgical innovation and the adoption of minimally invasive surgical techniques into current clinical and surgical practice.We highlight one recent example,natural orifice transluminal endoscopic surgery and how necessary it is to properly evaluate this new technology before it is accepted as a safe and effective surgical option. 展开更多

结合北京市可变信息板(VMS)应用现状,研究在不同道路状况下VMS对驾驶人路径选择行为的影响。通过SP调查收集相关数据,引入重力模型和Logit模型进行数据分析,利用VISSIM软件实现了静态交通模拟和动态交通模拟。研究表明,当部分驾驶人按照... 结合北京市可变信息板(VMS)应用现状,研究在不同道路状况下VMS对驾驶人路径选择行为的影响。通过SP调查收集相关数据,引入重力模型和Logit模型进行数据分析,利用VISSIM软件实现了静态交通模拟和动态交通模拟。研究表明,当部分驾驶人按照VMS提示信息选择替代路径后,道路拥堵状况得到缓解,路网平均延误时间减少23.63%;随着路段车流密度增大,VMS诱导效用递增,在第46min出现效用最大值;因拥堵造成平均车速在30km·h-1左右的城市快速路最适宜设立VMS以改善路网运行状况。 展开更多

[Objective] To investigate the appropriate low-carbon agriculture model in Southern Jiangsu Province. [Method] Through the analysis of regional features in Southern Jiangsu and several matured low-carbon agriculture development models at present, the low-carbon agriculture development modes suitable for Southern Jiangsu were investigated, and corresponding supporting measures for the development of the models were put forward. [Result] Low-carbon agriculture is the environment- friendly agriculture which achieves low emissions, low pollution, high efficiency and high-yield through efficient recycling of energy and resources and continuous im- provements on ecological environment. With a variety of development models, the specific development model for practical use should be determined according to the local conditions, and supported by corresponding supporting measures, to achieve the rapid development of low-carbon agriculture. [Conclusion] This study laid the foundation for the development of low-carbon agriculture model in Southern Jiangsu. 展开更多

为解决黄色闪烁警示灯有效性量化评估的问题,提高交叉口的综合运行水平,减少交通事故的发生。针对3种不同类型交叉口黄色闪烁警示灯的综合评估问题,提出了基于驾驶模拟技术的评价方法,实现以微观驾驶行为数据结合基于熵权的功效系数法... 为解决黄色闪烁警示灯有效性量化评估的问题,提高交叉口的综合运行水平,减少交通事故的发生。针对3种不同类型交叉口黄色闪烁警示灯的综合评估问题,提出了基于驾驶模拟技术的评价方法,实现以微观驾驶行为数据结合基于熵权的功效系数法对黄色闪烁警示灯进行有效性综合评价。以国道101部分路段为背景搭建T形、十字形、畸形3种类型交叉口国标组和专项组的模拟驾驶实验场景,采用驾驶模拟实验方法,从3个层面选取8项评价指标,进行综合评估。结果表明,同类型交叉口设置黄色闪烁警示灯后其总功效均显著性提高,交叉口的安全性、舒适性及预见性水平得到提高。功效系数法能够科学合理的实现方案间的相互对比,具有较强的实用性,为黄色闪烁警示灯的设置提供理论依据。 展开更多

A microsphere biosorbent with uniform size (CV = 1.52%), controllable morphology and component, and high mechanical strength was synthesized from chitosan by microfluidic technology combining with chemical crosslinking and solvent extraction. This chitosan microsphere (CS-MS) was prepared with a two-step solidification process, which was acquired by drying for the enhancement of mechanical property in final. The adsorption behavior of CS-MS towards copper (II) and main influencing factors on adsorption performance were investigated by batch experiments. Kinetic data high-lighted dominant chemical bonding along with electrons transferring in adsorption process. Isothermal analysis indicated that adsorption capacity was relevant to the number of active site. All these explorations provided a new direction for preparing highly comprehensive performance sorbent used in heavy metal treatment via microfluidic technology. 展开更多
