Prognosis value of Chinese Ocular Fundus Diseases Society classification for proliferative diabetic retinopathy on postoperative visual acuity after pars plana vitrectomy in type 2 diabetes

AIM: To compare the postoperative visual acuity among eyes with proliferative diabetic retinopathy(PDR) of different stages after pars plana vitrectomy(PPV) in type 2 diabetic patients. METHODS: A retrospective study was conducted for PDR eyes undergoing PPV in type 2 diabetic patients. All patients were divided into three groups based on Chinese Ocular Fundus Diseases Society(COFDS) classification for PDR: Group A(primary vitreous hemorrhage), Group B(primary fibrovascular proliferation) and Group C(primary vitreous hemorrhage and/or fibrovascular proliferative combined with retinal detachment). The postoperative visual acuity and the change between postoperative and preoperative visual acuity were compared among three groups. The associated risk factors for postoperative visual acuity were analyzed in the univariate and multiple linear aggression. RESULTS: In total, 195 eyes of 195 patients were collected in this study, including 71 eyes of 71 patients in Group A, 75 eyes of 75 patients in Group B and 49 eyes of 49 patients in Group C. The eyes in Group A got better postoperative best-corrected visual acuity(BCVA) compared to the eyes in Group B and C(0.48±0.48 vs 0.89±0.63, P<0.001;0.48±0.48 vs 1.04±0.67, P<0.001;respectively). The eyes in Group A got more improvement of BCVA compared to the eyes in Group B and C(1.07±0.70 vs 0.73±0.68, P=0.004;1.07±0.70 vs 0.77±0.78, P=0.024;respectively). In the multiple linear regression analysis, primary fibro-proliferative type(β=0.194, 95%CI=0.060-0.447, P=0.01), retinal detachment type(β=0.244, 95%CI=0.132-0.579, P=0.02), baseline log MAR BCVA(β=0.192, 95%CI=0.068-0.345, P=0.004), silicone oil tamponade(β=0.272, 95%CI=0.173-0.528, P<0.001) was positively correlated with postoperative log MAR BCVA. Eyes undergoing phacovitrectomy had better postoperative BCVA(β=-0.144, 95%CI=-0.389 to-0.027, P=0.025). CONCLUSION: PDR eyes of primary vitreous hemorrhage type usually have better visual acuity prognosis compared to primary fibrovascular proliferation type and retinal detachment type. COFDS classification for PDR may have a high prognostic value for postoperative visual outcome and surgical management indications.

International collaboration to promote global health:The 2017 Belt and Road Initiative Global Health International Congress&2017 Chinese Preventive Medicine Association--Chinese Society on Global Health Annual Meeting

With the rapid development of new technologies and global trade and increasing collaboration am ong countries worldwide,public health has become a global issue.Global health,as a new discipline,has been drawing more attention from both academ ia and governments.The Belt and Road Initiative(BRI),proposed by China in 2013,aimed to prom ote trade and resource exchange,including education,research,and health issues,with over 60 countries in Asia,Africa,and Europe.The BRI provides good opportunities for involved countries to address health problems jointly as well.In response to the BRI and to promote international collaboration on global health issues,"The 2017 Belt and Road Initiative Global Health International Congress&2017 Chinese Preventive M edicine Association-Chinese Society on Global Health Annual Meeting"was held on Septem ber 24-27,2017 in Xi’an,China.Thus far,this is the largest high-quality international conference held in China that focused on the BRI global health issues.This article summarized the background,key sessions and topics covered during the congress,and important events,and highlights different perspectives of the BRI and global health by invited experts from China and abroad.The conference included 14 sessions(three keynote speech forums and ten scientific sessions and a research poster session)and about 100 speakers,around 40 of which were leading experts outside of China.All the major sessions were held in English.More than 40 leaders and experts of health sectors from 13 countries presented their work in the congress.Approximately 400 delegates from 29 countries attended the congress.Delegates had extensive discussions about global health related issues,future cooperation and development in global health.The congress fostered international exchange and collaboration.

Chinese Society of Clinical Oncology(CSCO):Clinical guidelines for the diagnosis and treatment of pancreatic cancer

Pancreatic cancer is one of the leading causes of cancer-related mortality in both developed and developing coun-tries.The incidence of pancreatic cancer in China accounts for about a quater of the global incidence,and the epidemiological characteristics and therapeutic strategies differ due to social,economic,cultural,environmental,and public health factors.Non-domestic guidelines do not reflect the clinicopathologic characteristics and treat-ment patterns of Chinese patients.Thus,in 2018,the Chinese Society of Clinical Oncology(CSCO)organized a panel of senior experts from all sub-specialties within the field of pancreatic oncology to compile the Chinese guidelines for the diagnosis and treatment of pancreatic cancer.The guidelines were made based on both the Western and Eastern clinical evidence and updated every one or two years.The experts made consensus judg-ments and classified evidence-based recommendations into various grades according to the regional differences,the accessibility of diagnostic and treatment resources,and health economic indexes in China.Here we present the latest version of the guidelines,which covers the diagnosis,treatment,and follow-up of pancreatic cancer.The guidelines might standardize the diagnosis and treatment of pancreatic cancer in China and will encourage oncologists to design and conduct more clinical trials about pancreatic cancer.
