They most commonly affect the lungs, gastrointestinal tract, and pancreas, being rare conditions in the female genital tract. When present, these neoplasms often manifest with nonspecific signs and symptoms such as pain, itching, swelling, single-focus lesions, bleeding, and enlargement of inguinal lymph nodes, in addition to the presence of progressively enlarging vulvar nodules. Consequently, the diagnostic investigation involves histopathological examination and confirmation through immunohistochemistry. Objective: To present a comprehensive understanding of this rarely studied pathology. The primary objective is to provide valuable insights that could aid in the future development of universally applicable treatment guidelines. Case Presentation: A 57-year-old female, with no prior comorbidities, menopause at 36, who presented with a left vulvar nodule accompanied by intense pain and swelling, later diagnosed with small cell neuroendocrine carcinoma in the vulva. Conclusion: This case report highlights the importance of enhancing our knowledge regarding small cell neuroendocrine carcinoma in the vulva, given its scarcity in medical literature. The information presented here underscores the need for standardized diagnostic and treatment approaches, paving the way for future consensus on managing this uncommon but challenging neoplasm.展开更多

Their crop production activities are mostly rainfed subsistence with any surplus being sold to bring in some income. Timely decisions on farm practices such as farm preparation and planting are critical determinants of the seasonal outcomes. In Kenya, most small-scale farmers have no reliable source of information that would help them make timely and accurate decisions. County governments have extension officers who are mandated with giving farmers advisory services to farmers but they are not able to reach most farmers due to facilitation constraints. The mode and format of sharing information is also critical since it’s important to ensure that it’s timely, well-understood and usable. This study sought to assess access to geospatial derived and other crop production information by farmers in four selected counties of Kenya. Specific objectives were to determine the profile of small-scale farmers in terms of age, education and farm size;to determine the type of information that is made available to them by County and Sub-County extension officers including the format and mode of provision;and to determine if the information provided was useful in terms of accuracy, timeliness and adequacy. The results indicated that over 80% of the farmers were over 35 years of age and over 56% were male. Majority had attained primary education (34%) or secondary education (29%) and most farmers in all the counties grew maize (71%). Notably, fellow farmers were a source of information (71%) with the frequency of sharing information being mostly seasonal (37%) and when information was available (43%). Over 66% of interviewed farmers indicating that they faced challenges while using provided information. The results from the study are insightful and helpful in determining effective ways of providing farmers with useful information to ensure maximum benefits.展开更多

文摘Small mountainous rivers are characterized by large instantaneous fluxes and susceptible to extreme weather events, which can rapidly transport materials into the sea and have a significant impact on the ecological environment of estuaries and bays.In order to investigate the seasonal characteristics of nutrients in small mountainous rivers in the subtropical monsoon region and the output pattern to the sea during heavy precipitation, surveys on the mountainous rivers were carried out in Baixi watershed in August 2020(wet season),March 2021(dry season)and June 2021(Meiyu period). The results showed that the dissolved inorganic nitrogen(DIN)of the rivers has an average concentration of 752μg L^(−1)in the wet season and 1472μg L^(−1)in the dry season. The concentrations of dissolved inorganic phosphorus(DIP)in wet season and dry season were 63μg L^(−1)and 51μg L^(−1),respectively.Influenced by the changes of land use in sub-watersheds,DIN concentrations in the mainstream increased from 701μg L^(−1)in the upper reaches to 1284μg L^(−1)in the middle reaches. Two rainstorms during the Meiyu period in the watershed caused the pulse runoff in the river. The maximum daily runoff reached 70 times that before rains. The maximum daily fluxes of DIN and DIP were 109 and 247 times that before rains, respectively. In view that the watershed experienced several rainstorms in the wet season, the river, with pulse runoff, carries a large amount of nutrients into the sea in a short time, which will have a significant impact on the environment of Sanmen bay and its adjacent sea.

文摘The health and function of ecosystems are largely determined by the quality of habitat, and the optimal regulation of landscape patterns has become an important way to improve regional habitat quality. This article takes the Poyang Lake Basin of China as a case, reveals the spatial and temporal change of its habitat quality at the small watershed scale, and attempts to examine the multidimensional response of habitat quality to landscape pattern changes with respect to landscape compositions and landscape configuration. The results show:1)from 2000 to 2020,the overall landscape fragmentation of the basin decreased, the landscape aggregation in the central small watersheds changed significantly, and the spatial distribution of landscape elements in the central and southern small watersheds were relatively homogeneous.2)The overall habitat quality of the Poyang Lake Basin is at a middle to high level, with significant spatial differentiation, showing the distribution characteristics of‘high in the periphery, low in the center, high in the south and low in the north’.3)Both landscape compositions and landscape configurations influenced habitat quality, but there were obvious differences in the response degree. From the landscape composition and configuration dimension, the influence of landscape composition on habitat quality was greater than that of landscape configuration;from the multilevel landscape configuration dimension, compared to landscape level, landscape configuration at the class level impacted habitat quality more deeply. To the end, this article proposes a differentiated regulation strategy for habitat quality conservation in small watersheds from the perspective of landscape patterns to improve the ecological service level of Poyang Lake Basin.
文摘Studies on mainstreaming climate-smart agriculture(CSA)practices can increase smallholder farmers’capacity and awareness to improve food security and establish sustainable livelihoods through resilient agricultural systems, while achieving adaptation and mitigation benefits.Hence, valuable insights can be obtained from smallholder farmers in responding to present and forthcoming challenges of climate change impacts. However, there is little research work on trade-off and synergy assessments.Taking Geshy watershed in Southwest Ethiopia as a case study area, both quantitative and qualitative data analysis were undertaken in this study. The data were collected from 15 key informant interviews,6 focus group discussions, and 384 households to answer the following questions:(1)what are the top 5 preferred CSA practices for smallholder farmers in Geshy watershed when coping with the impacts of climate change?(2)What is the performance of the preferred CSA practices?And(3)which trade-offs and synergies are experienced upon the implementation of CSA practices? The study came up with the most preferred CSA practices such as the use of improved crop varieties,small-scale irrigation, improved animal husbandry, the use of efficient inorganic fertilizers, and crop rotation with legumes. The selected CSA practices showed that the productivity goal exhibit the best synergy, while the mitigation goal has trade-offs.The study also indicated that the use of improved crop varieties causes high synergies in all 3 goals of CSA practices;small-scale irrigation provides a medium synergy on productivity goal but high synergy for adaptation and mitigation goals;improved animal husbandry shows a high synergy with the adaptation goal,a relatively lower synergy with the productivity goal, and a trade-off with the mitigation goal;the use of efficient inorganic fertilizers shows maximum synergy for the productivity and adaptation goals;and crop rotation with legumes exhibits high synergy with the productivity and mitigation goals but a relatively lower synergy with the adaptation goal. These results can provide evidence to various stakeholder farmers in the value chain that the impacts of climate change can be addressed by the adoption of CSA practices. In general,CSA practices are considered indispensable.Smallholder farmers prefer CSA practices that help to increase crop productivity and household resilience to climate change impacts. The results generate a vital foundation for recommendations to smallholder farming decision-makers.It also sensitizes actions for innovative and sustainable methods that are able to upscale the preferred CSA practices in the agricultural system in Geshy watershed of Southwest Ethiopia and other regions.
文摘The sustainability of environmental management initiatives, such as watershed management programs, relies on the presence of effective institutions at the watershed level. However, there needs to be more empirical evidence from evaluating the effectiveness of watershed-level institutions. Therefore, this study presents a pioneering effort to evaluate the effectiveness of Nepal’s first watershed conservation committee at the watershed scale, focusing on the case of the Khageri Khola watershed in Central Nepal. The study involved conducting a household survey, key informant interviews, focus group discussions, and field observations to collect and analyze the data.Descriptive analysis, index value calculation, and chi-square statistics were then employed to summarize the results regarding local respondents’perceptions of twelve institutional characteristics, their rationalities, and their association with socio-demographic variables. The results reveal that the watershed conservation committee was perceived as performing well in managing the watershed.Specifically, good interaction, appropriate scale, technical, environmental, social, organizational, and government rationality were perceived as highly effective, with an average index value of less than 0.36.In contrast, clarity of objectives and economic rationality showed moderate effectiveness, with an average index value ranging from 0.36 to 0.65.However,the results suggested that adaptiveness, compliance capacity, and financial rationality merit increased attention, intending to improve their performance.Further, the results showed the association of socio-demographics with respondents’perceptions of various indicators of institutional characteristics and their rationalities. Therefore, the study provides valuable insights for policymakers, researchers, and development practitioners charged with designing sustainable and effective programs and institutions. To enhance the effectiveness and sustainability of watershed management programs,we recommend establishing a policy-guided institutional mechanism at the watershed scale. This mechanism should be based on various institutional characteristics and rationalities and should consider the extant variability in the socio-demographic and topographic characteristics of the watershed.

文摘Small nucleolar RNAs(snoRNAs)represent a class of non-coding RNAs that play pivotal roles in post-transcriptional RNA processing and modification, thereby contributing significantly to the maintenance of cellular functions related to protein synthesis.SnoRNAs have been discovered to possess the ability to influence cell fate and alter disease progression, holding immense potential in controlling human diseases. It is suggested that the dysregulation of snoRNAs in cancer exhibits differential expression across various cancer types, stages, metastasis, treatment response and/or prognosis in patients. On the other hand, colorectal cancer(CRC),a prevalent malignancy of the digestive system,is characterized by high incidence and mortality rates, ranking as the third most common cancer type.Recent research indicates that snoRNA dysregulation is associated with CRC,as snoRNA expression significantly differs between normal and cancerous conditions.Consequently, assessing snoRNA expression level and function holds promise for the prognosis and diagnosis of CRC.Nevertheless, current comprehension of the potential roles of snoRNAs in CRC remains limited. This review offers a comprehensive survey of the aberrant regulation of snoRNAs in CRC,providing valuable insights into the discovery of novel biomarkers, therapeutic targets, and potential tools for the diagnosis and treatment of CRC and furnishing critical cues for advancing research into CRC and the judicious selection of therapeutic targets.

文摘Transformer-based models have facilitated significant advances in object detection. However, their extensive computational consumption and suboptimal detection of dense small objects curtail their applicability in unmanned aerial vehicle(UAV)imagery.Addressing these limitations,we propose a hybrid transformer-based detector,H-DETR,and enhance it for dense small objects, leading to an accurate and efficient model.Firstly,we introduce a hybrid transformer encoder, which integrates a convolutional neural network-based cross-scale fusion module with the original encoder to handle multi-scale feature sequences more efficiently.Furthermore,we propose two novel strategies to enhance detection performance without incurring additional inference computation.Query filter is designed to cope with the dense clustering inherent in drone-captured images by counteracting similar queries with a training-aware non-maximum suppression.Adversarial denoising learning is a novel enhancement method inspired by adversarial learning, which improves the detection of numerous small targets by counteracting the effects of artificial spatial and semantic noise.Extensive experiments on the VisDrone and UAVDT datasets substantiate the effectiveness of our approach, achieving a significant improvement in accuracy with a reduction in computational complexity. Our method achieves 31.9%and 21.1%AP on the VisDrone and UAVDT datasets, respectively, and has a faster inference speed, making it a competitive model in UAV image object detection.
文摘The present letter to the editor is related to the study with the title“Automatic detection of small bowel(SB)lesions with different bleeding risk based on deep learning models”.Capsule endoscopy(CE)is the main tool to assess SB diseases but it is a time-consuming procedure with a significant error rate. The development of artificial intelligence(AI)in CE could simplify physicians’tasks. The novel deep learning model by Zhang et al seems to be able to identify various SB lesions and their bleeding risk, and it could pave the way to next perspective studies to better enhance the diagnostic support of AI in the detection of different types of SB lesions in clinical practice.

文摘To address the challenges of high complexity, poor real-time performance, and low detection rates for small target vehicles in existing vehicle object detection algorithms, this paper proposes a real-time lightweight architecture based on You Only Look Once(YOLO)v5m.Firstly,a lightweight upsampling operator called Content-Aware Reassembly of Features(CARAFE)is introduced in the feature fusion layer of the network to maximize the extraction of deep-level features for small target vehicles, reducing the missed detection rate and false detection rate.Secondly,a new prediction layer for tiny targets is added, and the feature fusion network is redesigned to enhance the detection capability for small targets. Finally, this paper applies L1 regularization to train the improved network, followed by pruning and fine-tuning operations to remove redundant channels, reducing computational and parameter complexity and enhancing the detection efficiency of the network.Training is conducted on the VisDrone2019-DET dataset. The experimental results show that the proposed algorithmreduces parameters and computation by 63.8% and 65.8%,respectively.The average detection accuracy improves by 5.15%,and the detection speed reaches 47 images per second, satisfying real-time requirements.Compared with existing approaches, including YOLOv5m and classical vehicle detection algorithms, our method achieves higher accuracy and faster speed for real-time detection of small target vehicles in edge computing.

文摘In recent years, there has been extensive research on object detection methods applied to optical remote sensing images utilizing convolutional neural networks.Despite these efforts, the detection of small objects in remote sensing remains a formidable challenge. The deep network structure will bring about the loss of object features, resulting in the loss of object features and the near elimination of some subtle features associated with small objects in deep layers.Additionally, the features of small objects are susceptible to interference from background features contained within the image, leading to a decline in detection accuracy. Moreover, the sensitivity of small objects to the bounding box perturbation further increases the detection difficulty. In this paper,we introduce a novel approach,Cross-Layer Fusion and Weighted Receptive Field-based YOLO(CAW-YOLO),specifically designed for small object detection in remote sensing. To address feature loss in deep layers,we have devised a cross-layer attention fusion module. Background noise is effectively filtered through the incorporation of Bi-Level Routing Attention(BRA).To enhance the model’s capacity to perceive multi-scale objects, particularly small-scale objects,we introduce a weightedmulti-receptive field atrous spatial pyramid poolingmodule.Furthermore, wemitigate the sensitivity arising from bounding box perturbation by incorporating the joint Normalized Wasserstein Distance(NWD)and Efficient Intersection over Union(EIoU)losses. The efficacy of the proposedmodel in detecting small objects in remote sensing has been validated through experiments conducted on three publicly available datasets. The experimental results unequivocally demonstrate the model’s pronounced advantages in small object detection for remote sensing, surpassing the performance of current mainstream models.

文摘Detection of maneuvering small targets has always been an important yet challenging task for radar signal processing. One primary reason is that target variable motions within coherent processing interval generate energy migrations across multiple resolution bins, which severely deteriorate the parameter estimation performance.A coarse-to-fine strategy for the detection of maneuvering small targets is proposed.Integration of small points segmented coherently is performed first, and then an optimal inter-segment integration is utilized to derive the coarse estimation of the chirp rate.Sparse fractional Fourier transform(FrFT)is then employed to refine the coarse estimation at a significantly reduced computational complexity.Simulation results verify the proposed scheme that achieves an efficient and reliable maneuvering target detection with-16dB input signal-to-noise ratio(SNR),while requires no exact a priori knowledge on the motion parameters.
文摘Background: Neuroendocrine neoplasms are those that develop from a neuroendocrine cell. The information presented here underscores the need for standardized diagnostic and treatment approaches, paving the way for future consensus on managing this uncommon but challenging neoplasm.
文摘Small-scale farming accounts for 78% of total agricultural production in Kenya and contributes to 23.5% of the country’s GDP. The results from the study are insightful and helpful in determining effective ways of providing farmers with useful information to ensure maximum benefits.

文摘Objective:To determine the genetic diversity of Plasmodium(P.)knowlesi isolates from Sabah,Malaysian Borneo and Peninsular Malaysia, targeting the S-type SSU rRNA gene and including aspects of natural selection and haplotype.Methods:Thirty-nine blood samples infected with P.knowlesi were collected in Sabah,Malaysian Borneo and Peninsular Malaysia. The S-type SSU rRNA gene was amplified using polymerase chain reaction, cloned into a vector, and sequenced. The natural selection and haplotype of the S-type SSU rRNA gene sequences were determined using DnaSP v6 and illustrated using NETWORK v10.This study's 39 S-type SSU rRNA sequences and eight sequences from the Genbank database were subjected to phylogenetic analysis using MEGA 11.Results:Overall,the phylogenetic analysis showed no evidence of a geographical cluster of P.knowlesi isolates from different areas in Malaysia based on the S-type SSU rRNA gene sequences. The S-type SSU rRNA gene sequences were relatively conserved and with a purifying effect.Haplotype sharing of the S-type SSU rRNA gene was observed between the P.knowlesi isolates in Sabah,Malaysian Borneo, but not between Sabah,Malaysian Borneo and Peninsular Malaysia. Conclusions:This study suggests that the S-type SSU rRNA gene of P.knowlesi isolates in Sabah,Malaysian Borneo, and Peninsular Malaysia has fewer polymorphic sites, representing the conservation of the gene. These features make the S-type SSU rRNA gene suitable for comparative studies, such as determining the evolutionary relationships and common ancestry among P.knowlesi species.

文摘Human activities in a transborder watershed are complex under the influence of domestic policies, international relations, and global events.Understanding the forces driving human activity change is important for the development of transborder watershed. In this study,we used global historical land cover data, the hemeroby index model, and synthesized major historical events to analyze how human activity intensity changed in the Heilongjiang River(Amur River in Russia)watershed(HLRW).The results showed that there was a strong spatial heterogeneity in the variation of human activity intensity in the HLRW over the past century(1900-2016).On the Chinese side, the human activity intensity change shifted from the plain areas for agricultural reclamation to the mountainous areas for timber extraction. On the Russian side, human activity intensity changes mostly concentrated along the Trans-Siberian Railway and the Baikal-Amur Mainline.Localized variation of human activity intensity tended to respond to regional events while regionalized variation tends to reflect national policy change or broad international events. The similarities and differences between China and Russia in policies and positions in international events resulted in synchronous and asynchronous changes in human activity intensity.Meanwhile, policy shifts were often confined by the natural features of the watershed. These results reveal the historical origins and fundamental connotations of watershed development and contribute to formulating regional management policies that coordinate population,eco-nomic,social,and environmental activities.

文摘Axonal remodeling is a critical aspect of ischemic brain repair processes and contributes to spontaneous functional recovery. Our previous in vitro study demonstrated that exosomes/small extracellular vesicles(sEVs)isolated from cerebral endothelial cells(CEC-sEVs)of ischemic brain promote axonal growth of embryonic cortical neurons and that microRNA 27a(miR-27a)is an elevated miRNA in ischemic CEC-sEVs.In the present study,we investigated whether normal CEC-sEVs engineered to enrich their levels of miR-27a(27a-sEVs)further enhance axonal growth and improve neurological outcomes after ischemic stroke when compared with treatment with non-engineered CEC-sEVs.27a-sEVs were isolated from the conditioned medium of healthy mouse CECs transfected with a lentiviral miR-27a expression vector.Small EVs isolated from CECs transfected with a scramble vector(Scra-sEVs)were used as a control.Adult male mice were subjected to permanent middle cerebral artery occlusion and then were randomly treated with 27a-sEVs or Scra-sEVs.An array of behavior assays was used to measure neurological function.Compared with treatment of ischemic stroke with Scra-sEVs,treatment with 27a-sEVs significantly augmented axons and spines in the peri-infarct zone and in the corticospinal tract of the spinal grey matter of the denervated side, and significantly improved neurological outcomes. In vitro studies demonstrated that CEC-sEVs carrying reduced miR-27a abolished 27a-sEV-augmented axonal growth.Ultrastructural analysis revealed that 27a-sEVs systemically administered preferentially localized to the pre-synaptic active zone, while quantitative reverse transcription-polymerase chain reaction and Western Blot analysis showed elevated miR-27a,and reduced axonal inhibitory proteins Semaphorin 6A and Ras Homolog Family Member A in the peri-infarct zone.Blockage of the Clathrin-dependent endocytosis pathway substantially reduced neuronal internalization of 27a-sEVs.Our data provide evidence that 27a-sEVs have a therapeutic effect on stroke recovery by promoting axonal remodeling and improving neurological outcomes. Our findings also suggest that suppression of axonal inhibitory proteins such as Semaphorin 6A may contribute to the beneficial effect of 27a-sEVs on axonal remodeling.
