This study sought to examine the association between HIV/AIDS sp...<strong>Background: </strong>The Sustainable Development Goals commitment to Ending HIV/AIDS by 2030 requires sustained adequate investment. This study sought to examine the association between HIV/AIDS spending and outcomes in Thailand between 2008 and 2019. <strong>Methods: </strong>A quantitative secondary data analysis with time-series was conducted using a retrospective dataset of HIV spending and some selected outcomes including the number of people living with HIV (PLHIV), incidence and prevalence of HIV/AIDS, the prevention of mother-to-child transmission (PMTCT) and AIDS-related deaths. Data were obtained from a diverse set of sources. Descriptive statistics and univariate regression model were used to analyze HIV expenditure and outcomes. <strong>Results: </strong>HIV spending per PLHIV rose by two folds from $347 in 2008 to more than $600 in 2019, mostly financed by domestic sources. This increase of domestic resources per PLHIV was significantly associated with better HIV-related outcomes especially in the reduction of PLHIV and AIDS-related deaths through increased number of people receiving antiretroviral therapy (ART). However, the spending per PLHIV varied across the three public health insurance schemes. Comparison of HIV expenditure and health outcomes across upper-middle-income countries shows Thailand is not highly ranked in terms of spending efficiency despite having made good progress. <strong>Conclusion: </strong>Domestic financing for HIV programs is indispensable for achieving the goal of ending AIDS. Despite significant improvement in HIV-related outcomes, challenges remain in achieving the 90-90-90 goal. The redesigning of payment methods should be considered to increase the efficiency of HIV financing. Other factors related to strengthening the health system should not be overlooked.展开更多
This paper is an empirical analysis of provisional unrestricted level relationship between Nigerian domestic output measured by Gross Domestic Product (GDP) and government spending proxied by capital and recurrent exp...This paper is an empirical analysis of provisional unrestricted level relationship between Nigerian domestic output measured by Gross Domestic Product (GDP) and government spending proxied by capital and recurrent expenditures in the presence of static regressors such as crude oil prices and federal government retained revenues. We estimate an ARDL (1,0,1) using a single-equation approach. Results show that government expenditures have negative but statistically insignificant effects on domestic output in the long-run. Similarly, negative short run effects are established amongst the variables. However, recurrent expenditure is statistically significant in the short-run. Whilst federal government retained revenue has a positive and significant effect, crude oil price exhibited negative relationships with domestic output both at level and in the short-run dynamics. Also a high speed of adjustment implies that Nigerian Gross Domestic Product is extremely sensitive to shocks on the government spending in the long-run. An upward trend forecast between 2014 and 2020 is an indication of the continued positive impact and the government retained revenue will exert on the domestic output in the long-run.展开更多

As an aspect of human capital,a positive association exists amongst health,productivity,and growth in output per capita.On the other hand,social infrastructure defined by the institution of governance has a direct eff...As an aspect of human capital,a positive association exists amongst health,productivity,and growth in output per capita.On the other hand,social infrastructure defined by the institution of governance has a direct effect on the environment upon which productive activities take place to determine outcomes.Nigeria like most African countries is bedevilled by the high prevalence of inadequate health financing and poor governance.Health financing for Nigeria consistently has fallen short of the AU health funding commitment of 15%of annual budgetary allocation to the health sector.Secondly,poor governance conditions available resources and shape the state of infrastructure,particularly health infrastructure and socioeconomic conditions.In turn,this determines individuals’level of exposure to health risks and their capacity to actively contribute to productive activity for growth stimulation and sustainability.Against this backdrop,this study added to the existing literature in the context of Nigeria,by theoretically applying the Solow augmented Mankiw-Romer-Weil structural model in the examination of the impact of government size and governance quality in the health sector,on economic growth.Autoregressive Distributed Lag(ARDL)model was adopted in the estimation.Findings show that governance quality adversely affects growth and this reduces the capacity of health spending to stimulate growth by an almost equal margin.As a result,this study recommends legislative backing to the AU health funding commitment in Nigeria.展开更多

Thr...To solve the drawback of the present electionic cash (E-cash) schemes that the size of bank's transcripts database expands without bound, the concept of validity duration of E-cash is introduced in this paper. Through improving the users' blind transforms of Brands' scheme, we design an efficient off-line E-cash system based on the restrictive blind signature and representation problem. The security analysis shows that the users can not forge a valid electronic coin or double spend it, in addition, the bank can not trace an honest users' payment behaviours. In compare with Brand's scheme, our scheme reduces storage costs and computation overhead of the users and the banks. Most of all, the electronic cashes will be authorized within a limit period only, the size of bank's transcripts database becomes controllable. We examined the effect of over-the-counter (OTC) conversion of prescription drugs on utilization at the drug class level using monthly drug utilization data from the US for the period 1999-2010 for 9 drug classes: ant...We examined the effect of over-the-counter (OTC) conversion of prescription drugs on utilization at the drug class level using monthly drug utilization data from the US for the period 1999-2010 for 9 drug classes: antihistamines, benign prostatic hyperplasia medication, cholesterol control drugs (statins), analgesics (triptans), contraception medications (emergency contraception), antiulcerants (proton pump inhibitors) non-sedating antihistamines, weight-loss remedies and erectile dysfunction remedies. We performed interrupted time series analysis to detect a break in the trend of drug utilization following OTC introduction. We found that the introduction of the first OTC drug increased drug utilization at the class level by an average of 30% or more. We concluded that OTC switches can be an important policy tool for improving public health in drug classes where a significant proportion of the population is untreated and where consumers can effectively manage treatment with limited physician supervision.展开更多
