BACKGROUND The neutrophil-to-lymphocyte ratio(NLR)is commonly utilized as a prognostic indicator in end-stage liver disease(ESLD),encompassing conditions like liver failure and decompensated cirrhosis.Nevertheless,som...BACKGROUND The neutrophil-to-lymphocyte ratio(NLR)is commonly utilized as a prognostic indicator in end-stage liver disease(ESLD),encompassing conditions like liver failure and decompensated cirrhosis.Nevertheless,some studies have contested the prognostic value of NLR in ESLD.AIM To investigate the ability of NLR to predict ESLD.METHODS Databases,such as Embase,PubMed,Web of Science,Cochrane Library,China National Knowledge Infrastructure,Weipu,and Wanfang,were comprehensively searched to identify studies published before October 2022 assessing the prognostic ability of NLR to predict mortality in patients with ESLD.Effect sizes were calculated using comprehensive meta-analysis software and SATAT 15.1.RESULTS A total of thirty studies involving patients with end-stage liver disease(ESLD)were included in the evaluation.Among the pooled results of eight studies,it was observed that the Neutrophil-to-Lymphocyte Ratio(NLR)was significantly higher in non-survivors compared to survivors(random-effects model:standardized mean difference=1.02,95%confidence interval=0.67-1.37).Additionally,twenty-seven studies examined the associations between NLR and mortality in ESLD patients,reporting either hazard ratios(HR)or odds ratios(OR).The combined findings indicated a link between NLR and ESLD mortality(randomeffects model;univariate HR=1.07,95%CI=1.05-1.09;multivariate HR=1.07,95%CI=1.07-1.09;univariate OR=1.29,95%CI=1.18-1.39;multivariate OR=1.29,95%CI=1.09-1.49).Furthermore,subgroup and meta-regression analyses revealed regional variations in the impact of NLR on ESLD mortality,with Asian studies demonstrating a more pronounced effect.CONCLUSION Increased NLR in patients with ESLD is associated with a higher risk of mortality,particularly in Asian patients.NLR is a useful prognostic biomarker in patients with ESLD.展开更多

Improving clinicians’ knowledge and understanding of surgical staging is critical?in the fight against the disease. However, a systematic evaluation of different ordinal regression models based on diverse predicted outcomes has not been given its due share in literature.?Objective:?To systematically assess the flexibility of odds ratios for three popular ordinal regression models i.e.?the Multinomial Logistic (ML) model, the Continuation Ratio (CR) model and Adjacent Category Logistic (ACL) model when applying cervical cancer data in surgical stage prediction.?Method:?We systematically, compared the performance of CR, ML and the ACL as the predictive mechanisms, and evaluate the most appropriate model in the cervical cancer setting. The study considered women who visited the Oncology department at the Moi Teaching and Referral Hospital’s Chandaria Cancer and Chronic Diseases Center and were diagnosed and surgically treated for cervical cancer from January 2014?to December 2018.?Results and Conclusion:?We presented the comparison between?3?different regression models for ordinal data within the cervical cancer setting. We found that the CR model without proportional odds yielded better results?comparing Akaike Information Criterion (AIC), log likelihood ratio and residual deviance. In addition, the key prognostic factor associated with invasive cervical cancer was the (International Federation of Gynecology and Obstetrics) FIGO clinical stage which in particular, had a higher influence on the surgical Stage 2 outcomes compared to the lesser surgical stage categories. It is difficult to build an effective water flooding displacement pressure system in the middle section of a horizontal well in an ultra-low permeability sandstone reservoir.To solve this problem,this study proposes t...It is difficult to build an effective water flooding displacement pressure system in the middle section of a horizontal well in an ultra-low permeability sandstone reservoir.To solve this problem,this study proposes to use packers,sealing cannula and other tools in the same horizontal well to inject water in some fractures and produce oil from other fractures.This new energy supplement method forms a segmental synchronous injection-production system in a horizontal well.The method can reduce the distance between the injection end and the production end,and quickly establish an effective displacement system.Changing the displacement between wells to displacement between horizontal well sections,and point water flooding to linear uniform water flooding,the method can enhance water sweeping volume and shorten waterflooding response period.The research shows that:(1)In the synchronous injection and production of horizontal well in an ultra-low-permeability sandstone reservoir,the water injection section should select the section where the natural fractures and artificial fractures are in the same direction or the section with no natural fractures,and the space between two sections should be 60?80 m.(2)In addition to controlling injection pressure,periodic water injection can be taken to reduce the risk of re-opening and growth of natural fractures or formation fracture caused by the gradual increase of water injection pressure with water injection going on.(3)Field tests have verified that this method can effectively improve the output of single well and achieve good economic benefits,so it can be widely used in the development of ultra-low permeability sandstone reservoirs.展开更多
