Objective:Transurethral resection of bladder tumor is one of the most common everyday urological procedures.This kind of surgery demands a set of skills that need training and experience.In this review,we aimed to investigate the current literature to find out if simulators,phantoms,and other training models could be used as a tool for teaching urologists.Methods:A systematic review was performed according to the Preferred Reporting Items for Systematic reviews and Meta-Analyses statement and the recommendations of the European Association of Urology guidelines for conducting systematic reviews.Fifteen out of 932 studies met our inclusion criteria and are presented in the current review.Results:The UroTrainer(Karl Storz GmbH,Tuttlingen,Germany),a virtual reality training simulator,achieved positive feedback and an excellent face and construct validity by the participants.The inspection of bladder mucosa,blood loss,tumor resection,and procedural time was improved after the training,especially for inexperienced urologists and medical students.The construct validity of UroSim®(VirtaMed,Zurich,Switzerland)was established.SIMBLA simulator(Samed GmbH,Dresden,Germany)was found to be a realistic and useful tool by experts and urologists with intermediate experience.The test objective competency model based on SIMBLA simulator could be used for evaluating urologists.The porcine model of the Asian Urological Surgery Training and Education Group also received positive feedback by the participants that tried it.The Simulation and Technology Enhanced Learning Initiative Project had an extraordinary face and content validity,and 60%of participants would like to use the simulators in the future.The 5-day multimodal training curriculum“Boot Camp”in the United Kingdom achieved an increase of the level of confidence of the participants that lasted months after the project.Conclusion:Simulators and courses or curricula based on a simulator training could be a valuable learning tool for any surgeon,and there is no doubt that they should be a part of every urologist's technical education. 展开更多

Virtual reality tools for training in gastrointestinal endoscopy:A systematic review

BACKGROUND Virtual reality(VR)has emerged as an innovative technology in endoscopy training,providing a simulated environment that closely resembles real-life scenarios and offering trainees a valuable platform to acquire and enhance their endoscopic skills.This systematic review will critically evaluate the effectiveness and feasibility of VR-based training compared to traditional methods.AIM To evaluate the effectiveness and feasibility of VR-based training compared to traditional methods.By examining the current state of the field,this review seeks to identify gaps,challenges,and opportunities for further research and implementation of VR in endoscopic training.METHODS The study is a systematic review,following the guidelines for reporting systematic reviews set out by the PRISMA statement.A comprehensive search command was designed and implemented and run in September 2023 to identify relevant studies available,from electronic databases such as PubMed,Scopus,Cochrane,and Google Scholar.The results were systematically reviewed.RESULTS Sixteen articles were included in the final analysis.The total number of participants was 523.Five studies focused on both upper endoscopy and colonoscopy training,two on upper endoscopy training only,eight on colonoscopy training only,and one on sigmoidoscopy training only.Gastrointestinal Mentor virtual endoscopy simulator was commonly used.Fifteen reported positive results,indicating that VR-based training was feasible and acceptable for endoscopy learners.VR technology helped the trainees enhance their skills in manipulating the endoscope,reducing the procedure time or increasing the technical accuracy,in VR scenarios and real patients.Some studies show that the patient discomfort level decreased significantly.However,some studies show there were no significant differences in patient discomfort and pain scores between VR group and other groups.CONCLUSION VR training is effective for endoscopy training.There are several well-designed randomized controlled trials with large sample sizes,proving the potential of this innovative tool.Thus,VR should be more widely adopted in endoscopy training.Furthermore,combining VR training with conventional methods could be a promising approach that should be implemented in training. 展开更多

The use of simulators as educational tools for medical procedures is spreading rapidly and many efforts have been made for their implementation in gastrointestinal endoscopy training. Endoscopy simulation training has been suggested for ascertaining patient safety while positively influencing the trainees' learning curve. Virtual simulators are the most promising tool among all available types of simulators. These integrated modalities offer a human-like endoscopy experience by combining virtual images of the gastrointestinal tract and haptic realism with using a customized endoscope. From their first steps in the 1980s until today, research involving virtual endoscopic simulators can be divided in two categories: investigation of the impact of virtual simulator training in acquiring endoscopy skills and measuring competence. Emphasis should also be given to the financial impact of their implementation in endoscopy, including the cost of these state-of-theart simulators and the potential economic benefits from their usage. Advances in technology will contribute to the upgrade of existing models and the development of new ones; while further research should be carried out to discover new fields of application. 展开更多

The composition of the system and the functions of the modules are described, then the three-dimensional graphic engine based on Open Scene Graph (OSG) and the way of scene organization based on hierarchical tree are introduced, moreover, the methods of fault simulation and detection instrument simulation based on algorithm component library are presented. Background Atrial fibrillation(AF)is the most common sustained cardiac arrhythmia that can cause severe heart problems.Catheter ablation is one of the most ideal procedures for the treatment of AF.Physicians qualified to perform this procedure need to be highly skilled in manipulating the relevant surgical devices.This study proposes an interactive surgical simulator with high fidelity to facilitate efficient training and low-cost medical education.Methods We used a shared centerline model to simulate the interaction between multiple surgical devices.An improved adaptive deviation-feedback approach is proposed to accelerate the convergence of each iteration.The periodical beating of the human heart was also simulated in real time using the position-based dynamics(PBD)framework to achieve higher fidelity.We then present a novel method for handling the interaction between the devices and the beating heart mesh model.Experiments were conducted in a homemade simulator prototype to evaluate the robustness,performance,and flexibility of the proposed method.Preliminary evaluation of the simulator was performed by medical students,residents,and surgeons.Results The interaction between surgical devices,static vascular meshes,and beating heart mesh was stably simulated in a frame rate suitable for interaction.Conclusion Our simulator is capable of simulating the procedure of catheter ablation with high fidelity and provides immersive visual experiences and haptic feedback. 展开更多

System Restoration Tools: System Restoration Navigator Integrated into EPRI Operator Training Simulator (SRN/OTS)

System operators and planners develop and implement restoration plans based on off-line simulation studies, and accumulated experience and knowledge. One of the challenges in developing a restoration plan is to sift through numerous possible restoration scenarios and paths, in order to identify those that are technically feasible. When implementing a restoration plan in an on-line environment following a blackout, the operators need to adapt to the actual outage scenarios and available resources, and be constantly mindful of anticipated voltage and frequency excursions that must remain within system and equipment tolerances. In recognition of these challenges, EPRI has developed System Restoration Navigator (SRN), to provide decision support to system restoration planning and operations engineers in developing, evaluating and revising system restoration strategies, guidelines, plans and step-by-step procedures. During 2013-2014, EPRI developed SRN version 3.0, which is designed to facilitate its integration into a commercial operator training simulator (OTS) (AKA a dispatch training simulator, DTS). The integration of SRN 3.0 with an OTS allows operators to obtain experience in simulating, developing, experimenting with and revising system restoration plans, and to address related regulatory standards. The integration expands the usability of SRN 3.0 by providing the OTS platform for training purposes and for the purpose of interfacing SRN 3.0 with operational power system models to be able to explore near real time application of SRN 3.0. This 2013-2014 development work also included the integration of SRN 3.0 into EPRI OTS, and its application on the Florida Reliability Coordinating Council (FRCC) power system. Readily accessible equipment, aswell as structured training curriculum should be provided to guarantee adequate practice hours and skillproficiency. Dry-lab training is typically adopted before animal model surgery, usually comprising ofpurpose-built bulky simulators that is neither accessible nor portable. In this technical note, we designed ahome-made simulator, using two 4 L water jugs as operating space that are communicated inside, plus anobservation hole taped in between to mimic the triangular working space of laparoscopic surgery. Imagingwas achieved via smartphone camera, which was wirelessly connected to a laptop and a projector for realtime display on multiple screens, using built-in multi-screen collaboration software. A self-regulated andproficiency-based training curriculum was adopted. This dry-lab simulator is low-cost, highly portable andeasily replicable for basic laparoscopic skills training for the beginners to intermediate surgeons, whichmay serve as a good way for the standardized residency and specialist training program. 展开更多

This simulator is composed of independent simulation server and operator consoles and can be used for network analysis, network operation, fault management and evaluation. TDAS DB is duplicated online to the simulation server keeping the data security. The system can model distribution network penetrated with distributed generations (DG) using the real data from the TDAS DB. Network fault scenarios are automatically generated by calculating fault current and generating fault indicators. Also, manual entry of cry wolf alarm is available. Moreover, operation solution for scenario of fault isolation and service restoration is generated automatically so that trainee can check their operation result. 高速列车突出隧道过程中横风效应研究

为研究横风效应对突然驶出隧道过程中高速列车气动特性的影响,基于列车流场的三维、可压缩性的非定常特性,建立隧道-列车-横风三维数值模模型,研究横风效应对驶出隧道过程中高速列车流场分布和列车表面压力的影响,揭示列车气动荷载变化... 为研究横风效应对突然驶出隧道过程中高速列车气动特性的影响,基于列车流场的三维、可压缩性的非定常特性,建立隧道-列车-横风三维数值模模型,研究横风效应对驶出隧道过程中高速列车流场分布和列车表面压力的影响,揭示列车气动荷载变化机理。通过与动模型试验结果进行对比,验证数值模拟的合理性。研究结果表明:横风下列车驶出隧道过程中流场分布具有显著空间效应,受列车与隧道相互作用影响,隧道出口附近流场具有明显非定常特性;相较于无风情形,横风下列车底部压力变化幅值增大60%,列车迎风面、顶部压力变化幅值分别增大38.1%和28.6%,背风面压力变化幅值差异为4.8%,背风面压力分布受横风影响最小;横风效应导致列车气动特性发生显著变化,气动荷载变化幅值远比无风情形的大,无风时尾车横向力、升力变化幅值最大,横风作用下头车横向力变化幅值最大,倾覆风险最大。 展开更多

目的 探究情景模拟联合以问题为导向的教学法(problem-based learning,PBL)在脊柱侧弯住培教学中的应用效果。方法 将2020年1月—2021年12月在南通大学附属医院骨科参加住培的40名学员作为研究对象,并随机分为对照组(n=20,给予PBL教学... 目的 探究情景模拟联合以问题为导向的教学法(problem-based learning,PBL)在脊柱侧弯住培教学中的应用效果。方法 将2020年1月—2021年12月在南通大学附属医院骨科参加住培的40名学员作为研究对象,并随机分为对照组(n=20,给予PBL教学法),观察组(n=20,给予情景模拟联合PBL教学法)。比较分析2组学员考核成绩、满意度及综合能力评价情况。结果在考核成绩方面,住培前2组学员考核成绩差异无统计学意义(P> 0.05),住培后观察组学员理论与实践的成绩分别是(91.04±3.14)分、(92.33±3.57)分,均高于对照组学员的(81.75±2.97)分、(82.64±2.69)分,且观察组综合得分(91.47±3.26)分高于对照组学员的(82.01±2.54)分,差异均有统计学意义(P<0.05)。观察组与对照组对住培教学的满意度(95.00%vs. 70.00%)的差异有统计学意义(P <0.05)。在综合能力评价方面,观察组学员综合评价总分(89.46±1.17)分,高于对照组学员的(70.27±1.22)分,差异有统计学意义(P <0.05)。结论 在脊柱侧弯住培教学中实施情景模拟联合PBL教学法可以有效提高学员的理论知识水平以及实践操作能力,并且学员的综合能力显著提高。 展开更多

在长期的高速列车运营过程中,极易形成前后车辆的不同形式偏置,造成列车气动性能改变,甚至可能引发行车平稳性问题,极大影响乘坐舒适性和安全性。以高速列车尾车作为研究对象,探究尾车上下偏置时,高速列车尾部流场变化以及气动特性。基... 在长期的高速列车运营过程中,极易形成前后车辆的不同形式偏置,造成列车气动性能改变,甚至可能引发行车平稳性问题,极大影响乘坐舒适性和安全性。以高速列车尾车作为研究对象,探究尾车上下偏置时,高速列车尾部流场变化以及气动特性。基于SST k-ω双方程湍流模型,采用数值仿真方法研究了350 km/h高速列车尾车无偏置、尾车下降20 mm、尾车下降40 mm、尾车下降60 mm、尾车上升20 mm、尾车上升40 mm以及尾车上升60 mm 7种工况下列车的气动性能,分析高速列车气动阻力的变化规律,揭示了不同垂向位移下高速列车尾部流场特性以及列车表面压力分布情况。研究结果表明:高速列车尾部垂向位移对列车整体气动阻力影响较小,但对高速列车气动阻力分布以及流场特性造成一定影响。当尾车偏置位移达到60 mm时,列车车体气动阻力相对于无偏置工况分别降低了-1.11%和2.64%,转向架气动阻力相对无偏置情况下分别降低了11.35%和-17.43%。此外,尾车偏置对列车近尾流区域流场结构有一定影响,尾车鼻锥下方排障器周围漩涡结构由双漩涡结构向单漩涡结构转变;鼻尖处漩涡结构随着尾车高度下降而增大,随着尾车高度上升而逐渐变小。本研究结果丰富了列车尾部偏置情况下的相关研究,对列车检修以及CR450高速列车设计等具有重要指导意义。 展开更多
