The Causes of Conversion from Laparoscopy to Laparotomy in Patients with Laparoscopic Repair of Perforated Peptic Ulcer 被引量:4

Laparoscopic ulcer repair is a feasible and safe procedure. The aim of this study was to research the efficacy of laparoscopic repair of peptic ulcer and to discuss the causes of conversion from laparoscopy to laparotomy. Methods: We collected 34 patients with perforated peptic ulcer underwent laparoscopic surgery from October 2003 to October 2008. Thirty four patients with perforated peptic ulcer underwent laparoscopic intervention and 6 cases were converted to laparotomy. The demographics, laboratory data, perioperative data, morbidity and mortality were compared. Results: In demographics of two groups, there were no significant differences in sex, age, location, and mean duration of symptoms of acute abdominal pain. However, there were significant differences in median size of perforation, mean duration of history of peptic ulcer related pain, and the experiences of surgeon. There were no significant differences in the laboratory data and perioperative data of two groups. In morbidity?and mortality of two groups, there were no significant differences in leakage, wound infection, intra-abdominal abscess, ileus, urinary tract infection, pneumonia, and mortality, but there was significant difference in overall morbidity in two groups. Conclusions: Laparoscopic repair of perforated peptic ulcer is safe and could be used in routine clinical practice. However, patients with larger perforations (>10 mm), longer duration of history peptic ulcer related pain (>2 years), and learning curve of surgeon could be associated with conversion rate. It is associated with higher morbidity in patients with conversion from laparoscopy to laparotomy. 展开更多

Conversion of SrSO4 to acidic strontium oxalate hydrate(H[Sr(C2O4)1.5(H2O)]) in aqueous H2C2O4 solutions proceeds as a consecutive reaction. In the first step of the consecutive reaction, SrSO4 reacts with H2C2O4 and pseudomorphic conversion to SrC2 O4·H2O occurs. In the second step, SrC2 O4·H2O reacts with H2C2O4 to form H[Sr(C2 O4)1.5(H2O)]. Sr(HC2 O4)(C2 O4)0.5·H2 O crystallizes during cooling of the reaction mixture to room temperature if the solution reaches the saturation concentration of (H[Sr(C2O4)1.5(H2O)]. The aims of this study are the derivation of reaction rate equations and the determination of the kinetic parameters such as pre-exponential factor, apparent activation energy and order of H2C2O4 concentration for each reaction step.Fractional conversions of SrSO4 were calculated using the quantitative amounts of dissolved S and Sr. It was determined that the reaction rate increased at the initial time of reaction by increasing the temperature using solutions with approximately same H2C2O4 concentrations. The reaction extends very slowly after a certain time in solutions with low H2C2O4 concentration and ends by the formation of a protective layer of SrC2O4-H2O around the surfaces of solid particles. Fractional conversion of SrSO4 is increased by increasing concentration of H2C2O4 at constant temperature. Kinetic model equations were derived using shrinking core model for each step. 展开更多

First, we discuss the theoretical model of a 4/3 conversion scheme and then design the hardware structure implemented in parallel and illustrate the subsystem structure in detail. Surface OH radical concentration in Beijing City was measured by impregnated filter trapping technique-high performance liquid chromatography (IFT-HPLC). The observed concentration of OH radical showed obvious diurnal and seasonal variations, with maximum readings at noon or afternoon, similar to 80x10(6)OH/cm(3) in summer and similar to 20x10(6)-40x10(6)OH/cm(3) in fall. On the basis of measured data, the reaction rates related to the photochemical process of HO(x) (OH+HO(2)) were derived and characteristics of atmospheric chemical processes in the city were analyzed. The results showed that conversion rates of atmospheric OH and HO(2) in the summer of Beijing City were about 700x10(6) molecule/(cm(3) . s) and 600x10(6)molecule/(cm(3) . s), respectively. And the net production of OH in the air of the city mainly originated from the photolysis of the gaseous HNO(2), and the main sink of OH were the photochemical reactions with VOCs, NO(2), HCHO and CO. This paper concerns the influence of temperature and local concentration of oxygen on the conversion efficiency of carbon into CO, CO2, CH4, C3H8, C2H4, C2H2, C2H6, C6H6, during the thermal degradation of plastic bags and millet stalks. The experimental device used is the tubular kiln, coupled to an analyzer Fourier Transform Infrared (FTIR) and a Non Dispersive Infrared analyzer (NDIR). Temperatures are considered between 800 and 1000°C. Local concentrations of oxygen during thermal degradation are 0%, 10% and 21%. On the one hand results obtained on the influence of temperature show that for each type of thermal degradation and whatever the temperature of the combustion, the rate of conversion of carbon remains substantially the same. In the case of plastic bags, the rate of carbon converted during pyrolysis is about 90% of carbon converted during reductive combustion. On the other hand, with millet stalks, the carbon converted represents only 60% of the rate of carbon converted during combustion to 10% oxygen. 1 to 2% of carbon not analyzed is in the form of aromatic compounds that are found most often in the soot and/or tar from this combustion system. Moreover, whatever the temperature, the overall efficiency of carbon conversion increases linearly with the local concentration of oxygen. During the thermal degradation of plastic bags, we see that the reducer environment has fostered the conversion of 7% of carbon more while the presence of oxygen in double proportion promotes the conversion of 27% carbon. Regarding the influence of the local content of oxygen, it is clear that for plastic bags, the reactions of oxidation of CO into CO2 tend to be favored for the benefit of those of hydrocarbons into CO. The formation of CO and CO2 by oxidation of light hydrocarbons is primarily from gaseous compounds CH4 and C2H4. At 950°C, we have also acetylene (C2H2) which is involved in the production of carbon oxides. At 1000°C, benzene (C6H6) heavily involved in the formation of CO and CO2. However, with millet stalks, more the local content of oxygen increases, more combustion is better, that is to say that the oxidation reactions producing CO2 are faster than the oxidation reactions of hydrocarbons into CO. The rate of carbon converted into CO and CO2 is higher for millet stalks than for plastic bags, due to this oxygen levels higher in millet stalks than in plastic bags. Similarly, for the millet stalks, from pyrolysis to combustion (at 10 and 21% oxygen), there is practically no hydrocarbon emitted. 展开更多

The high mortality rate in hepatocellular carcinoma(HCC)is partially due to the fact that a significant number of patients are diagnosed at an intermediate or advanced stage,with surgical treatment options unavailable.Conversion therapy,which involves both locoregional and systemic treatments,has the potential to downstage tumors in selected patients with initially unresectable HCC,thereby making surgical treatment a possibility and potentially increasing long-term survival.To optimize the conversion rate,it is necessary to maximize successful conversions and clearly define the target population for conversion treatment through a collaborative effort.In this review article,we summarize the clinical experience and evidence for conversion therapy in patients with‘potentially resectable’HCC from four perspectives:1)defining the target population for conversion therapy,2)selecting the appropriate conversion strategy,placing emphasis on the utilization of combination therapy that exhibits a significant objective response rate,3)determining the timing and urgency of surgical resection,4)promoting the adoption of a multidisciplinary team model.The authors are optimistic that with the continuous progress in treatment and a deeper understanding of HCC,the success rate of HCC conversion therapy will increase,and the overall survival of HCC patients will be prolonged. 展开更多

On the basis of continuous tests and batch tests, conversion regular patterns of acetate, propionate and butyrate in activated sludge at different heights of the UASB reactor were conducted. Results indicated that the conversion capacity of the microbe is decided by the substrate characteristic when sole VFA is used as the only substrate. But when mixed substrates are used,the conversion regulations would have changed accordingly. Relationships of different substrates vary according to their locations. In the whole reactor, propionate's conversion is restrained by acetate and butyrate of high concentration. On the top and at the bottom of the reactor, conversion of acetate, but butyrate, is restrained by propionate. And in the midst, acetate's conversion is accelerated by propionate while that of butyrate is restrained. It is proved, based on the analysis of specific conversion rate, that the space distribution of the microbe is the main factor that affects substrates' conversion. The ethanol type fermentation of the acidogenic phase is the optimal acid type fermentation for the two phase anaerobic process. 展开更多

Interannual variations in energy conversion and interaction between the mesoscale eddy field and mean flow in the Kuroshio south of Japan 被引量:2

Using 19-year satellite altimetric data, variations in the eddy kinetic energy, energy exchanges and interaction between the eddy fi eld and mean fl ow are discussed for the Kuroshio south of Japan. In the seasonal cycle, the eddy kinetic energy level is a minimum in December/January and a maximum in April/May. In addition to seasonal variations, the eddy kinetic energy undergoes interannual changes. The energy transfers mainly from the mean fl ow to the eddy fi eld in the Kuroshio south of Japan, and dominant energy exchanges mainly occur along the Kuroshio path south of Japan in each year from 1993 to 2011. In addition, there is often barotropic instability south of Honshu. Regarding interactions between the eddy fi eld and mean fl ow, cyclonic and anticyclonic accelerations are also found along the Kuroshio path and they fl ank each other. There is cyclonic acceleration always imposed on southeast of Kyushu, and anticyclonic acceleration dominates south of Honshu from 2001 to mid-2005. Reynolds stress is used to explain the dynamic process of energy exchange. Furthermore, lag-correlation and linear regression analysis show that variability of the energy conversion rate and Reynolds stress involve responses to eddy acceleration at two time scales. The enhanced eddy acceleration induces large Reynolds stress, and enhanced Reynolds stress or barotropic instability further enforces energy transfer from the mean fl ow to the eddy fi eld. 展开更多

In order to achieve high-efficiency alkali conversion and impurity removal of high titanium slag under the condition of low alkali concentration,a new way of oxygen-rich alkali conversion in KOH solution was proposed.The conversion law of element occurrence state and the influence of the conversion conditions on the titanium conversion rate and removal rate of silicon and aluminum were studied.The results showed that the KOH solution converted the titanium oxide in high titanium slag into whisker-like potassium titanate.Silicon and aluminum elements were dissolved into the solution.Under the following conditions,KOH concentration of 6 mol/L,conversion temperature of 260℃,initial oxygen partial pressure of 2 MPa,liquid−solid ratio of 35 mL/g,conversion time of 4 h,and high titanium slag particle size of 48−74μm,the conversion rate of titanium was 97.0%,and the removal rates of silicon and aluminum were 90.2%and 76.2%,respectively.Oxygen-rich alkali conversion product was converted to rutile with a TiO_(2) grade of 99.1%by acid hydrolysis conversion. 展开更多

The electrochemical conversion of carbon dioxide(CO_(2))has been attracting increasingly research interest in the past decade,with the ultimate goal of utilizing electricity from renewable energy to realize carbon neutrality,as well as economic and energy benefits.Nonetheless,the capture and concentrating of CO_(2) cost a substantial portion of energy,while almost all the reported researches showed CO_(2) electroreduction under high concentrations of(typically pure)CO_(2) reactants,and only very few recent studies have investigated the capability of applying low CO_(2) concentrations(such as~10%in flue gases).In this work,we first demonstrated the electroreduction of 0.03%CO_(2)(in helium)in a homemade gas‐phase electrochemical electrolyzer,using a low‐cost copper(Cu)or nanoscale copper(nano‐Cu)catalyst.Mixed with steam,the gas‐phase CO_(2) was directly delivered onto the gas‐solid interface with the Cu catalyst and reduced to CO,without the need/constraint of being adsorbed by aqueous solution or alkaline electrolytes.By tuning the catalyst and experi‐mental parameters,the conversion efficiency of CO_(2) reached as high as~95%.Furthermore,we demonstrated the direct electroreduction of 0.04%CO_(2) from real air sample with an optimized conversion efficiency of~79%,suggesting a promising perspective of the electroreduction ap‐proach toward direct CO_(2) conversion. 展开更多

We investigated the conversion of methane in coke-oven gas by steam reforming process in an electric tubular flow at 14 kPa with temperature varying from .500 ℃ to 9.50 ℃, and developed a kenetic model for, ignoring the effects of adsorption and diffusion. The optimal dynamic conditions for methane conversion 14 kPa are as follows: the ratio of the amount of water to the amount of methane is from 1.1 to 1.3; the reaction temperature is from 1 223 K to 1 273 K. The methane conversion rate is larger than 95% when the ratio of the amount of water to the amount of methane is 1.2 at a temperature above 1 223 K with the residence time up to 0.75 s. 展开更多

We propose a Rate-Distortion (RD) optimized strategy for frame-dropping and scheduling of multi-user conversa- tional and streaming videos. We consider a scenario where conversational and streaming videos share the forwarding resources at a network node. Two buffers are setup on the node to temporarily store the packets for these two types of video applications. For streaming video, a big buffer is used as the associated delay constraint of the application is moderate and a very small buffer is used for conversational video to ensure that the forwarding delay of every packet is limited. A scheduler is located behind these two buffers that dynamically assigns transmission slots on the outgoing link to the two buffers. Rate-distortion side information is used to perform RD-optimized frame dropping in case of node overload. Sharing the data rate on the outgoing link between the con- versational and the streaming videos is done either based on the fullness of the two associated buffers or on the mean incoming rates of the respective videos. Simulation results showed that our proposed RD-optimized frame dropping and scheduling ap- proach provides significant improvements in performance over the popular priority-based random dropping (PRD) technique. 展开更多

Laser-assisted simulation technique has played a crucial role in the investigation of dose rate effects of silicon-based devices and integrated circuits,due to its exceptional advantages in terms of flexibility,safety,convenience,and precision.In recent years,wide band gap materials,known for their strong bonding and high ionization energy,have gained increasing attention from researchers and hold significant promise for extensive applications in specialized environments.Consequently,there is a growing need for comprehensive research on the dose rate effects of wide band gap materials.In response to this need,the use of laser-assisted simulation technology has emerged as a promising approach,offering an effective means to assess the efficacy of investigating these materials and devices.This paper focused on investigating the feasibility of laser-assisted simulation to study the dose rate effects of wide band gap semiconductor devices.Theoretical conversion factors for laser-assisted simulation of dose rate effects of GaN-based and SiC-based devices were been provided.Moreover,to validate the accuracy of the conversion factors,pulsed laser and dose rate experiments were conducted on GaN-based and SiC-based PIN diodes.The results demonstrate that pulsed laser radiation andγ-ray radiation can produce highly similar photocurrent responses in GaN-based and SiC-based PIN diodes,with correlation coefficients of 0.98 and 0.974,respectively.This finding reaffirms the effectiveness of laser-assisted simulation technology,making it a valuable complement in studying the dose rate effects of wide band gap semiconductor devices. 展开更多

为优化马铃薯高效再生及遗传转化体系,研究鄂马铃薯3号茎段外植体在不同激素浓度配比下愈伤组织及分化情况,获得最佳愈伤组织及分化激素配比,再采用根癌农杆菌的遗传转化进行筛选,获得最佳转化体系,为马铃薯优质品种的基因工程育种提供... 为优化马铃薯高效再生及遗传转化体系,研究鄂马铃薯3号茎段外植体在不同激素浓度配比下愈伤组织及分化情况,获得最佳愈伤组织及分化激素配比,再采用根癌农杆菌的遗传转化进行筛选,获得最佳转化体系,为马铃薯优质品种的基因工程育种提供依据。结果表明,不同激素浓度配比下愈伤组织及分化率差异较大,其中2.0 mg/L ZR+0.1 mg/L IAA激素配比下可高效再生出芽,出芽率高达100%,芽长势粗壮。通过构建植物表达载体pCambia1301,以GV3101为介导菌株,预培养2~3 d,共培养2~3 d,350 mg/L羧苄青霉素,50 mg/L卡那霉素筛选浓度转化效果较好,转化效率可达65%,其中转化苗在0.8 mg/L IBA+0.2 mg/L NAA激素配比的培养基中可形成再生植株,再生率达80%,苗茎粗壮,根系发达。 展开更多
