Methods Respiratory physicians received questionnaire at randomization just after they managed a patient presenting to outpatient department with AE-CB/COPD, on general conditions and symptoms of patients, type of examinations, diagnosis, type of antibiotics used and mode of administration. Results of sputum bacteria culture were followed up. Results Among the 1583 AE-CB/COPD, 63.04% were male and 35.19% were female. 54.6% of them were older than 60 years. 81.87% of the patients produced sputum. Sputum bacteria culture, chest X-ray and chest CT were carried out to 21.3%, 66.3% and 11.1% patients, respectively. 355 strains were isolated from patients whose sputum bacteria culture was positive. Streptococcus pneumoniae, Haemophilus influenzae and Moraxella catarrhalis were the three most isolates. 84.7% of the 1583 AE-CB/COPD received 1692 times of antibiotic prescribing. The most frequently prescribed antibiotic were B-lactams (51.36% of all antibiotic prescribing), macrolides (14.01%) and quinolones (31.03%). Much more macrolides were prescribed in the area where more patients pay the medicines at his own expense than those in the area where more patients share public health service. Conclusion In China, respiratory physicians can reasonably select antibiotics to manage acute exacerbation of chronic bronchitis/ chronic obstructive pulmonary disease in pulmonary outpatient department. Sputum culture is done in part of the patients, but susceptibility tests are missing. One issue revealed by the survey is that the list of prescribing medications laid down by government have great influence on antibiotic use.展开更多

A weekly average of 10.4 tonnes of waste is produced by chicken meat hatcheries. The cost of disposal (average $127/tonne) and availability of disposal sites is an emerging issue. The majority of hatchery waste is sent to land fill or for composting, with some rendered for use as pet food. Hatchery wastewater is mostly used for irrigation or disposed directly into the sewer. Most of the hatcheries have no environmental issues with hatchery waste on site but some report odour problems. Some hatcheries would like to treat the waste on site so that it could be sold as a commodity or to use methods to separate liquid from solid waste and recycle water.展开更多

The majority present with disease confned to the ovary and have an excellent prognosis after surgical removal. A small proportion subsequently has recur-rent disease or progression to invasive cancer. Tumor recurrence can occur up to 20 years after surgical resection. There are no robust clinical, histological or molecular markers that distinguish high risk cases and no satisfactory treatment for patients with progressive disease. This results in great variability in management in different centres. We conducted a national survey on the management of borderline ovarian tumors in cancer centres representing different regions in the United Kingdom. In this article we review the literature for the current concepts in diagnosis, treatment and follow up of BOTs and we report the results of the survey of current practice in the United Kingdom. On that basis we provide recommendations for the management of patients with BOTs.展开更多

Study Design: We conducted a survey of OB/GYNs (n = 220) and HEMs (n = 30) to describe the characteristics of current treatment strategies for postpartum hemorrhage. Surveys were administered via a structured questionnaire on a secure internet website from 5 - 12 October 2009. Results: The majority of OB/GYN and HEM respondents were practicing in a community hospital environment (77%). Of the OB/GYNs, the majority practiced at hospitals with over 2000 deliveries per year (77%). A majority (58%) of OB/GYNs were affiliated with hospitals that lacked a massive transfusion protocol to treat severe postpartum hemorrhage. Subsequent to uterine massage and additional oxytocin, the majority of OB/GYNs (73%), preferred the administration of Methergine? as the next level of intervention for postpartum hemorrhage. There was considerable variability in response to specific treatment strategies for several hypothetical case scenarios;however, the large majority of OB/GYNs favored obstetrical procedures over interventional radiology or administration of rFVIIa. A large majority (77%) of physicians who are familiar with rRVIIa as treatment for postpartum hemorrhage reported being very satisfied with the agent for this indication. Conclusions: An established, systematic treatment strategy among OB/GYNs emerged only in the case of mild postpartum hemorrhage.展开更多

As far as Germany is concerned, geothermal energy and coal bed/mine methane are considered as mineral resources according to the German mining law. This demands the participation of mine surveyors/mineral resources managers from the planning to the production stage and even beyond it. The main tasks that have to be fulfilled are, among others, applications for exploration and production licences as well as the evaluation of the resource’s potential. This paper indicates the tasks of mine surveyors/mineral resources managers in the wide area of the renewable energies in Germany, especially concerning the use of geothermal energy and methane.展开更多

An online interactive survey of US medical and pharmacy directors was conducted in 2012. In addition to a 10-point Likert scale (10 = agree completely, 1 = disagree completely), qualitative responses and interpretive analysis were used to explore beliefs about certain statements. The results showed that the 30 respondents (20 medical directors and 10 pharmacy directors) rated current progress in obtaining usable CER (comparative-effectiveness research) at only an average of 4.17 on the 10-point scale. They hoped to regularly utilize CER information in formulary decision making by 2015 (average rating, 6.03). The rating of evidence-based medicine use in coverage decision making today was somewhat higher, at an average of 7.08 (medical directors, 7.38; pharmacy directors, 6.40). The survey participants believe that emerging CER results will greatly affect the following areas: optimization/improvement of clinical guidelines, medical/pharmacy benefit management, assessments of the value and appropriateness of interventions, and pharmaceutical research and development. Therefore, payers expect CER to play an increasing role in helping them determine the value of new therapies.展开更多

Conclusions: An established, systematic treatment strategy among OB/GYNs emerged only in the case of mild postpartum hemorrhage.
文摘Due to the public’s increased interest in renewable energies and the “Erneuerbare Energien Gesetz” (EEG) from February 2000, geothermal energy and other alternative or renewable energies gain more and more importance. This paper indicates the tasks of mine surveyors/mineral resources managers in the wide area of the renewable energies in Germany, especially concerning the use of geothermal energy and methane.
文摘The paper is to determine the types of approaches preferred by medical and pharmacy directors of US payer organizations to enhance the Pharmacy & Therapeutic decision-making process and how medications accepted onto the formulary should be covered. Therefore, payers expect CER to play an increasing role in helping them determine the value of new therapies.
文摘This paper presents a survey of technologies for personal data self-management interfacing with administrative and territorial public service providers. It classifies a selection of scientific technologies into four categories of solutions:Personal Data Store(PDS),Identity Manager(IdM),Anonymous Certificate System and Access Control Delegation Architecture. Each category, along with its technological approach,is analyzed thanks to 18 identified functional criteria that encompass architectural and communication aspects,as well as user data lifecycle considerations. The originality of the survey is multifold.First,as far as we know, there is no such thorough survey covering such a panel of a dozen of existing solutions.Second,it is the first survey addressing Personally Identifiable Information(PII)management for both administrative and private service providers.Third, this paper achieves a functional comparison of solutions of very different technical natures. The outcome of this paper is the clear identification of functional gaps of each solution. As a result, this paper establishes the research directions to follow in order to fill these functional gaps.
文摘Xiaoqing River is one of the key rivers in the Yellow River Basin, and its management satisfaction is the content that the government should consider when formulating policies. This paper concentrates on residents’ satisfaction of water environment management in Jinan section of Xiaoqing River, using questionnaires to find out the problems and effects of Xiaoqing River management. Based on the correlation analysis of the questionnaire data, the results show that five factors including the impact of water pollution, understanding of Xiaoqing River governance, willingness to participate in Xiaoqing River governance, policy publicity, and government regulation have a positive impact on the satisfaction of Xiaoqing River water environment governance. Finally, the paper puts forward some countermeasures and suggestions to increase residents’ satisfaction from five aspects, such as increasing publicity efforts, paying attention to the cultivation of public participation consciousness, etc.

文摘Background:Tuberculosis(TB)patient management(TPM)is crucial to improve patient compliance to treatment. The coverage of TPM delivered by TB dispensaries or Centers for Disease Control and Prevention(CDC)was not high under the previous CDC model of TB control in China. In the integrated TB control model in China,TB patient management(TPM)was mainly delivered by lay health workers(LHWs)in primary health care(PHC)sectors. This study aims to investigate TPM delivery in resource-limited western China and to identify factors affecting TPM delivery by LHWs under the integrated TB control model. Methods:A stratified random sampling was used to select study sites.Pulmonary TB(PTB)patients≥15 years old from selected counties/districts in Guizhou Province were surveyed from August 2015 to May 2016.Structured questionnaires were used to collect data.Aχ^2 test and logistic regression were used to identify factors associated with self-administered treatment(non-TPM).Results:In total,638 PTB patients were included in the final analysis.Close to 30%of patients were ethnic minorities.More than 30%of patients were from counties with high TB burden, and 24.9%of patients had poor compliance to treatment.Only 37.1%of patients received TPM delivered by LHWs under the integrated TB control model throughout the treatment period. The main reasons for unwillingness to manage reported by patients included social stigma and no perceived need.Being ethnic minorities(OR=3.35)was a main factor associated with lower likelihood of receiving TPM,while living in areas with middle or high TB burden may increase the likelihood of receiving TPM(OR=0.17 and 0.25,respectively).Among current management approaches, more than 85%of patients chose phone reminder as their preferred TPM by LHWs.Conclusions:TPM under the integrated model in West China is still low and need further improvement, and the impeding factors of TPM need to be addressed.Strengthening patient-centered and community-based TPM and developing more feasible approaches of TPM delivery should be explored in future research in this region.
