They occur in adults of 60 years of age or more. The objective of this work is to describe the clinical-pathological characteristics, the immunophenotype and the differential diagnosis. Clinical Case: From the files of the surgical pathology unit we found four cases of primary diffuse large B cell lymphoma of the central nervous system in a 6-year period. Three corresponded to women over 47 years of age and the other to a 42-year-old man. The time of evolution was between 2 and 4 months. The symptoms were headache, blurred vision, hemiparesis, and seizures. Localization was in the pineal region, the frontal, parietal regions, and the right thalamus. Morphologically, large lymphoid cells with a diffuse growth pattern and necrosis were observed. Immunohistochemical markers, such as CD 20 and bcl2 were positive, one was positive to CD3. Expression of bcl6 and CD 10 was positive in one case, and MUM-1 was positive in three cases. All the cases were negative for Epstein-Barr virus. Conclusions: The diffuse large-B cell lymphoma of the central nervous system is rare. Its average age of presentation is at 60 years or older. The localization is in the pineal, frontal, parietal and thalamic regions. Three cases were originated by activated B lymphocyte (MUM-1 expression) and other from the Germinal Center (GC) (CD 10 expression). The clinical course was bad. The four patients died shortly after the diagnosis.展开更多

Analysis of the effect on conduction system of these drugs was also performed. Methods Forty three patients with AF were randomly assigned to receive intravenous therapy with 0.25mg/kg diltiazem (n = 21) or 0.4rag cedilanid-D (n = 22). If not effective at 120 minutes (〈 20% decrease in pretreatment ventricular rate or can not convert to sinus rhythm= another dose of diltiazem or 0.2mg cedilanid-D was administered. Blood pressure and electrocardiographic recordings were performed before and after 5, 10, 20, 30, 60 minutes of drug administration. Further recordings were performed at 120 minutes in noneffective patients, and at 180 minutes in patients who received second time drug administration. To evaluate the effect on conduction system of these two drugs by measuring PA, AH and HV intervals using His bundle electrogram test another nineteen sinus rhythm patients were randomized to dihiazem (n=9) and cedilanid (n= 10) group. His bundle electrogram recordings were performed before and after 5, 10, 20 and 30 minutes of drug administration. Statistical significance was assessed with the use of t test, Fisher's exact test, ANOVA and LSD methodology. Results At baseline and after 5, 10, 20, 30, 60 minutes of drug administration the heart rates (mean±SD) were(133±15), (92±20), (87±22), (85 ±20), (85±21), (85 ±23)beats/minute in diltiazem group respectively and( 140±21 ), ( 122±24), (118±25), (110±26), (112±25), (110±28) beats/ minute in cedilanid-D group respectively. Heart rate reduction was higher in diltiazem group than cedilanid group during 5 (41±20 vs 17±14,P 〈 0.01); 10 (46±21 vs 22±20, P〈0.01); 20 (48±21 vs 29±22, P〈0.01 ) ; 30(48±22 vs 27±22,P〈0.01 )and 60 minutes (48±23 vs 29±24, P〈 0.05). Both drugs had no effect on both systolic and diastolic blood pressure (P 〉0.05) and no major side effects were noticed. Diltiazem maintained effective ventrieular rate in 20 patients, whereas eedilanid-D maintained in 15 patients within 180 minutes (95.2%vs 68.2%,P〈 0.05). There were no statistical significance in baseline heart rate, age and weight between the two groups. Both diltiazem and cedilanid-D can increase AH interval, but have no effect on HV and PA intervals in sinus rhythm patients. Conclusions Both dihiazem and eedilanid-D decrease ventrieular heart rate, but heart rate reduction is significantly higher in diltiazem group, thus should be considered as a drug of choice for emergency control of ventrieular rate. Under clinical monitoring this dose of diltiazem seems to be safe and applicable in AF patients with congestive heart failure. Both drugs have no effect on PA and HV intervals but increase the AH interval thereby can reduce ventricular rate.展开更多

Conventional geothermal systems have the heat, permeability and fluid, requiring only drilling down to °C, normal heat flow or decaying radiogenic granite as heat sources, and used in district heating. Medium-temperature (MT) 100°C - 190°C, and high-temperature (HT) 190°C - 374°C resources are mostly at plate boundaries, with volcanic intrusive heat source, used mostly for electricity generation. Single well capacities are °C - 500°C) and a range of depths (1 m to 20 Km), but lack permeability or fluid, thus requiring stimulations for heat extraction by conduction. HVAC is 1 - 2 m deep and shallow geothermal down to 500 m in wells, both capturing °C, with °C are either advanced by geothermal developers at <7 Km depth (Enhanced Geothermal Systems (EGS), drilling below brittle-ductile transition zones and under geothermal fields), or by the Oil & Gas industry (Advanced Geothermal Systems, heat recovery from hydrocarbon wells or reservoirs, Superhot Rock Geothermal, and millimeter-wave drilling down to 20 Km). Their primary aim is electricity generation, relying on closed-loops, but EGS uses fractures for heat exchange with earthquake risks during fracking. Unconventional approaches could be everywhere, with shallow geothermal already functional. The deeper and hotter unconventional alternatives are still experimental, overcoming costs and technological challenges to become fully commercial. Meanwhile, the conventional geothermal resources remain the most proven opportunities for investments and development.展开更多

Both are chronic conditions that can manifest in various patterns (subtypes) and are linked to extracutaneous involvement in pediatric patients. Juvenile SSc poses a higher risk of morbidity and mortality, with patients facing life-threatening complications such as lung, heart, and visceral organ fibrosis, and vasculopathy. In contrast, mortality is extremely rare in juvenile LS, but patients are susceptible to significant morbidity, leading to severe disfigurement and functional impairment. Treatment for scleroderma aims to control inflammation and address specific issues. An early diagnosis significantly enhances the overall outcome. Despite this challenging context, short-term considerations continue to guide their planning and management rather than more integrated, longer-term perspectives, thus preventing viable, sustainable development. Over the years, the design of humanitarian settlements has not been adapted to local contexts and perspectives, nor to the dynamics of urbanization and population growth and data. In addition, the current approach to temporary settlement harms the environment and can strain limited resources. Inefficient land use and ad hoc development models have compounded difficulties and generated new challenges. As a result, living conditions in settlements have deteriorated over the last few decades and continue to pose new challenges. The stakes are such that major shortcomings have emerged along the way, leading to disruption, budget overruns in a context marked by a steady decline in funding. However, some attempts have been made to shift towards more sustainable approaches, but these have mainly focused on vague, sector-oriented themes, failing to consider systematic and integration views. This study is a contribution in addressing these shortcomings by designing a model-driving solution, emphasizing an integrated system conceptualized as a system of systems. This paper proposes a new methodology for designing an integrated and sustainable human settlement model, based on Model-Based Systems Engineering and a Systems Modeling Language to provide valuable insights toward sustainable solutions for displaced populations aligning with the United Nations 2030 agenda for sustainable development.展开更多

This is mainly due to an increase in the number of forcibly displaced people over the last few decades, and the difficulties of sustainably providing social services that meet the required standards. The development of refugee settlements assumed that forcibly displaced people would return to their places or countries of origin. Unfortunately, displacement situations are prolonged indefinitely, forcing these people to spend most of their lives in conditions that are often deplorable and substandard, and therefore unsustainable. In most cases, the establishment of refugee settlements is triggered by an emergency caused by an influx of forcibly displaced people, who need to be accommodated urgently and provided with some form of international assistance and protection. This leaves little or no time for proper planning for long-term development as required. In addition, the current approach to temporary settlement harms the environment and can strain limited resources with ad hoc development models that have exacerbated difficulties. As a result, living conditions in refugee settlements have deteriorated over the last few decades and continue to pose challenges as to how best to design, plan, and sustain settlements over time. To contribute to addressing these challenges, this study proposes a new methodology supported by Model-Based Systems Engineering (MBSE) and a Systems Modeling Language (SysML) to develop a typical sustainable human settlement system model, which has functionally and operationally executed using a Systems Engineering (SE) approach. To assess the sustainability capacity of the proposed system, this work applies a matrix of crossed impact multiplication through a case study by conducting a system capacity interdependence analysis (SCIA) using the MICMAC methodology (Cross-impact matrix multiplication applied to classification) to assess the interdependency that exist between the sub-systems categories to deliver services at the system level. The sustainability analysis results based on capacity variables influence and dependency models shows that development activities in the settlement are unstable and, therefore, unsustainable since there is no apparent difference between the influential and dependent data used for the assessment. These results illustrate that an integrated system could improve human settlements’ sustainability and that capacity building in service delivery is beneficial and necessary.展开更多

The international literature has contributed to the understanding of the essential aspects to be verified regarding the quality of EIS, offering a wide spectrum of good practice examples related to the content of the studies. Even so, there is a need for empirical studies that allow the identification of specific aspects related to the context of application of the EIS, which could lead to the identification of opportunities to improve both the quality of the reports and also the effectiveness of EIA. Therefore, the present paper is focused on the quality review of a number of EIS submitted to the Brazilian Federal Environmental Agency (Ibama) to instruct the assessment of electric power transmission systems. Based on the application of the EIS quality review package as proposed by Lee and Colley (1992), the outcomes reveal opportunities for improving the scope of EIA, analysis of alternatives, prediction of magnitude and the assessment of impact significance. Finally, the development and/or adaptation of a similar tool for the systematic review of the quality of EIA reports is recommended.展开更多

These thresholds can be obtained in various ways, but most often they are based on previous landslide data. This approach introduces several limitations. For instance, there is a requirement for the location to have been previously monitored in some way to have this type of information recorded. Another significant limitation is the need for information regarding the location and timing of incidents. Despite the current ease of obtaining location information (GPS, drone images, etc.), the timing of the event remains challenging to ascertain for a considerable portion of landslide data. Concerning rainfall monitoring, there are multiple ways to consider it, for instance, examining accumulations over various intervals (1 h, 6 h, 24 h, 72 h), as well as in the calculation of effective rainfall, which represents the precipitation that actually infiltrates the soil. However, in the vast majority of cases, both the thresholds and the rain monitoring approach are defined manually and subjectively, relying on the operators’ experience. This makes the process labor-intensive and time-consuming, hindering the establishment of a truly standardized and rapidly scalable methodology on a large scale. In this work, we propose a Landslides Early Warning System (LEWS) based on the concept of rainfall half-life and the determination of thresholds using Cluster Analysis and data inversion. The system is designed to be applied in extensive monitoring networks, such as the one utilized by Cemaden, Brazil’s National Center for Monitoring and Early Warning of Natural Disasters.展开更多

In contrast to existing research that focuses on the strengths, weaknesses, and improvements of RCS, this study uses literature study to reveal the dynamic evolution of RCS through three phases, with RCS spreading from developed coastal areas to central and western inland regions. RCS’s diffusion path involves vertical diffusion between central and local levels and horizontal diffusion among local governments. Moreover, RCS has also achieved conceptual spillover, gradually expanding into other governance domains, such as the Lake Chief System, the Field Chief System, the Forestry Chief System, and the integration of multiple chief roles. However, it is essential to scrutinize the phenomenon of applying similar governance mechanisms to different areas, as it may result in challenges such as overburdening local governments, insufficient public participation, oversimplification of differences in natural resource endowments, and limited applicability. This study also provides suggestions on how to address these challenges. The study contributes theoretical insights and policy implications, providing a foundation for practical policy innovation.展开更多

文摘Objectives This randomized study was designed to compare the safety and efficacy of intravenous diltiazem versus intravenous cedilanid-D (deslanoside) for ventricular rate control in patients with atrial fibrillation (AF). Both drugs have no effect on PA and HV intervals but increase the AH interval thereby can reduce ventricular rate.

文摘A good quality Environmental Impact Statement (EIS) is key for the effectiveness of Environmental Impact Assessment (EIA) processes and consequently to the acceptability of projects subject to EIA. Finally, the development and/or adaptation of a similar tool for the systematic review of the quality of EIA reports is recommended.
文摘This study outlines the essential nursing strategies employed in the care of 10 patients experiencing vascular vagal reflex, managed with artificial liver support systems. It highlights a holistic nursing approach tailored to the distinct clinical manifestations of these patients. Key interventions included early detection of psychological issues prior to initiating treatment, the implementation of comprehensive health education, meticulous monitoring of vital signs throughout the therapy, prompt emergency interventions when needed, adherence to prescribed medication protocols, and careful post-treatment observations including venous catheter management. Following rigorous treatment and dedicated nursing care, 7 patients demonstrated significant improvement and were subsequently discharged.
文摘A significant portion of Landslide Early Warning Systems (LEWS) relies on the definition of operational thresholds and the monitoring of cumulative rainfall for alert issuance. The system is designed to be applied in extensive monitoring networks, such as the one utilized by Cemaden, Brazil’s National Center for Monitoring and Early Warning of Natural Disasters.
文摘The River Chief System (RCS) has evolved from local innovative practices to a national water governance strategy to address the current challenges in China’s water environmental management. The study contributes theoretical insights and policy implications, providing a foundation for practical policy innovation.
