Case Report: Surgical Correction of Transposition of the Great Arteries with Aortopulmonary Window

Introduction:Transposition of the great arteries(TGA)with aortopulmonary window is a rare type of congenital heart disease with limited experience.We reported a neonate aged 25 days receiving the arterial switch operation and assisted with extracorporeal membrane oxygenation.Conclusion:TGA with aortopulmonary window can be safely correctly with the arterial switch operation.

F.Scott Fitzgerald’s The Great Gatsby successfully portrays the“Jazz Age”.In the early 20th century,the United States transitioned from a production-based society to a consumer-based society,leading to an era of unparalleled prosperity.However,in the context of consumer culture,individuals often transition from being masters of their possessions to becoming enslaved by them.As a result,their behaviours and values become increasingly associated with extravagance and consumption.This paper uses Baudrillard’s theory of consumer society and textual analysis to explore the impact of consumer culture on human beings through the symbolization of men and objectification of women in The Great Gatsby.The meanings embedded in the novel are revealed from a different perspective.

Anatomic Correlates of Mitral Systolic Anterior Motion in Transposition of the Great Arteries Following Atrial Switch Operation

Introduction:We sought to investigate whether the development of sub-pulmonic systolic anterior motion(SAM)may be inherent to the anatomy of the the mitral valve(MV)or affected by external factors,such as a dilated right ventricle or chest abnormalities in d-looped transposition of the great arteries post atrial switch operation(d-TGA/AtS).Methods:Analysis was performed of clinical and cardiac imaging studies acquired on 19 adult patients with d-TGA/AtS(age 42±6 years old,56%male)between 2015–2019.Echocardiography data included mitral apparatus anatomy,and CT/MRI data included biventricular dimensions,function,and Haller index(HI)for pectus deformity.Results:Patients with leaflet SAM(n=6)compared to patients without SAM(n=13)had higher MV protrusion height(2.3±0.5 vs.1.5±0.4 cm,p≤0.01)and longer anterior MV leaflet length(3.1±0.4 cm vs.2.6±0.3 cm p≤0.05),when compared to those without.CT/MRI showed higher sub-pulmonic left ventricular ejection fraction(LVEF)in the SAM group(71%±8%vs.54%±7%,respectively).RV size and function,significant chest deformity(HI>3.5),presence of a ventricular lead pacemaker,and septal thickness did not play a role in development of SAM.Conclusions:An elongated mitral apparatus is associated with the development of SAM,and the development of left ventricular outflow tract obstruction(LVOTO),in d-TGA/AtS.LV hyperkinesia is associated with SAM.Systemic RV dimensions,septal thickness,and degree of chest deformity did not differ significantly between subjects with SAM and those without.

Systemic right ventricle complications in levo-transposition of the great arteries: A case report and review of literature

BACKGROUND Congenitally corrected levo-transposition of the great arteries(L-TGA)is a congenital heart disease in which the ventricles and great arteries are transposed from their typical anatomy.In L-TGA,the double discordance,atrioventricular and ventriculoarterial,create an acyanotic milieu which allows patients to survive their early decades,however,progressive systemic right ventricle(sRV)dys-function creates complications later in life.sRV dysfunction and remodeling predisposes patients to intracardiac thrombus(ICT)formation.CASE SUMMARY A 40-year-old male with L-TGA presented with symptoms of acute decom-pensated heart failure.In childhood,he had surgical repair of a ventricular septal defect.In adulthood,he developed sRV dysfunction,systemic tricuspid valve(sTV)regurgitation,and left-bundle branch block for which he underwent cardiac resynchronization therapy.Transthoracic echocardiogram showed a sRV ejection fraction of 40%,severe sTV regurgitation,and a newly identified sRV ICT.ICT was confirmed by ultrasound-enhancing agents and transesophageal echocardio-graphy.Our patient was optimized with guideline-directed medical therapy and diuresis.Anticoagulation was achieved with a vitamin K antagonist(VKA)and he was later referred for evaluation by advanced heart failure and heart transplant services.CONCLUSION Anticoagulation with VKA is the mainstay of treatment in the absence of conclusive data supporting direct oral anticoagulant use in ICT in patients with congenital heart disease.This case illustrates the natural history of L-TGA and highlights the importance of surveillance and monitoring with dedicated cardiac imaging to identify complications.

Selective Technology Importation and the Convergence in the Late Qing Dynasty: Evidence from Wired Telegraph

Using the adoption and expansion of wired telegraph in China during the late 19^(th) century,this paper investigates the effect of cost reductions for knowledge exchange on China’s industrial growth before the outbreak of the War of Resistance Against Japanese Aggression(1931-1945),thus testing Baldwin’s theory of“the Great Convergence”in which developing countries are empowered by information and communication technologies.Based on panel data of 1858-1937,we found that wired telegraph access had a significantly positive effect,as well as a long-term growth effect,on the entry of industrial enterprises.Our mechanism analysis indicates that wired telegraph access accelerated early-stage industrialization in localities by encouraging market integration,human capital accumulation,and auxiliary commercial organizations.Only a few countries firmly asserted their telegraph sovereignty and set up their own workforce educational system during telegraph adoption.This explains why the Great Convergence arising from technology importation only occurred in a small number of countries.Our findings contribute to understanding the source of China’s modern industrial progress,as well as why global inequities remain.

Research on the Interannual Variability of the Great Whirl and the Related Mechanisms

Based on AVISO （archiving, validation and interpretation of satellite data in oceanography） data from 1993 to 2010, QuikSCAT （Quick Scatterometer） data from 2000 to 2008, and Argo data from 2003 to 2008, the interannual variability of the Great Whirl （GW） and related mechanisms are studied. It shows that the origin and termination times of the GW, as well as its location and intensity, have significant interarmual variability. The GW appeared earliest （latest） in 2004 （2008） and vanished ear- liest （latest） in 2006 （2001）, with the shortest （longest） duration in 2008 （2001）. Its center was most southward （northward） in 2007 （1995）, while the minimum （maximum） amplitude and area occurred in 2003 and 2002 （1997 and 2007）, respectively. The GW was weaker and disappeared earlier with its location tending to be in the southwest in 2003, while in 2005 it was stronger, van- ished later and tended to be in northeast. The abnormal years were often not the same among different characters of the GW, and were not all coincident with ENSO （El Nifio-Southern Oscillation） or IOD （Indian Ocean Dipole） events, indicating the very com- plex nature of GW variations. Mechanism investigations shows that the interannual variability of intraseasonal wind stress curl in GW region results in that of the GW. The generation of the GW is coincident with the arrival of Rossby waves at the Somali coast in spring; the intensity of the GW is also influenced by Rossby waves. The termination of the GW corresponds well to the second one of the top two peaks in the baroclinic energy conversion rate in GW region, and the intensity and the position of the GW are also closely related to the top two baroclinic energy conversion rates.

Focusing on Gatsby's dream in The Great Gatsby, I attempt to analyze the reason for the disillusionment of American Dream at the historical time of the 1920 s. From the micro perspective, Gatsby has four times of disillusionment altogether. The first one was from his birth to his leaving home, the second disillusionment came after his under Dan Cody's patron for several years, ended by Cody's sudden death leaving Gatsby no money actually inherited. His third disillusionment was the frustration of his love for Daisy as Daisy married to Tom Buchanan. Finally, his fourth disillusion brought him away altogether with his death, after the car accident committed by Daisy and Gatsby was mistakenly regarded as the murderer. In a macro perspective,Gatsby's death in the novel symbolizes the disillusionment of American Dream in the Jazz Age.

Organic contamination in the Great Wall bay,Antarctica in austral summer

During 1992/1993 austral summer,oil, BHCs,DDTs,PCBs etc.were determined in the sea water, sediments and organisms samples of the Great Wall bay,Antarctica.In surface sea water,the mean concentrations of oil were 0. 26-0. 93 μg/L.DDTs were all lower than the DL of the method,that was<0.04ng/L. BHCs mean concentrations were<0.69-<1.94 ng/L. In other samples,oil,BHCs,DDTs,and PCBs were all relatively higher in patella than that in sediments and seaweed. PAHs could not be detected in all samples. By comparison,the concentrations level of organic contaminants in the Great Wall bay was low yet.

A Study of Planetary Theory in the Great Expansion System(Dayan li大衍历,727 CE):The Case of Mars

The planetary theory in the Great Expansion System(Dayan li大衍历,727 CE)is investigated,with a detailed example of Mars.In ancient Chinese astrology,the position of one planet and the relative positions of different planets had important astrological significance.Thus,planetary theory is an important part of Chinese mathematical astronomy.The Great Expansion System,which was compiled by Yixing一行of the Tang唐dynasty(618–907 CE),provided many innovations in planetary theory.Based on the extant Treatises on Mathematical Harmonics and Astronomy(Lüli zhi律历志)in Chinese official histories,the Great Expansion System was the first Chinese astronomical system to include tables of the planetary equation of center and procedures for correcting the influence of the planetary equation of center on the position of a planet.It was also the first Chinese system to design a table of the planetary phases of motion for calculating the mean position of a planet,which was the basis for calculating the true position of the planet.In addition,Yixing proposed the concept of the precession of planetary perihelion and gave the values of the precession of planetary perihelion for the first time in ancient China.The innovations of the Great Expansion System regarding planetary theory established its important position in the history of Chinese astronomical systems.Mars is taken as a case study to investigate the planetary theory in the Great Expansion System,including the astronomical constants related to Mars,two important astronomical tables,namely the table of the equation of center and the table of the phase motion of Mars in one synodic period,and the procedures for calculating the position of Mars on any given day using the planetary and solar equations of center.Two questions are addressed.First,how did Yixing correct the influence of the equation of center of Mars on the time of mean conjunction and the mean position of Mars?Second,how did Yixing calculate the true position of Mars on any given day?The original text of the Great Expansion System is analyzed to show how Yixing developed the planetary theory in the Sui and early Tang periods and constructed a complete method for predicting the true positions of planets using the planetary and solar equations of center.

Evaluation of the Long-Term Changes in Fatigue of Cancer Survivors Who Received Outpatient Chemotherapy in the Area Affected by the Great East Japan Earthquake

Objective: The aim of the study is to evaluate the long-term changes in fatigue of cancer patients receiving outpatient chemotherapy after a disaster such as the Great Earthquake. Methods: From 2012 to 2014, we measured subjective fatigue, autonomic function, and physical activity with reference to the criteria for chronic fatigue syndrome. Results: The participants were 13, 9 males (69.2%) and 4 females (30.8%). The most common cancer among the participants was lung cancer 4 patients (30.8%). In 2012, eight patients (61.6%) received anti-cancer drugs and three (23.0%) received the drug by intravenous drip and oral administration. Physical fatigue (p = 0.004), mental fatigue (p = 0.002), comprehensive fatigue (p = 0.012), and comprehensive evaluations (p = 0.032) were significantly different. Significant differences in the autonomic nervous function were observed during the three years from 2012 to 2014 (log LF [p = 0.017], log HF [p = 0.023] and log LF/HF [p = 0.039]). Similar to Subjective fatigue, Significant differences in physical activity were observed during the three years from 2012 to 2014 (DA [p = 0.042], TST [p = 0.037], SE [p = 0.018], AW [p = 0.023] and Naps [p = 0.014]). Conclusions: The participants fatigue worsened at one time, but showed improvement over time. In the event of an emergency such as a disaster, it is necessary to evaluate the physical and mental condition based on objective indicators of cancer patients receiving anti-cancer drug treatment and provide nursing support.

Six new chrysophycean stomatocysts ornamented with reticulum from the Great Xing'an Mountains, China

Six new chrysophycean stomatocysts ornamented with reticulum were illustrated based on SEM observation. They were described following the guidelines of the International Statospore Working Group (ISWG). All samples were collected from the Great Xing'an Mountains, China. Their taxonomic characteristics and habitats were described to provide new information on the biodiversity of chrysophycean stomatocysts. As is common with many morphotypes, their biological affinities remain unknown.

Isolation of the Left Subclavian Artery in D-Transposition of the Great Arteries with Right Aortic Arch

Abnormal branching of the aorta associated with the right aortic arch(RAA)has been reported as isolation of left subclavian artery(ILSA),isolation of left common carotid artery,isolation of brachiocephalic artery.ILSA is a rare aortic branch anomaly that originates in the left subclavian artery from the pulmonary artery via ductus arteriosus.Several reports have described ILSA associated with 22q11.2 deletion syndrome and tetralogy of Fallot.Here,we present a very unusual case of RAA with ILSA associated with D-transposition of the great arteries and inferior vena cava interrupted with azygos continuation.

Implantation of an S-ICD in a Patient with a DDD Pacemaker and Congenitally Corrected Transposition of the Great Arteries

Subcutaneous implantable cardioverter-defi brillator(S-ICD)therapy has become a viable alternative to conventional transvenous ICD implantation.Patients with congenitally corrected transposition of the great arteries(ccTGA)have a high risk of sudden cardiac death due to malignant arrhythmia.The interaction between the S-ICD system and the transvenous pacemaker system is not fully understood.We report a case of S-ICD implantation in a patient with ccTGA and a DDD pacemaker.The patient was a 30-year-old man with a previously placed pacemaker with diagnoses of congenital heart disease,ccTGA(SLL),left atrioventricular valve insuffi ciency,and third-degree atrioventricular block.He presented with an out-of-hospital cardiac arrest,and an S-ICD was implanted to prevent sudden cardiac death.Defi brillation checks were performed successfully.We tested the compatibility of the DDD pacemaker with the S-ICD and found that there was no interference between them.In conclusion,an S-ICD system is a reasonable and safe option in patients with ccTGA.
