To improve the management of patients with heart failure and anemia at the University Hospital of Brazzaville, a cross-sectional study of patients diagnosed with heart failure condition (left or global heart failure) ...To improve the management of patients with heart failure and anemia at the University Hospital of Brazzaville, a cross-sectional study of patients diagnosed with heart failure condition (left or global heart failure) was conducted over a period of nine months from January 1 to September 30, 2017. A total of 171 patients were included during the study period. Study participants were divided into two groups: Group A included patients with an additional anemic condition (n = 57) and Group NA patients without anemia (n = 114). Anemia was defined as a hemoglobin rate of < 12 g/dL for men and <11 g/dL for women. All eligible patients admitted to the Department of Cardiology were included in the study. The frequency of anemia was 33.3%, with a mean hemoglobin level of 9.4 ± 1.5 g/dL. Men accounted for 46.9% of cases (n = 79) and women 53.8% (n = 92). The mean age of eligible patients was 57.5 ± 16.5 years. Of these, 46.2% (n = 75) had a secondary educational level and 53.8% (n = 92) had a low socioeconomic status. Heart failure was global in 153 cases (89.5%). Patients were on NYHA III-IV functional class in 112 cases (65.5%), with a statistically significant difference between anemic and non-anemic patients (p = 0.0001). The main underlying heart diseases were dilated cardiomyopathy (75.1%), hypertensive heart disease (10.5%), ischemic heart disease (6.5%), and valvular disease (4.7%). The comparison between the two groups (A and NA) showed a longer hospital length of stay (18.4 ± 8.9 versus 12.9 ± 7.6 days;p = 0.00001) and a higher mortality rate (4 versus 2 deaths). The re-hospitalization rate was more important in group A (n = 4) than in group NA (n = 1). Anemia is a common condition in patients with heart failure. It worsens the clinical features and prognosis.展开更多

To improve the effectiveness of Korean initial attack resources with a range of policy goals, we create a scenario optimization model that minimizes the expected number of fires not receiving a predefined response. In this study, the predefined response indicates the number of firefighting resources that must arrive at a fire before the fire escapes and becomes a large fire. We use spatially explicit GIS-based information on the ecology, fire behavior, and economic characterizations important in Korea. The data include historical fire events in the Republic of Korea from 1991 to 2007, suppression costs, and spatial information on forest fire extent. Interviews with forest managers inform the range of we address in the decision model. Based on the geographic data, we conduct a sensitivity analysis by varying the parameters systematically. Information on the relative importance of the components of the settings helps us to identify “rules of thumb” for initial attack resource allocations in particular ecological and policy settings.展开更多

In the Republic of Korea(ROK), twenty-seven climate technologies were selected to boost the greening of existing industries and to develop new green industries to promote a sustainable climate technology development strategy. Rechargeable battery technology, carbon capture and storage(CCS) technology, smart grids, and sewage treatment are all research areas expected to have tangible outcomes in the forthcoming years. As such, they were included in a comprehensive R&D plan for climate technology advancement, which places an emphasis on climate technology development and commercialization strategy. In this study, the R&D plan of the ROK is reviewed by examining its six core climate technology programs: solar cells, fuel cells, bioenergy, rechargeable battery technology, information technology(IT) applications for the power sector, and CCS technology in detail. The climate policy in the ROK aims to find new economic growth engines and to develop new business opportunities while actively participating in international efforts to combat climate change.展开更多

Background:Following the West Africa Ebola virus disease(EVD)outbreak(2013-2016),WHO developed a preparedness checklist for its member states.This checklist is currently being applied for the first time on a large and...Background:Following the West Africa Ebola virus disease(EVD)outbreak(2013-2016),WHO developed a preparedness checklist for its member states.This checklist is currently being applied for the first time on a large and systematic scale to prepare for the cross border importation of the ongoing EVD outbreak in the Democratic Republic of Congo hence the need to document the lessons learnt from this experience.This is more pertinent considering the complex humanitarian context and weak health system under which some of the countries such as the Republic of South Sudan are implementing their EVD preparedness interventions.Main text:We identified four main lessons from the ongoing EVD preparedness efforts in the Republic South Sudan.First,EVD preparedness is possible in complex humanitarian settings such as the Republic of South Sudan by using a longer-term health system strengthening approach.Second,the Republic of South Sudan is at risk of both domestic and cross border transmission of EVD and several other infectious disease outbreaks hence the need for an integrated and sustainable approach to outbreak preparedness in the country.Third,a phased and well-prioritized approach is required for EVD preparedness in complex humanitarian settings given the costs associated with preparedness and the difficulties in the accurate prediction of outbreaks in such settings.Fourth,EVD preparedness in complex humanitarian settings is a massive undertaking that requires effective and decentralized coordination.Conclusion:Despite a very challenging context,the Republic of South Sudan made significant progress in its EVD preparedness drive demonstrating that it is possible to rapidly scale up preparedness efforts in complex humanitarian contexts if appropriate and context-specific approaches are used.Further research,systematic reviews and evaluation of the ongoing preparedness efforts are required to ensure comprehensive documentation and application of the lessons learnt for future EVD outbreak preparedness and response efforts.展开更多

In order to study the calculation methods of bending behavior of Chinese reinforced concrete beams from 1912 to 1949, tests on the mechanical performance of 66 rebars from different modem Chinese concrete buildings, t...In order to study the calculation methods of bending behavior of Chinese reinforced concrete beams from 1912 to 1949, tests on the mechanical performance of 66 rebars from different modem Chinese concrete buildings, the concrete compressive strength of 12 modem Chinese concrete buildings, and the concrete cover thickness of 9 modem Chinese concrete buildings are carried out; and the actual material properties and structural conformations of modem Chinese concrete buildings are obtained. Then, the comparison on calculation methods of bending behavior including the original Chinese calculation method, the present Chinese calculation method, the present American calculation method and the present European calculation method is studied. The results show that the original Chinese calculation method of bending behavior is based on the allowable stress calculation method, and the design safety factors are 3.55 to 4. 0. In term of the calculation area of longitudinal rebars of reinforced concrete beams, without considering earthquake action, the original Chinese structural calculation method is safer than the present Chinese structural calculation method, the present European structural calculation method, and the present American structural calculation method. The results can provide support for the structural safety assessments of modem Chinese reinforced concrete buildings.展开更多

At the end of the text, there are few charts. Macedonia gained independence in 1991, in the course of transition from planned to market economy, many socio-economic parameters changed: Demography become evident that fertility rate dropped significantly. In the previous 20 years, number of infants born decreased somewhat 5-6 times compared to 1980. Number of divorce grew rapidly. Also, in the Republic of Macedonia, there followed a parallel process of immigration to western countries, especially western Europe, USA and Australia as well as other countries from the developed world. This migration happened as a result of mainly economic reasons, i.e., countries in which Macedonians migrate have much higher real incomes when compared with Macedonia, and comparatively much higher living standard. One also can find reason for migration in political reasons also, and family reunification. Low fertility is associated with low income and unemployment. People in Macedonia had suffered greatly in transition times with high unemployment, low wages, and corruption, which largely derogated their chances of better future and career and contributed to the demographic aging and low fertility rates.展开更多

Unlike the traditional approach to explaining the Good in the Republic as "rational order" or a unity of Forms, this ]paper argues that the Good is the unique transcendent principle, like the apex of a hierarchy, but does not encompass the whole structure. According to its Ontological position, its multiple facets (functions) include the Ontological foundation of uniting "to be" and "ought to be," the ultimate source of knowledge, and the Ideal goal of uniting the common good and individual goods. The practical dimension of the Good is highlighted in exploring the lifelong study of the Philosopher-Kings and their political personality. It is also pointed out that "sudden enlightenment" plays an important role in their path toward the Good. Finally, this paper proposes that the Good should be the a priori beginning of education and the end of the practice of virtues in the community.展开更多

In term of the calculation area of longitudinal rebars of reinforced concrete beams, without considering earthquake action, the original Chinese structural calculation method is safer than the present Chinese structural calculation method, the present European structural calculation method, and the present American structural calculation method. The results can provide support for the structural safety assessments of modem Chinese reinforced concrete buildings.
文摘In this paper, it explained the process of demographic aging in Republic of Macedonia. There is relevant information for the gradually process of aging. At the end of the text, there are few charts. Macedonia gained independence in 1991, in the course of transition from planned to market economy, many socio-economic parameters changed: Demography become evident that fertility rate dropped significantly. In the previous 20 years, number of infants born decreased somewhat 5-6 times compared to 1980. Number of divorce grew rapidly. Also, in the Republic of Macedonia, there followed a parallel process of immigration to western countries, especially western Europe, USA and Australia as well as other countries from the developed world. This migration happened as a result of mainly economic reasons, i.e., countries in which Macedonians migrate have much higher real incomes when compared with Macedonia, and comparatively much higher living standard. One also can find reason for migration in political reasons also, and family reunification. Low fertility is associated with low income and unemployment. People in Macedonia had suffered greatly in transition times with high unemployment, low wages, and corruption, which largely derogated their chances of better future and career and contributed to the demographic aging and low fertility rates.
文摘This paper focuses on Plato's "Form of the Good," or "the Good," with an interest in Plato's riddle that "the Good is the One." Unlike the traditional approach to explaining the Good in the Republic as "rational order" or a unity of Forms, this ]paper argues that the Good is the unique transcendent principle, like the apex of a hierarchy, but does not encompass the whole structure. According to its Ontological position, its multiple facets (functions) include the Ontological foundation of uniting "to be" and "ought to be," the ultimate source of knowledge, and the Ideal goal of uniting the common good and individual goods. The practical dimension of the Good is highlighted in exploring the lifelong study of the Philosopher-Kings and their political personality. It is also pointed out that "sudden enlightenment" plays an important role in their path toward the Good. Finally, this paper proposes that the Good should be the a priori beginning of education and the end of the practice of virtues in the community.
