This study proposes the use of the MERISE conceptual data model to create indicators for monitoring and evaluating the effectiveness of vocational training in the Republic of Congo. The importance of MERISE for structuring and analyzing data is underlined, as it enables the measurement of the adequacy between training and the needs of the labor market. The innovation of the study lies in the adaptation of the MERISE model to the local context, the development of innovative indicators, and the integration of a participatory approach including all relevant stakeholders. Contextual adaptation and local innovation: The study suggests adapting MERISE to the specific context of the Republic of Congo, considering the local particularities of the labor market. Development of innovative indicators and new measurement tools: It proposes creating indicators to assess skills matching and employer satisfaction, which are crucial for evaluating the effectiveness of vocational training. Participatory approach and inclusion of stakeholders: The study emphasizes actively involving training centers, employers, and recruitment agencies in the evaluation process. This participatory approach ensures that the perspectives of all stakeholders are considered, leading to more relevant and practical outcomes. Objective: To analyze the effect of combined extracorporeal shock wave and rehabilitation training treatment in patients with muscle articulation chronic pain (MACP). Methods: Ninety-seven MACP patients admitted to our hospital from September 2021 to September 2023 were randomly selected and were divided into Group A (control group, 46 cases, rehabilitation training treatment) and Group B (observation group, 51 cases, extracorporeal shock wave with rehabilitation training treatment), and outcomes of the two groups were compared. Results: The treatment efficiency, post-treatment clinical indexes (upper and lower limb function scores, activities of daily living (ADL) scores, visual analog scale (VAS) scores), and short-form 36 (SF-36) scores of Group B were better than those of Group A (P < 0.05). Conclusion: Combined extracorporeal shock wave and rehabilitation training treatment for MACP patients improved their limb function, daily activities, quality of life, and reduced pain. 展开更多

BACKGROUND Type 2 diabetes mellitus(T2DM)is a chronic metabolic syndrome characterized by insulin resistance and hyperglycemia that may lead to endothelial dysfunction,reduced functional capacity and exercise intolerance.Regular aerobic exercise has been promoted as the most beneficial non-pharmacological treatment of cardiovascular diseases.High intensity interval training(HIIT)seems to be superior than moderate-intensity continuous training(MICT)in cardiovascular diseases by improving brachial artery flow-mediated dilation(FMD)and cardiorespiratory fitness to a greater extent.However,the beneficial effects of HIIT in patients with T2DM still remain under investigation and number of studies is limited.AIM To evaluate the effectiveness of high intensity interval training on cardiorespiratory fitness and endothelial function in patients with T2DM.METHODS We performed a search on PubMed,PEDro and CINAHL databases,selecting papers published between December 2012 and December 2022 and identified published randomized controlled trials(RCTs)in the English language that included community or outpatient exercise training programs in patients with T2DM.RCTs were assessed for methodological rigor and risk of bias via the Physiotherapy Evidence Database(PEDro).The primary outcome was peak VO_(2 ) and the secondary outcome was endothelial function assessed either by FMD or other indices of microcirculation.RESULTS Twelve studies were included in our systematic review.The 12 RCTs resulted in 661 participants in total.HIIT was performed in 310 patients(46.8%),MICT to 271 and the rest 80 belonged to the control group.Peak VO_(2 ) increased in 10 out of 12 studies after HIIT.Ten studies compared HIIT with other exercise regimens(MICT or strength endurance)and 4 of them demonstrated additional beneficial effects of HIIT over MICT or other exercise regimens.Moreover,4 studies explored the effects of HIIT on endothelial function and FMD in T2DM patients.In 2 of them,HIIT further improved endothelial function compared to MICT and/or the control group while in the rest 2 studies no differences between HIIT and MICT were observed.CONCLUSION Regular aerobic exercise training has beneficial effects on cardiorespiratory fitness and endothelial function in T2DM patients.HIIT may be superior by improving these parameters to a greater extent than MICT. 展开更多

Purpose-The design goal for the tracking interval of high-speed railway trains in China is 3 min,but it is difficult to achieve,and it is widely believed that it is mainly limited by the tracking interval of train arrivals.If the train arrival tracking interval can be compressed,it will be beneficial for China's high-speed railway to achieve a 3-min train tracking interval.The goal of this article is to study how to compress the train arrival tracking interval.Design/methodologylapproach-By simulating the process of dense train groups arriving at the station and stopping,the headway between train arrivals at the station was calculated,and the pattern of train arrival headway was obtained,changing the traditional understanding that the train arrival headway is considered the main factor limiting the headway of trains.Findings-When the running speed of trains is high,the headway between trains is short,the length of the station approach throat area is considerable and frequent train arrivals at the station,the arrival headway for the first group or several groups of trains will exceed the headway,but the subsequent sets of trains will havea headway equal to the arrival headway.This convergence characteristic is obtained by appropriately increasing the running time.Originality/value-According to this pattern,there is no need to overly emphasize the impact of train arrival headway on the headway.This plays an important role in compressing train headway and improving high-speedrailwaycapacity. 展开更多

Analysis of the Effect of High-Dose Segmental Citrate Anticoagulation in High Flux Hemodialysis

Objective:To analyze the clinical effect of high-dose citrate in segmental extracorporeal anticoagulation for high-throughput hemodialysis.Methods:The subjects included in this study were admitted to the hospital for maintenance hemodialysis treatment from January 2021 to January 2023.All patients had a high risk of bleeding and received 4%trisodium citrate anticoagulant treatment,administered at a rate of 200 mL/h before and after the dialyzer.The anticoagulant effects achieved by the patients were observed and analyzed.Results:The total number of patients who received high-dose segmented citrate extracorporeal anticoagulation dialysis treatment was 50,with each patient undergoing 100 treatments.During the treatment,2 patients had to end the treatment early due to transmembrane pressure exceeding 30 mmHg and an increase in venous pressure exceeding 250 mmHg;the treatment times for these patients were 20 minutes and 200 minutes,respectively.The remaining patients successfully completed the 4-hour treatment.Blood pH and calcium ion concentration in the venous pot were monitored.It was observed that before dialysis,after 2 hours of dialysis,and at the end of dialysis,the blood pH of the patients remained within a relatively normal range.Although some patient levels changed after dialysis,they remained within the normal range.No adverse reactions(such as numbness of the limbs or convulsions)were observed during the anticoagulant treatment.Conclusion:Administering 4%trisodium citrate at a rate of 200 mL/h before and after the dialyzer achieves a good anticoagulant effect,maintains the patient’s blood gas levels within the normal range at the end of dialysis,and causes no adverse reactions. 展开更多

Teaching and training through online tools were used by most Higher Education Institute(HEIs)worldwide during COVID-19 to cater to the needs of students who stay far away from educational institutions.After the pandemic,the virtual model of education and training became a trend.The students and teachers are also more used to this trend which is giving more opportunities to both learners and instructors.One of the notable benefits of virtual teaching and learning platform is that it provides a flexible environment to gain knowledge,skills,and attitude simultaneously along with formal off-line education.From the earlier studies it is found that the majority of studies have focused on traditional,offline training methods and only a few studies have focused on virtual training.Therefore,the present study examined the effectiveness of virtual training using the Kirkpatrick model.For this purpose,a research instrument based on the Kirkpatrick model was constructed and distributed among the UG and PG students in Mangalore City.A total of 143 responses were collected,of which 132 were completed responses and all of them were considered for analysis.Descriptive statistics and one sample t-test are employed to analyse and interpret the data.The findings revealed that virtual training is more effective compared to traditional and offline training methods.Henceforth,the training and education provided through virtual platforms made significant contributions to the employability of youngsters. 展开更多

Based on the CIPP(context,input,process,and product)model,this paper constructs an index system suitable for evaluating the teaching effect of VBSE(virtual business social environment)practical training from four aspects:context evaluation,input evaluation,process evaluation,and product evaluation.Taking the students participating in VBSE practical training in X university as the survey population,381 valid sample data were obtained through an online questionnaire survey,and the index weights were determined by factor analysis method.The score value of the VBSE practical training teaching effect was calculated based on the evaluation mean value of three indexes.The results showed that the context evaluation score was 1.56,the input evaluation score was 1.54,the process evaluation score was 1.51,and the product evaluation score was 1.48.Subsequently,this paper put forward some countermeasures from the aspects of optimizing course arrangement,improving hardware facilities,and enhancing team cooperation to provide a guideline for improving the effect of VBSE practical training. 展开更多

Cancer is a major cause of morbidity and mortality worldwide,and the incidence is increasing,highlighting the need for effective strategies to treat this disease.Exercise has emerged as fundamental therapeutic medicine in the management of cancer,associated with a lower risk of recur-rence and increased survival.Several avenues of research demonstrate reduction in growth,proliferation,and increased apoptosis of cancer cells,including breast,prostate,colorectal,and lung cancer,when cultured by serum collected after exercise in vitro(i.e.,the cultivation of cancer cell lines in an experimental setting,which simplifies the biological system and provides mechanistic insight into cell responses).The underlying mechanisms of exercise-induced cancer suppressive effects may be attributed to the alteration in circulating factors,such as skeletal muscle-induced cytokines(i.e.,myokines)and hormones.However,exercise-induced tumor suppressive effects and detailed information about training interventions are not well investigated,constraining more precise application of exercise medicine within clinical oncology.To date,it remains unclear what role different training modes(i.e.,resistance and aerobic training)as well as volume and intensity have on exercise-condi-tioned serum and its effects on cancer cells.Nevertheless,the available evidence is that a single bout of aerobic training at moderate to vigorous intensity has cancer suppressive effects,while for chronic training interventions,exercise volume appears to be an influential candidate driving cancer inhibitory effects regardless of training mode.Insights for future research investigating training modes,volume and intensity are provided to further our understanding of the effects of exercise-conditioned serum on cancer cells. 展开更多

This study aims to understand the in-service training needs,training transfer,and training effectiveness of teachers in local normal universities in China,while also proposing a continuous professional development plan for teachers.Descriptive research methods were employed,with questionnaires serving as the primary data collection tool.The survey revealed that the educational background of teachers in local normal universities is characterized by an unreasonable distribution of academic qualifications,with a shortage of teachers holding postgraduate and higher degrees.Additionally,the training effectiveness for female teachers was found to be lower than that of male teachers.There is a lack of personalization in training needs,which tends to be more generalized.The transfer effect of training is moderate,and there is a need for further enhancement at the behavioral level of training effectiveness.A continuous professional development plan for teachers has been formulated to improve training effectiveness and promote professional development. 展开更多

Effects of High-Intensity Intermittent Training on Metabolic Parameters and Irisin Levels in High-Fat-Fed Rats

Objective:To investigate the effects of high-intensity intermittent training(HIIT)on preventing significant weight gain and provide scientific theoretical support and practical guidance for reducing the occurrence of obesity.Methods:Twenty-four Sprague-Dawley rats were randomly divided into four groups:the control sedentary group(CS),the high-fat sedentary group(HS),the high-fat continuous exercise group(HE),and the high-fat intermittent exercise group(HI).The HE and HI groups underwent five days of continuous low-intensity exercise and eight weeks of high-intensity intermittent exercise.Weekly monitoring included measurements of food intake and body weight.An automatic biochemical analyzer was used to assess blood lipid and glucose levels,while ELISA kits measured serum insulin and irisin content.H&E staining was used to observe adipocyte size.Results:In the HS group,body weight,blood lipid levels,blood glucose levels,and adipocyte size significantly increased,while the QUICKI index decreased.In the HI group,body weight,blood lipid levels,blood glucose levels,and adipocyte size decreased,and the QUICKI index increased.The effects of high-intensity intermittent exercise were superior to those of continuous low-intensity exercise.In the HI group,serum irisin levels did not change significantly after exercise,while in the HE group,there was a slight upward trend in irisin levels.Conclusion:A high-fat diet induced abnormal metabolism in rats.HIIT effectively prevents metabolic abnormalities induced by a high-fat diet,and its effects are more pronounced than those of low-intensity exercise.HIIT stimulates the secretion of blood irisin,affecting secretion levels,and may represent a novel mechanism for maintaining metabolic homeostasis.This has important implications for controlling significant weight gain. 展开更多

In order to study the treatment of the children with attention deficit hyperactivity disorder (ADHD), the integrated visual and auditory continuous performance test (IVA-CPT) was clinically applied to evaluate the effectiveness of electroencephalogram (EEG) biofeedback training. Of all the 60 children with ADHD aged more than 6 years, the effective rate of EEG biofeedback training was 91.6 % after 40 sessions of EEG biofeedback training. Before and after treatment by EEG biofeedback training, the overall indexes of IVA were significantly improved among predominately inattentive, hyperactive, and combined subtype of children with ADHD (P<0.001). It was suggested that EEG biofeedback training was an effective and vital treatment on children with ADHD. 展开更多

There are 453 patients treated with PCSK-9 inhibitors combined with statins in PCSK-9 inhibitor group and 2,610 patients treated with statins-based lipid lowering therapies in statins-based group. The lipid control rate and incidence of major adverse cardiovascular events(MACE) over six months were compared between two groups.A propensity score-matched(PSM) analysis was used to balance two groups on confounding factors. Survival analysis using Kaplan-Meier methods was applied for MACE.RESULTS In a total of 3,063 patients, 89.91% of patients had received moderate or high-intensity statins-based therapy before PCI, but only 9.47% of patients had low-density lipoprotein cholesterol(LDL-C) levels below 1.4 mmol/L at baseline. In the PSM selected patients, LDL-C level was reduced by 42.57% in PCSK-9 inhibitor group and 30.81%(P < 0.001) in statins-based group after six months. The proportion of LDL-C ≤ 1.0 mmol/L increased from 5.29% to 29.26% in PCSK-9 inhibitor group and 0.23% to 6.11% in statins-based group, and the proportion of LDL-C ≤ 1.4 mmol/L increased from 10.36% to 47.69% in PCSK-9 inhibitor group and 2.99% to 18.43% in statins-based group(P < 0.001 for both). There was no significant difference between PCSK-9 inhibitor and statins-based treatment in reducing the risk of MACE(hazard ratio = 2.52, 95% CI: 0.49-12.97, P = 0.250).CONCLUSIONS In the real world, PCSK-9 inhibitors combined with statins could significantly reduce LDL-C levels among patients with very high risk of ASCVD in China. The long-term clinical benefits for patients received PCSK-9 inhibitor to reduce the risk of MACE is still unclear and requires further study. 展开更多

Variations in obvious form, stress-strain curve, peak strength, peak strain and elastic modulus with temperature were analyzed and the essence of rock failure modes was explored. The results indicate that, compared with granite after the high temperature treatment, the brittle-ductile transition critical temperature is lower, the densification stage is longer, the elastic modulus is smaller and the damage is larger under high temperature. In addition, the peak stress is lower and the peak strain is greater, but both of them change more obviously with the increase of temperature compared with that of granite after the high temperature treatment. Furthermore, the failure modes of granite after the high temperature treatment and under high temperature show a remarkable difference. Below 100 ℃, the failure modes of granite under both conditions are the same, presenting splitting failure. Aerobic capacity, which is expressed as peak oxygen consumption (VO2peak), is well-known to be an independent predictor of all-cause mortality and cardiovascular prognosis. This is true even for people with various coronary risk factors and cardiovascular diseases. Although exercise training is the best method to improve VO2peak, the guidelines of most academic societies recommend 150 or 75 min of moderate- or vigorous- intensity physical activities, respectively, every week to gain health benefits. For general health and primary and secondary cardiovascular prevention, high-intensity interval training (HIIT) has been recognized as an efficient exercise protocol with short exercise sessions. Given the availability of the numerous HIIT protocols, which can be classified into aerobic HIIT and anaerobic HIIT [usually called sprint interval training (SIT)], professionals in health-related fields, including primary physicians and cardiologists, may find it confusing when trying to select an appropriate protocol for their patients. This review describes the classifications of aerobic HIIT and SIT, and their differences in terms of effects, target subjects, adaptability, working mechanisms, and safety. Understanding the HIIT protocols and adopting the correct type for each subject would lead to better improvements in VO2peak with higher adherence and less risk. 展开更多

In 2009, 386,324 people, 1 in 6 Americans, died as a result of coronary artery disease (CAD) alone [1]. They also estimate 150,000 people have “silent” heart attacks each year [1]. Even though the number of cardiovascular disease deaths has declined in the last 10 years, they still accounted for 32.3% of American deaths [1]. As a result, the AHA updated their 2020 goals to improve the nation’s cardiovascular health by 20% [1]. One of these methods is through the use of cardiac rehabilitation. Cardiac rehabilitation (CR) is a health promotion strategy to help return cardiac patients to their previous level of functioning, increase health, decrease comorbidities and promote education and lifestyle change. For select patients, another alternative exercise plan may exist to gain even better results. High intensity interval training (HIIT) has shown positive training results for athletes and many studies show that it may also be an effective exercise modality for many cardiac patients instead of the traditional circuit training method. This article will review current literature on the effects of HIIT on CR patients as well as a sample HIIT protocol for instituting this treatment with appropriate patients. 展开更多

The aim of this study was to compare the effects of two types of high intensity interval training (HIIT) on body mass and physiological indexes in inactive female students. 27 inactive female students (mean age 24.81 ±0.66 yr, height 162.09 ±0.67 cm, weight 59.41 ±0.33 kg) were voluntarily selected and randomly assigned to three groups (each group = 9 subjects) of high intensity interval training (type I), high intensity interval training (type 2) and control. HIIT (type 1) (8 seconds of sprint running and 12 seconds of active recovery) was performed for 4 weeks, 3 sessions per week, 6-9 min. per session with above 90% maxHR. HIlT (type 2) (40-m shuttle run test with maximum speed) was performed with above 90% max HR while the control group did not participate in any training. Data analysis with dependent t test showed that HIIT (type 1) and HIlT (type 2) significantly increased VO2max while they decreased fat percentage and BMI. The intergroup data were analyzed by one-way ANOVA at P 〈 0.05.
