Since the first CHINARE over 30 years ago, considerable work has been undertaken to promote the development of techniques for the observation of surface and upper-air meteorological elements, and satellite image and data reception systems at Chinese Antarctic stations and onboard Chinese icebreakers have played critical roles in this endeavor. The upgrade of in situ and remote sensing measurement methods and the improvement of weather forecasting skill have enabled forecasters to achieve reliable on-site weather forecasting for the CHINARE. Nowadays, the routing of icebreakers, navigation of aircraft, and activities at Chinese Antarctic stations all benefit from the accurate weather forecasting service. In recent years, transdisciplinary research has emerged ...There is a growing need for both science and practice domains to collaboratively and systematically seek knowledge-based strategies for sustainable development. In recent years, transdisciplinary research has emerged as a new approach that enables joint problem solving among scientists and stakeholders in various fields. In this paper, we aim to introduce transdisciplinary research for supporting the integration of the concept of eco- system services into land and water management in the Tarim River Basin, Xinjiang, Northwest China. While a large number of ecosystem service studies have helped to raise the awareness for the value of nature in China, a number of challenges remain, including an improved understanding of the relationships between ecosystem structure, functions and services, and the interaction of the various ecosystem services. A meaningful valuation of ecosystem services also requires the consideration of their strong spatial heterogeneity. In addition, ways to introduce the con- cept of ecosystem services into decision-making in China need to be explored. Thus, successful integration of the concept of ecosystem services into actual land and water management requires a broad knowledge base that only a number of scientific disciplines and stakeholders can provide jointly, via a transdisciplinary research process. We regard transdisciplinary research as a recursive process to support adaptive management that includes joint knowledge generation and integration among scientists and stakeholders. System, target, and transformation knowledge are generated and integrated during the process of (1) problem (re)definition, (2) problem analysis and strategy development, and (3) evaluation of the impact of the derived strategy. Methods to support transdisciplinary research comprise participatory modelling (actor-based modelling and Bayesian Networks modelling) and partici- patory scenario development. Actor-based modelling is a semi-quantitative method that is based on the analysis of problem perspectives of individual stakeholders as depicted in perception graphs. With Bayesian Networks, com- plex problem fields are modelled probabilistically in a simplified manner, using both quantitative data and qualitative expert judgments. These participatory methods serve to integrate diverse scientific and stakeholder knowledge and to support the generation of actually implementable management strategies for sustainable development. For the purpose of integrating ecosystem services in land and water management in the Tarim River Basin through trans- disciplinary research, collaboration among scientists and institutional stakeholders from different sectors including water, agriculture, forestry, and nature conservation is required. Valuation of ecosystem services is favorable for people realizing the importance of natural ecosystem to human being, and considering its long-term influence on sustainable development of human society when making decisions. Besides, it is an attempt of ecology, geography and other natural sciences to influence social decision process by economic methods. The Value of the World's Ecosystem Services and Natural Capital, written by Robert Costanza et al., in 1997, is generally regarded as a masterpiece for the research of valuing ecosystem services. However, the classifying standard of ecosystem services, the method of various services summation and the purpose for static global value, had confronted many criticisms. Still to now, accurately valuing ecosystem services is not easy, because of the complexity of natural ecosystem, the weak capability of economic system to pricing ecosystem services and the lack of more study in this field. Based on the criticisms summarizing and comprehensive analysis, further study suggestions of ecosystem services valuation is presented: multi-scale integrated and community participation applied in evaluation, dynamic changes of material value and intangible value of ecosystem services, investigation and simulation studies of the marginal value when ecosystem services changed, associated with related areas like ecological compensation research, and research on the market-oriented evaluation of ecosystem's intangible values.展开更多
Objectives:In the hospital setting,poor dietary intake interacts with disease and represents a major and modifiable cause of malnutrition.Understanding barriers to adequate dietary intake is an important strategy to g...Objectives:In the hospital setting,poor dietary intake interacts with disease and represents a major and modifiable cause of malnutrition.Understanding barriers to adequate dietary intake is an important strategy to guide the development of interventions to improve nutrition intake.The aim of this study reported in this paper was to explore patient,family and health care professionals'perceptions of barriers to and enablers of adequate nutrition care and dietary intake of medical inpatients.Methods:An exploratory qualitative study design incorporating group and individual interviews of patients(n=14),their family members(n=4),and health care professionals(n=18)was undertaken.Participants were recruited pragmatically,using a mix of convenience and purposive sampling.A theoretically informed,semi-structured interview schedule was based on observations of practice and the Theoretical Domains Framework.Interviews were audio-recorded,transcribed verbatim and analysed inductively using a general inductive approach.Results:Three key themes emerged from analysing participant interviews.Siloed approaches to nutrition care reflected the diverse range of health care professionals responsible for nutrition care but who often worked in isolation from their colleagues.Competing work priorities for nurses reflected the challenge in prioritise nutrition care which was often constrained because of other care needs or work-related pressures.Helping patients to eat highlighted that nurses were often the only health care professional who would provide assistance to patients at mealtimes and lack of available staff could negatively influence patients'nutrition intakes.Conclusions:We have identified many complex and interrelated barriers which preclude adequate dietary intake in acute medical patients.These predominantly reflect issues inherent in the hospital culture and environment.Multi-faceted and sustainable interventions that support a facilitating nutrition culture and multidisciplinary collaboration,inclusive of patients and families,are needed to address these underlying barriers.展开更多
Objective:Unmet supportive care needs(SCNs)impact pediatric cancer patients and their parents.This study aimed to explore the unmet SCNs from the perspective of Chinese children with cancer and their parents through l...Objective:Unmet supportive care needs(SCNs)impact pediatric cancer patients and their parents.This study aimed to explore the unmet SCNs from the perspective of Chinese children with cancer and their parents through lived experiences.Methods:The data of this study was collected using face-to-face semi-structured interviews.The participants were recruited from the oncology units of three children’s hospitals in China’s cities(Shanghai,Guangzhou,and Hefei)from October 2020 to December 2021.Data were analyzed using Colaizzi’s sevenstep phenomenological analysis method.Results:Eight pediatric cancer patients and twenty-four parents were enrolled in the study.Four main themes and eight subthemes(both children’s and parent’s perspectives)were generated:1)meeting the ongoing needs along the cancer trajectory(can you tell me what comes next;our needs are growing);2)communicating with a family focus(they only talk to my parents;let each family member have a voice);3)providing care beyond the treatment(I am bigger than my body[the children’s needs for emotional consolidation and information about their prognosis];there are things beyond treatment);4)getting support from the community(I am not a monster[the children were unhappy about being treated differently];we want to connect with the resources near us).Conclusion:This study revealed multiple unmet SCNs from the perspective of Chinese children with cancer and their parents.The findings call for comprehensive and in-depth supportive care beyond treatment,integration of the family member voice in pediatric cancer care,and a coordinated pediatric cancer support mechanism in the Chinese healthcare system.展开更多
Background:Forests contribute to human wellbeing through the provision of important ecosystem services.Methods:In this study,we investigated how the perceived importance of ecosystem services may impact the overall ...Background:Forests contribute to human wellbeing through the provision of important ecosystem services.Methods:In this study,we investigated how the perceived importance of ecosystem services may impact the overall benefit provided by managed watersheds at the Hubbard Brook Experimental Forest over a 45-year period,using standardized measures of service capacity weighted by service importance weights derived from a survey of beneficiaries.Results:The capacity of watersheds to regulate water flow and quality was high in all watersheds throughout the study period,whereas cultural services such as scenic beauty declined after harvest.Impacts on greenhouse gas regulation depended on the efficiency with which harvested biomass was used.Surveys revealed that stakeholders placed high value on all ecosystem services,with regulating and cultural services seen as more important than provisioning services.When service metrics were weighted by survey responses and aggregated into a single measure,total service provision followed the same overall trend as greenhouse gas regulation.Where biomass use was less efficient in terms of greenhouse gas emissions,harvesting resulted in an overall "ecosystem service debt";where use was more efficient,this "ecosystem service debt" was reduced.Beneficiaries' educational backgrounds significantly affected overall assessment of service provision.Beneficiaries with college or university degrees incurred smaller "ecosystem service debts" and were less negatively affected by harvesting overall.Conclusions:This study highlights the importance of including empirical measures of beneficiary preference when attempting to quantify overall provision of ecosystem services to human beneficiaries over time.展开更多

Background: Mutual Support Groups (MSG) provides to the patient, the ability to effective self-management behaviors, such as taking prescribed medications, following diet and exercise regimens, self-monitoring, and co...Background: Mutual Support Groups (MSG) provides to the patient, the ability to effective self-management behaviors, such as taking prescribed medications, following diet and exercise regimens, self-monitoring, and coping emotionally with the rigors of living with diabetes. Physicians, nurses and health promoters from public primary Health Care Centers (HCC) are responsible for providing follow-up care through MSGs. However, although the MSG program has been carried out, in the last decade Mexico presents the most alarming statistics in the prevalence and complications of Diabetes Mellitus type II (DM-II), suggesting a low impact of MSG in the strategy to support the effective control of the disease. Objetive: The aim of this work was to assess whether knowledge or benefits of effective management to control of DM-II, also to identify strengths and limitations of MSGs, in six different Health Care Centers (HCC), in San Luis Potosi, Mexico. This research provides an overview of patients’ perception, and significant issues that demand to adjust MGSs strategies, with the goal of exceed the expectations of current health statistics. Methods: A qualitative evaluation was carried out, with an ethnographic approach design. The study included 28 diabetic persons, (21 women and 7 men from ruraland urban carecenters). Semi-structured interviews, non-participant observation, and structured content analysis were used. Results: Support groups give patients a way to address emotional issues, and learn about their disease and self-care, although some patients consider participation an inconvenient obligation. Support group users mention barriers such as lack of continuity in support group activities, inconvenient meeting times, and the difficulty of commuting to attend group sessions. They also mention that overworked health care workers find it difficult to provide leadership to keep the group going. These issues have multiple implications for the success of strategies to control the disease. Conclusions: This study shows the need to apply a participatory model to disease support group strategies to reorganize their actions in such a way as to meet the needs and requirements of patients and to ensure their participation and help them control their disease.展开更多

Our aim was to investigate the association between the timing o...Background: Treatment guidelines in the 1990s established surgery followed by chemoradiotherapy as the standard treatment for stage II/III rectal cancer. Our aim was to investigate the association between the timing of adjuvant treatment and patient survival in practice and identify demographic/clinical factors associated with treatment patterns. Methods: All residents of Alberta diagnosed with stage II/III rectal adenocarcinoma in 2000-2005 who had surgery were included in the study. Demographic and clinical data were obtained from the Alberta Cancer Registry and linked to hospital data and socioeconomic data from the 2001 Canadian Census. Overall and cancer-specific hazard ratios of death were estimated using Cox proportional hazards models. Results: 1243 patients were included in the study;636 (51%) patients received treatment consistent with guidelines. Patients who received adjuvant chemotherapy 12 - 16 weeks after surgery or more than 16 weeks/ did not receive it had a 43% and 58% higher risk of rectal cancer death, respectively, compared to those who received it within 8 weeks of surgery. Conclusion: Adjuvant chemotherapy for stage II/III rectal cancer should be initiated within 12 weeks after surgery to maximize treatment benefits. Efforts to increase the proportion of patients treated within 12 weeks after surgery are needed.展开更多

文摘The ecosystem services are material base and natural capital for sustainable development of human being. Valuation of ecosystem services is favorable for people realizing the importance of natural ecosystem to human being, and considering its long-term influence on sustainable development of human society when making decisions. Besides, it is an attempt of ecology, geography and other natural sciences to influence social decision process by economic methods. The Value of the World's Ecosystem Services and Natural Capital, written by Robert Costanza et al., in 1997, is generally regarded as a masterpiece for the research of valuing ecosystem services. However, the classifying standard of ecosystem services, the method of various services summation and the purpose for static global value, had confronted many criticisms. Still to now, accurately valuing ecosystem services is not easy, because of the complexity of natural ecosystem, the weak capability of economic system to pricing ecosystem services and the lack of more study in this field. Based on the criticisms summarizing and comprehensive analysis, further study suggestions of ecosystem services valuation is presented: multi-scale integrated and community participation applied in evaluation, dynamic changes of material value and intangible value of ecosystem services, investigation and simulation studies of the marginal value when ecosystem services changed, associated with related areas like ecological compensation research, and research on the market-oriented evaluation of ecosystem's intangible values.

文摘Objectives:In the hospital setting,poor dietary intake interacts with disease and represents a major and modifiable cause of malnutrition.Understanding barriers to adequate dietary intake is an important strategy to guide the development of interventions to improve nutrition intake.The aim of this study reported in this paper was to explore patient,family and health care professionals'perceptions of barriers to and enablers of adequate nutrition care and dietary intake of medical inpatients.Methods:An exploratory qualitative study design incorporating group and individual interviews of patients(n=14),their family members(n=4),and health care professionals(n=18)was undertaken.Participants were recruited pragmatically,using a mix of convenience and purposive sampling.A theoretically informed,semi-structured interview schedule was based on observations of practice and the Theoretical Domains Framework.Interviews were audio-recorded,transcribed verbatim and analysed inductively using a general inductive approach.Results:Three key themes emerged from analysing participant interviews.Siloed approaches to nutrition care reflected the diverse range of health care professionals responsible for nutrition care but who often worked in isolation from their colleagues.Competing work priorities for nurses reflected the challenge in prioritise nutrition care which was often constrained because of other care needs or work-related pressures.Helping patients to eat highlighted that nurses were often the only health care professional who would provide assistance to patients at mealtimes and lack of available staff could negatively influence patients'nutrition intakes.Conclusions:We have identified many complex and interrelated barriers which preclude adequate dietary intake in acute medical patients.These predominantly reflect issues inherent in the hospital culture and environment.Multi-faceted and sustainable interventions that support a facilitating nutrition culture and multidisciplinary collaboration,inclusive of patients and families,are needed to address these underlying barriers.

文摘Objective:Unmet supportive care needs(SCNs)impact pediatric cancer patients and their parents.This study aimed to explore the unmet SCNs from the perspective of Chinese children with cancer and their parents through lived experiences.Methods:The data of this study was collected using face-to-face semi-structured interviews.The participants were recruited from the oncology units of three children’s hospitals in China’s cities(Shanghai,Guangzhou,and Hefei)from October 2020 to December 2021.Data were analyzed using Colaizzi’s sevenstep phenomenological analysis method.Results:Eight pediatric cancer patients and twenty-four parents were enrolled in the study.Four main themes and eight subthemes(both children’s and parent’s perspectives)were generated:1)meeting the ongoing needs along the cancer trajectory(can you tell me what comes next;our needs are growing);2)communicating with a family focus(they only talk to my parents;let each family member have a voice);3)providing care beyond the treatment(I am bigger than my body[the children’s needs for emotional consolidation and information about their prognosis];there are things beyond treatment);4)getting support from the community(I am not a monster[the children were unhappy about being treated differently];we want to connect with the resources near us).Conclusion:This study revealed multiple unmet SCNs from the perspective of Chinese children with cancer and their parents.The findings call for comprehensive and in-depth supportive care beyond treatment,integration of the family member voice in pediatric cancer care,and a coordinated pediatric cancer support mechanism in the Chinese healthcare system.

文摘Background:Forests contribute to human wellbeing through the provision of important ecosystem services.Methods:In this study,we investigated how the perceived importance of ecosystem services may impact the overall benefit provided by managed watersheds at the Hubbard Brook Experimental Forest over a 45-year period,using standardized measures of service capacity weighted by service importance weights derived from a survey of beneficiaries.Results:The capacity of watersheds to regulate water flow and quality was high in all watersheds throughout the study period,whereas cultural services such as scenic beauty declined after harvest.Impacts on greenhouse gas regulation depended on the efficiency with which harvested biomass was used.Surveys revealed that stakeholders placed high value on all ecosystem services,with regulating and cultural services seen as more important than provisioning services.When service metrics were weighted by survey responses and aggregated into a single measure,total service provision followed the same overall trend as greenhouse gas regulation.Where biomass use was less efficient in terms of greenhouse gas emissions,harvesting resulted in an overall "ecosystem service debt";where use was more efficient,this "ecosystem service debt" was reduced.Beneficiaries' educational backgrounds significantly affected overall assessment of service provision.Beneficiaries with college or university degrees incurred smaller "ecosystem service debts" and were less negatively affected by harvesting overall.Conclusions:This study highlights the importance of including empirical measures of beneficiary preference when attempting to quantify overall provision of ecosystem services to human beneficiaries over time.

文摘Background:Various efforts have been made by the Indonesian government to improve mental health services.In 2014,the government established Law no 18,which is about mental health and the treatment of people with mental illness covered by the universal health coverage.However,many people still experience difficulty in accessing mental health services.In Indonesia,family plays the role ofa caregiver to people with mental illness.Objective:This study aims to identify the perceived barriers on mental health services by families whose members suffers from mental illness.Methods:This study is a qualitative research study with a phenomenological approach.Sampling was conducted by purposive sampling with a sample size of 12 participants.Data were collected using semistructured in-depth interviews.Thematic analysis was performed using Colaizzi steps.Results:The obtained results presented three themes.Theme 1,mental health service affordability;theme 2,mental health service availability;and theme 3,negative attitudes (stigma).Conclusion:Families whose members suffered from mental illness still experienced barriers in relation to mental health services even with universal health coverage,Improved mental health services are related to the health insurance coverage,affordability,availability of mental health services and stigma reduction in the health professionals and wide community.
文摘Background: Mutual Support Groups (MSG) provides to the patient, the ability to effective self-management behaviors, such as taking prescribed medications, following diet and exercise regimens, self-monitoring, and coping emotionally with the rigors of living with diabetes. Physicians, nurses and health promoters from public primary Health Care Centers (HCC) are responsible for providing follow-up care through MSGs. However, although the MSG program has been carried out, in the last decade Mexico presents the most alarming statistics in the prevalence and complications of Diabetes Mellitus type II (DM-II), suggesting a low impact of MSG in the strategy to support the effective control of the disease. Objetive: The aim of this work was to assess whether knowledge or benefits of effective management to control of DM-II, also to identify strengths and limitations of MSGs, in six different Health Care Centers (HCC), in San Luis Potosi, Mexico. This research provides an overview of patients’ perception, and significant issues that demand to adjust MGSs strategies, with the goal of exceed the expectations of current health statistics. Methods: A qualitative evaluation was carried out, with an ethnographic approach design. The study included 28 diabetic persons, (21 women and 7 men from ruraland urban carecenters). Semi-structured interviews, non-participant observation, and structured content analysis were used. Results: Support groups give patients a way to address emotional issues, and learn about their disease and self-care, although some patients consider participation an inconvenient obligation. Support group users mention barriers such as lack of continuity in support group activities, inconvenient meeting times, and the difficulty of commuting to attend group sessions. They also mention that overworked health care workers find it difficult to provide leadership to keep the group going. These issues have multiple implications for the success of strategies to control the disease. Conclusions: This study shows the need to apply a participatory model to disease support group strategies to reorganize their actions in such a way as to meet the needs and requirements of patients and to ensure their participation and help them control their disease.
文摘In this paper, the exact Bayesian limits, taking conjugate and noninformative prior distribution, and the exact fiducial limits for the mean of the lognormal distribution are presented. They can be found iteratively by one-dimension integral on a finite interval. The new algorithm is very convenient and with high accuracy. It can meet the practical engineering need excellently. However, the primitive algorithm is rather cumbersome. By the way, the very close approximate limits with a simple algorithm are derived. They can be applied immediately to engineering. Otherwise, they can also be used as a search interval to find the root of equation for the exact limits.
文摘Background: Treatment guidelines in the 1990s established surgery followed by chemoradiotherapy as the standard treatment for stage II/III rectal cancer. Our aim was to investigate the association between the timing of adjuvant treatment and patient survival in practice and identify demographic/clinical factors associated with treatment patterns. Methods: All residents of Alberta diagnosed with stage II/III rectal adenocarcinoma in 2000-2005 who had surgery were included in the study. Demographic and clinical data were obtained from the Alberta Cancer Registry and linked to hospital data and socioeconomic data from the 2001 Canadian Census. Overall and cancer-specific hazard ratios of death were estimated using Cox proportional hazards models. Results: 1243 patients were included in the study;636 (51%) patients received treatment consistent with guidelines. Patients who received adjuvant chemotherapy 12 - 16 weeks after surgery or more than 16 weeks/ did not receive it had a 43% and 58% higher risk of rectal cancer death, respectively, compared to those who received it within 8 weeks of surgery. Conclusion: Adjuvant chemotherapy for stage II/III rectal cancer should be initiated within 12 weeks after surgery to maximize treatment benefits. Efforts to increase the proportion of patients treated within 12 weeks after surgery are needed.

文摘Objectives: This study aimed to describe work and activities of community networks focusing on the improvement of the quality of life (QOL) of older people in Thailand.The understanding of the work can help enhancing the community development and strengthening of local communities and their networks.Methods: Qualitative methods including in-depth interview,observation,and focus group discussion were employed to the study.64 participants participated to the study and were recruited from 4 key actors within the community.Content analysis was used to analyze the obtained data.This study was conducted in 6 local administrative organizations (LAOs) which selected from the outstanding areas of the project.Each LAO represents one sub-district of the regions of Thailand namely;(1) the upper north,(2) the lower north,(3)the upper eastern,(4) the lower eastern,(5) the central and (6) the south.Results: The findings of this study were categorized into three main themes: (1) Social capital including people in the community,social groups,and organizations,(2) Mutual help/collaboration activities composed of six sets of activities related to social capitals working on the improvement of QOL of older people,and (3) Impacts of the mutual help/collaboration activities on older people and local communities who help to improve of QOL of older people.Conclusion: The findings are important features for the community development.These themes should be recommended for community nurses,health related groups and organizations for the improvement of QOL of older people in the community.
文摘Madness has attracted and frightened for centuries,and talking about this means discussing how this diversity was built and managed in different social contexts and historical periods.Not all societies have had,and still have,the same relationship with madness.It is only with the affirmation of the Modern State,and of Capitalism,that the idea of“normality”indispensable to be able to conceive diversity as something dangerously distant and different from the norm takes over.In our post-modern society,people with mental illness in Italy can resort to specialists and social-health services.But the heterogeneous answers given after the approval of law 180 appear to be increasingly diversified.In this research,much attention will be paid to how the social and health services,located in different areas of Italy(Messina,Rome,Trento)face the current growing risk of social,housing and economic isolation of these fragile subjects.The aim of the research is to explore the possibility of a new relationship between the social-health service and the local community.On the one hand,research investigates what the contribution of the services could be.On the other what the spaces of protagonism and participation of the community could be in inclusion process account.In order to better understand the differences between these two dimensions,a qualitative research approach was chosen through the conduct of in-depth interviews.In this way it was possible to investigate:(1)the partial representations characteristic of the single individual,family members,operators and stackholders in general;(2)the services around the topic dealt with is articulated.From the first results of the research it emerges that the territory can no longer be considered as an abstract entity,but becomes the social space within which the construction of a new community welfare can and must take place.
