HCT involves intravenous infusion of hematopoietic progenitor cells from human leukocyte antigen(HLA)-matched donor(allogeneic) or from the patient(autologous). Before HCT, the patient is prepared with high dose chemotherapy and/or radiotherapy to destroy residual malignant cells and to reduce immunologic resistance. After HCT, chemotherapy is used to prevent graft rejection and graft versus host disease(Gv HD). Neurological complications are related to the type of HCT, underlying disease, toxicity of the conditioning regimens, immunosuppression caused by conditioning regimens, vascular complications generated by thrombocytopenia and/or coagulopathy, Gv HD and inappropriate immune response. In this review, neurological complications are presented according to time of onset after HCT:(1) early complications(in the first month)-related to harvesting of stem cells, during conditioning(drug toxicity, posterior reversible encephalopathy syndrome), related to pancytopenia,(2) intermediate phase complications(second to sixth month)-central nervous system infections caused by prolonged neutropenia and progressive multifocal leukoencephalopathy due to JC virus,(3) late phase complications(after sixth month)-neurological complications of Gv HD, second neoplasms and relapses of the original disease.展开更多

Despite current multimodal-treatment approach, median-survival time and progression-free survival(PFS) remains short. Glioblastomas display a variety of molecular alterations, which necessitates determining which of these have a prognostic significance. This is a case of a 45-yearold patient who presented with progressive slurring of speech and features of raised intracranial pressure. Computed tomography(CT) scan revealed a large heterogeneously enhancing lesion in the left front-temporalperisylvian region with solid, cystic areas, suggestive of malignant glioma. Partial tumor-excision was followed by concurrent chemo-radiotherapy. Histopathologically, the tumor was astrocytoma grade-IV. Patient had an extended PFS of 12 mo, with an overall survival of 26 mo. Primary-GBM was confirmed using molecular markers and the immunophenotypic signature was defined by evaluating systemic expression of human telomerase reverse transcriptase, interleukin-6, neutrophil-lymphocyte ratio, tissue inhibitor of metalloproteinases-1, human chitinase-3-like-protein-1(YKL-40) and high mobility group-A1. Current findings suggest that this signature can identify worst outcomes, independent of clinical criteria.展开更多

Guided by the experience of traditional Chinese medicine (TCM), several new anticancer drugs have been found. Indirubin from indigofera tinctoria (青黛) is useful for the treatment of chronic myelocytic leukemia. On the basis of structure-activity relationship studies of these compounds, a second generation drug (N-methyl isoindigotin) has been developed. Clinical studies demonstrated that it is more effective than indirubin, 1O-hydroxy campothecin derived from Camptotheca acuminata (喜树) have exhibited definite activity on rodent tumors and it is used for phase Ⅱ studies.
