慢性心力衰竭(以下简称心衰),是各种心脏疾病的终末期阶段,其发病机制与免疫炎症密切相关。免疫细胞激活后,产生大量炎症因子,损伤心肌细胞,导致心肌纤维化、心室重构等病理改变。“毒损心络”是心衰发生、发展的主要病机,贯穿心衰发展的始终,以不同的形式影响心衰的发展及预后。该文通过查阅相关文献,基于毒邪与络病学说,从免疫炎症与心衰的关系及中药防治进行系统梳理,提出心衰早期,以心气虚为主,兼心气阴虚,尚未化毒、入络;心衰中期,心气亏虚为本,痰浊、瘀血、水饮留滞络脉,有化毒、入络趋势;心衰晚期,痰、瘀、水蕴结成毒,损伤心络。中药以复方治疗为主,多具有益气温阳、化瘀利水、解毒通络之功效,通过恢复M1/M2、Th1/Th2及Th17/Treg细胞之间的动态平衡,调节促炎/抗炎因子,从根本上祛除或缓解因毒邪产生的病理状态,减轻炎症,调整心之气血阴阳,达到扶正祛邪、保护心功能的作用,为临床防治心衰提供指导。

新医科背景下基于校院合作的妇产科护理学混合式教学模式的构建研究

目的探讨新医科背景下基于校院合作的混合式教学模式在妇产科护理学课程中的应用与效果。方法选择我校2019级110名护理本科生为对照组、2020级113名护理本科生为观察组。对照组采用传统教学模式,观察组采用新医科背景下基于校院合作的妇产科护理学混合式教学模式。课程结束后,比较两组的期末考试成绩、自我导向学习能力、评判性思维能力和教学满意度。结果观察组的期末考试成绩及优良率高于对照组(P<0.05)。观察组的自我导向评定量表各维度得分及总分均高于对照组(P<0.01)。观察组的评判性思维能力各维度得分及总分均高于对照组(P<0.05)。观察组的各项教学满意度评分均高于对照组(P<0.05)。结论新医科背景下基于校院合作的妇产科护理学混合式教学模式能有效提高学生的学习成绩、自我导向学习能力和评判性思维能力,进而能提高护理本科生整体教学质量。

据《论语·宪问》,孔子主张“以直报怨”,而对“以德报怨”则并未明确表示赞成或反对。通过考察《论语》中关于“直”、“怨”的各种论述,结合历代学者的相关观点,可以得出:“以直报怨”中的“直”,应该包含诚实、无私、符合通权达变原则的正直、等值、合乎礼的直率等丰富的含义;“以德报怨”的情况比较复杂,其中有的体现了人们不计前嫌、公而忘私的高尚品德,有的则反映了一些人的虚伪和充满私心的算计,因此,对于“以德报怨”,我们不能简单地予以肯定或否定;“以直报怨”既包含“以德报怨”中值得肯定或提倡的内容,又包含“以德报怨”中所没有的对怨恨对象采取相应报复措施的内容,因此是一种更为全面、合理的报怨方式。

白癜风是一种难治性皮肤色素脱失性疾病,可发于全身各部位皮肤,皮损处颜色逐渐减退、变白。历代医家认为“风邪侵袭”其发病的关键所在,近年来,大量医家在风邪致病的基础上,认为毒邪内蕴是白癜风缠绵难愈的重要因素。风性善于行而数变,白斑发病迅速多与风邪有关;毒邪凶、顽、难、痼、杂的特点,亦阻滞局部气血,经络不通,白斑易诊难消。通过查阅整理大量文献,详细论述风邪、毒邪引起白癜风发病的机理以及中医药治疗方法,为中医辨证治疗白癜风的理论研究和临床应用提供新思路。

从“虚瘀劫肝”论原发性肝癌中医药防治思路

理论创新是中医药发展的源泉,在新理论指导下形成的新方药或新诊治方案是中医药临床防治重大疑难疾病的新突破口。正邪交争,正不胜邪是慢性疾病持续进展的内在驱力和一般特性,慢性肝病持续恶性进展囊括了:肝纤维、肝硬化、肝癌等逐步恶化的病理进程。其在遵循中医“正不胜邪”基本病机的基础也必须有其特殊性和指向性,也可理解为在慢性肝病持续进展的病理状态下将一般性的病机进行指向性和特征性的阐发。肝正虚主要是指维持正常肝再生修复的相关机制和途径异常或紊乱,肝邪胜则是指肝损伤因素持续存在及其下游触发的恶性肝再生状态进行性亢进;而“瘀”作为正邪交争过程中产生的最主要病理产物也在不断推动疾病的恶性发展。在此基础上,从“虚瘀劫肝”论肝癌的病理新认识及补虚(补肾生髓、健脾)祛瘀(疏肝、柔肝、化肝)阻断慢性肝病持续恶性进展以防治肝癌的基本思路,期望有益于疑难肝脏病症的临床防治。

宣肺止嗽合剂治疗急性支气管炎(风邪犯肺证)随机双盲多中心研究

目的观察宣肺止嗽合剂对急性支气管炎(风邪犯肺证)患者中医证候及生活质量的影响。方法采用随机、双盲、安慰剂对照研究,在19个平行中心进行观察,将240例急性支气管炎(风邪犯肺证)患者分为试验组和对照组,每组120例。试验组口服宣肺止嗽合剂,对照组口服宣肺止嗽合剂模拟剂,疗程均为7 d。比较两组中医证候(日间咳嗽、夜间咳嗽、咳痰、咽痒)积分和莱塞斯特咳嗽生命质量问卷(the Leicester cough questionnaire,LCQ)评分。结果最终完成试验者232例,其中试验组118例,对照组114例。治疗后,两组中医证候积分降低,LCQ中生理性得分、心理性得分、社会性得分和总分均升高,试验组改变更为明显(P<0.01)。两组上述指标在治疗后、治疗后较基线的差值均有统计学意义(P<0.01)。全分析集(full analysis set,FAS)及符合方案集(per protocol set,PPS)结果一致。结论宣肺止嗽合剂可以明显改善急性支气管炎(风邪犯肺证)患者中医证候,提高其生活质量。

宣肺止嗽合剂对感染后咳嗽(风邪犯肺证)中医证候和生活质量的影响观察:一项随机、安慰剂对照、双盲、多中心临床研究

目的评价宣肺止嗽合剂对感染后咳嗽(风邪犯肺证)中医证候和生活质量的影响。方法采用随机、双盲、安慰剂对照、多中心的试验,纳入240例患者,按1∶1的比例分为试验组120例和对照组120例。试验组口服宣肺止嗽合剂,对照组口服宣肺止嗽合剂模拟剂,两组均20 mL/次,每日3次,疗程7 d。比较两组中医证候(日间咳嗽、夜间咳嗽、咳痰、咽痒)积分和莱塞斯特咳嗽生命质量问卷(生理性得分、心理性得分、社会性得分和总分)评分,并采用全分析(full analysis set,FAS)集及符合方案(per protocol set,PPS)集进行分析。结果238例进入FAS集(试验组和对照组均为119例),227例进入PPS集(试验组114例、对照组113例)。PPS与FAS分析均显示,治疗后,两组中医证候积分降低,莱塞斯特咳嗽生命质量问卷评分升高,试验组改变更为明显(P<0.01)。两组上述指标在治疗后、治疗后-基线的差值均有统计学意义(P<0.01)。结论宣肺止嗽合剂可以明显改善感染后咳嗽(风邪犯肺型)患者中医证候,提高其生活质量。

“遇邪”与清代假命案的形成与处理——以《巴县档案》为中心的讨论

有清一代,凡最终确定并不构成关系人犯罪的命案称为假命案。在对假命案的处理过程中,涉案人及州县官员均有借用“遇邪”之类理据来促成案件和解的行为。虽然百姓以死者“遇邪”为由和解命案的背后多隐含金钱因素的推动,但知县为了顺利结案,对于这一结果往往乐见其成,甚至在审案过程中还存在一些主动引导涉案人确认死者遇邪身亡之举。然而,由于朝廷以儒家思想规范社会,总体上不容妖邪鬼怪之说,州县在上呈详文中又往往隐去“遇邪”的死因记录,以致档案记载的司法状况多呈混乱。在当时社会环境下,此类司法实践存在一定的合理性,但同时也必须看到,这种司法实践会造成案情中许多对社会生活史和法制史研究有重要意义的真实细节的遗失。
