The current increase in the speed of natural-origin structure growth may be linked to their superior mechanical properties and environmental resilience. Biological composite structures with helicoidal schemes and designs have remarkable capacities to absorb impact energy and withstand damage. However, there is a dearth of extensive study on the influence of fiber redirection and reorientation inside the matrix of a helicoid structure on its mechanical performance and reactivity. The present study aimed to explore the static and transient responses of a bio-inspired helicoid laminated composite(B-iHLC) shell under the influence of an explosive load using an isomorphic method. The structural integrity of the shell is maintained by a viscoelastic basis known as the Pasternak foundation, which encompasses two coefficients of stiffness and one coefficient of damping. The equilibrium equations governing shell dynamics are obtained by using Hamilton's principle and including the modified first-order shear theory, therefore obviating the need to employ a shear correction factor. The paper's model and approach are validated by doing numerical comparisons with respected publications. The findings of this study may be used in the construction of military and civilian infrastructure in situations when the structure is subjected to severe stresses that might potentially result in catastrophic collapse. The findings of this paper serve as the foundation for several other issues, including geometric optimization and the dynamic response of similar mechanical structures.展开更多

The increase in its prevalence in the country is the result of multiple factors including economic stress imposed by precariousness, poor living conditions, sources of anxiety, anguish, depression and other behavioral disorders. Economic stress is a globalizing concept that integrates into a purely hermeneutic approach, a particular functioning of the nervous system of an individual who faces employment problems and precarious remuneration conditions. The non-satisfaction by an individual of his basic needs due to insufficient financial means can cause him to become irritable, aggressive, and socially and symbolically isolated, thereby increasing the desire to resort to morbid life models such as excessive consumption of narcotics and other psychoactive substances often associated with high blood pressure. The fight against the emergence of BPH is a complex, multifaceted and multifactorial reality that requires taking into account economic stress. The main objective of this survey is to describe the situation of economic stress within the Cameroonian population, which imposes precariousness and life models at risk of high blood pressure. Specifically, we determined the level of household income and the sources of income. Methods: A cross-sectional survey with a descriptive aim among five hundred households in the Central Region of Cameroon was conducted. A probabilistic technique called simple randomness was used. The number of households to be surveyed was determined indirectly using the Cochrane formula. Data collection in face-to-face mode using a physical questionnaire took place from July 1 to August 31, 2023, after obtaining ethical clearance from the Regional Health Research Ethics Committee, Human from the Center and an administrative authorization for data collection. Regarding their processing, the data was grouped during processing in Excel sheets. Normality and reliability tests of the collected data were carried out. For this, the Chi-square test was used for data with a qualitative value and that of Kolmogorov-Sminorf for data with a quantitative value. Descriptive analysis was possible using R software version 3.2, SPSS version 25.0, XLSTAT 2016, PAST and EXCEL programs from Microsoft Office 2013. Results: The main results highlight economic stress, with 45.60% of households surveyed earning less than US$154 per month;55% of household heads were women in single-parent families;14% of household heads were unemployed, 22% worked in the private sector and 19% were self-employed. This general economic situation leads to precarious living conditions, thereby increasing the risk of high blood pressure among the Cameroonian population.展开更多

Methods: The current study evaluated the rate of binge drinking among university students as they returned to normal life post-lockdown and whether binge drinking affected their mental health and quality of life. A cross-sectional study included 1382 university students (18 - 24 years) of one private Southwestern university in Brazil. Alcohol Use Disorders Identification, the Beck Depression Inventory-II, and the World Health Organization Quality of Life Brief were used. Data were collected after the COVID-19 pandemic lockdown period (from October 2021 to May 2022). Results: 52.6% of the students reported binge drinking, and 35.6% reported symptoms of depression. Binge drinkers were 1.41 times more likely to experience depressive symptoms when compared with non-binge drinkers, and binge drinking was a risk factor for lower quality of life in all three domains (physical, psychological and environmental). Conclusion: Our study highlights an increased prevalence of binge drinking in university students after the pandemic lockdown. The rise in binge drinking rates was linked to depressive symptoms and worsening of quality of life.展开更多

Over 16 million Americans suffer from smoking-related illnesses, and one-third of individuals with mental health issues are also cigarette smokers. Both physical and mental health are associated with measures of overall quality of life, a critical outcome that can influence treatment, intervention, and prevention decision-making. However, the direct relation between smoking and quality of life is not clear and requires further investigation. The present study utilized data from 34,309 adults who participated in Wave 5 of the Population Assessment of Tobacco and Health (PATH) to examine the relation between smoking and quality of life while considering the mediating effects of physical and mental health, and the moderating effect of social satisfaction. Using Conditional Process Analyses to integrate mediation and moderation approaches, the present study found that smoking had indirect associations with quality of life mediated through both physical and mental health and that social satisfaction moderated these relationships. Additionally, smoking had an independent direct effect on quality of life that was not moderated by social satisfaction. Results suggest the negative impacts associated with smoking reach beyond its effect on physical, social and mental well-being.展开更多

Muslim cancer patients and health professionals are faced with a dilemma between the necessity of fasting and its possible adverse effects during Ramadan on the health status of cancer patients. We believe that addressing the issue of fasting in cancer patients is of global interest. Objectives: Prevalence of fasting in cancer patients during Ramadan;Predictive factors of Ramadan fasting in cancer patients; The effect of fasting during Ramadan on the quality of life of cancer patients. Materials and Methods: A structured questionnaire was used to interview adult cancer patients who received chemotherapy IV—or oral treatment at the medical oncology department at the Hassan II University Hospital in Fez during the month of Ramadan 1443 Hijri (April to May 2022). The quality of cancer patients’ lives during their fasting practice in Ramadan was evaluated via the QLQ-C30 questionnaires. Results: 48% of patients did not fast during Ramadan, 22% and 30% were partial and complete fasters respectively. Patients, with OMS 0 to 1, who receive outpatient treatment, non-metastatic patients and patients on oral treatment were more frequent in the completely fasted group. All these factors showed statistically significant values (p < 0.05). 44% of patients sought advice from the treating oncologist if they could fast, while only 15% of patients during Ramadan asked an imam if they could be excused from fasting. A comparison of the QLQ-C30 data showed a significant improvement in emotional functioning with fasting, however, patients had clinically more but not significantly fatigue after fasting. Conclusion: Female gender, good WHO, non-metastatic disease, and oral or outpatient treatment were more likely to be fasting. This study argues that fasting did not influence the quality of life, and appears to significantly improve emotional functioning in cancer patients during Ramadan.展开更多

文摘The safe and reliable operation of lithium-ion batteries necessitates the accurate prediction of remaining useful life(RUL).However, this task is challenging due to the diverse ageing mechanisms, various operating conditions, and limited measured signals.Although data-driven methods are perceived as a promising solution, they ignore intrinsic battery physics, leading to compromised accuracy, low efficiency, and low interpretability. In response, this study integrates domain knowledge into deep learning to enhance the RUL prediction performance. We demonstrate accurate RUL prediction using only a single charging curve.First,a generalisable physics-based model is developed to extract ageing-correlated parameters that can describe and explain battery degradation from battery charging data. The parameters inform a deep neural network(DNN)to predict RUL with high accuracy and efficiency. The trained model is validated under 3 types of batteries working under 7 conditions, considering fully charged and partially charged cases.Using data from one cycle only, the proposed method achieves a root mean squared error(RMSE)of 11.42 cycles and a mean absolute relative error(MARE)of 3.19%on average, which are over45%and 44%lower compared to the two state-of-the-art data-driven methods, respectively.Besides its accuracy, the proposed method also outperforms existing methods in terms of efficiency, input burden, and robustness. The inherent relationship between the model parameters and the battery degradation mechanism is further revealed, substantiating the intrinsic superiority of the proposed method.

文摘BACKGROUND Ischemic stroke(IS)is a widely recognized disease characterized by high preva-lence,mortality,morbidity,disability,and recurrence rates. It ranks prominently in terms of mortality, constituting 60%-80%of stroke cases.AIM To explore the impact of comprehensive nursing care on the quality of life and swallowing function in individuals diagnosed with IS.METHODS This study comprised 172 patients with IS admitted to our hospital between February 2018 to March 2021.The participants were divided into two groups, namely the control group(n=80)receiving routine care and the research group(n=92)receiving comprehensive care.Various assessment scales, including the standard swallowing function assessment scale(SSA),National Institutes of Health Stroke scale(NIHSS),European stroke scale(ESS),self-rating anxiety scale(SAS),self-rating depression scale(SDS),Barthel index(BI),and the motor func-tion assessment scale(MAS),were employed to evaluate the improvement in swallowing function, neurological deficits, clinical outcomes, anxiety, depression, daily living activities, and motor function before and after care.Furthermore, the study compared the occurrence of adverse reactions during the nursing period, life quality before and after the intervention, rehabilitation compliance, and nursing satisfaction between the two groups.RESULTS After the nursing intervention, the research group exhibited significantly improved SSA and NIHSS scores compared to the control group(P<0.05).Moreover,both groups demonstrated significant reductions in SAS and SDS scores(P<0.05),with the research group showing more obvious advantages(P<0.05).Compared to the control group, the research group displayed significantly better ESS,BI,and MAS scores(P<0.05),coupled with a lower incidence of adverse reactions(P<0.05).Additionally,the research group demonstrated markedly higher levels of life quality, rehabilitation compliance, and nursing satisfaction compared to the control group(P<0.05).CONCLUSION Comprehensive nursing effectively improved swallowing function, quality of life, and patient satisfaction,high-lighting its clinical significance.
文摘Investigating natural-inspired applications is a perennially appealing subject for scientists. The findings of this paper serve as the foundation for several other issues, including geometric optimization and the dynamic response of similar mechanical structures.

文摘The travel time of rock compressional waves is an essential parameter used for estimating important rock properties, such as porosity, permeability, and lithology.Current methods, like wireline logging tests, provide broad measurements but lack finer resolution.Laboratory-based rock core measurements offer higher resolution but are resource-intensive.Conventionally,wireline logging and rock core measurements have been used independently. This study introduces a novel approach that integrates both data sources. The method leverages the detailed features from limited core data to enhance the resolution of wireline logging data.By combining machine learning with random field theory, the method allows for probabilistic predictions in regions with sparse data sampling. In this framework,12 parameters from wireline tests are used to predict trends in rock core data. The residuals are modeled using random field theory. The outcomes are high-resolution predictions that combine both the predicted trend and the probabilistic realizations of the residual.By utilizing unconditional and conditional random field theories, this method enables unconditional and conditional simulations of the underlying high-resolution rock compressional wave travel time profile and provides uncertainty estimates. This integrated approach optimizes the use of existing core and logging data. Its applicability is confirmed in an oil project in West China.

文摘For dissimilar metal welds(DMWs)involving nickel-based weld metal(WM)and ferritic heat resistant steel base metal(BM)in power plants, there must be an interface between WM and BM,and this interface suffers mechanical and microstructure mismatches and is often the rupture location of premature failure. In this study,a new form of WM/BM interface form, namely double Y-type interface was designed for the DMWs.Creep behaviors and life of DMWs containing double Y-type interface and conventional I-type interface were compared by finite element analysis and creep tests, and creep failure mechanisms were investigated by stress-strain analysis and microstructure characterization.By applying double Y-type interface instead of conventional I-type interface, failure location of DMW could be shifted from the WM/ferritic heat-affected zone(HAZ)interface into the ferritic HAZ or even the ferritic BM,and the failure mode change improved the creep life of DMW.The interface premature failure of I-type interface DMW was related to the coupling effect of microstructure degradation, stress and strain concentrations, and oxide notch on the WM/HAZ interface. The creep failure of double Y-type interface DMW was the result of Type IV fracture due to the creep voids and micro-cracks on fine-grain boundaries in HAZ,which was a result of the matrix softening of HAZ and lack of precipitate pinning at fine-grain boundaries. The double Y-type interface form separated the stress and strain concentrations in DMW from the WM/HAZ interface, preventing the trigger effect of oxide notch on interface failure and inhibiting the interfacial microstructure cracking. It is a novel scheme to prolong creep life and enhance reliability of DMW,by means of optimizing the interface form, decoupling the damage factors from WM/HAZ interface, and then changing the failure mechanism and shifting the failure location.
文摘In 1993,the World Bank released a global report on the efficacy of health promotion, introducing the disability-adjusted life years(DALY)as a novel indicator. The DALY,a composite metric incorporating temporal and qualitative data,is grounded in preferences regarding disability status. This review delineates the algorithm used to calculate the value of the proposed DALY synthetic indicator and elucidates key methodological challenges associated with its application. In contrast to the quality-adjusted life years approach, derived from multi-attribute utility theory, the DALY stands as an independent synthetic indicator that adopts the assumptions of the Time Trade Off utility technique to define Disability Weights.Claiming to rely on no mathematical or economic theory,DALY users appear to have exempted themselves from verifying whether this indicator meets the classical properties required of all indicators, notably content validity, reliability, specificity, and sensitivity. The DALY concept emerged primarily to facilitate comparisons of the health impacts of various diseases globally within the framework of the Global Burden of Disease initiative, leading to numerous publications in international literature.Despite widespread adoption, the DALY synthetic indicator has prompted significant methodological concerns since its inception, manifesting in inconsistent and non-reproducible results.Given the substantial diffusion of the DALY indicator and its critical role in health impact assessments,a reassessment is warranted. This reconsideration is imperative for enhancing the robustness and reliability of public health decisionmaking processes.
文摘BACKGROUND Hemorrhoids are among the most common and frequently encountered chronic anorectal diseases in anorectal surgery. They are venous clusters formed by con-gestion,expansion,and flexion of the venous plexus in the lower part of the rec-tum.Mixed hemorrhoids bleed easily and recurrently, and this can result in severe anemia.Hence, they may have a negative effect on the health of the patient and surgical treatment is required.Milligan-Morgan hemorrhoidectomy has been widely used since 1937 for the treatment of grade III and IV hemorrhoids.How-ever,most patients experience different degrees of postoperative pain that may cause anxiety.with mixed hemorrhoids post-surgery.METHODS The clinical data of patients with mixed hemorrhoids who underwent Milligan-Morgan hemorrhoidectomy were collected retrospectively. The basic character-istics of the enrolled patients with mixed hemorrhoids were recorded, and based on the Goligher clinical grading system, the hemorrhoids were classified as grades III or IV.The endpoint of this study was the disappearance of pain in all patients.Quantitative data were presented as mean±SD,such as age, pain score, and QoL score.Student’s t-test was used to compare the groups.RESULTS A total of 164 patients were enrolled. The distribution of the visual analog scale pain scores of all patients at 3,7,14 and 28 d after surgery showed that post-surgery pain was significantly reduced with the passage of time.Fourteen days after the operation, the pain had completely disappeared in some patients.Twenty-eight days after the surgery, none of the patients experienced any pain.Comparing the World Health Or-ganization Quality of Life-BREF self-reporting questionnaire scores of patients between 14 and 28 d after surgery,we observed that the quality-of-life scores of the patients post-surgery had significantly improved. There were six items that were compared at 14-and 28-d post-surgery.The mean QoL score 28 d after surgery(4.79±0.46)was higher than that at 14 d post-surgery(3.79±0.57).The mean health condition score 28 d after surgery(4.80±0.41)was also higher than that at 14 d post-surgery(4.01±0.62).The mean physical health score 28 d after surgery(32.10±2.96)was significantly higher than that at 14 d post-surgery(23.41±2.85).The mean psychological health score 28 d after surgery(27.22±1.62)was significantly higher than that at 14 d post-surgery(21.37±1.70).The mean social relations score 28 d after surgery(12.21±1.59)was significantly higher than that at 14 d post-surgery(6.32±1.66).The mean surrounding environment score 28 d after surgery(37.13±2.88)was significantly higher than that at 14 d post-surgery(28.42±2.86).The differences in quality-of-life scores at day 14 and day 28 post-surgery were ob-served to be statistically significant(P<0.001).CONCLUSION Milligan-Morgan hemorrhoidectomy can significantly improve the postoperative QoL of patients.Age,sex, and the number of surgical resections were important factors influencing Milligan-Morgan hemorrhoidectomy.

文摘Early-life stress(ELS)refers to a period of environmental/social deprivation, physica l,sexual or emotional abuse, neglect, severe and/or chronic trauma in the prenatal/early postnatal stage, which is closely related to many adverse psychiatric disorders later in life, such as depressive disorder, substance abuse, dysthymia, panic, anxiety, and suicidal behavior in adulthood(Waters and Gould,2022).
文摘Context/Objective: High blood pressure (HBP) currently represents the most widespread chronic non-communicable disease in Cameroon. This general economic situation leads to precarious living conditions, thereby increasing the risk of high blood pressure among the Cameroonian population.
文摘BACKGROUND The quality-adjusted life year(QALY)is a metric that is increasingly used today in the field of health economics to evaluate the value of different medical treatments and procedures.Surgical waiting lists(SWLs)represent a pressing problem in public healthcare. The QALY measure has rarely been used in the context of surgery. It would be interesting to know how many QALYs are lost by patients on SWLs.AIM To investigate the relationship between QALYs and SWLs in a systematic review of the scientific literature.METHODS The study was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Statement. An unlimited search was carried out in PubMed,updated on January 19,2024.Data on the following variables were investigated and analyzed:Specialty, country of study, procedure under study, scale used to measure QALYs,the use of a theoretical or real-life model, objectives of the study and items measured, the economic value assigned to the QALY in the country in question, and the results and conclusions published.RESULTS Forty-eight articles were selected for the study. No data were found regarding QALYs lost on SWLs.The specialties in which QALYs were studied the most in relation to the waiting list were urology and general surgery, with 15 articles each. The country in which the most studies of QALYs were carried out was the United States(n=21),followed by the United Kingdom(n=9)and Canada(n=7).The most studied procedure was organ transplantation(n=39),including 15 kidney,14 liver,5 heart,4 lung, and 1 intestinal.Arthroplasty(n=4),cataract surgery(n=2),bariatric surgery(n=1),mosaicplasty(n=1),and septoplasty(n=1)completed the surgical interventions included.Thirty-nine of the models used were theoretical(the most frequently applied being the Markov model,n=34),and nine were real-life.The survey used to measure quality of life in 11 articles was the European Quality of Life-5 dimensions, but in 32 articles the survey was not specified. The willingness-to-pay per QALY gained ranged from$100000 in the United States to€20000 in Spain.CONCLUSION The relationship between QALYs and SWLs has only rarely been studied in the literature. The rate of QALYs lost on SWLs has not been determined.Future research is warranted to address this issue.
文摘Background: The restrictive lockdown measures that were introduced as a response to the COVID-19 pandemic imposed constraints that affected university students’ drinking habits and mental health. The rise in binge drinking rates was linked to depressive symptoms and worsening of quality of life.
文摘Cigarette smoking has known negative effects on physical and mental health. Results suggest the negative impacts associated with smoking reach beyond its effect on physical, social and mental well-being.

文摘Tree interactions are essential for the structure, dynamics, and function of forest ecosystems, but variations in the architecture of life-stage interaction networks(LSINs)across forests is unclear. Here,we constructed 16 LSINs in the mountainous forests of northwest Hebei,China based on crown overlap from four mixed forests with two dominant tree species. Our results show that LSINs decrease the complexity of stand densities and basal areas due to the interaction cluster differentiation. In addition,we found that mature trees and saplings play different roles, the first acting as“hub”life stages with high connectivity and the second,as“bridges”controlling information flow with high centrality.Across the forests, life stages with higher importance showed better parameter stability within LSINs.These results reveal that the structure of tree interactions among life stages is highly related to stand variables. Our efforts contribute to the understanding of LSIN complexity and provide a basis for further research on tree interactions in complex forest communities.
文摘Otosclerosis is a well-known disease which mainly affects middle-aged female patients, causing conductive or mixed hearing loss.Diagnosis is essentially made through a complete audiometric battery testing, while treatment substantially relies on stapes surgery. In this study,we analyzed audiometric and Quality-of-Life data from 33 patients in order to refine the surgical indications(unilateral versus bilateral stapes surgery)and assess the best technique(stapedotomy versus partial stapedectomy)and timing of treatment. The original questionnaire OTOS,in the Italian language, was administered to all patients. In this study, most of the patients were female(18/33),with a mean age of 53 years old. Both surgical techniques appear to be effective with no significant difference in post-operative Air Bone Gap.OTOS questionnaire appeared to be effective in assessing quality of life in patients treated by stapes surgery.Post-operative QoL appeared to be similar in bilaterally and unilaterally operated patients, suggesting that a contralateral intervention after effective surgery should be undertaken only if expressly requested by the patient.
文摘Introduction: Ramadan is the ninth lunar month of the Muslim calendar, and fasting during Ramadan is one of the five pillars of Islam. This study argues that fasting did not influence the quality of life, and appears to significantly improve emotional functioning in cancer patients during Ramadan.
